Stroboscopic findings in patients with benign laryngeal lesions: a brief report

Abstract: Background: Laryngeal videostroboscopy is an important noninvasive diagnostic tool in patients with dysphonia. More than 50% of patients with dysphonia have a benign laryngeal lesion on vocal fold examination. The aim of this study was to evaluate patients with benign laryngeal lesions by videostroboscopy.Methods: This cross-sectional study was done on 159 patients with dysphonia in Amiralam Hospital in Tehran, Iran during 2006-2007. All the patients underwent stroboscopic examination of the vocal folds, including their movement. We also evaluated the patients for mucosal status, mucosal wave and patterns of glottal closure.Results: Eighty-two patients participating in the study were female and 77 were male. Reflux laryngitis and muscle tension dysphonia were the most observed disorders in the patient population. Patients with sulcus vocalis and intracordal cysts had the worst mucosal wave patterns.Conclusion: Laryngeal videostroboscopy is a useful tool for the diagnosis and treatment planning in patients with benign laryngeal lesions.
Evaluation of Safety Standards in Operating Rooms of Tehran University of Medical Sciences (TUMS) Hospitals in 2010

Abstract: Background and Aim: The operating room is one of the main units in a hospital, where the most important phase of treatment is performed. Due to the physical properties, use of gases, and different electric devices in the operating room, the safety of both the patients and the staff should be considered carefully. This study aims to estimate the safety standards of the operating rooms in Tehran University Hospitals and to provide solutions to the existing problems. Materials and Methods: This is a descriptive, cross-sectional study conducted in the operating rooms of TUMS hospitals. The research instrument was a checklist used by interviewers while observing the place. The safety standards were studied in terms of physical environment, protection against fire , personnel safety, patients safety, and infection control. The data were analyzed by SPSS software. Results: The overall safety of the operating rooms of TUMS hospitals was found to be 84.9 percent. The figures for infection control and personnel safety, however, were lower. Conclusion: The operating rooms of Tehran University of Medical Sciences hospitals were studied in terms of five safety standards. Considering the physical environment, fire protection, and patient safety, they were quite safe but as to infection control they were relatively safe. However, they were found to be unsafe regarding the safety of their staff. Tehran Heart Center with an overall safety of 97.5% for its operating rooms can be considered as a model for the other TUMS treatment centers.
Topical mitomycin-C for treatment of laryngotracheal stenosis after the treatment with laser and dilatation

In this survey, the effect of topical mitomycin-C after laser therapy and dilatation for preventing laryngotracheal stenosis is considered."n"nMethods: Twenty three cases suffered from laryngotracheal stenosis were studied in a clinical trial between years 2006-2007 in Amir-alam educational Hospital in Tehran, Iran. The patients were divided into two groups. In 12 patients (case group), we used topical mitomycin-C (1mg/ml for 4 min) after dilatation and laser therapy, and in 11 of them (control group), this solution was not used. All patients were controlled by endoscopy one and three months later, and then followed up. The results of this study were compared statistically."n"nResults: In the control group after treatment (dilatation and laser), the average changed- vertical and horizontal lengths of stenosis were 0.47±0.77mm and 0.50±0.60mm respectively. However in the control group these changed averages were 2.28±1.15mm and 2.23±1.03mm, respectively after treatment (p<0.0001). The average changed cross section between the two groups were different significantly (p<0.0001), too."n"nConclusions: In the laryngotracheal stenosis, the use of topical mitomycin-C could reduce the recurrence of the stenosis after dilatation and after the laser procedures. In addition, the need for repeated therapy will be reduced.
Seroepidemiologic survey of influenza C virus infection

Abstract: This is a report of the first serological survey of influenza C virus in Iran, performed during a one year period (March 1997-May 1998). This study was accomplished in the National Influenza Center-Division of Virology in Tehran University of Medical Scinces. 1080 samples of serum (689 samples from Tehran and 391 samples from other provinces) were assayed for the presence of antibodies against influenza C virus (C/Paris/1/67) by haemagglutination inhibition (HI) test. 43.7% of people tested in Tehran and 40.7% of people tested from other provinces had protective antibodies against influenza C virus. Distribution of seropositives in various age groups had a somewhat similar pattern as what has been reported from other countries. The results of this study indicates that the lowest level of protective antibody titer is found at childhood and the level increases with age. The protective antibody titer level off for 20-30 years old age group and decreases in older age groups. These results indicates a primary contact in childhood, reinfection in adulthood. The influenza C virus is simultaneously circulating in Iran with other types of influenza viruses (types A and B).
Estimation of Septal Wall Thickness by Processing Sequential Echocardiographic Images

Abstract: Background: Non-invasive quantitative analysis of the heart wall thickness is a fundamental step in diagnosis and discrimination of heart disease. Thickness measurements in 2D echocardiographic images have many applicationsin research and clinic for assessment of wall stress, wall thickening and viability parameters. The measurement of interventricular septum wall thickness by conventional manual method is more dependent on sonographer’s experiment.This encouraged researchers to develop a semi-automatic computer algorithm to accessinterventricular septum segments thickness. Patients and Methods: We proposed and developed a computerized algorithm for wall thickness measurements in 2D echocardiographic image frames. In this program, wall thickness measurement is based on intensity profilefunction and adaptive bilateral thresholding operation. For validation, thicknesses of septum base and mid segments were estimated in constituent image frames using proposed technique followed by comparing them with conventional manual results from same images of the cardiac cycle by statistical methods. Results: In our sample image frames (240 corresponding segments; with different range of image quality), a bias of 0.10 mm and 0.12 mm with SD differences of ±0.81 mm and ±0.72 mm and correlation coefficients of 0.87 and 0.89 were found in base and mid segments, respectively. Interobserver variability using the computer-assisted method (CAM) and conventional manual technique (CMM) were 4.0% and 4.7% for the basal and 2.8% and 3.9% for the middle segments. Conclusion: The method introduced in the present study permits precise thickness assessment of base and mid segments of the interventricular septum wall with high concordance with CMM
Evaluation of Regional Displacement in Myocardium during a Heart Cycle Based on Mathematical Analysis of TDE Images

Abstract: Introduction & Background: Tissue Doppler Echocar-diography (TDE) method has been recently devel-oped, which permits quantification of intramural myocardial velocities by detection of consecutive phase shifts of the ultrasound signal arising from the myocardium. Time resolution in this system is high; therefore, we can evaluate regional function of myo-cardium muscle. In clinic, peak velocities of iso-volumic, systolic, and diastolic phases of heart cycle are conventionally measured to diagnose and dis-criminate heart diseases. Other studies have shown the sensitivity and specificity of this method to be 70 and 80 percent. However, there is no report of the absolute displacement variation of myocardium in total heart cycle. Materials & Methods: The purpose of this study was an integral assessment of time-velocity profile from TDE images during systolic and diastolic phases of heart cycle as a new method for evaluation of total absolute displacement of myocardium muscle based on numerical integral. We also used results of some integral function for evaluation of this method. For this reason, we proposed a computer algorithm based on trapezoid sums and translated it to a computer program using image processing toolbox of Matlab-7 software. For assessment of approximation of this method, we examined equal widths of 1-, 2-, 3-, 4-, 5-, 10-, 15-, 20-, 25-, and 30- pixel steps for estima-tion of the area under time-velocity curves (each pixel was equal to 4ms). The results of 1 to 5 pixels were close to each other (coefficient of variation: 0.2%). For assessment of accuracy, polynomial of or-ders one to four and exponential function were plot-ted using the mathematical toolbox of Mathlab-7, and the results saved as bitmap images. Then, we calcu-lated the approximate value of area under curve using the proposed algorithm and calculated the error value as the absolute difference between approximate and real results divided by real results ( 0.5%). Echocar-diographic recordings (time-velocity profiles) were obtained in basal and middle segments of interven-tricular septum of a 44-year-old man suffering from coronary artery disease (CAD) using standard four-chamber view, according to the guidelines of the American Society of Echocardiography at end expira-tion with 3 MHz sector transducer at 210 frames per second, and electrocardiogram were recorded simultaneously. The time-velocity curves were saved as full-color images and were transferred to a personal computer for off-line analysis. Abstract: Time-cost trade off problem (TCTP), known in the literature as project crashing problem (PCP) and project speeding up problem (PSP) is a part of project management in planning phase. In this problem, determining the optimal levels of activity durations and activity costs which satisfy the project goal(s), leads to a balance between the project completion time and the project total cost. A large amount of literature has studied this problem under various behavior of cost function. But, in all of them, influence of discount has not been in-vestigated. Hence, in this paper, TCTP would be studied considering the influence of discount on the re-source price, using genetic algorithm (GA). The performance of proposed idea has been tested on a medium scale test problem and several computational experiments have been conducted to investigate the appropriate levels of proposed GA considering accuracy and computational time.
Clinical Features and Differential Diagnoses in Laryngeal Mucoepidermoid Carcinoma

Abstract: Mucoepidermoid carcinoma is the most common malignant tumor of salivary glands. However, it is a rare entity in larynx. Laryngeal cases are frequently misdiagnosed with other malignancies and they are under-reported. So, recognizing the clinical and histological features of this tumor is essential. Laryngeal mucoepidermoid carcinoma can arise in supraglottis, glottis and subglottis. Generally, it presents as a submucosal mass; therefore, progressive symptoms without any identifiable lesion in laryngoscopy must be well considered. The prognosis is somehow dependent on the histological features. In high-grade tumors, recurrence is more common and radical surgery with radiotherapy is recommended. In this paper, we provide a thorough literature review on mucoepidermoid carcinoma in the larynx. The most important distinguishing features of mucoepidermoid carcinoma and its two major differential diagnoses (squamous cell carcinoma and adenosquamous carcinoma) are clearly stated and pitfalls in true diagnosis of this tumor are discussed.
