CALIDAD DE LAS LEYES: ALGUNOS PUNTOS CRíTICOS

Abstract: El autor se pregunta sobre las posibilidades y límites de mejorar la calidad de las leyes y el significado de las expresiones “calidad de la legislación” y “calidad de las leyes”. Seguidamente, a partir de una muestra de estudios sobre el tema, identifica el método de diagnóstico utilizado en ellos, hace un compendio de los principales problemas que afectan la calidad de las leyes y las soluciones propuestas para superarlos, y las pone en relación conlas áreas de estudios que integran la Teoría de la Legislación. La principal conclusión es que la “calidad de las leyes” está asociada a factores consustanciales al Derecho moderno como su vinculación a las fuerzas políticas actuantes en la sociedad, su carácter formal-racional, su producción centralizada y burocrática y el lenguaje técnico-especializado y cuasi-formalizado en que se expresan.

Abstract: El objetivo básico de este trabajo es identificar las principales causas que, desde el punto de vista institucional, normativo, teórico y metodológico, afectan la calidad de las leyes en Cuba. Para conseguirlo, se hace una descripción gen eral del régimen político establecido en la Constitución de 1976, y se utilizan los instrumentos analíticos desarrollados por la teoría y la técnica de la legislación en los últimos treinta a os, y sus antecedentes en los siglos XVIII y XIX. De las conclusiones obtenidas se deriva un conjunto de recomendaciones que, a juicio del autor, podrían contribuir a mejorar la calidad de las leyes en Cuba.

Abstract: The recruitment can be treated as a decision making problem, this is evidenced in the analysis of its definitions is done in this paper. In addition to the comparative nature and decision-making that characterizes this process, the activities as part of it is made have a high participation and reliance on human judgment, which it a process in which there is a high subjectivity in the information handled, further information can be both qualitatively and quantitatively, this can result in a process with a high level of uncertainty. That is why there is a marked tendency to give solutions to the problems of personnel selection using decision-making methods that integrate the use of soft computing techniques. In this paper an analysis of trends and methods used in solving recruitment problems. We propose a recommendation to the conditions in which the methods studied may have a better result in it application. La selección de personal puede ser tratado como un problema de toma de decisión, esto está evidenciado en el análisis de sus definiciones que se hace en el presente trabajo. Además de la naturaleza comparativa y de decisión que caracteriza este proceso, las actividades que como parte de él se realizan tienen una elevada participación y dependencia del juicio humano, lo que de él un proceso en cual existe una elevada subjetividad en la información que se maneja, información que además que puede ser tanto cualitativa como cuantitativa, esto se puede redundar en un proceso con un alto nivel de incertidumbre. Es por ello que existe una marcada tendencia a dar soluciones a los problemas de selección de personal empleando métodos de toma de decisión que integran el uso de técnicas de soft computing. En el presente trabajo se realiza un análisis de las tendencias y los métodos más empleados en la solución de problemas de selección de personal. Se plantea una recomendación de las condiciones en las que los métodos estudiados pueden tener un mejor resultado en su aplicación.

Abstract: aim: due to the importance of the cardiovascular disease in costa rica, and the lack of information about cardiac rehabilitation programs in our country, the objective of this study is to determine the results of a phase ii cardiac rehabilitation program at the national center of rehabilitation. methods: retrospective study based on a review of the clinical records of patients with coronary artery disease referred for the phase ii cardiac rehabilitation program from march 2009 to may 2010. of a total of 158 patients, 131 finished the phase ii. we compared the results of maximal functional capacity estimated by a treadmill stress testing, and lipid profile at the beginning and the end of the 10 - 12 week program. results: we found a significant statistical increase in the functional capacity of metabolic equivalents (mets) in average of 29,7% at the end of the 10-12 week program (p<0,05) estimated by a treadmill stress testing; the group that obtain the bigger increase was the “high risk” patients with 64,37% and with less increase the younger patients with 19,15%. we found a lowering in average of total cholesterol of 4.44mg/dl (-2.9%) (p = 0.145), ldl-coholesterol of 0.96 mg/dl (-1.1%) (p=0.746), triglycerides of 19.41mg/dl (-11.2%) (p=0.016) and increase in hdl-cholesterol of 0.95mg/dl (2.5%) (p= 0.181). conclusions: the phase ii cardiac rehabilitation program of the cenare reported an increase in the functional capacity of patients with coronary artery disease in average of 29,7%, been this increase higher in the “high risk” patients and a lesser increase in the ones with less age. we did not find significant changes in the lipid profile at the end of this training period.

Abstract: Objetivo: Debido a la importancia de la enfermedad cardiovascular en Costa Rica y a la escasa información de los programas de rehabilitación cardiaca a nivel nacional, el presente estudio buscó describir los resultados del Programa de Rehabilitación Cardiaca, Fase II, en el Centro Nacional de Rehabilitación. Métodos: Se trata de un estudio retrospectivo de revisión de expedientes clínicos de pacientes portadores de enfermedad arterial coronaria, remitidos a un programa de rehabilitación cardiaca desde marzo de 2009 a mayo de 2010. De un total de 158 pacientes, 131 finalizaron la fase II. Se comparó la capacidad funcional máxima -estimada mediante una prueba de esfuerzo en banda sin fin- y el perfil de lípidos. Estas pruebas se realizaron al inicio y al final de esta fase II, la cual consta de 10 a 12 semanas de duración. Resultados: Se obtuvo un aumento significativo de la capacidad funcional estimada en Equivalentes Metabólicos (METs), con un promedio equivalente a un 29.7% al finalizar las 10 a 12 semanas (p<0,05), valorado mediante una prueba de esfuerzo en banda sin fin; mayor en pacientes de “riesgo alto” con 64,37% y menor en pacientes de menos edad equivalente a 19,15%. Se encontró una disminución promedio en el colesterol total de 4.44mg/dl (-2.9%) (p = 0.145), en las LDL de 0.96 mg/dl (-1.1%) (p=0.746), en los triglicéridos de 19.41mg/dl (-11.2%) (p=0.016) y un aumento de las HDL en 0.95mg/dl (2.5%) (p= 0.181). Conclusiones: El programa reportó una mejoría de la capacidad funcional de los pacientes portadores de enfermedad arterial coronaria en promedio de 29.7%, con un mayor impacto en pacientes de “alto riesgo” y menor en los pacientes de menor edad. Con respecto al perfil lípido, no se obtuvieron cambios significativos en la medición al cabo de este periodo de entrenamiento. Abstract: a review of the topic on extrapulmonary articular tuberculosis was made for having found a male patient with this pathology of pure presentation with no other systemic manifestations. this is the first patient diagnosed at the community comprehensive hospital of san andrés, municipality of caracollo, cervado province, department of oruro, bolivia. on reviewing the topic it was proven the infrequency of this form of presentation in spite of the fact that extrapulmonary tuberculosis is common in the region due to the existence of the following factors: cold, height with little tenor of oxygen, malnutrition, dust storms, many mining workers, etc. the patient was presented and his satisfactory evolution as regards the base pathology was showed. it was concluded that tuberculosis in its articular form is rare in appearance and that its diagnosis is difficult due to the absence of echographic or radiographic patterns defined. the anatomopathological diagnosis is the only one that can determine the aetiology of the tuberculosis of a proliferative injury of the synovia. as synovectomy is a generally disabling intervention, an early diagnosis and an adequate medical treatment on time will prevent functional future limitations. it is recommended to perform a diagnostic echography in every carrier of knee synovitis of long evolution, and to conduct specific studies for the detection of tuberculosis in those patients among whom complex images of regular snowflake-like edges are observed.

Abstract: a bibliographic review was carried out on the oxidative stress taking into account the normal physiology of this metabolic mechanism, the causes of its alteration and the damaged produced, when the defense organic system are unbalanced to evidence the link between the oxidative damage to cells and the physiology and pathology of the injuries produced by major burns, from the analysis of the printed bibliographic national and international sources and in digital format on this subject. the significance to design more effective strategies is emphasized that take into account the magnitude of the damage produced by the oxidative stress in the treatment of burnt patients to modulate the oxidative damage to cells. we conclude that in the medical practice it is not possible to avoid to patients the new situation of redox state. the healthcare efforts must to be directed once again to help to organism in restoration of redox balance by means of an appropriate and early recovery with practices of active surgical tactics minimizing the external damages, supply appropriately the nutrients and to respect the politic of use of systemic antibiotics, by means of the use of parental antioxidants to reduce the hospital stay with the support of psychologists and physiatrist trained in the therapy used.
Desbalance redox en pacientes quemados mayores

Abstract: introduction: according to current researches regarding the burnt patient, it is interesting to assess the oxidative stress alterations in them to seek its link with the physiopathologic features of this nosologic entity and its possibility to establish life prognosis. objective: to assess the behavior of pro-oxidative and oxidative markers of patients with severe burns and to identify the possible relationship among the values of these markers with the group where the patients are located within a cuban classification of life prognosis. methods: sample included 15 burnt patients, distributed in: six severe, six very severe and three critically severe, according the cuban classification of life prognosis, determining the plasma concentrations of malonyldialdehyde, superoxide, dismutase, catalase and lipid peroxidation during the first twenty four hours, at sixth day and at twenty one days after lesion. data were processed using the statistica 6.1 program. results: marker's values changed the pathological way, with a significant level between each study moment. the oxidative stress in these patients appeared during the first hours after trauma and its behavior not depended on severity of patients according to the cuban classification of the life prognosis. conclusions: the oxidative lack of balanced present in the burnt patient covers from the phase of hypovolemic shock up to the adaptation according the kirsbaum physiopathologic classification. the variation of the oxidative stress markers not behaves in a homogenous way in the study groups of the cuban classification of life prognosis.
Miliar tuberculosis with heart commitment in the immediate puerperium: a case presentation.

Abstract: The miliar tuberculosis is a form of progressive illness, resultant of the dissemination of theMycobacterium tuberculosis. It affects children, old men and immune deppressed patients.The pregnancy and puerperium are two states that increase the risk of contracting thisillness. The case of a patient that begins with dyspnea, pulmonary rales and oedemas ininferior members to the 20 days of a caesarean operation, is presented. In the thorax Rx itwas detected cardiomegaly and multinodulars lung images. The echocardiogram showedsevere pericardial effusion, with fibrin remains in its interior, without signs of cardiac tamponade. Antituberculosis and heart failure treatments were administered. The clinicalevolution was favourable.The form of presentation of this illness and their similarity with other causes of heart failureunderlines the necessity of their early diagnosis, fundamentally in the countries with highdegree of infection.
