Abstract: in the last years an increase in the prevalence of obesity and metabolic syndrome has been observed around the world. causes for this increase are mainly related with sedentarism and inadequate diet. obesity and metabolic syndrome are a primary cause for the development of cardiovascular disease and type 2 diabetes mellitus. furthermore, insulin resistance plays a central role in the development of vascular and metabolic alterations associated with metabolic syndrome and obesity: atherogenic dyslipidemia, hypertension, pro-thrombotic and inflammatory states. activation of sympathetic nervous system and renin-angiotensin-aldosterone system play a main role in the majority of these alterations. thus, blockade of angiotensin ii actions with aceis or ara ii is an obligatory therapeutical approach for these type of patients. it has to be considered that many patients with multiple risk factors need more than one drug to control hypertension. combination of drugs blocking angiotensin ii with diuretics is the most common combination. however, due to the diabetogenic potential of thiazidic diuretics, this combination is not the most appropriate for hypertensive patients with diabetes, metabolic syndrome or alterations of glucose metabolism. consequently, the combinations of aceis with calcium antagonists has revealed as an interesting alternative combination. accomplish, star and star-let studies, demonstrated important benefits in diabetic patients or in patients with insulin resistance.

Abstract: En los últimos a os se ha observado un aumento de la prevalencia de obesidad y síndrome metabólico a nivel mundial, cuyas causas habrían de buscarse fundamentalmente en el sedentarismo y la alimentación inadecuada. La obesidad y el síndrome metabólico son una primera causa para el desarrollo de enfermedad cardiovascular y diabetes mellitus tipo 2. La resistencia a la insulina desempe a un papel central en el desarrollo de las alteraciones vasculares y metabólicas que acompa an al síndrome metabólico y la obesidad: dislipidemia aterogénica, hipertensión y estado protrombótico e inflamatorio. La activación del sistema nervioso simpático y del sistema renina-angiotensina-aldosterona también participa en la mayoría de estas alteraciones. Por ello, el bloqueo de las acciones de la angiotensina II con los inhibidores de la enzima convertidora de angiotensina (IECA) o los antagonistas de los receptores de angiotensina II (ARA-II), se considera una aproximación terapéutica obligatoria en estos pacientes. Es preciso considerar que muchos pacientes con múltiples factores de riesgo requieren más de un fármaco para controlar su hipertensión. La combinación de medicamentos bloqueadores de la angiotensina II y diuréticos es la más frecuente; sin embargo, en pacientes hipertensos con diabetes, síndrome metabólico o alteraciones del metabolismo de la glucosa, ésta no parece ser la más adecuada debido al potencial diabetogénico de los diuréticos tiazídicos. Por ello, la unión de IECA y calcioantagonistas se ha revelado como una combinación alternativa interesante. Los estudios ACCOMPLISH, STAR y STAR-LET, han demostrado beneficios importantes en pacientes diabéticos o con resistencia a la insulina. Abstract: a descriptive study was conducted among middle and higher educational level students from the municipality of las tunas that were surveyed to determine the following variables: age, sex, age at which they have the first sexual relation, their level of knowledge, involvement and protection in relation to sexually transmitted diseases. the results obtained coincided with the reviewed literature and it was observed an involvement of students under 15 of 28 %, an effective protection of 52 % and a predominance of females.

Abstract: Se realizó un estudio descriptivo en estudiantes de la ense anza media y superior del municipio de Las Tunas, a los cuales se le aplicó una encuesta donde se precisaron variables como: edad, sexo y edad de las primeras relaciones sexuales; además del nivel de conocimiento, de implicación y de protección de estos en relación con las enfermedades de transmisión sexual. Coincidieron los resultados obtenidos con la literatura revisada, siendo la implicación con los menores de 15 a os con un 28 %; la protección efectiva en un 52 % predominando el sexo femenino. Abstract: A randomized, double blind, cross-over, placebo-controlled, clinical trial was designed to assess the effects of Candesartan (up to 32 mg/day during 6 months) on the Homeostasis Model Assessment (HOMA) index, lipid profile, protrombotic state, oxidative stress and plasma levels of inflammatory markers. The participants will be recruited in the "Fundación Cardiovascular de Colombia". Subjects who fullfil selection criteria will receive permanent educational, nutritional and exercise support during their participation in the study. After a 15 days-run-in period with placebo and life-style recommendations, the patients who have a treatment compliance equal or greater than 80% will be randomlly assigned to one of the treatment groups. Group A will receive Candesartan during 6 months and placebo during 6 months. Group B will receive placebo during the first 6 months, and then, Candesartan during the last 6 months. Control visits will be programed monthly and all parameters of interest will be evaluated every 6 months.Treatment with Candesartan, could improve the HOMA index, the response to the oral glucose tolerance test and reduce the plasma levels of adipoquines, oxidative stress and prothrombotic markers, in non diabetic, non hypertense subjects with dysglycemia and abdominal obesity, recruited from a population at high risk of developing insulin resistance. These effects are independent of the changes in arterial blood pressure. It has been estimated that more than 171 million people suffer from this disease and that this number could increase to 366 million by 2030, of which 298 million would be from developing countries [2].
