Impacto do volume de gordura aspirado na resistência insulínica após lipoaspira o Impact of the aspirated volume of fat tissue in the insulin resistance after liposuction

Abstract: OBJETIVO: investigar a resistência insulínica imposta pela lipoaspira o, correlacionando sua intensidade com a extens o da opera o. MéTODOS: A amostra foi formada de 20 pacientes do sexo feminino sem comorbidades, com idade entre 21 e 43 anos, índice de massa corporal entre 19 e 27 Kg/m2, submetidas à lipoaspira o isolada ou associada à prótese de mamas. Foram coletados os indicadores de resistência insulínica no início e término da cirurgia para o cálculo do Homeostasis Model Assessment (HOMA-IR). As variáveis operatórias foram tempo de lipoaspira o, tempo de prótese de mamas, áreas corporais lipoaspiradas e gordura total aspirada. RESULTADOS: O tempo de lipoaspira o foi 94 a 278 min (média=174 min), tempo de prótese de mamas de 20 a 140 min (média=65 min), gordura total aspirada de 680 a 4280 g (média=1778 g). A análise estatística foi realizada por uma linha de corte de 1500 g de gordura aspirada e revelou uma resistência insulínica pelo índice de HOMA significativamente mais intensa no grupo >1500 g (aumento de 123%) em rela o ao grupo d"1500 g (aumento de 53%,) a partir dos dados basais (p=0,02). As demais variáveis operatórias n o apresentaram correla o significativa. CONCLUS O: A resistência insulínica apresenta aumento significativo na lipoaspira o, correlacionada ao volume de gordura aspirado. OBJECTIVE: To investigate insulin resistance imposed by liposuction, correlating its intensity with the extent of the operation. METHODS: The sample consisted of 20 female patients without comorbidities, aged between 21 and 43 years, body mass index between 19 and 27 kg/m2, undergoing liposuction alone or associated with breasts' prosthesis. We assessed insulin resistance at the beginning and end of the procedure by calculating the Homeostasis Model Assessment (HOMA-IR). The operative variables were length of liposuction, breast prosthesis time, body areas submitted to liposuction and total fat aspirated. RESULTS: The liposuction time was 94-278 min (mean = 174 min), duration of breast prosthesis 20-140 min (mean = 65 min) and total fat aspirated 680-4280 g (mean = 1778 g). Statistical analysis was performed by considering a division line of 1500 g of aspirated fat and there was a significantly increased insulin resistance by HOMA index greater in the group > 1500 g (123% increase) than in the group d" 1500 g (an increase of 53 %) from the baseline data (p = 0.02). Abstract: multidetector coronary computed tomography angiography is a precise method for evaluating post-coronary revascularization arterial and venous bypass grafts, and is somehow superseding coronary catheterization that is an invasive and more expensive technique. the present iconographic essay is aimed at anatomically demonstrating the bypass grafts most frequently utilized, how to differentiate between arterial and venous grafts and how to find them. the studies were performed in 64-row multidetector computed tomography equipment, with breath hold, controlled heart rate and appropriate protocols with later mpr, mip and 3d reconstructions according to electrocardiogram. the localization of the bypass grafts and anastomoses at computed tomography studies focused on chest and coronary arteries may represent a difficulty in the images analysis by the radiologist who is not familiar with the matter, so the knowledge of the surgical techniques adopted and possible courses of the saphenous bypass grafts and arterial grafts can aid in the analysis of both studies, avoiding diagnostic errors.
Custom-Made Mouthguards: Electromyographic Analysis of Masticatory Muscles and Cardiopulmonary Tests in Athletes of Different Sports  [PDF]

Abstract: The purpose of this study is to analyse the neuromuscular balance of the masticatory muscles and the influence on the athlete respiratory parameters induced by custom-made mouthguard. Twenty-six athletes (24 males and 2 females), of different disciplines, average age of 32.12 ± 12.05, were recruited. Each athlete received a custom-made mouthguard in the Ethylene-Vinyl-Acetate (EVA) thermoplastic material and using surface electromyography, the masseter muscles and the frontal beams of the temporal muscles were analysed, with and without mouthguard. The athletes were then subjected to a medical examination and cardiopulmonary test, in the condition with and without mouthguard. The use of mouthguard caused an improvement of all the electromyographic indexes analysed. In particular, GLOBAL INDEX (p = 0.0021), BAR (p = 0.0005), IMPACT (p = 0.0076) and ASIM (p = 0.0290) showed an enhancement statistically significant. For the cardiopulmonary test indexes as maximum oxygen uptake (VO2max), minute ventilation (VE), breathing reserve (BR), not statistically differences (p > 0.05) were reported. The custom-made mouthguard improved the neuromuscular balance of the masticatory muscles, symmetrizing the masseters and temporalis muscles work. It produced a better balancing distribution of occlusal loads both in the anterior-posterior direction and in the lateral direction and offered the possibility to produce more muscular work. Moreover, it did not constitute an impediment that can significantly disturb the athlete’s breathing during physical activity.

Abstract: For many years, measuring the blood pressure was part of the routine of doctors and nurses. Lately, since the idea of a new professional category deeply involved with community work taking the blood pressure the Community Health Agents -, this practice has become controversial in our country. In this paper we analyze the favorable opinions and criticisms regarding this proposal and study the literature dealing with the topic. We verified a strong demand of the public for this practice, including from the local and municipal health councils. The training of the CHA in blood pressure taking and educational actions in households was evaluated and specific schedules and guidelines were created and applied. The results obtained were: reproducibility of the technique used by the CHA, the number of people, who never before had their blood pressure measured, the number of cases considered high or borderline, the number of hypertensive people with poor control of their blood pressure and the adherence of users to the instructions of the CHA. A avalia o da press o arterial integra a prática semiológica de médicos e a rotina dos cuidados de enfermagem. Recentemente, essa a o ressurgiu em nosso país como polêmica assim que se cogitou a possibilidade de uma nova categoria profissional inserida profundamente nas comunidades, a dos Agentes Comunitários de Saúde (ACS), praticá-la. Tanto as vantagens alegadas quanto as críticas s o analisadas, bem como revisada a literatura sobre o tema. Detectou-se forte demanda populacional para que essa prática fosse implantada, inclusive nos conselhos municipal e local de saúde.Geraram-se dados que avaliam o treinamento de ACS para realizar medidas de press o arterial e atividades educacionais nos domicílios com a cria o e aplica o de um protocolo específico, tendo como resultados: a reprodutibilidade da técnica, o número de usuários que nunca tenham medido press o arterial antes, o número de medidas consideradas limítrofes e altas, o número de hipertensos mal controlados e a ades o dos usuários às orienta es das ACS.
Leibniz e a retomada das formas substanciais

Abstract: Considerando o percurso total do pensamento de G. W. Leibniz, o presente trabalho remete ao período da escrita do Discurso de Metafísica (1686), que antecede ao estado final de sua filosofia, popularmente conhecido como o “sistema das m nadas”. Embora esta obra inaugure a maturidade do pensamento leibniziano, expondo boa parte dos pressupostos fundamentais de sua metafísica, a formula o do conceito de substancia simples ou m nada ainda n o se faz presente. Todavia, suas origens e a dire o na qual avan am as indaga es de Leibniz sobre a determina o ontológica do real s o sugeridas pela restaura o do conceito de forma substancial, que o autor retoma da filosofia escolástica num sentido específico, simultaneamente integrando-o à sua teoria da substancia e convertendo-o num ícone de sua oposi o à filosofia cartesiana. Tendo encontrado a ocasi o oportuna para fazer chegar suas idéias ao afamado teólogo e intelectual francês Antonie Arnauld, Leibniz sintetiza nos artigos do Discurso os principais aspectos de sua filosofia, agregando às considera es sobre a fé, o bem e a liberdade algumas posi es peculiares em rela o ao contexto filosófico do final do séc. XVII. Dentre elas, a retomada do antigo conceito escolástico de forma substancial consistirá numa ousada reivindica o pela prioridade da metafísica frente à envolvente influência do mecanicismo nas modernas concep es filosóficas sobre a natureza e as substancias corpóreas. Se, por um lado, Leibniz é conhecido pelo seu ímpeto de harmoniza o das diferentes tendências filosóficas de sua época, por outro, suas apropria es dos diferentes conceitos e pontos de vista oriundos destas vertentes sempre resultaram em algo novo e mais profundo, distinguindo-se por fim de sua origem. Com rela o ao tema da forma substancial n o é diferente, uma vez que, embora constitua um marco de sua oposi o à primazia do mecanicismo na filosofia, a recupera o deste conceito t o recha ado por seus contemporaneos n o implica nem na recusa do autor à física moderna, tampouco numa proposta de retorno à mentalidade escolástica, sen o que aparece totalmente inserida em sua teoria da substancia cumprindo um papel vital na arquitet nica de seu sistema filosófico. Trata-se, portanto, de retomar brevemente o contexto da teoria da substancia, tal como se apresenta no Discurso de metafísica, e, em seguida, tentar compreender os passos desta retomada das formas substanciais no interior do pensamento leibniziano.

Abstract: In this editorial, we reflect on the evaluation criteria that is under discussion to be adopted for the assessment of CAPES (Brazilian Federal Agency for the Support and Evaluation of Graduate Education) Environmental Sciences area – CACiamb. This criteria aims to increase qualified production by simulating CAPES quality strata A1, A2, B1 and B2 production of Academic Master Degree Programs with 12 permanent professors based on the criteria established by the Interdisciplinary Area Committee – CAInter in 2007. As well as expectations for the adoption of periodic assessment of free access bases, along with fostering the use of scientific journals published online by Graduate Programs.

En este sentido, los parques ecoarqueológicos son el reflejo de esa nueva oferta en la que muestras del pasado conviven con espacios de descanso con el objetivo de dar a conocer de forma sencilla y divertida periodos anteriores de las ciudades y pueblos. Evaluación de políticas públicas de provisión de fármacos para diabetes mellitus tipo 2 en Argentina: estudio de caso

Abstract: in argentina, the provision of drugs for patients suffering from type 2 diabetes mellitus who lack health insurance is carried out through public programs. in the province of buenos aires, the national program remediar and the provincial program prodiaba (from the spanish programa de prevención, diagnóstico y tratamiento del paciente diabético) coexist. this study estimates the percentage of adults in the municipality of bahia blanca (province of buenos aires) who suffer from type 2 diabetes mellitus and lack health insurance, thus satisfying their need for oral antidiabetic treatments within the public sector. it is a quantitative study that assesses the need and demand for public provision. the results indicate that: 1) the greatest percentage of demand is satisfied at the primary health care level; 2) the province of buenos aires funds the largest share of the pills, followed by the municipal and the national levels; 3) the local government intervenes to satisfy the demand and 4) the total public provision covers approximately 25% of the overall need in relation to the average consumption. this shows that despite the presence of these public programs, the provision is insufficient and thus requires the intervention of the local government even though economic theory does not recommend the decentralization of drug purchases.
Costos de enfermedades: Una revisión crítica de las metodologías de estimación

Abstract: Studies on the cost of illness arise to advance knowledge and estimation of the economic burden of disease. These identify, quantify and value all the economic resources linked to a disease, and classify them as direct, indirect and intangible costs. The aim of this paper is to critically describe the different methods for estimating each of these costs. The valuation of the costs of an illness involves a vector of prices and quantities. The quantities arise from the resources used or lost. Prices are estimated from the market value of the good or service or, if it is a non-market good or service, then prices are estimated from proxy variables or specific methods of estimation of individual preferences. The main methodological and theoretical weaknesses found deal with the estimation of the ‘time’ resource, the presence of externalities and market imperfections and the violation of some assumptions of economic theory.
