Abstract: vaccination of infants with conjugated haemophilus influenzae type b (hib) vaccines has been proven to reduce hib meningitis by 95% and pneumoniae by 20%. the routine use of hib vaccine is facilitated by the introduction of combination vaccines into the epi (expanded plan of immunization). the objective of this study was to compare the immunogenicity and reactogenicity of an extemporaneously mixed dtpw/hib (diphtheria-tetanus-whole cell pertussis) combination, using the technology of two brazilian manufacturers, against a licensed dtpw/hib european combination in 108 infants vaccinated at 2, 4 and 6 months according to the local national schedule. the brazilian combination was highly immunogenic with hib seroprotection rates (anti-prp > 0.15 mg /ml of 98% after 2 doses and 100% after 3). also for tetanus and pertussis the new brazilian combination was as immunogenic as the european counterpart, except the diphtheria seroprotection rates and titers were lower. there was also no clinically relevant difference in reactogenicity. if these feasibility results are confirmed, the brazilian dtpw/hib combination should help to boost the uptake of hib vaccination in brazil.

Abstract: objective:medication errors are currently a worldwide public health issue and it is one of the most serious prescription errors. the objective of the study was to evaluate the practice of prescribing high-alert medications and its association with the prevalence of medication errors in hospital settings. methods: a retrospective cross-sectional study was conducted including 4,026 prescription order forms of high-alert medications. there were evaluated all prescriptions received at the pharmacy of a reference hospital in the state of minas gerais, southeastern brazil, over a 30-day period in 2001. prescription were checked for legibility, patient name, type of prescription, date, handwriting or writing, prescriber identification, drug prescribed, and use of abbreviations. prescription errors were classified as writing or decision errors and how the type of prescription affected the occurrence of errors was assessed. results: most prescriptions were handwritten (45.7%). in 47.0% of handwritten, mixed and pre-typed prescriptions had patient name errors; the prescriber name was difficult to identify in 33.7%; 19.3% of them were hardly legible or illegible. of a total of 7,148 high-alert drugs prescribed, 3,177 errors were found, and the most frequent one was missing information (86.5%). errors occurred mostly in prescriptions of heparin, phentanyl, and midazolam. intensive care and neurology units had the highest number of errors per prescription. non-standard abbreviations were frequent and widespread. overall it was estimated 3.3 errors per prescription order form. pre-typed prescriptions were less likely to have errors compared to mixed or handwritten prescriptions. conclusions: the study results show there is a need for standardizing the prescription process and eliminating handwritten prescriptions. the use of pre-typed or edited prescriptions may reduce errors associated to high-alert medications.
Mitochondrial control region genetic diversity and maternal ancestry of a Brangus-Ibage cattle populations

Genetics and Molecular Biology , 2005, DOI: 10.1590/S1415-47572005000100011
Abstract: the genetic diversity of 277 nucleotides in the mitochondrial dna control region (nt 15,964 to 16,240 in reference sequence) was analyzed in crossbreed beef cattle (brangus-ibage, 5/8 bos primigenius taurus x 3/8 bos primigenius indicus) as well as in some nellore samples (b. p. indicus). fifty-seven mutations were found in brangus-ibage comprising 18 haplotypes (haplotype diversity, h = 0.851 ± 0.041 and nucleotide diversity, ntd = 0.009 ± 0.006) and 66 in nellore (h = 1.00 ± 0.27, ntd = 0.014 ± 0.012). these data indicated sequence identities of 99.6 and 92.1% between the b. p. taurus' reference sequence and brangus-ibage and nellore, respectively. the comparison of our data with sequence data for 612 individuals recovered from genbank showed a total of 205 haplotypes defined by 99 polymorphic sites. most of the variability (53%) was due to differentiation within breeds. the phylogenetic tree constructed using the neighbor-joining method showed clearly the well-known dichotomy between b. p. taurus and b. p. indicus. the brangus-ibage clustered with b. p. taurus lineages; however, the displacement of nellore from b. p. indicus branch probably indicates a substantial b. p. taurus maternal ancestry in some nellore samples (obtained from genbank) and reflects the primarily male-driven introduction of this breed in brazil.

Abstract: diagnosis of the nonclassical form of 21-hydroxylase deficiency (nc-21oh) is based on clinical hyperandrogenism and basal 17-hydroxyprogesterone (17ohp) levels >5ng/ml or >10ng/ml after acth-stimulation. we present two children with precocious pubarche and a hormonal diagnosis of nc-21oh, whose molecular study of the cyp21a2 gene confirmed the diagnosis in just one case. after that, we re-evaluated the diagnosis of 58 patients with acth-stimulated 17ohp levels >10ng/ml with molecular studies of cyp21a2 gene. only 37 (67%) of these patients had mutations detected in both alleles: basal 17ohp levels were >5ng/ml in 70%, normal in 11% and between 2 and 5ng/ml in the remaining patients. the lowest acth-stimulated 17ohp level in this group was 17ng/ml. we then analyzed 59 obligate carriers for 21-hydroxylase deficiency and found two asymptomatic subjects with acth-stimulated 17ohp levels of 11 and 15ng/ml, respectively. based on these findings, we suggest that the current cut-off value for acth-stimulated 17ohp level overestimates the diagnosis of nc-21oh.
Diversity of microsatellites linked to the FSHbeta gene, their usefulness for individual identification and association with reproductive performance
Duarte, Letícia Becker Homrich;Moraes, José Carlos Ferrugem;Weimer, Tania de Azevedo;
Ciência Rural , 2005, DOI: 10.1590/S0103-84782005000100023
Abstract: the genetic diversity of three microsatellites (ilsts027, mbo22, bm4325) mapped on the bovine chromosome 15 and linked to the follicle-stimulating hormone beta gene (fsh?) was investigated in cows of a brangus ibagé herd and the efficiency of these markers for individual identification and parentage control was estimated. possible associations between each molecular marker and cow reproductive performance were also analyzed. six alleles were detected in bm4325 and ilsts027 and 12 in mb022, the most frequent being bm4325*101, bm4325*103, ilsts027*169 and mb022*229. polymorphic information content ranged from 0.58 to 0.88 while expected heterozygosity ranged from 65% to 89%, with an average mean value of 77%. although only three markers were studied, the combined values indicate a high power of exclusion of a false parentage (94%) and of individual identification (3.8 x 10-4). the association analyses based on statistics parameters [mb022 (n=104, ci=545.3±127.0, wfc=349.9±53.4), bm4325 (n=106, ci=542.2±124.9, wfc=350.5±54.4) and ilsts027 (n =105, ci=543.4 ± 124.5, wfc=350.1±54.5)] indicated no positive association between each microsatellite and weight at first calving. calving interval (ci) also seemed not to be influenced by the ilsts027 or mb022 system. however, carriers of at least one bm4325*101 allele presented a ci about 54 days shorter than the other animals (p=0.04; n=106). this marker could be useful for marker-assisted selection, allowing the improvement of reproductive performance, at least in the brangus ibagé herd.
Genetic polymorphism and relationships among several swine populations of landrace, large white and duroc breeds
Tagliaro, Cláudia Helena;Franco, Maria Helena Lartigau Pereira;Weimer, Tania Azevedo;
Ciência Rural , 1997, DOI: 10.1590/S0103-84781997000300021
Abstract: the data of three protein polymorphisms were used to investigate the genetic relationships among the landrace, large white and duroc swine breeds reared in brazil, 12 other populations of these same breeds from various countries and a population of belgium landrace. the dendrogram, constructed from matrix of genetic distance coefficients, disclosed three large groups clustered by breed. among them, the landrace and the large white showed in average closer resemblance (d= 0.203) than between them and duroc (d= 0.241). it the three breeds, the smallest genetic distances were found between brazilian and cuban pig populations (landrace: d = 0.060; large white: d = 0.052; duroc: d = 0.065), although there were not reports of pig exchanges between these two countries.
