Ocular Toxoplasmosis: A Case Report  [PDF]

Abstract: Report a case of a male patient with a macular scar compatible with ocular toxoplasmosis (OT) in right eye (RE) and review the relevant literature on this disease. A patient, who attended for a routine contact lens follow up, presented with amblyopic exotropia without any ocular disease. Best-corrected visual acuity of the affected eye was 20/40 with constant and mono-fixation exotropia. Ophthalmoscopic assessment revealed a macular scar compatible with OT. OT is the leading cause of infection in the posterior segment. Inactive cases could be asymptomatic and diagnosis requires a complete eye examination, providing a correct diagnosis and patient management.
Repeatability of 3D OCT Pachymetry in Eyes with and without Contact Lens Induced Corneal Swelling  [PDF]

Abstract:

Introduction: Corneal thickness measurement is an indirect assessment of the physiological corneal function in human eye. This value, especially in the central area, is an important data for clinical diagnosis, treatment and monitoring of anterior segment pathologies. There are several technologies for non-invasively measuring corneal thickness like optical coherence tomography (OCT). However, there is little information available describing the effect of contact lens induced corneal swelling in OCT measures. The purpose of this study was to evaluate the repeatability of OCT corneal pachymetry in eyes with and without contact lens induced corneal swelling. Material and Methods: This study included five visits over one week in six healthy subjects: baseline and after sleeping with four different CL of +0.50 D, +2.00 D, +5.00 D and +8.00 D (Acuvue 2, Johnson & Johnson Vision Care) on four different days. Corneal pachymetry OCT 3D (Topcon) were measured three consecutive times during each visit. Besides, this measurement was repeated in 12 meridians (Scans #1 to #12), between 0°and 165°with 15° intervals. Results: Corneal pachymetry was significant different between before and after contact lens wear (p < 0.001 ANOVA). High repeatability was found without corneal swelling [Coefficient of variation (CV) = 0.68% p = 0.93] and with corneal swelling [CL -0.50 D (CV = 0.78% p = 0.95; 590 ± 46 μm CI 95% 548 to 596), with CL + 2.00 D (CV = 0.72% p = 0.97; 601 ± 46 μm CI 95% 595 to 607), with CL + 5.00 D (CV = 0.78% p = 0.66; 608 ± 50 μm CI 95% 601 to 615) and with CL + 8.00 D (CV = 0.77% p = 0.97; 607 ± 44 μm CI 95% 601 to 613)]. There were no differences (p = 1.00) in central corneal thickness along the 12 scans corneal [Scan #0 (593 ± 50 μm CI 95% 582 to 603), #1 (592 ± 50 μm CI 95% 581 to 603), #2 (591 ± 50 μm IC 95% 580 to 602), #3 (590 ± 49 μm CI 95% 579 to 600), #4 (590 ± 50 μm CI 95% 579 to 600), #5 (591 ± 50 μm CI 95% 581 to 602), #6 (590 ± 51 μm CI 95% 580 to 601), #7 (589 ± 50 μm CI 95% 578 to 600), #8 (590 ± 50 μm CI 95% 579 to 600), #9 (589 ± 50 μm CI 95% 579 to 600), #10 (592 ± 50 μm CI 95% 581 to 602) and #11 (591 ± 50 μm CI 95% 580 to 601).

Evaluación de tres niveles de nacedero Trichantera gigantea (10%, 20%, 30%) en ceba de conejos

Abstract: En el corregimiento de El Lauro, Palmira, Colombia, se realizó un experimento para evaluar tres niveles de Trichantera gigantea (10%, 20%, 30%) en dietas balanceadas y peletizadas para conejos con un tratamiento testigo de concentrado comercial. Se emplearon 64 conejos Nueva Zelanda blancos con 35 días de edad, distribuidos en un dise o completamente al azar, con 4 tratamientos, 4 repeticiones y 4 conejos por unidad experimental. La unidad experimental estaba constituida por 4 conejos alojados en jaulas de 1.0 m largo, 0.5 m alto, 0.5 m ancho. Se evaluaron las variables: consumo de alimento, ganancia de peso, conversión alimenticia y tiempo en semanas para la ceba. El mayor consumo de alimento se presentó en el tratamiento con 30 % de Trichantera (110 g/d), la mejor ganancia diaria (32.29 g/d) y mejor conversión (3.49) con el tratamiento testigo. El tratamiento con 30 % de Trichantera registró una ganancia diaria similar al testigo (32.12) y conversión aceptable (4.2) y fue el tratamiento de mayor beneficio neto. Los menores tiempos de ceba se presentaron en el testigo (41.3 días) y tratamiento con 30 % de Trichantera (46.9 días). At "El Lauro" village, Palmira, Colombia was done an experiment for evaluating three levels of Trichantera gigantea (10, 20, 30 %) in pelleted and balanced diets for rabbits, using commercial feed as a control treatment. Sixty-four New Zeland rabbits 35 days old were distributed in a complete randomized design, using four treatments, four replications and four rabbits for experimental unit. They were located in wire cages, 1.0 m long, 0.5 high and 0.5 m wide. The variables evaluated were: feed compsumption weight gain, feed efficiency and time to reach the final weight. The great feed comsumption was for the 30 % Trichantera (110 g/d), the best daily gain (32.3 g/d) and feed efficiency (3.49) for the control treatment. The 30 % treatment showed a similar result for daily gain (32.12) and a fair feed efficiency (4.2); it had the great net profit. Lower times for final weight were obtained for the control and 30 % Trichantera (43.3 and 46.9 days respectively).
Impact of flu on hospital admissions during 4 flu seasons in Spain, 2000–2004

Abstract: Hospital discharge registers containing pneumonia, chronic bronchitis, heart failure and flu from all public hospitals in Spain were reviewed for the years 2000 to 2004. Epidemic periods were defined by data from the Sentinel Surveillance System. Excess hospitalisations were calculated as the difference between the average number of weekly hospitalisations/100,000 in epidemic and non-epidemic periods. Flu epidemics were defined for seasons 2001/2002, 2002/2003, 2003/2004.A(H3N2) was the dominant circulating serotype in 2001/2002 and 2003/2004. Negligible excess hospitalisations were observed during the 2002/2003 epidemic where A(H1N1) was circulating. During 2000/2001, flu activity remained below threshold levels and therefore no epidemic period was defined. In two epidemic periods studied a delay between the peak of the influenza epidemic and the peak of hospitalisations was observed. During flu epidemics with A(H3N2), excess hospitalisations were higher in men and in persons <5 and >64 years higher than 10 per 100,000. Pneumonia accounted for 70% of all flu associated hospitalisations followed by chronic bronchitis. No excess flu-specific hospitalisations were recorded during all seasons.Flu epidemics have an impact on hospital morbidity in Spain. Further studies that include other variables, such as temperature and humidity, are necessary and will deepen our understanding of the role of each factor during flu epidemics and their relation with morbidity.Influenza virus causes an acute disease of the respiratory pathways, with high potential for person to person transmission. Abstract: this paper describes the case of a woman who complained of pelvic pain. she underwent a transvaginal ultrasound which revealed the presence of bilateral ovarian teratomas. she then underwent a laparoscopy in which 7 teratomas and one simple cyst were found and resected from the right ovary; 6 additional teratomas were found within the ovary following left-hand side salpingoophorectomy. this case is reported to increase the body of literature concerning multiple ovarian lesions and because no other reports about such a large amount of multiple teratomas which have been handled by laparoscopy despite the size and the number of the tumors could be found after conducting extensive research.

Abstract: Se describe el caso de una mujer que consultó por dolor pélvico. Tenía una ecografía transvaginal que reportaba teratomas bilaterales de ovario. Fue llevada a cirugía laparoscópica durante la cual se extirparon 7 teratomas y 1 quiste folicular simple del ovario derecho y se realizó salpingooforectomía izquierda, encontrando 6 teratomas intraováricos. Los objetivos de esta publicación son reportar un caso atípico de múltiples lesiones ováricas sincrónicas y discutir algunos aspectos controversiales del manejo de los teratomas por laparoscopia. Abstract: El artículo analiza la organización de la atención sanitaria pública en Espa a, mediante un marco conceptual que permite integrar los aspectos más relevantes de la misma. Asimismo, se profundiza en los aspectos organizativos de los hospitales descritos como Burocracias Profesionales, Meritocracias y Organizaciones Intensivas en Conocimientos, aspectos que condicionan la gestión de los mismos. Finalmente, mediante las aportaciones de la teoría de los Recursos y Capacidades se concluye que la gestión sanitaria implica la gestión del conocimiento que reside en el capital humano altamente especializado, el capital organizativo acumulado y la reputación conseguida con el paso del tiempo, todo ello en un entorno tecnológico de alta complejidad y elevado dinamismo.How public health is organized in Spain is analyzed through using a theoretical framework which includes the most relevant issues regarding this topic. Likewise, this paper deals with organizational aspects from hospitals which determine the way they are managed, such as Professional Bureaucracies, Meritocracies, and Knowledge-Intensive Organizations. Finally, drawing on Resource-Based View contributions, this article concludes that health management, which occurs in a technological, highly complex and dynamic environment, involves the management of knowledge derived from three sources: highly specialized human capital, accumulated organizational capital, and reputation.
The Role of Serum Biomarkers in Predicting Fibrosis Progression in Pediatric and Adult Hepatitis C Virus Chronic Infection

Abstract: Background/Aims Liver biopsy represents the gold standard for damage evaluation, but noninvasive serum markers that mirror liver fibrosis progression are actual goals both in adults and especially in children. The aim was to determine specific serum markers that correlate with liver fibrosis progression during chronic HCV infection. Methods Liver biopsies and concomitant serum samples from 22 pediatric and 22 adult HCV patients were analyzed. Histological parameters were evaluated. On serum TGF-?1, tissue inhibitor of matrix metalloprotein inhibitor-1 (TIMP-1), hyaluronic acid (HA) and aminoterminal peptide of procollagen type III (PIIINP) were tested. Results Significant fibrosis (F≥2) and advanced fibrosis (F≥3) represented 64% and 20%, respectively in children; while 54% F≥2 and 23% F≥3 in adults. Hyaluronic acid (p = 0.011) and PIIINP (p = 0.016) were related to worse fibrosis stages only in adults, along with TIMP-1 (p = 0.039) just in children; but TGF-?1 was associated with mild fibrosis (p = 0.022) in adults. The AUROC of TIMP-1 in children to discriminate advanced fibrosis was 0.800 (95%IC 0.598–0.932). In adults, the best AUROCs were that of HA, PIIINP and TGF-?1 [0.929 (IC95% 0.736–0.994), 0.894 (IC95% 0.689–0.984) and 0.835 (IC95% 0.617–0.957)], respectively. In children, according to the cut off (165.7 ng/mL) value for TIMP-1, biopsies could have been avoided in 72% (18/25). Considering the cut off for HA (109.7 ng/mL), PIIINP (9.1 μg/L), and TGF-?1 (10,848.3 pg/mL), biopsies could have been avoided in 87% (19/22) of adult patients by using HA and 73% (16/22) using PIIINP or TGF-?1. Conclusions In adults given the diagnostic accuracy of HA, PIIINP, TGF-?1, their combination may provide a potential useful tool to assess liver fibrosis. This first pediatric study suggests that TIMP-1 is clinically useful for predicting liver fibrosis in HCV patients.

Abstract: alzheimer's disease ( ad) is a chronic illness characterized by the presence of cognitive and physical impairments and emotional and/or behavioral disturbances. nowadays, over 24 million people around the world are diagnosed with ad and it is estimated that in 2040 this number will rise to 81 million. the objective of this article was to review in detail the different techniques and/or cognitive treatments that have been developed in the rehabilitation of cognitive impairments of population with ad, and the existing papers that test their efficacy. the main results evidence the use of techniques such as cognitive stimulation, errorless learning, spaced retrieval, visual imagery, vanishing cues and external aids. most of the reviewed treatments used these techniques combined, with patients in the initial stages of ad; many of the papers reviewed demonstrated long term maintenance of the positive results of some treatments.
Rehabilitación Cognitiva en pacientes con Enfermedad de Alzheimer

Abstract: La enfermedad de Alzheimer (EA) es una enfermedad crónica que se caracteriza por la presencia de síntomas cognitivos, problemas físicos y alteraciones emocionales y/o comportamentales. Actualmente más de 24 millones de personas en el mundo han sido diagnosticadascon EA, y se calcula que para 2040 el número será de 81 millones. El objetivo de este artículo es hacer una revisión detallada de las diferentes técnicas y/o tratamientos cognitivos que se han venido utilizando en la rehabilitación de las alteraciones cognitivas de personas con EA, así como de los estudios existentes que evalúan su eficacia. Los principales resultadosde la revisión evidencian la aplicación de tratamientos cognitivos mediante técnicas como estimulación cognitiva, aprendizaje sin error, recuperación espaciada, imaginería visual, desvanecimiento de pistas y ayudas externas. La mayoría de tratamientos revisados utilizaron técnicas de manera combinada, las cuales se implementaron en etapas iniciales de la EA; varios de los estudios revisados demostraron el mantenimiento a largo plazo de las ganancias obtenidas en algunos tratamientos.
