Abstract: in over 100 war conflicts that have occurred in the last 10 years, m ore than 80 percent of the victims were civilians. the scope of recent studies has been increasingly focused on psychological and social effects among civilians who are the target and main victims of contemporary wars. this article is an explanation of why war is considered a disaster, a revision of modern terms for war, disaster and the psychological effects of war on civilians, the training of children as soldiers, and the resulting mental disorders. it touches upon sexual violence problem as a war weapon. women, adolescents and young children are often violated and forced into sexual slavery or any other type of sexual abuse, causing humiliation and psychological, ethical and moral damage, which can affect social groups, communities, social classes, or races. the deterioration or destruction of the economic, social and political infrastructure of the countries in conflict hinders the satisfaction of the citizens' basic needs. the most common psychological effect among war victims is posttraumatic stress, but there are also other mental disorders which must be taken into consideration

Abstract: a conceptual and methodological analysis of psychiatry as a science is made, taking the cuban psychiatrists' criteria as a starting point, referring to it as a branch of the medical sciences, in which the object of study is the mental illness or the process health-mental illness, being its actions the diagnosis, treatment, ethiology and pathogenesis, rehabilitation and prevention, as well as its epidemiological study that is related to the social and interdisciplinary levels. starting with redirecting psychiatry towards the community, a reflection is made of the meaning and implications of this science. it is documented the application of the community mental health attention model that is applied through the community centers of mental health, which constitute the cornerstone of the cuban model of community attention. thus, community psychiatry represents the new paradigm of current cuban psychiatry. it is recognized that the philosophical foundation that sustains current cuban psychiatry is the marxist-leninist theory

Abstract: Se realiza un análisis metodológico conceptual de la Psiquiatría como ciencia partiendo de los criterios de los psiquiatras cubanos, coincidiendo en que es una rama de las Ciencias Médicas, en la cual el objeto de estudio es la enfermedad mental o el proceso salud enfermedad mental y sus acciones es el diagnóstico, tratamiento, etiopatogenia, rehabilitación y prevención de las mismas, así como su estudio epidemiológico y que incluye el carácter social e interdisciplinario. A partir de la reorientación de la Psiquiatría hacia la comunidad se hace una reflexión del significado e implicaciones de la misma. Se argumenta como en Cuba se aplica el Modelo de la Atención Comunitaria de Salud Mental a través de los Centros comunitarios de Salud Mental que constituyen la piedra angular del modelo cubano de atención comunitaria, con los que constituye la Psiquiatría comunitaria el nuevo paradigma de la Psiquiatría cubana actual. Abstract: En más de 100 conflictos bélicos que se han producido en los últimos diez a os, más del 80 por ciento de las víctimas son civiles. Se ha producido un desarrollo acelerado en las investigaciones concernientes a las consecuencias psico-sociales de la guerra en la población civil, la cual es el blanco principal de las víctimas en la guerra contemporánea. Este trabajo constituye una revisión bibliográfica sobre la guerra como desastre, acerca de los conceptos “modernos” de “guerra total”, desastre y las consecuencias psicológicas de la guerra en la población civil, la utilización de los ni os como soldados lo cual acarrea desórdenes mentales marcados y se aborda el problema de la violencia sexual como arma de guerra. Las mujeres, las jóvenes, e incluso ni as, son a menudo sometidas, violadas, secuestradas y forzadas a la esclavitud sexual o cualquier tipo de abuso sexual; son vejadas, humilladas y da adas en lo más profundo de sus sentimientos psicológicos, y valores morales, lo que también trasciende al grupo social, comunidad, clase social o etnia a la que pertenecen. El deterioro o la destrucción de la infraestructura económica, social y política de las naciones en conflicto impide la atención de las necesidades básicas de la población. Abstract: background: to identify epidemiological aspects of the osteoporotic hip fractures (foc) in the province of zamora in 1993, and the basic and standardised accumulative incidence rates amongst the spanish population. methods: 215 foc cases were studied in the province throughout 1993. 203 were admitted to hospital, 188 of which were treated surgically by the traumatology department. the surveyed variables were: sex, age, location (rural, urban), type of (cervical, trochantereal) fracture, surgical procedure (partial prosthesis, ender stems, others), seasonal factors and average hospital stay. results: global accumulative incidence: 100.6 per 100,000 inhabitants. with respect to people over 50, the rate was 246.5 per 100,000 inhabitants (according to the 1991 census). female predominance (3.78:1). average age: 81.3?8.8 years. 50% distribution of fracture type, both at global and female level. dual incidence of trochantereal fracture suffered by males. average stay: 15.35 days. standardised accumulative incidence according to age and sex for the spanish population in 1991 would be 202.99 fractures per 100,000 inhabitants and year, which would result in a total number of 23,740 fractures per year. conclusions: the province of zamora currently has one of the highest rates of foc in spain, both with respect to overall population as well as people over 50 years of age. average age, predominance of the female sex, distribution of the type of fracture and average hospital stay are similar to the figures published in our country.

Abstract: ictal onset localization is a important factor in presurgical evaluation of epilepsy. this paper describes the localization of a seizure onset recorded by magnetoencephalography (meg) from a 12-year-old male patient who suffered from complex partial drug-resistant seizures. mri revealed a 20mm diameter lesion located in left hippocampus. scalp eeg showed left temporal theta waves. interictal meg registrations detected isolated spike-wave activity posterior and inferior to the mri lesion. ictal meg showed continuous spike-wave activity (2 hz). dipole localization sited seizure onset in the inferior left temporal gyrus, the same localization of the interictal meg activity. this ictal activity spreads bilaterally to frontal areas. intrasurgical electrocorticography recording confirmed interictal meg results.

Abstract: En Colombia es evidente la necesidad de innovar en el sector público. En este artículo se presenta el caso específico de la innovación del gobierno en el sector minero. Para esto, se iniciará con la identificación del potencial minero en Colombia y las barreras para su explotación. En seguida se explorarán los planes de innovación del gobierno como una estrategia para sobrepasar aquellas barreras que impiden aprovechar este potencial.//The need to innovate the public sector in Colombia is evident. This article presents the specific case of governmental innovation within the mining sector. It begins with the identification of mining potential in Colombia and the barriers that impede its exploitation. It is concluded by exploring governmental innovation plans that seek to overcome those barriers to take full advantage of Colombia's potential.

Approach to early-onset colorectal cancer: Clinicopathological, familial, molecular and immunohistochemical characteristics

Abstract: AIM: To characterize clinicopathological and familial features of early-onset colorectal cancer (CRC) and compare features of tumors with and without microsatellite instability (MSI).METHODS: Forty-five patients with CRC aged 45 or younger were included in the study. Clinical information, a three-generation family history, and tumor samples were obtained. MSI status was analyzed and mismatch repair genes were examined in the MSI families. Tumors were included in a tissue microarray and an immunohistochemical study was carried out with a panel of selected antibodies.RESULTS: Early onset CRC is characterized by advanced stage at diagnosis, right colon location, low-grade of differentiation, mucin production, and presence of polyps. Hereditary forms represent at least 21% of cases. Eighty-one percent of patients who died during follow-up showed a lack of expression of cyclin E, which could be a marker of poor prognosis. β-catenin expression was normal in a high percentage of tumors.CONCLUSION: Early-onset CRC has an important familial component, with a high proportion of tumors showing microsatellite stable. Cyclin E might be a poor prognosis factor.

Abstract:

Klippel-Trenaunay Syndrome (KTS) is a rare triad of congenital vascular malformations involving extensive Port wine stains, soft tissue or bone hypertrophy and underlying venous and/or lymphatic malformation affecting limb, pelvicor abdominal organs. Pregnancy is known to exacerbate KTS complications and can put women at increased obstetrical risk due to deep venous thrombosis and other thromboembolic events. We report a case of a pregnant woman with KTS with the personal antecedent of two episodes of pulmonary thromboembolism.
