Abstract: General Practice/Family holds tremendous latent potential, and is thus a specialty with a bright future ahead. It could well establish itself as the specialty of the future if it is able to overcome the barriers that currently make of General Practice/Family Medicine an unpopular career choice. It is important to train confident, competent and polyvalent family physicians, but it is also necessary to overhaul payment schemes, to invest in primary care infra-structure and organization, and to continue to attract more and more bright and motivated trainees.In the 24th January 2009 issue of the Portuguese weekly newspaper "Expresso", Dr Luís Pisco, then President of the Portuguese Association of General Practitioners said: "today's freshers are the most brilliant students from their schools, and wish to pursue a technologically-driven specialty and make substantial amounts of money[1]."This isn't a new phenomenon. General Practice has always lagged behind hospital specialties in terms of pulling power for the future generations of specialists.In a recent article, Dr Juan Gérvas defined Primary Health Care as "all the accessible and comprehensive services available to citizens for the follow-up of their health problems. Specialists, Hospitals and emergency departments provide services focused on problems, whereas primary health care is focused on person-centred care[2]."According to the European Society of General Practice/Family Medicine (WONCA Europe), "General practitioners/family doctors are specialist physicians trained in the principles of the discipline. They are personal doctors, primarily responsible for the provision of comprehensive and continuing care to every individual seeking medical care regardless of age, sex and illness[3]. Abstract: it is common place within academic and political discussions on higher education to affirm its key role in the development of any society. however, this assertion does not solve the question about what kind of education, to what development, for what society. this implies the recognition of the historical and political character of that relationship, since the answer to the question on what education for what society has not been nor can it always be the same; and whatever the answer, it always has to be part of a political project that views the problems of the society as a whole. with those assumptions on the horizon, we propose a reflection on the situation and perspectives of higher education in latin america: it is a view that, first, assesses the changes occurred during the last years, to then consider the present context of higher education in the region and the challenges which must generate new answers in order to be faced. we attempt to make a contribution to the debate that poses in the center the question of what education we can and must promote in order to build a society with an autonomous cultural and productive development; with dialog and participation in the international scenario; and also with equity, integration and solidarity.

Abstract: the present study analyses the effects of the phytocannabinoids (chemical components of marijuana) and endogenous cannabinoids (produced by the organism). evaluation of the therapeutic possibilities establishes that the antiemetic effect is the only one that has been accepted. an opinion regarding the legalization of marijuana is formulated based on its permanent effects, which are related to its usage at an early age.

Abstract: to assert that higher education is important for the development of any society is yet a common place in the political and academic discussions about the matter. however this affirmation does not resolve the problem of what kind of education, for what kind of development and what kind of society; therefore it is necessary to recognize the historical and political nature of this relationship. based on those suppositions, the article proposes a reflection about the current situation and about the prospects of higher education in latin america: it has to do with a new look that offers a balance of the changes that have been happening during the last few years, in order to consider the current context of higher education in the region and the challenges that make necessary the generation of new answers. the author pretends to add a contribution to the debate that focuses on the question about what kind of education we can and must encourage in order to build up a society that entails autonomous productive and cultural developments with dialogue and participation at international level and with equity, integration and solidarity.

Abstract: dural arteriovenous fistulas are direct arteriovenous connections within the duramater either in the cranial or spinal region. they account for 10 to 15% of all intracranial arteriovenous malformations and they have been consistently reported since forty years. the clinical picture is variable with mild symptoms as tinnitus or secondary to ocular venous congestion, or severe ones, like neurologic symptoms or hemorrhage. they can present at any age but most of the patients are over 50 years old. the anatomy of this malformation is variable and sometimes difficult. natural history is not completely known, therefore the best treatment is still a matter of debate. in order to achieve a favorable treatment, many publications have been focused in the classification of these malformations based on a relation between the anatomy and the natural history

Abstract: objective. to determine the operational cost of a hospital's surgical unit, compare those results with standards, and determine the incidence of the costs; to produce an effective methodology for this purpose that can be applied to all kinds of medical units using any low-cost widely available computer database system, with that methodology being utilized for ongoing management control. methods. this study was carried out in 1999 in the orthopedics and traumatology unit of the "domingo funes" public hospital, in the province of córdoba, argentina. of the 817 patients who underwent diagnostic or therapeutic procedures in that unit during the year, we selected the 154 of them who were hospitalized and required surgical or instrumental procedures. the study used the tracer event method described by kessner. through a consensus process, medical specialists and health economists selected and defined these indicators, which were categorized in relation to their complexity (high, medium, limited, or none). direct and indirect costs were assessed in order to study their contribution to total cost. total cost was estimated using nine variables that were applied to the different tracer events. results. the largest portion of total cost, both real and standard, was based on the "bed-day" variable. of this bed-day cost for all tracer events, 51.2% was due to an indirect cost of "social hospitalization" (keeping patients hospitalized for such reasons as transportation difficulties or delays in receiving requested medical supplies) and 48.8% to a direct cost of true therapeutic hospitalization. this bed-day cost was 117% higher for the real costs as compared to the standard costs. the indirect costs of social hospitalization and of therapeutic supplies made up nearly half of the total real cost. excluding the indirect costs, the real cost of all the tracer events exceeded the standard cost by only 6.4%. including the indirect costs of social hospitalization and of therapeutic sup

Abstract: With an estimated membership if 4.5 million and a growing economic clout, the cooperative sector has come to an age as an emerging social force and an important actor in the Philippine political scene. A political role does not negate cooperativism because the former is an outgrowth of the cooperative mission of building capabilities to respond to the needs of its members and to intervene in shaping its environment. At the practical level, are very intimately interconnected, being regulated by the same laws and beset by common issues and problems. Moreover, they share a very natural and conservative basis of commonality, i.e. the practice and promotion of self help and cooperation. Despite this common identity, not all of them consider themselves as belonging to the "cooperative movement." There is nothing, however, in cooperativism that limits what coops can do in the social and political arena provided that they have a visionary and conscientizing framework and continue to embrace the civic orientation towards equality, justice, transparency, and solidarity. Actually, cooperatives have several political options such as participation in local legislative and policy-making bodies, lobbying, joining the party-list system, coalescing with an existing national party, supporting pro-coop candidates and parties, having sectoral representation in the House of Representatives and launching a national coop party. Whatever the options may be taken, the time for a distinct coop political movement has undoubtedly ripened as coops can no longer remain as mere bystanders or spectators in politics

Abstract: La afirmación sobre la importancia que tiene la educación superior en el desarrollo de cualquier sociedad es ya un lugar común en las discusiones políticas y académicas sobre la materia. No obstante, dicha aserción no resuelve la pregunta sobre qué tipo de educación, para qué desarrollo y para qué sociedad; para ello es indispensable reconocer el carácter histórico y político de esa relación. Con estos supuestos como base, en el siguiente trabajo proponemos una reflexión sobre la situación y las perspectivas de la educación superior en América Latina: se trata de una mirada que esboza un balance sobre los cambios ocurridos en los últimos a os, para considerar, a continuación, el actual contexto de la educación superior de la región y los desafíos frente a los cuales deberán generarse nuevas respuestas. Pretendemos sumar un aporte al debate que pone en el centro la pregunta sobre qué educación podemos y debemos promover para construir una sociedad con desarrollo productivo y cultural autónomos, con diálogo y participación en el escenario internacional, con equidad, integración y solidaridad.
