Abstract: worldwide, tracheal transplantation is a novel procedure; it poses very complex challenges that must be assumed by an interdisciplinary group in order to obtain optimal results. because of their specialized knowledge, anesthesiologists must be actively involved in every step of the management of patients undergoing such transplantation, namely: planning, intraoperative and intensive care and hospitalization after the latter. a general overview of anesthesiologists' role is presented based on the experience of 11 tracheal transplantations in medellín, colombia.

Abstract: El trasplante de tráquea es un procedimiento novedoso no solo en Colombia sino también en el ámbito mundial. Los retos que plantea al grupo tratante son de tal complejidad que deben ser asumidos en forma interdisciplinaria para obtener los mejores resultados posibles. Por ello el anestesiólogo debe integrarse muy activamente en todas las fases de atención del paciente que va a ser trasplantado: planeación, manejo en el quirófano, cuidado intensivo y hospitalización; de este modo puede enfocar integral y oportunamente el tratamiento haciendo uso de sus conocimientos especializados acerca de la vía aérea y del comportamiento hemodinámico. Se presenta en este artículo una visión panorámica del papel del anestesiólogo con base en la experiencia de 11 trasplantes traqueales en Medellín, Colombia. Abstract: inverted papilloma of the upper urinary tract is a rare tumor and generally considered to be a benign. we present two case of benign ureteral inverted papiloma. they were discovered after to study a upper urinary infection and a gross hematuria respectively. both cases appear to be associated with asynchronous and synchronous bladder carcinoma, so it is very important a strict follow- up.

Abstract: background: autoimmune hepatitis (aih) is a chronic and immune disease with typical clinic and laboratory characteristics. liver/kidney microsomal antibody type 1 (lkm-1) and liver cytosol antibody type 1 (lc1) are the markers of type 2 aih and differentiates the subtypes of aih. anti- lkm-1 antibodies are present in aih type 2 and help to differentiate aih type 1from aih 2. incidence and prevalence of autoimmune hepatitis and lkm-1 antibodies in colombia are unknown. objectives: our goal was to analyze a group of patients with aih and to study the frequency of anti-lkm-1 antibody in these patients. in addition, we described the autoimmune diseases and rheumatologic signs and symptoms associated to this group of patients with autoimmune hepatitis. methods: semi-quantitative quanta lite probe to detect anti-lkm-1 antibodies was developed by elisa method from inova laboratories in 32 colombian patients with autoimmune hepatitis (aih). the diagnosis was classified of probable or definitive aih according of the classification of international autoimmune hepatitis group. patient with less than 10 points, dementia, toxic or drug induced hepatitis and hepatitis of metabolic origin were excluded. results: no one of the patients was positive for anti lkm-1 antibody. the mean age was 39.03 (1,2-80), women 78.1 and men 21.9. level of education of these patients: high school 37.5% and university 31.3%. the most frequent symptom was fatigue in 68.8%, jaundice 62%, pruritus 24.9%, arthralgia and abdominal pain 34.4%. antinuclear antibodies positive more than 1:80 were found in 53.8% of patients. anti- smooth muscle antibody more than 1:40 were positive in 40.6% of patients. sjogr?n?s syndrome and autoimmune thyroiditis were the most frequently autoimmune diseases found.

Abstract: Football and the Internet have shown early signs of a prosperous marriage. Web sites with football content are among the most popular, with football followers and web users sharing remarkably similar demographics. Despite these compelling observations, limited empirical research has been undertaken exploring how to maximise the opportunities for competitive advantage that the Internet can provide to football clubs. This research was a response to this void, and was undertaken by manually accessing and evaluating the web sites of European Football Clubs. Quality of web sites was determined using an original Web Assessment Index, which focuses on four categories: accessibility, speed, navigability and content. A detailed report of the results arising from this investigation is presented and systematically analyzed. These findings will be useful for both researchers and practitioners who seek to understand the issues relevant to football club management.
Manejo de la vía aérea en trasplante de tráquea

Abstract: A pesar de los grandes avances en cirugía de resección traqueal y en dispositivos como las endoprótesis, existe pacientes en quienes dichas técnicas no son viables debido al compromiso tan extenso de su estenosis; por lo tanto, su problema no se ha podido resolver alterando la calidad de vida y muchas veces poniendo en riesgo la vida de los pacientes, secundario a una obstrucción de la vía aérea. A continuación, se describe la experiencia del grupo de la Universidad de Antioquia y el Hospital Universitario San Vicente de Paúl, en el manejo de la vía aérea durante el transplante de tráquea realizado a 5 pacientes en la institución, ya que dicho procedimiento se convierte en un reto para el anestesiólogo por compartir el campo quirúrgico con el cirujano. Gracias al entrenamiento previo en cadáveres y en modelos experimentales animales y al trabajo multidisciplinario llevado a cabo, es que se llega a mostrar buenos resultados. In spite of great advances in tracheal resection and endoprosthesis, there are many patients in which stenosis is so vast that they are not eligible for those procedures. They have bad quality of life and risk of airway obstruction and denth. The experience of the airway management group of Universidad de Antioquia/Hospital Universitario San Vicente de Paúl of tracheal transplantation (5 patients) is described. The management of this patients is a challange for the anesthesiologist, as the surgical field is shared with the surgeon. Due to previous training in cadavers and in animal models and multidisciplinary work, it is possible to show good outcomes.
