Focal peliosis hepatis in a colon cancer patient resembling metastatic liver tumor

Abstract: Peliosis hepatis (PH) is a rare benign condition characterized by the presence of multiple, randomly distributed, blood filled cystic areas of variable size within the liver parenchyma. PH is difficult to recognize and may be mistaken for neoplasm, metastases or multiple abscesses. A 75-year-old female with a previous history of colon cancer was admitted when a liver mass in the right liver lobe was found 11 mo after surgery during the follow-up period. Computed tomography and magnetic resonance imaging scan of the abdomen were performed. The initial possible diagnosis was metastatic hepatocellular carcinoma. The patient underwent excision of the hepatic segment where the nodule was located. The pathological diagnosis of the surgical specimen was PH. PH should be considered in the differential diagnosis of new liver lesions in patients whose clinical settings do not clearly favor metastasization. Clinicians and radiologists must recognize these lesions to minimize the probability of misdiagnosis and inappropriate treatment.

Abstract: Based on the ionic group theory, the SHG coefficients of KB5 crystal have been calculated, which leads to the following values d31=2.61×1010 esu, d32=0.07×1010 esu and d33=3.26×1010 esu. They agree fairly well with the expeimental ones. Our calculation results indicate that the hydrogen bond of (B5O10) ionic group has a large effect on the coefficients of KB5 crystal, we are therefore able to determine to how large an extent the H belongs to the ionic group. The probability of the H belonging to the ionic group is about 40-60%. Furthermore, the reasons for too small SHG coefficient of KB5 have been analysed. In addition to the small microscopic SHG coefficients of (B5O10) group, the space arrangement of (B5O10) group is in an unfavourable manner so that the largest microscopic SHG coefficients, x123, are cancelled each other. Finally, some structural requirments are proposed to search for the new type non-linear optical crystals among the B-O compounds, composed of six-membered rings, each having three borons in trigonal and tetrahedral coordination.

Abstract: In the crystal structure of the title organic salt, C16H19N2+·C9H11O3S ·H2O, the cations pack head-to-tail within a sheet and are aligned in opposite directions in neighboring sheets. The benzene ring of the anion makes an angle of 76.99 (6)° with the plane of the cationic chromophore. The cations are situated in the ab plane, whereas the benzene rings of the anions lie in the ac plane.
Wnt5a participates in hepatic stellate cell activation observed by gene expression profile and functional assays

Abstract: AIM: To identify differentially expressed genes in quiescent and activated hepatic stellate cells (HSCs) and explore their functions. METHODS: HSCs were isolated from the normal Sprague Dawley rats by in suit perfusion of collagenase and pronase and density Nycodenz gradient centrifugation. Total RNA and mRNA of quiescent HSCs, and culture-activated HSCs were extracted, quantified and reversely transcripted into cDNA. The global gene expression profile was analyzed by microarray with Affymetrix rat genechip. Differentially expressed genes were annotated with Gene Ontology (GO) and analyzed with Kyoto encyclopedia of genes and genomes (KEGG) pathway using the Database for Annotation, Visualization and Integrated Discovery. Microarray data were validated by quantitative real-time polymerase chain reaction (qRT-PCR). The function of Wnt5a on human HSCs line LX-2 was assessed with lentivirus-mediated Wnt5a RNAi. The expression of Wnt5a in fibrotic liver of a carbon tetrachloride (CCl4)-induced fibrosis rat model was also analyzed with Western blotting. RESULTS: Of the 28 700 genes represented on this chip, 2566 genes displayed at least a 2-fold increase or decrease in expression at a P < 0.01 level with a false discovery rate. Of these, 1396 genes were upregulated, while 1170 genes were downregulated in culture-activated HSCs. These differentially expressed transcripts were grouped into 545 GO based on biological process GO terms. The most enriched GO terms included response to wounding, wound healing, regulation of cell growth, vasculature development and actin cytoskeleton organization. KEGG pathway analysis revealed that Wnt5a signaling pathway participated in the activation of HSCs. Wnt5a was significantly increased in culture-activated HSCs as compared with quiescent HSCs. qRT-PCR validated the microarray data. Lentivirus-mediated suppression of Wnt5a expression in activated LX-2 resulted in significantly impaired proliferation, downregulated expressions of type I collagen and transforming growth factor-β1. Wnt5a was upregulated in the fibrotic liver of a CCl4-induced fibrosis rat model. CONCLUSION: Wnt5a is involved in the activation of HSCs, and it may serve as a novel therapeutic target in the treatment of liver fibrosis.
Pulmonary Lymphangioleiomyomatosis: A Clinicopathological Analysis of Ten Cases.

Abstract: Background: Lymphangioleiomyomatosis (LAM) of the lung is a very rare disease ofunknown etiology which occurs particularly in women of childbearing age.So far, there have been very few reports of LAM in Taiwan.Methods: Data on all patients with a clinical or pathological diagnosis of LAM inChang Gung Memorial Hospital from 1990 to 2001 were collected from thepathology files or by consultation with thoracic surgeons, chest physicians,and radiologists of this institution. Cases were confirmed by lung biopsy andhigh-resolution computed tomographic scanning. Clinical data were obtainedfrom patients' charts. Additional immunostaining for HMB-45 antigen, estrogenreceptor, and progesterone receptor was carried out for cases with availableparaffin blocks.Results: In total, 10 patients confirmed with a diagnosis of LAM and adequate clinicalfollow-up information were found from 1991 to 2001. All were femalesof fertility age. The follow-up periods ranged from 18 to 167 months. Sixpatients received hormonal therapy. According to the pulmonary functiontests and clinical symptoms, 6 patients were in stable condition and 4 showedslow progression of disease by the time of the last follow-up. Two patientsdied after lung transplantation and abdominal surgery for retroperitonealLAM, respectively, at another medical center.Conclusions: The clinicopathological features of our series are similar to those reportedfrom other countries. The clinical course was slowly progressive with no significantresponse to hormonal therapy. Lung biopsies are important for adiagnosis, but the size and location of the biopsy specimens greatly affect thediagnostic reliability.
Minimally Invasive Surgery for Coronary Artery Disease with Associated Lung Cancer

Abstract: Simultaneous surgical management of patients with co-existing ischemic heart diseaseand lung carcinoma remains controversial. Traditionally, these operations have been staged,with coronary artery revascularization first, followed by pulmonary resection at a later date.Under this procedure, tumor resection is delayed, and these staged procedures may increasemorbidity and cost. Our experience with minimally invasive surgery in the cardiac and thoracicfields suggests that both diseases can be resolved within the same operation. Thisreport presents a 65-year-old man with coronary artery disease (CAD), and left lung cancer.He received concomitant coronary artery bypass grafting for CAD and pulmonary resectionfor lung cancer via a left parasternal minithoracotomy. No complaints or clinical signs wereobserved during a 6 month follow-up. Therefore, major cardiac and thoracic procedures thatare performed via a minimally invasive approach using conventional instruments, can resultin a safer, quicker, and more economical procedure. The results of the a minimally invasiveapproach demonstrated the technical feasibility of treating a triple vessel cardiac diseasewith lung cancer.

Abstract: Functional separation of the lungs may be accomplished by several methods. Patientswith a stiff neck and limited mouth opening restrict options to a one-lung ventilation. Wereport the use of the WuScope video system, a new tool for facilitating double-lumen endotrachealtube placement for one-lung ventilation, in a patient who suffered from ankylosingspondylitis with a stiff, flexed neck and limited mouth opening for his recurrent spontaneouspneumothorax.
Correlation of Anaplastic Lymphoma Kinase Overexpression and the EML4-ALK Fusion Gene in Non-Small Cell Lung Cancer by Immunohistochemical Study

Abstract: Background: Recently the echinoderm microtubule-associated protein-like 4-anaplastic lymphoma kinase (EML4-ALK) fusion gene with transforming activity was identified in non-small cell lung cancer (NSCLC). In addition, NSCLC patients with the EML4-ALK fusion gene had a dramatic response and longer progression free survival after ALK inhibitor treatment than those without this fusion gene. However, the incidence and clinical and molecular characteristics of the EML4-ALK fusion gene in NSCLC patients of Taiwan are still unclear.Methods:Sixty-four fresh frozen tumor specimens were obtained from the tissue bank of Chang Gung Memorial Hospital for RNA extraction and EML4-ALK fusion gene detection. Paraffin sections of lung tumors from all of these patients were available and were analyzed for ALK protein expression by immunohistochemical (IHC) study. The results were correlated with clinical and molecular biomarkers.Results: Three of the 64 tumors (4.7%) had the EML4-ALK fusion gene. Two were adenocarcinomas, and one was adenosquamous carcinoma. Twenty patients with non-squamous cell carcinomas had epidermal growth factor receptor (EGFR) mutations, so the EML4-ALK fusion gene was found in 14.3% of EGFR wild type non-squamous cell carcinomas. Two tumors were variant 3 (3a+3b with 3b predominant) and had strong staining (3+) for ALK by IHC stains. One tumor was variant 1 and had moderate staining (2+) for ALK. None of the ALK wild type tumors had strong staining for ALK. When compared with other clinical and molecular features, only the IHC stain for ALK was significantly correlated with the EML4-ALK fusion gene (p = 0.0002).Conclusions: ALK overexpression detected by IHC study could be a promising detection method for the EML4-ALK fusion gene and is worth further confirmation with more samples.
Effective Mechanism for Social Recommendation of News

Abstract: Recommendation systems represent an important tool for news distribution on the Internet. In this work we modify a recently proposed social recommendation model in order to deal with no explicit ratings of users on news. The model consists of a network of users which continually adapts in order to achieve an efficient news traffic. To optimize network's topology we propose different stochastic algorithms that are scalable with respect to the network's size. Agent-based simulations reveal the features and the performance of these algorithms. To overcome the resultant drawbacks of each method we introduce two improved algorithms and show that they can optimize network's topology almost as fast and effectively as other not-scalable methods that make use of much more information.
Time-aware Collaborative Filtering with the Piecewise Decay Function

Abstract: In this paper, we determine the appropriate decay function for item-based collaborative filtering (CF). Instead of intuitive deduction, we introduce the Similarity-Signal-to-Noise-Ratio (SSNR) to quantify the impacts of rated items on current recommendations. By measuring the variation of SSNR over time, drift in user interest is well visualized and quantified. Based on the trend changes of SSNR, the piecewise decay function is thus devised and incorporated to build our time-aware CF algorithm. Experiments show that the proposed algorithm strongly outperforms the conventional item-based CF algorithm and other time-aware algorithms with various decay functions.
