Socioeconomic disparities in breast cancer survival: relation to stage at diagnosis, treatment and race

Abstract: Women diagnosed with breast cancer during 1998-2002 in the 13 SEER cancer registry areas were followed-up to the end of 2005. The association between an area-based measure of SES and cause-specific five-year survival was estimated using Cox regression models. Six models were used to assess the extent to which SES differences in survival were explained by clinical and demographical factors. The base model estimated the hazard ratio (HR) by SES only and then additional adjustments were made sequentially for: 1) age and year of diagnosis; 2) stage at diagnosis; 3) first course treatment; 4) race; and 5) rural/urban residence.An inverse association was found between SES and risk of dying from breast cancer (p < 0.0001). As area-level SES falls, HR rises (1.00 → 1.05 → 1.23 → 1.31) with the two lowest SES groups having statistically higher HRs. This SES differential completely disappeared after full adjustment for clinical and demographical factors (p = 0.20).Stage at diagnosis, first course treatment and race explained most of the socioeconomic disparity in breast cancer survival. Targeted interventions to increase breast cancer screening and treatment coverage in patients with lower SES could reduce much of socioeconomic disparity.Previous studies have documented substantial disparities in breast cancer survival in relation to socioeconomic status (SES) as measured either at the individual [1,2] or area levels [3-6] in the United States, with women in lower SES groups showing poorer survival. Factors that may mediate these disparities include differences in the stage at diagnosis, access to and quality of care delivered and other correlates of low SES [7]. Women with less education and those who are unemployed, reside in a poor area, or are uninsured or under-insured are more likely to be diagnosed at later stages [8-10], and are less likely to receive optimal cancer care [11,12]. Quantum criticality at the Chern-to-normal insulator transition

Abstract: Using the non-commutative Kubo formula for aperiodic solids [1-3] and a recently developed numerical implementation [4], we study the conductivity $\sigma$ and resistivity $\rho$ tensors as functions of Fermi level and temperature, for models of strongly disordered Chern insulators. The formalism enabled us to converge the transport coefficients at temperatures low enough to enter the quantum critical regime at the Chern-to-trivial insulator transition. We find that the $\rho_{xx}$-curves at different temperatures intersect each other at one single critical point, and that they obey a single-parameter scaling law with an exponent close to the universally accepted value for the unitary symmetry class. However, when compared with the established experimental facts on the plateau-insulator transition in the Integer Quantum Hall Effect, we find a universal critical conductance $\sigma_{xx}^c$ twice as large, an ellipse rather than a semi-circle law, and absence of the quantized Hall insulator phase.
The noncommutative Kubo Formula: Applications to Transport in Disordered Topological Insulators with and without Magnetic Fields

Abstract: The non-commutative theory of charge transport in mesoscopic aperiodic systems under magnetic fields, developed by Bellissard, Shulz-Baldes and collaborators in the 90's, is complemented with a practical numerical implementation. The scheme, which is developed within a $C^*$-algebraic framework, enable efficient evaluations of the non-commutative Kubo formula, with errors that vanish exponentially fast in the thermodynamic limit. Applications to a model of a 2-dimensional Quantum spin-Hall insulator are given. The conductivity tensor is mapped as function of Fermi level, disorder strength and temperature and the phase diagram in the plane of Fermi level and disorder strength is quantitatively derived from the transport simulations. Simulations at finite magnetic field strength are also presented.
A New Type of Double-Lumen Catheter to Replace Current One in RCA  [PDF]

Abstract: Objective: To replace the peripheral venous puncture for blood sampling with taking blood samples extracorporally from arterial line before predilution during RCA-CRRT performed. Methods: A new type of double-lumen femoral catheter was used instead of the current tubes. The new type of double-lumen tube had a greater distance from the inner venous ports to the inner arterial ports than current tubes. The minimum distance from the venous port to the arterial port was greatly lengthened. Replacement solution contained citrate, zero Ca2+, zero bicarbonate, low Na+. Blood samples were synchronously collected from the arterial line before the infusion of citrate replacement fluid and from the peripheral vein. The iCa concentration data of two groups were analyzed to observe the difference between iCa concentration levels in the arterial line and in peripheral vein; the anticoagulant effect of RCA and possible complications were observed, such as bleeding, clottings and hypocalcaemia. Results: 28 times of RCA-CRRT were performed on17 AKI and CRF patients with active bleeding or at the high risk of bleeding; 336 blood samples were collected. Statistics showed that the difference of iCa concentration between arterial line group and the peripheral vein group was not significant (P = 0.9), there is a high degree of similarity between the iCa concentration of arterial line blood and the peripheral venous blood. None of the patients developed citrate toxicity or metabolic alkalosis. None induced bleeding, or bleeding aggravated. No obvious clotting occurred. Systemic calcium concentration was achieved in the ideal range. Conclusion: In clinical practice, the data of iCa concentration from arterial line can be used to replace that from peripheral vein when the new type of double-lumen femoral catheter is placed in femoral vein. RCA-CRRT therapy is safe and effective.
A car-following model with stochastically considering the relative velocity in a traffic flow

Abstract: In this paper, the car-following model with stochastically considering the relative velocity in a traffic flow is presented through investigating the vehicle a cceleration process with uncertainty. The stability criterion superior to the Ba ndo's model is derived by the linear stability analysis. The density wave is inv estigated analytically with the perturbation method. The results show that the o ccurrence of traffic jamming transitions can be described by the kink solution o f the modified Korteweg-de Vries equation. The effects of stochastic relative ve locity on the traffic flow are simulated and analyzed. It is shown that the chan ge rate of velocity in the new model is smaller than that of the Bando's model u nder the same condition and depends on the probability p. As the probability p i ncreases, a small initial disturbance does not magnify and influence on the traf fic flow, but fades away for a long time. The stable region in the new model is larger than in the Bando's model in the phase diagram. The existent initial hyst eresis in the new model will approach to a small region with the increase of tim e in the headway-velocity plot, which is exactly different from that in the Band o's model.In contrast,the hysteresis region in Bando's model will be continuousl y extended to cause traffic flow instability.
Lattice models of the optimal traffic current

Abstract: Based on the lattice model of traffic flow, the lattice models which consider the next-nearest-neighbour flow on traffic and pay attention to the following vehicle as well as the preceding vehicle are presented, respectively. The stability criterion is derived by the linear stability analysis. The evolution of density wave has been investigated analytically with the perturbation method. The results show that the occurrence of traffic jamming transitions is described by the kink solution of the modified KdV equation. The theoretical results are in good agreement with the simulation. Moreover, the simulation indicates that the lattice models considering the next-nearest-neighbour interaction can stabilize the traffic flow, but the other model is just in opposition to the former model.
Analysis of the stability and density waves for traffic flow

Abstract: In this paper, the optimal velocity model of traffic is extended to take into account the relative velocity. The stability and density waves for traffic flow are investigated analytically with the perturbation method. The stability criterion is derived by the linear stability analysis. It is shown that the triangular shock wave, soliton wave and kink wave appear respectively in our model for density waves in the three regions: stable, metastable and unstable regions. These correspond to the solutions of the Burgers equation, Korteweg-de Vries equation and modified Korteweg-de Vries equation. The analytical results are confirmed to be in good agreement with those of numerical simulation. All the results indicate that the interaction of a car with relative velocity can affect the stability of the traffic flow and raise critical density.
DNA immunoadsorption for childhood-onset systemic lupus erythematosus  [PDF]

Abstract: We present a retrospective review of DNA immunoadsorption (DNA-IA) therapy on clinical symptoms as well as indicators in pediatric cases with systemic lupus erythematosus (SLE), and follow up the short-term curative effects. 16 SLE cases were treated by DNA-IA for 3 times every other day. We observed the changes on clinical manifestations and immunological indicators, in order to compare the alteration of these indicators including clinical manifestations, Systemic Lupus Erythematosus Disease Active Index (SLEDAI) scores, 24 hurinary protein excretion, autoantibodies, serum IgG and complement C3. 13 cases were followed up regularly, within 3 months after DNA-IA therapy, 12 cases of clinical manifestations improved (92.3%). SLEDAI scores in 10 cases decreased from (16.20 ± 12.54) to less than 5 (76.9%), 8 cases of ANA, anti-DNA antibodies were negative (61.5%), 13 cases with IgG level in serum recovered to normal (10.39 ± 4.38) g/L, C3 level rose to normal (1.06 ± 0.23) g/L. 3 to 6 months after IA, clinical manifestations and laboratory examinations in all cases got maximum improved. 9 months after IA, SLEDAI score in 2 cases (15.4%) rose to more than 5, anti-DNA antibody in 2 cases (15.4%) became positive, and 1case (7.7%) with serum C3 decreased again. 2 cases died from multiple organs dysfunction within 3 to 6 months after IA. No serious complications were found during DNA-IA. We recommend that DNA immunoadsorption is a safe and effective therapy for active childhood-onset SLE, which could improve clinical symptoms, eliminate ANA and anti-DNA antibodies. Combining with corticosteroids and immunosuppressive drugs, DNA-IA could significantly reduce the activity of disease and protect vital organs function in the short term.
Urban environmental problems transition and environmental education towards sustainable cities in China

Abstract: With the rapid economic development and the speeding up of urbanization, the urban environmental problems in China are experiencing a progressive transition, which is characterized by steady transferring to the living (or consumption) oriented pollution pattern from the production oriented pollution pattern. Regarding this transition, the environmental education recognized as an important measure for approaching the objective of sustainable development, should transfer its focus correspondingly in urban area, which is to transfer to the habit cultivating pattern on the basis of the traditional knowledge inputting environmental education pattern. In practice, the urban environmental education should emphasize more on developing or cultivating the sustainable living mode of the urban residents, especially the students in elementary and primary schools and women. By this transform, the environmental education may be guided further towards sustainable development, and to serve the achievement of sustainable cities better in China.
Determination of Relaxation Time of a Josephson Tunnel Junction

Abstract: We propose a non-stationary method to measure the energy relaxation time of Josephson tunnel junctions from microwave enhanced escape phenomena. Compared with the previous methods, our method possesses simple and accurate features. Moreover, having determined the energy relaxation time, we can further obtain the coupling strength between the microwave source and the junction by changing the microwave power.
