Partial pressure of end-tidal carbon dioxide predicts successful cardiopulmonary resuscitation in the field

Abstract: In their report on 737 patients who sustained out-of-hospital cardiac arrest, collected over an interval of 9 years in a well-organized emergency medical system, Kolar and colleagues confirmed that the measurement of end-tidal carbon dioxide tension (PetCO2) is predictive of the outcomes of cardiopulmonary resuscitation [1]. The authors provide impressive data supporting the conclusion that, in their population, failure to increase PetCO2 to levels exceeding 14 mmHg (1.5 kPa) after 20 minutes of attempted resuscitation served as a reliable guide for terminating unsuccessful cardiopulmonary resuscitation. The population studied, however, differed in some respects from the majority of earlier demographic reports that the authors cited with hospital survival <3%. More than 53% survived. The majority of instances of cardiac arrest reported by them was witnessed, and as many as one-third of victims received bystander cardiopulmonary resuscitation – favoring improved outcomes. Fatal outcomes, as anticipated, were associated with a doubling of the response time of professional rescuers, presumably in the absence of bystander utilization of automated external defibrillators, especially since a majority of survivors had shockable ventricular fibrillation or ventricular tachycardia.As the authors pinpoint, PetCO2 has evolved into a technically facile and singularly useful monitor to guide cardiopulmonary resuscitation. PetCO2 provides an indirect measurement of the cardiac output generated by chest compression and thereby guides the effectiveness of the procedure, including chest compression, to achieve better outcomes. It also allows uninterrupted chest compression because it promptly signals the return of spontaneous circulation [2]. PetCO2 is likely to promptly identify asphyxia, in contrast to primary cardiac causes of arrest as previously reported by one of the present authors [3]. Diversidad y abundancia relativa de corales, octocorales y esponjas en el Parque Nacional Jaragua, República Dominicana

Abstract: diversity and relative abundance of corals, octocorals and sponges at jaragua national park, dominican republic. the jaragua national park is located in a remote area to the sw coast of the dominican republic. fishing and mining are the major human activities. the main reef formations of the park include: (a) long bank reefs (spur and groove) growing as bands over the platform and running in a sw-nw direction at 12-25 m depth, (b) well developed, deep, fringing reefs at the platform edge (drop-off) areas which could extend from 10 to 45 m depth, and (c) small patch reefs and poorly developed coral-octocoral-sponge-algal communities in shallow platforms near shore, rocky bottoms, and over the submerged walls of the uplifted reef. nine reef localities were surveyed between cabo beata and bahia honda using scuba diving to inventory the diversity and relative abundance of scleractinian corals, octocorals and sponges. fringing reefs were surveyed starting at the bottom (30 m) and swimming in a zig-zag pattern (50 m on each side) to shallower areas. bank reefs were surveyed by swimming in zig-zag across the spur-groove formation along 500 m. sponges were the most diverse group with 83 species in 50 genera followed by the scleractinian corals with 56 species in 26 genera and the octocorals with 47 species in 15 genera. new records included eight coral species, 29 octocoral species and 59 sponges. the diversity, species composition and abundance of particular groups varied across the different localities. northern reefs within the park and the los frailes island offshore had the highest live cover, relative abundance and diversity for the three groups. in general, the jaragua national park had the highest diversity of corals, octocorals and sponges reported for the dominican republic and rank amongst the highest reported for the northern caribbean. it is recommended that the area be protected and that fishing activities be regulated or eliminated altogether. rev. biol. trop

Euflexxa (high molecular weight hyaluronate), a bioengineered high molecular weight hyaluronan, has been shown to be a safe and effective treatment for patients with knee osteoarthritis. There is also support for the use of hyaluronate injection for the treatment of chronic shoulder pain associated with osteoarthritis or rotator cuff damage. This small-scale exploratory study was conducted to evaluate the safety and efficacy of high molecular weight hyaluronate for the treatment of chronic shoulder pain associated with osteoarthritis. Methods: Subjects with glenohumeral osteoarthritis and chronic pain (n = 27) received one injection per week for 3 weeks of high molecular weight hyaluronate and were assessed for changes in pain (100 mm visual analog scale [VAS]), range of motion, and the subject’s and physician’s global assessment over 26 weeks. Subjects were also assessed for pain, stiffness, and physical functioning using the Western Ontario and McMaster Universities Arthritis Index (WOMAC). Finally, responses were evaluated using modified Outcome Measures in Rheumatoid Arthritis Clinical Trials (OMERACT)-Osteoarthritis Research Society International (OARSI) Proposition D criteria. Safety was assessed by recording adverse events. Results: High molecular weight hyaluronate significantly improved pain (VAS, WOMAC), range of motion, stiffness, and physical functioning scores; 77.8% of subjects were rated as having an OMERACT-OARSI Proposition D high response. There were no serious adverse events, and none were considered to be related to treatment. Conclusion: Treatment with high molecular weight hyaluronate improves pain, stiffness, and range of motion, and may have an acceptable safety and tolerability profile. A randomized, double-blind, placebo-controlled clinical trial may be warranted to investigate further the efficacy and safety of intra-articular high molecular weight hyaluronate for treatment of chronic shoulder pain in patients with glenohumeral osteoarthritis.

Abstract: The title compound, octaaqua(hydrogendifluorido)dicopper(II) hexafluoridoaluminate dihydrate, was obtained under hydrothermal conditions. The structure is isotypic with that of the analogous FeIII compound, [Cu2(HF2)(H2O)8][FeF6]·2H2O. The coordination sphere of the CuII atom is formed by one F and three water O atoms at short distances < 2 and is augmented by two additional water O atoms at significantly longer distances, leading to a considerably distorted octahedral environment. By edge-sharing, these octahedra form dimeric [Cu2(HF2)(H2O)8]3+ units that are bonded to [AlF6]3 anions (overline{1} symmetry) and to crystal lattice water molecules via hydrogen bonds. Besides F—H...F interactions between the dimeric cationic units, O—H...F and O—H...O hydrogen bonds (both in part bifurcated) are observed.
The cyclo-tetraphosphate Cd2P4O12, a member of the isotypic series M2P4O12 (M = Mg, Mn, Fe, Co, Ni, Cu)

Abstract: The title compound, Cd2P4O12, dicadmium cyclo-tetraphosphate, crystallizes isotypically with the members of the series MII2P4O12, where M = Mg, Mn, Fe, Co, Ni or Cu. Two CdO6 octahedra, one with 2 and one with overline{1} symmetry, share corners with the centrosymmetric P4O124 ring anion that is built up from four corner-sharing PO4 tetrahedra. The isolated ring anions are arranged in layers parallel to (10overline{1}) with the CdO6 octahedra situated between these layers. The main difference between the individual MII2P4O12 structures pertains to the different sizes of the MO6 octahedra whereas the geometric parameters of all cyclo-P4O124 anions are very similar.

Abstract: The crystal structure of the title compound, sodium strontium orthoarsenate(V) nonahydrate, is isotypic with NaSr(PO4)(H2O)9 and the minerals nabaphite [NaBa(PO4)(H2O)9] and nastrophite [Na(Sr,Ba)(PO4)(H2O)9]. The Na and Sr atoms are located on threefold rotation axes and are in the centres of slightly distorted Na(H2O)6 octahedra and Sr(H2O)9 tricapped trigonal prisms, respectively. A framework structure is established via edge-sharing of these polyhedra. Disordered AsO4 tetrahedra (with threefold symmetry) are situated in the interstitial space of the framework. Although reasonable H-atom positions of the water molecules were not established, close O...O contacts between the disordered AsO4 tetrahedra and the water molecules suggest strong O—H...O hydrogen bonding.

Abstract: Dieser Beitrag befasst sich mit einem Exploratory Workshop on Improving the Quality of Qualitative Research, der – gef rdert von der European Scientific Foundation (ESF) – im Juni 2007 in Kristiansand, Norwegen, stattgefunden hat. Hierzu versammelten sich europ ische Wissenschaftler und Wissenschaftlerinnen unterschiedlicher disziplin rer Herkunft und methodologischer Verortung, um den State of the Art und die Qualit t qualitativer Forschung in ihren jeweiligen Bezugsfeldern zu diskutieren. Der Fokus galt insbesondere der mangelnden Glaubwürdigkeit bzw. fortdauernden Kritik qualitative Forschungsmethodik insbesondere durch quantitativ-orientierte Kolleginnen und Kollegen. Dies aufgreifend wurden unterschiedliche Methodologien, Verfahren und Forschungsdesigns diskutiert mit Blick auf M glichkeiten der Verbesserung qualitativer Methodik und auf einen Ausgang aus der Legitimationskrise qualitativer Forschung. Vorschl ge hierzu betrafen u.a. Verbesserungen der Anleitungen zur Durchführung qualitativer Studien (MAEDER) oder von Forschungsdesigns (GOBO, SILVERMAN) bzw. Vorschl ge zu einer konsequenteren Triangulation von Daten und Verfahren (KONECKI). Von anderen wurden Vorschl ge in Richtung eines systematischeren Einsatzes z.B. von Reflexivit t (BUSCATTO) oder lebensgeschichtlichem Material (BERTAUX) gemacht; wieder andere favorisierten innovative Methoden wie applied theatre (HUNDT) oder einen breiteren Einsatz von Audio-Techniken (HEATH). PRIOR unterstrich die Bedeutung von Repositorien für qualitative Forschung, DREW eine deutlichere Orientierung in Richtung von Konversationsanalysen. Wieder andere Beitr gen befassten sich mit der Unterschiedlichkeit von Informant(inn)en in verschiedene Typen von Untersuchungen: RYEN berichtete aus seiner Arbeit mit sog. "Schlüsselpersonen", w hrend WEIL Chancen und Schwierigkeiten kollaborativer Forschung diskutierte. KVALE schlie lich fokussierte auf qualitative Forschung als "Handwerk". Abstract: Dieser Beitrag bietet einen überblick über die Entwicklung und den jetzigen Zustand der qualitativen Forschung als einer akademischen Disziplin in Israel. Er dokumentiert ihre Bedeutung für die Lehre und ihre Rolle in den h heren Bildungseinrichtungen Israels, indem er die Ergebnisse einer kleinen Umfrage beschreibt und die Netzwerke skizziert, die die qualitative Forschung in Israel bef rdern. Der Beitrag fasst die Ergebnisse einer Durchsicht aller Artikel der bedeutendsten israelischen Zeitschrift Megamot zusammen. Daneben zeigt er, welche Methoden von israelischen Forschern in welchen Forschungsbereichen eingesetzt werden. Schlie lich er rtert er die Frage, ob nationale Erfahrungen die Auswahl sozialwissenschaftlicher Methoden leiten und kommt zum Schluss, dass sich diese Erfahrungen zwar auf die Forschungsbereiche, nicht aber auf die spezifischen Methoden auswirken. By means of a survey carried out especially for this issue of FQS, the paper documents the prevalence of qualitative courses in Israel's institutions of higher learning and other networks which promote qualitative re-search. In addition, the article presents the findings of a separate survey carried out for this issue of all articles published in the past decade in Israel's foremost behavioural science journal "Megamot". It documents the methods used by Israeli scholars, as well as their new and emerging areas of inter-est. The article demonstrates that in recent years there has been a steady increase in interest in qualitative research in Israel. Finally, the paper discusses whether national experience shapes the selection of social scientific methods, and comes to the conclusion that it influences the research area studied but does not necessarily affect the methods used.
