Reconstruction of the infected war defects of the tibia: A fragment elongation according to the Ilizarov technique

Abstract: Introduction/Aim. The treatment of tibia defects complicated with chronic osteomyelitis is difficult, often requiring one or more surgical interventions with prolonged periods of functional incapacity. Methods. We treated 20 patients with tibia defects, who had been wounded during the war operations in the former Yugoslavia, complicated with chronic osteomyelitis by applying the Ilizarov apparatus. In 10 patients with the average defect of 4.7 cm, interfragmentary diastasis of 1.5 cm, and 3.1 cm of shortening we applied the bilocal synchronous compressive - distractive method (BSCD). In the remaining 10 patients with average defect of 6.4 cm, interfragmentary diastasis of 5.5 cm, and 1.6 cm of shortening we applied bilocal alternating distractive - compressive osteosynthesis (BADC). Results. The average followup was 93 months. In the group A, the average distraction index was 10.6, maturation index 39.8, and external fixation index 52.5. In the group B, the average distraction index was 11.7, maturation index 47.1, and external fixation index 60.1. The average time from the application to the apparatus removing was in the group A 6.5 months, and in the group B 11.9 months. There were 27 complications: 11 problems, 8 disturbs, and 8 true complications. Pin-track inflammation of the soft tissue was noted most frequently (6 patients). Conclusions. One stage of repairing inflamation and the restitution of defect in lower leg tissue was the advantage of this type of treatment. All of the patients recovered. There was not any bad result, either in osteal or in functional outcome.

Abstract: The tourism industry is a great potential for the development of Serbia. The main characteristics of the existing and potential tourist offer of Serbia are interesting and diverse natural resources and cultural and historical heritage. Felix Romuliana, established palace of the Roman emperor Galerius, is located in the valley of the Black Timok, near Zaje ar and the village of Gamzigrad in eastern Serbia. The palace was built in the late third and early fourth century, as a testamentary construction. This is where the Roman emperor was buried and included among the gods. It is the best preserved example of Roman palatial architecture which in 2007 was added to the List of World Heritage of UNESCO. One of the key tasks of this paper is to point out ways of promoting and popularizing this tourism potential that can be used as a resource for the development of cultural tourism and as such strengthen the position of Serbia''s tourist offer in Europe. The aim of this paper is contained in the presentation of the site Felix Romuliana and extraction of the most important attractiveness through valorization, which on the bases of historical and cultural significance may be activated for tourism purposes. Review of thirty patients with bone chondroblastoma

Abstract: Background/Aim. To analyse the results of the treatment of the patients with the diagnosis of chondroblastoma, to confirm the possible malignancy and to recommend the best and the safest method of the treatment. Methods. We reviewed the cases of 30 patients with chondroblastoma who were treated between 1975 and 2004. Data were obtained using complete medical documentation, physical examinations, radiographic findings, and the available additional diagnostic procedures. Results. We found that the proximal part of the tibia, proximal part of the humerus, and distal part of the femur were the most common sites of the tumor in 63% of the cases. The higher prevalence of chondroblastoma in male patients was found, especially in the second decade of life. The patients were treated with different surgical procedures after histologically confirmed chondroblastoma. In 1 of the patients, radiation therapy was performed because the lesion recurred, after which the malignant transformation of chondroblastoma occurred. We found two more malignant chondroblastomas, one of which had been diagnosed as a primary tumor. Seven patients had a local recurrence, one of them had a second recurrence and the malignant transformation of chondroblastoma. The only solution was a below knee amputation. Conclusion. Chondroblastoma of bone is a rare lesion with the high local recurrence rate. We emphasized the need for an adequate and rapid diagnosis, including histological verification. The treatment was strictly surgical. The basic goal of the treatment was to avoid tumor penetration into articular cavity and/or local soft tissues. Malignant chondroblastoma of bone should be treated with radical surgical resection, avoiding any adjuvant therapy.
The use of Geothermal Energy Resources in the Tourism Industry of Vojvodina (Northern Serbia)

Abstract: Exploitation of geothermal energy in Vojvodina is still at an unjustly low level taking into account the abundance of resource locations, some of which are ranked among the most affluent in Europe. Moreover, development of geothermal exploitation started in Serbia at about the same time as in other countries whose geothermal energy facilities are now at the highest technological level and which are leaders in this field. The largest use of geothermal energy in Vojvodina is present in the non-energetic area, especially in spas and sports–recreational centers. Other, seasonal consumers of geothermal energy are from the field of industry and agricultural production where the energy is used for heating of cattle and poultry farms, greenhouses and other facilities. However these consumers use only a small portion of available geothermal resources. The main users are those from the tourism industry. The goal of this paper is to give an overview and an analysis of the use of geothermal energy resources, mainly geothermal waters, in the tourism industry of Vojvodina. It shows how these resources are used and also for what are they used by the tourism industry. The paper covers only geothermal resources that are currently being used by the tourism industry. The potential for future usage in this area is also briefly discussed
Misperceptions of peer gambling norms among adolescents: Analysis of a national sample in Finland  [PDF]

Abstract: Introduction: Misperceptions of peer drinking norms are widely documented in the US student populations and are associated with increased personal consumption. Few studies have examined misperceptions of peer gambling, and none of these have been conducted among adolescents in the European context. In a national sample, we examined misperceptions of peer gambling in Finland. Tenets of the social norms approach form a framework for discussion of the findings. Methods: Participants were 4526 adolescents aged 12 to 18 years who completed the nationwide Adolescent Health and Lifestyle Survey in 2011. Main measures were gambling behavior and the perception of same age-sex peers’ gambling. Misperception was an overestimation of the frequency of gambling by others compared to the actual frequency of gambling. Results: Adolescents held substantial misperceptions of peer gambling, imagining they gambled much more than they actually did. Age, sex, and gambling behavior were correlated with the perceptions. The extent of misperception was greatest among monthly gamblers, whereas non-gamblers and daily gamblers were more accurate in their perceptions. Estimations of peers’ gambling frequency were more accurate in boys than in girls and among those aged 12 years than among older adolescents. Conclusion: Our findings suggest that adolescent gambling prevention efforts could benefit from adopting a social norms approach; that is, correcting gambling-related misperceptions might discourage gambling and protect adolescents from adopting more severe gambling patterns.
Challenging the Cosmological Constant

Abstract: We outline a dynamical dark energy scenario whose signatures may be simultaneously tested by astronomical observations and laboratory experiments. The dark energy is a field with slightly sub-gravitational couplings to matter, a logarithmic self-interaction potential with a scale tuned to $\sim 10^{-3} {\rm eV}$, as is usual in quintessence models, and an effective mass $m_\phi$ influenced by the environmental energy density. Its forces may be suppressed just below the current bounds by the chameleon-like mimicry, whereby only outer layers of mass distributions, of thickness $1/m_\phi$, give off appreciable long range forces. After inflation and reheating, the field is relativistic, and attains a Planckian expectation value before Hubble friction freezes it. This can make gravity in space slightly stronger than on Earth. During the matter era, interactions with nonrelativistic matter dig a minimum close to the Planck scale. However, due to its sub-gravitational matter couplings the field will linger away from this minimum until the matter energy density dips below $\sim 10^{-12} {\rm eV}^4$. Then it starts to roll to the minimum, driving a period of cosmic acceleration. Among the signatures of this scenario may be dark energy equation of state $w \ne -1$, stronger gravity in dilute mediums, that may influence BBN and appear as an excess of dark matter, and sub-millimeter corrections to Newton's law, close to the present laboratory limits.
A New Dimension Hidden in the Shadow of a Wall

Abstract: We propose a new way to hide the fifth dimension, and to modify gravity in the far infra-red. A gravitating tensional membrane in five dimensions folds the transverse space into a truncated cone, stoppered by the membrane. For near-critical tension, the conical opening is tiny, and the space becomes a very narrow conical sliver. A very long section, of length comparable to the membrane radius divided by the remaining conical angle, of this sliver is well approximated by a narrow cylinder ending on the membrane. Inside this cylindrical throat we can reduce the theory on the circle. At distances between the circle radius and the length of the cylinder, the theory looks 4D, with a Brans-Dicke-like gravity, and a preferred direction, while at larger distances the cone opens up and the theory turns 5D. The gravitational light scalar in the throat can get an effective local mass term from the interplay of matter interactions and quantum effective potentials on the cone, which may suppress its long range effects. We discuss some phenomenologically interesting consequences.
Hollywood industry: Correlation between film production and political discourse

Abstract: The paper focuses on the relation between Hollywood industry and political establishment of the USA, particularly US foreign policy and the military intervention as its specific form. Only the biggest and the most significant US military interventions were considered: World War Two, Korean War, Vietnam War, military interventions in Latin America, in the Middle East, Asia, Africa and on Balkan, concerning their treatment in Hollywood movies without analyzing them in broader socio-political context. In addition, the anticommunism in Hollywood is also considered, which was perhaps the most perennial content of the US foreign policy.
Effect of recasting on the elastic modulus of metal-ceramic systems from nickel-chromium and cobalt-chromium alloys

Abstract: Background/Aim. Elastic modulus of metal-ceramic systems determines their flexural strength and prevents damages on ceramics during mastication. Recycling of basic alloys is often a clinical practice, despite the possible effects on the quality of the future metal-ceramic dentures. This research was done to establish recasting effects of nickel-chromium and cobalt-chromium alloys on the elastic modulus of metalceramic systems in making fixed partial dentures. Methods. The research was performed as an experimental study. Six metal-ceramic samples of nickel-chromium alloy (Wiron 99) and cobalt-chromium alloy (Wirobond C) were made. Alloy residues were recycled through twelve casting generations with the addition of 50% of new alloy on the occasion of every recasting. Three- point bending test was used to determine elastic modulus, recommended by the standard ISO 9693:1999. Fracture load for damaging ceramic layer was recorded on the universal testing machine (Zwick, type 1464), with the speed of 0,05 mm/min. Results. The results of this research revealed significant differences between elasticity modules of metal-ceramic samples in every examined recycle generation. Recasting had negative effect on the elastic modulus of the examined alloys. This research showed the slight linear reduction of elastic modulus up to the 6th generation of recycling. After the 6th recycling there was a sudden fall of elastic modulus. Conclusion. Recasting of nickelchromium and cobalt-chromium alloys is not recommended because of the reduced elastic modulus of these alloys. Instead of reusing previously recasted alloys, the alloy residues should be returned to the manufacturer. .
