Postoperative Pain Management in Urologic Patients

Abstract: The quality of postoperative anesthesia in urologic patients has been analyzed. Correlation between pain intensity and postoperative complications has been revealed. Nonsteroid antiinflammatory and narcotic drugs don’t provide high-quality anesthesia after urological operations. Continued postoperative pain is the cause of development of pathological reactions and complications. An important role of prevention and treatment of postoperative pain comes to prolonged epidural analgesia composed of multimodal pain management. Usage of similar tactics significantly decreases the risk of negative reactions for organs and systems in response to operative stress, excludes the necessity of narcotic drugs, prevents the development of complications during the early postoperative period and provides comfortable conditions for patients.
Sex characteristics of the microcirculation in healthy subjects and in patients with the first degree arterial hypertension.

Abstract: Aim. To study the sex characteristics of microcirculation in healthy volunteers and in patients with degree 1 arterial hypertension (HT).Material and methods. Patients (n=71; 34 men and 37 women) with degree 1 HT participated in the study. Besides 28 normotensive men and 41 women were included. The microcirculation study was performed with LAKK-02 analyzer on the palmar surface of the third finger of the right hand. The average values of the microcirculation index, the standard deviation of perfusion fluctuations (σ), the variation coefficient of tissue blood flow (Kv) were evaluated.Results. The microcirculation index in HT men 26.64 (21.00–32.45) perfusion units (p.u.) was significantly higher than this in HT women 20.91 (16.98–24.30) p.u., p=0.031. The variability of microcirculation index was significantly higher in HT patients than this in healthy subjects. Capillary blood flow reserve in healthy women was higher than this in healthy men. Amplitude, reflexing myogenic factor of blood flow regulation was significantly lower in HT men than this in HT women: 13.37 (10.51–16.43) vs 14.69 (11.35–19.13), respectively, p=0.023. Amplitudes, reflecting respiration effects on peripheral blood flow in HT men were significantly higher than these in HT women. Amplitudes, reflecting pulse effect on the tissue perfusion in HT women were significantly higher than these in HT men. Influence of sympathetic adrenergic innervation on the blood flow was higher in healthy men than this in healthy women.Conclusion. Received data suggests the intensification of microcirculation regulation mechanisms in men with HT degree 1. It was found that sympathetic adrenergic effect on the microcirculation regulation was significantly less in healthy women than this in healthy men.

Abstract: Results  are presented on synthesis   and crystal growth of  Gd2S3-Dy2S3 solid solution sulfides and study of their thermoelectric properties in the range of temperatures 80-400 K. Gd0.2Dy0.8S1.48 composition has the best values of thermoelectric efficiency 0.39 x 10-3/K at 400 K.

The Use of Cryodestruction in Ingrown Nail Treatment in Polyclinic

Abstract: The aim of the investigation is to study the possibilities of using modern combined cryodestruction technique to treat ingrown nail in polyclinic surgeries. Materials and methods. There have been analyzed the results of complex cryosurgical treatment of ingrown nail in 17 patients with various degree of pathological process. 17 patients of the control group underwent traditional surgical treatment.Conclusion. Cryosurgical method of treating patients with ingrown nail is least traumatic, cost-effective. It contributes to rapid rehabilitation, improvement of patient’s life quality, vocational rehabilitation, and has a good cosmetic effect.
Prognosis of Prolonged Clinical Course Variants of Neonatal Conjugated Hyperbilirubinemia

Abstract: The purpose of the study is to reveal risk factors that have a reliable effect on prolonged clinical course variants of prolonged conjugated hyperbilirubinemia.Materials and Methods. There has been carried out a prospective study of clinical biological indexes in 100 children aged from 4 weeks to 3 months with manifestations of a prolonged variant of conjugated hyperbilirubinemia and 40 children without these manifestations.Results. Based on the analysis of dynamics of total bilirubin level decrease and the characteristics of prolonged course of hyperbilirubinemia, there have been distinguished three variants of pathological condition: benign, undulating and resistant. And prognostic criteria of forming prolonged variants of neonatal conjugated hyperbilirubinemia have been determined.
Peculiarities of a Penial Hemodynamics in Patients with a Cerebrovascular Pathology (New Aspects)

Abstract: Aim of investigation is assessment of the hemodynamic disturbance influence in patients with a cerebrovascular pathology on the erectile functions.Materials and methods. 171 patients with a dyscirculatory encephalopathy, 96 — with initial manifestations of a cerebral circulation insufficiency, 92 — with consequences of ischemic insult were examined with a method of ultrasonic doplerography with a spectral analysis.Results. The arterial inflow disturbances in patients with a dyscirculatory encephalopathy were revealed in a half of cases, however they were faintly expressed, and a functional possibility of cavernous bodies is conserved in majority of cases. A venous hemodynamics disturbance in a penis in patients with the cerebrovascular diseases is revealed in more than a quarter of humans with ischemic insult and in 8.7% of humans with a dyscirculatory encephalopathy. The critical disturbances of a venous hemodynamics in a penis are not revealed in patients with initial manifestations of a cerebral circulation disturbance.A participation of erectile dysfunction in a pathogenesis at the vascular factor cerebrovascular diseases is confirmed, and moreover both arterial and venous systems of a penis circulation are affected, frequently in combination with each other.

Abstract: Characteristics of daily dynamics of seaweeds abundance and biomass were determined for steppe biogeocoenosis and agrocoenosis of Biosphere reserve “Askaniya-Nova” in spring of 2011. Fluctuation ranges in seaweeds abundance and biomass have been registered.Analyzing the indices of total number and algae biomass in studied biogeocoenoses it should be noted that the maximal values of alga number in virgin soil steppe exceeded minimal in 3,3 times; biomasses - in 2,1. For virgin soil steppe of post-fire-induced development the relation between maximum and minimal value of total number of algae was up to 2,1; biomass - 2,4. For agrocoenosis we noted the largest ranges in variation of number and biomass. In conditions of dry-land arable land the maximum values of total number of alga exceeded minimal in 21,9 times; biomasses - in 8,7; for irrigated arable land - in 12,5 and 5,6 respectively.In soil samples, selected within the limits of virgin soil biogeocoenoses of biosphere reserve “Askania-Nova” and agrocoenosis of dry-land and irrigated arable land in biosphere reserve by direct count, the algae species of Bacillariophyta, Cyanophyta, Chlorophyta, Xanthophyta and Eustigmatophyta have been found. The largest contribution to number and biomass of algae belonged to Bacillariophyta. The number and biomass of agrocoenosis algae is more dynamic feature, than for algae of virgin soil biogeocoenoses.

Abstract: We studied the development of the macromycetes on anthropogenic edaphotop of dumping sites in western wooden-steppes of Ukraine. We considered principal ecological factors, namely acidity, temperature, and connectivity of edaphotop; humidity, temperature, and air humidity, atmospheric pressure, wind speed, equivalent dose of ionizing radiation in the surface ground of macromycetes habitats. The species diversity of macromycetes in dumping sites of western wooden-steppes of Ukraine was considerably low (Simpson index: 0,91; Shannon index: -1,9). Species evenness was calculated by indices of Simpson and Shannon was rather low: 0,43 and -2,71 correspondingly; this indicated limited development of fungi. The main factors were violation of natural environment of macromycetes biotopes and anthropogenic pressure on their development as a result of aerobic and anaerobic processes that occur within the dumping sites. We founded that macromycetes could be bioindicators of the anthropogenic edaphotop of the dumping sites.
Designing of steel frameworks from thin-walled cold-formed profiles in SCAD Office

Abstract: This paper is devoted to computer-aided designing of steel frameworks from thin-walled cold-formed profiles. It has been shown that the wide utilization of building structures from thin-walled cold-formed profiles produced domestically is restricted, first of all, by shortcoming of national building standards and requirements, as well as insufficient domestic experience in economic and reliable designing of such kind of steel structures. The main attention has been paid to the structural analysis and determination of the internal forces in structural members from thin-walled cold-formed profiles taken into account restricted torsion deformations. Some scientific issues under the buckling behavior of the structural members from thin-walled cold-formed profiles, as well as post-buckling behavior of these elements after the local buckling of the compressed cross-sectional parts has been presented.As example, computer-aided design for the steel framework of typical warehouse building of company Llentabhallen Sp.z.o.o. using SCAD Office has been presented. In this example the particularities of the 3D finite element modeling of the steel frameworks from cold-formed thin-walled profiles has been also presented.
