Sonography of the Primary Cutaneous Melanoma: A Review

Abstract: The diagnosis and management of primary cutaneous melanoma have traditionally relied on clinical and histological characteristics. Nevertheless, in recent years there has been a significant growth in the usage of ultrasound for studying the cutaneous layers. Thus, the present paper focuses on the primary lesion, its sonographic characteristics, the potential benefits of early imaging, and the new developments on the ultrasound field applied to cutaneous melanoma. 1. Introduction Cutaneous malignant melanoma constitutes 4 to 11% of all skin cancers but is responsible for more than 75% of skin cancer-related deaths producing more than 8000 deaths per year in the United States [1]. To date the diagnosis is clinical, and the usage of ultrasound in the study of cutaneous melanoma has been focused more on the locoregional staging than on the primary lesion, and the prognosis being mainly assessed by clinical and histological features. The staging of melanoma is mostly based on the Breslow classification that relies on sequential tumor infiltration (depth) by histology within the different cutaneous layers, providing a measurement of the microscopic invasion from the stratum granulosum of the epidermis to the deepest portion of the tumor [2]. The Breslow index has been reported to correlate well with the prognosis of the disease. Moreover, according to the thickness of the primary tumor, important decisions are taken such as the size of the excision and the free margins or the requirement for a sentinel lymph node procedure. Nevertheless, there are controversial reports about the appropriate size of the excision that should be performed on melanoma. Hence, a small (but potentially important) difference in overall survival between wide and narrow excision margins has been reported that cannot be confidently ruled out. Literature has mentioned that recurrence-free survival is favored with wide excision (Hazard Ratio 1.13; ; 95% confidence interval 0.99 to 1.28) but the results have not reached statistical significance ( level). Furthermore, randomized trial evidence seems to be insufficient to address optimal excision margins for primary cutaneous melanoma [3]. Reports on recurrence rates in melanoma have shown a wide range depending on the stage of the primary tumor; thus, they can vary from 7.1% in stage I to 51% in stage III [4]. Moreover, older patients with thicker tumors and angiolymphatic invasion appear to be at higher risk for local and in-transit recurrence [5]. Abstract: Resumen: Antecedentes. La Psoriasis es una enfermedad frecuente en la práctica dermatológica. En EE.UU., 7 millones de personas (2-3%) la padecen. El 5% de los pacientes con psoriasis ungueal no tienen manifestaciones cutáneas. Entre 10-20% de los pacientes con psoriasis cutánea tienen artritis psoriatica. De los pacientes con artritis psoriatica, el 53-86% presentan compromiso ungueal. Objetivo. Estudiar los cambios visibles al ultrasonido de alta resolución, en la u a de los pacientes psoriaticos. Material y método. Se utilizo equipo de ultrasonido Philips ATL 5000 equipado con SonoCT, XRES, campo de visión extendida y transductores de 15-7 MHz y 12-5 MHz. Se estudiaron 15 pacientes, 9 controles normales y 6 psoriaticos con compromiso ungueal. Mujeres n=9 y hombres n=6, promedio de edad 47,7 a os. Se exploraron las 2 manos en todos los pacientes (150 u as) y se midieron en cada paciente las distancias entre ambas placas ungueales como también entre la placa ungueal ventral y el margen óseo de la falange distal en la u a del dedo índice derecho. Resultados. Se describe la anatomía ultrasonográfica normal de la u a y cuatro patrones de alteraciones visibles en el compromiso por psoriasis que incluyen: compromiso focal hipere-cogénico de la placa ventral sin compromiso de placa dorsal, perdida de definición de la placa ventral con placa dorsal indemne, ondulación de ambas placas y perdida de definición de ambas placas. Se encontró una diferencia significativa (p= 0.02) entre el promedio de distancia placa ventral ungueal y margen óseo de la falange distal para u as normales (1,5 mm) y para psoriasis (3,0 mms). Conclusión: El ultrasonido de alta resolución permite un estudio no invasivo de la u a, describiendo su anatomía y cambios morfológicos en pacientes normales y psoriaticos con compromiso ungueal. Ello puede ser útil en la monitorización de los efectos del tratamiento de psoriasis en sus distintas mani-festaciones, ya que existen cambios imperceptibles en su magnitud y ubicación para el clínico, que pueden observarse sin esperar el recambio ungueal total. Abstract: abstract: background. psoriasis is a common disease in dermatology practice. in usa, 7 million of people (2-3%) have psoriasis. approximately 5% of the patients with nail psoriasis don't show cutaneous psoriasis and 10-20% of them have psoriatic arthritis. in patients with psoriatic arthritis, 53-86% have nail involvement. objective. to study the nail changes with high-resolution ultrasound (15mhz). methods and material. we utilized philips atl 5000 ultrasound equipment with sonoct, xres, extended field of vision and a linear probe of 15-7 mhz. we studied 15 patients, 9 healthy control individuals and 6 psoriatic patients with nail involvement, (women n=9; men n= 6, mean age= 47.7 years). all the nails of both hands were observed in both groups (150 nails). we measured in every patient the distance between ventral and dorsal nail plates and also between the ventral plate and the bone margin of the distal phalanx at the right index finger. results. the normal ultrasound anatomy of the nail in high resolution was described and also the morphological changes in the psoriatic nails that included 4 types of alterations: focal hyperechoic compromise of the ventral plate without involvement of the dorsal plate, loosening of the borders of the ventral plate, wavy plates, and absence of definition of both plates. there was a significant difference (p= 0.02) between the mean distance ventral plate_osseous margin of the distal phalanx in psoriatic patients (3,0 mms) and healthy control individuals (1,5 mm). conclusion. high resolution ultrasound permits non invasive study of the nail, to describe and measure the morphological characteristics and changes of the nail in healthy control individuals and psoriatic patients, which can be useful for monitoring treatment effects of psoriasis in its different manifestations. imperceptibles nail changes for the clinician in magnitude and location, can be detected without waiting the whole total replacement of the nail. to our concern th

Abstract: La necrosis grasa del recién nacido (NGRN) es una rara enfermedad que afecta al tejido adiposo y que se desarrolla durante las primeras semanas de vida. Se caracteriza por nodulos y/o placas eritemato-violáceas, de consistencia duro-elástica, únicas o múltiples, en dorso, hombros, glúteos, brazos, muslos y mejillas, que evolucionan espontáneamente hacia la regresión. Si bien pueden desarrollarse complicaciones, éstas son infrecuentes, siendo la más importante la hipercalcemia. La patogenia de la NGRN es desconocida, aunque se han identificado factores predisponentes para su desarrollo. Presentamos el caso de un recién nacido de pretérmino de 36 semanas, sexo masculino, que presentó un cuadro clínico compatible con NGRN. El diagnóstico se sospechó en base a la clínica, a los antecedentes perinatales y se confirmó con el estudio histopatológico. El paciente evolucionó satisfactoriamente con regresión de las lesiones y sin presentar complicaciones hasta la fecha. It is characterized by reddish-purple nodules and/or plaques with a hard elastic consistency, isolated or múltiple, localized at the back, shoulders, buttocks, arms, thighs and chin. They evolve spontaneously to regression and unfrequently develop complications, being hypercalcemia the most observed. The pathogenesis of newborn fat necrosis is unknown, although certain risk factors for its development have been identified. Case-report: A 36 weeks preterm male newborn with a clinical picture compatible with fat necrosis. The diagnosis was made according to the clinical findings and his perinatal history, confirming it with pathology. The patient evolution was complete improvement and regression of the lesions, without complications.

Abstract: the importance of the "argentine events" for the weekly march, and especially to the country they hailed from, uruguay, has been sufficiently recognized (vior, 2003; rocca, 2006). here i am interested in stopping, in a first move in what the end of peronism have opened for some contributors of the publication. that is, the recovery of relations between argentina and uruguay as well as among intellectuals, especially regarding to the possibilities of regional economic integration first and then american. clearly, what he opened was a paradox defined around what i call here the "question rioplatense": the idea that the peronism fall would allow recover a historical links with argentina at the same time served as principal distinction between the cultural characterizations between both countries, especially among intellectuals. in a second move, i'm interested to check if march can be thought as part of the circuit of publications contributed to the reshaping of argentine intellectual field after the liberating revolution.

Hacia una sociología de los cuerpos y las emociones desde Latinoamérica" editada por CLACSO-CICCUS. El libro recorre a través de sus artículos las prácticas de la dominación como formas de regulación corporal, las prácticas del re-apasionamiento, las indagaciones sobre el placer y las formas de esperanza son parte de las múltiples perspectivas que desarrollan los autores en los trabajos compilados.Las páginas de esta publicación constituyen una búsqueda pero también un recorrido: este trabajo es parte de las producciones presentadas en las “Jornadas Interdisciplinarias sobre Cuerpo(s), Subjetividad(es) y Conflicto(s).
