VIH/SIDA y determinantes sociales estructurales en municipios del Valle del Cauca-Colombia

Abstract: this study aimed to determine the association between the prevalence of hiv/aids, according to self-report for the period 2000-2005 and some structural determinants in the municipalities of the department of valle del cauca, colombia. an observational, descriptive and ecological study was conducted. the data were taken from the census of population (2005) conducted by the national administrative statistics department (dane). variables related to social structural determinants were the quality of housing, sanitary infrastructure, the minimum level of human capital and access to health care services. evidence statistically significant (p=1%, p=5%) of the association between these determinants and the prevalence of hiv in nine of 21 municipalities studied was found. the structural determination of hiv/aids deserves additional analysis that should be conducted from the perspective of economic and social rights of people affected by the virus, all in guaranteeing the right to health.

Abstract: the aim of this study was to analyze the diagnosis of sexually transmitted diseases (std) according to self-reporting by a sample of colombian adult population. a secondary source analysis was carried out using data of the national health survey-2007. the sample consisted of 29,760 persons between ages 18 and 69. at national level, a self-reported diagnosis of std of 0.6% was found; the highest percentages occurred in the pacific and atlantic regions, and the lowest in bogotá. findings showed that women (or=1.7), adults in the 18 24 age range (or=11.9), and population affiliated to the subsidized regime (or=2.1) were more likely to report std diagnosis. social determinants linked to the socio-economic position, poverty, access to health services and the roles of gender must be taken into consideration when analyzing stds in colombia.

Abstract: introduction: to do a review of the bibliography published in statistical methodology specialized journals about the procedures developed to estimate the prevalence and performance of clinical diagnostic tests. methods: this review includes 52 articles about procedures developed to estimate the prevalence and performance of clinical diagnostic test parameters found in journals specialized in statistical methodology applied to health areas. results: three important statistical problems were found: the presence of a number of parameters higher than the pieces of information available to do the estimation procedure, known as lack of the ability to identify, the presence of individuals with negative outcome in the screening tests for comparison, whose actual health conditions are unknown (verification bias) and the dependence between test results. conclusions: a wide range of possibilities to address the three problems that characterized the estimation problem was found. to address the verification bias problem many alternatives consider the use of the latent variables and others use different parameters. many strategies to address the lack of the ability to identify and the dependence between test outcomes were found too. most of the proposed alternatives are characterized by their complexity and difficult practical applicability.

Identification of Global Gene Expression Shifts Using Microarray Data from Different Biological Conditions  [PDF]

Abstract: Gene expression data have been very useful during the past two decades for the detection of differentially expressed genes when two (or more) biological conditions are compared. Studies seeking for differentially expressed genes are based on testing gene by gene for a mean differential expression between two conditions. Nevertheless, the global shift in gene expression when taking into account all genes present on a microarray experiment, has not yet been investigated and could provide different information on genes that could be affected by the condition under research. Such a global approach would help identifying a gene expression threshold, characteristic of a certain condition and therefore could be used for diagnosis together with the list of differentially expressed genes detected by classical methods. Moreover, characterizing genes below or above such a threshold could give new insights into the molecular mechanisms implicated functionally in each condition. Here, we present a simple methodology, based on heuristics, gene filtering, variable transformation and descriptive statistics in order to identify such global gene expression shifts and the characteristic threshold so the same can be applied by any professional that works with expression gene data and not only by statisticians. Our procedure is illustrated on a real gene expression data set comparing pathogen inoculated tomatoes with non-inoculated tomatoes. This methodology can be used for the identification of the threshold values when we have continuous variable data sets from two populations with overlapped distributional forms (histograms) in most of their percentiles.
Proposed Scales for Measuring Suicidal Ideation in Adult Cancer Patients  [PDF]

Abstract: Suicidal ideation, which can occur in oncology patients, is associated with suffering and a reduced quality of life. Empirical studies indicate that the incidence of suicide in people with cancer is approximately twice that of the general population. Objective: This cross-sectional descriptive study was designed to establish the prevalence of suicidal ideation in adult oncology patients based on a combination of scales to measure this variable. Method: A battery of three instruments (Scale for Suicidal Ideation. Item 9 of Beck’s Depression Inventory, and a semi-structured interview) was used to make estimates that are not based on just one test and can thus make a comprehensive evaluation of suicidal ideation in the study sample. Four approximations, based on different items from suicidal ideation assessment scales, were developed to determine the most sensitive combination for identifying the presence of suicidal ideation in adult cancer patients. Results: The highest prevalence of suicidal ideation found with the proposed scales was 24.5% (CI = 16%-33%) with Scale 1; 23.6% (CI = 15.2%-32%) with Scale 2; and 19.1% (CI = 11.3%-26.9%) with Scale 4. The lowest prevalence of suicidal ideation was 17.3% (CI = 9.8%-24.8%) with Scale 3. Conclusions: Suicidal behavior in the oncology patient should be assessed; however, there is a need to select sensitive instruments that can be applied easily and are suitable for the target group, which faces difficulties in participating in extensive evaluations.
La producción de salud infantil en Colombia: una aproximación

Abstract: this paper achieves a national approach to the main factors that influence the child health production, as well as the determinants of the demand for health inputs in colombia. data was taken form the national health and demographic survey 2005. in order to correct endogeneity problems in the inputs, the empiric analysis uses instrumental variables, and the estimation was done in two non-linear stages. in general, it was found that the long term health status for children, measured as z-score, is determined by: a) preventive care during pregnancy, b) availability of household resources and, c) child characteristics, such as age and gender.

Abstract: this paper explores the linkage between international migration and school attendance for colombia during 2005. data from national demographic and health survey, carried out by profamilia, were used. the potential presence of endogeneity was ruled out by estimating probit models using non-linear-two-stages methods. no evidence was found on migration increasing the probability of attending school.

Abstract: El objetivo de este trabajo es explorar el vínculo entre la migración internacional y la asistencia escolar en Colombia, en el a o 2005. Para ello se usaron los datos de la Encuesta Nacional de Demografía y Salud (ENDS), aplicada por Profamilia en el mismo a o. Considerando posibles problemas de endogeneidad se estimaron modelos Probit usando el método de dos etapas no lineales. No se encontró evidencia estadística de que la migración internacional hubiera afectado la probabilidad de asistir a la escuela.

Abstract: colombia, like most latin american countries and being awareof the negative social and economic impact of child labor, has been committed to fight for the eradication of this phenomenon. this is evidenced by the colombian government's ratification in 2007 of the ilo convention 182 concerning the prohibition and immediate action against the worst forms of child labor. at the regional level, as part of the development plan for the department of valle del cauca 2008 -2011, the government is currently carrying out a strategy to eliminate child labor in its 42 municipalities. taking this background into account, this paper investigates the determinants of child labor and school attendance in the department of valle, based upon the national survey on the quality of life of 2003. the analysis employs a bivariate probit econometric model that allows a joint and interrelated study of different decisions, in this case school attendance and labor.
