Abstract: Demandas judiciais, por meio das quais se requer o fornecimento gratuito de medicamentos, multiplicam-se, e o Poder Judiciário, reiteradamente, tem proferido decis es em que avalia determinados parametros (quest es controvertidas), que surgiram de forma natural das teses expostas em juízo, julgando procedentes ou n o os pedidos que exigem o acesso, sem qualquer custo, aos fármacos. Este trabalho, à luz das teorias que tratam do ativismo judicial em meio à interpreta o e à aplica o do direito, analisará as principais quest es polêmicas que têm sido apreciadas nos tribunais pátrios, para contribuir com o debate existente atualmente sobre o tema.

Abstract: deep venous thrombosis and pulmonary embolism are complications of surgical procedures, the most of times underdiagnosticated. ln laparoscopic surgery, the carbon dioxide insuflation into the peritoneal cavity is probably the cause of venous stasis of the lower limbs. the purpose of this study was to verify the effects of pneumoperitoneum in the blood circulation of the lower limbs. thirty female pigs were divided in three groups: 1- without pneumoperitoneum; i - with 10 mmhg; and ii - with 15 mmhg of abdominal pressure levels. within the clinical parameters were analysed heart rate, cardiac output and mean arterial pressure to certify that the haemodynamic changes would not be gilty for the modifications of lower limbs circulation. this analysis did not find significant changes in the groups comparation. the results showed that were not found significant changes in the pressure, diameter and flow rate in the femoral artery in the presence of pneumoperitoneum. the average femoral venous pressure found in group i was 7.2 mmhg, in group ii was 10.2 mmhg (p<0.05), and in group 11i was 14 mmhg (p<0.05). the average femoral venous flow rate was 9.8 cm/sec in group i; 6.8 cm/sec in group ii (p<0.05) and 5.5 cm/sec in group 11i (p<0.05).the analisys of the femoral vein diameter found that the average value of group i was 4.2 cm; group ii was 5 cm (p<0.05) and group iii was 5.4 cm (p<0.05). the pneumoperitoneum was found to be associated with a significant increase of the pressure and diameter of the femoral vein, and a significant decrease of its flow rate. every parameter analysed returned to the previous values after the peritoneal gas empting.

Abstract: background: cardiovascular diseases are the most frequent complications associated with obesity. the weight loss determine blood pressure decrease and improvement in parameters of lipid and glucose levels. weight loss and maintenance reduce the risk factors for cardiovascular disease, making reversible cardiac changes resulting from obesity. aim: to evaluate the effect of weight loss on physical capacity and the structural and functional heart of obese patients undergoing to bariatric surgery. methods: forty-three patients who underwent gastric roux-en-y bypass, were analyzed by ecg, and echocardiography preoperatively and one year after the bypass. statistical analysis used the wilcoxon and student t test, shapiro-wilk test with a significance level of 5%. results: the weight decreased from 116.5 kg to 21.5 ± 15.9 ± 80 kg, bmi of 41.8 kg/m 2 ± 4.4 to 28.4 kg/m 2 ± 3.8, heart rate decreased from 77.9 ± 9.6 bpm to 70.9 ± 7.8 bpm, systolic pressure of 130 ± 20 mmhg to 120 mmhg ± 10, diastolic 80 mmhg ± 10 mmhg to 80 ± 0. the exercise test showed increases in distance traveled of 378.9 m ± ??126.5 m to 140.4 ± 595m; coefficient in metabolic equivalents (mets) from 6.7 ± 2.4 ml o 2 / kg / min to 8.3 ± 2.6 ml o 2 / kg / min, oxygen consumption (vo 2) 23.1 ± 8.4 to 10.3 ± 30. the echocardiogram showed a reduction in the interventricular septum of 12 mm to 10 mm ± 2 ± 1; posterior wall of 11 mm to 10 mm ± 2 ± 1; ventricular mass of 273 ± 85 g to 216g ± 60. there was improvement in diastolic function with increase in e '/ a'e e / a and ejection fraction with an increase of 70.2% ± 7.2 to 72.9 ± 6.4%. conclusion: obese patients undergoing bariatric surgery showed improvement in physical capacity and in the structure/functioning of the heart.

Abstract: gastrinomas are generally localized in pancreas, duodenum and lymphonodes, within the so called "gastrinoma's triangle" . in 5% of the cases, it may arise from liver, stomach, ovarium, kidneys, parathyroid, omentum, jejunum and heart. we describe a case of a fifteen-year-old boy with a primary gastrinoma of the liver, treated by right-hepatectomy.

Abstract: the most common causes of acute abdominal pain are acute apendicites or cholecistites among several other causes, but uncommon ethiology should be remembered. the authors report a case of a 34 year-old man with necrosis of the epiploon that simulated an acute apendicites. the correct diagnosis and the treatment were done by laparoscopy.
Complica es da blefaroplastia superior Superior eyelid blepharoplasty and complications

Abstract: A blefaroplastia tem sido realizada com grande frequência devido à valoriza o da cirurgia estética. As complica es relacionadas a este procedimento s o raras. Este relato de caso tem como objetivo enfatizar um tipo de complica o que pode ocorrer. T.J.S.P., 45 anos de idade, sexo feminino, branca, procedente de Botucatu, apresentava dermatocálase bilateral. Optou-se pela realiza o de blefaroplastia superior bilateral, realizada removendo-se pele e gordura da pálpebra superior. No pós-operatório foi observado ptose palpebral à esquerda. Optou-se pela cirurgia para reinser o do músculo levantador da pálpebra superior à esquerda. Um mês após a reinser o, observou-se retra o palpebral superior nos dois olhos. A paciente foi, ent o, submetida à cirurgia para a corre o de retra o palpebral por desinser o do músculo de Müller. Dois meses após esta última cirurgia, a paciente apresentava ptose palpebral à direita e piora da retra o palpebral à esquerda. Nova cirurgia para corre o da retra o palpebral foi feita, colocando-se enxerto livre de esclera para alongamento do levantador da pálpebra superior esquerda, com bom resultado. Foi apresentada uma paciente submetida à blefaroplastia e que desenvolveu no pós-operatório quadro de ptose, seguida de retra o palpebral. Estes dois quadros podem ocorrer como complica o de blefaroplastia e o cirurgi o deve estar atento para manejar de forma apropriada, a fim de obter o resultado que a paciente espera ter. The blepharoplasty has been performed with great frequency due to the appreciation of aesthetic surgery. The complications related to this procedure are rare. This case report aims to emphasize complications that can occur. T.J.S.P., 45 years old, female, white, resident of Botucatu City, presented dermatocalase bilaterally. We opted to perform upper blepharoplasty bilateral, by removing skin and fat of the upper eyelid. In the postoperative period was observed left eyelid ptosis. We decided to reinsert the levator muscle of the left upper eyelid. One month after the ptosis correction, retraction was observed in both upper eyelid. The patient was then submitted to surgery for the correction of eyelid retraction by disinsertion of the muscle of Muller. Two months after this last surgery, the patient presented right eyelid ptosis and worsening of eyelid retraction to the left. New surgery for correction of the eyelid retraction was made, using free graft sclera for elongation of the left eyelid levator muscle, with good results. Abstract: purpose: to investigate the safety and efficacy of intraoperative infiltration of 5-fluorouracil (5-fu) as an adjuvant drug in pterygium treatment. methods: of 125 consecutive patients, 125 eyes with primary and recurrent pterygium underwent pterygium excision with intraoperative 5-fu (25 mg/ml) infiltration. the superior and inferior conjunctiva was approximated to cover the scleral bed and 0.2 ml 5-fu was injected at the end of the surgical procedure. the gender, occupation, pterygium characteristics and the follow-up at 7, 21, 60 e 180 days after surgery were evaluated and the data were statistically analyzed. results: with follow-up of 180 days the patients had no serious complications observed during or after surgery. the relapse rate was 35.8% and occurred in primary (35.7%) and recurrent (36.4%) lesions with no statistical difference. conclusions: this study suggests that intraoperative infiltration of 5-fu is safe. however the high recurrence rate indicated that other studies would be necessary to show the concentration/dose to better prevent it.
Accuracy of magnetic resonance in identifying traumatic intraarticular knee lesions

Abstract: purpose: to evaluate the diagnostic accuracy of magnetic resonance imaging of the knee in identifying traumatic intraarticular knee lesions. method: 300 patients with a clinical diagnosis of traumatic intraarticular knee lesions underwent prearthoscopic magnetic resonance imaging. the sensitivity, specificity, positive predictive value, negative predictive value, likelihood ratio for a positive test, likelihood ratio for a negative test, and accuracy of magnetic resonance imaging were calculated relative to the findings during arthroscopy in the studied structures of the knee (medial meniscus, lateral meniscus, anterior cruciate ligament, posterior cruciate ligament, and articular cartilage). results: magnetic resonance imaging produced the following results regarding detection of lesions: medial meniscus: sensitivity 97.5%, specificity 92.9%, positive predictive value 93.9%, positive negative value 97%, likelihood positive ratio 13.7, likelihood negative ratio 0.02, and accuracy 95.3%; lateral meniscus: sensitivity 91.9%, specificity 93.6%, positive predictive value 92.7%, positive negative value 92.9%, likelihood positive ratio 14.3, likelihood negative ratio 0.08, and accuracy 93.6%; anterior cruciate ligament: sensitivity 99.0%, specificity 95.9%, positive predictive value 91.9%, positive negative value 99.5%, likelihood positive ratio 21.5, likelihood negative ratio 0.01, and accuracy 96.6%; posterior cruciate ligament: sensitivity 100%, specificity 99%, positive predictive value 80.0%, positive negative value 100%, likelihood positive ratio 100, likelihood negative ratio 0.01, and accuracy 99.6%; articular cartilage: sensitivity 76.1%, specificity 94.9%, positive predictive value 94.7%, positive negative value 76.9%, likelihood positive ratio 14.9, likelihood negative ratio 0.25, and accuracy 84.6%. conclusion: magnetic resonance imaging is a satisfactory diagnostic tool for evaluating meniscal and ligamentous lesions of the knee, but it is unable to clearly i

Abstract: objective: to assess nutritional and metabolic evolution and inflammatory activity in severe obese patients submitted to bariatric surgery. methods: this prospective study evaluated 56 patients (50 female and 6 male), mean age 40 ± 9,9 years, submitted to rygb. nutritional, metabolic, and inflammatory parameters were assessed prior to and 12 months postsurgery. results: it was verified significant decreases in weight loss in relation to baseline values from 138 ± 28,8 to 90 ± 19,5 kg (p< 0,0001), glucose levels from 116 ± 47,3 to 84 ± 9,8 mg/dl (p< 0,0001), triacylglycerol levels from 137 ± 61,4 to 84 ± 38,6 mg/dl (p< 0,0001), and also in total cholesterol from 189 ± 41,6 to 166 ± 36,4 mg/dl (p< 0,0001) and ldl-cholesterol from 119 ± 36,1 to 104 ± 30,7 mg/dl (p< 0,0005). c-reactive protein levels reduced from 11,33 ± 10,82 to 3,62 ± 4,49 mg/dl (p< 0,0001). although maintenance of iron levels was verified after one year, there was a significant decrease in hemoglobin from 13 ± 1,3 to 12 ± 1,4 g/dl (p< 0,01), and reduction in ferritin levels, especially in women who showed a decrease from 101,2 ± 123,3 to 85,0 ± 101,9 (p< 0,03). conclusion: therefore, weigh loss in patients with severe obese after rygb showed improvement in both metabolic and inflammatory status and may reduce substantially co-morbidities associated with increased cardiovascular risk.
