Greater omentum gastrointestinal stromal tumor with PDGFRA-mutation and hemoperitoneum

Abstract: Although gastrointestinal stromal tumor (GIST) occurs generally in the digestive tract, omental GIST is very rare. We report the first case of an adult greater omental GIST with a new platelet-derived growth factor receptor α gene (PDGFRA)-mutation with hemoperitoneum. A 43-year-old man was admitted to our hospital complaining of acute abdominal pain. Abdominal contrast-enhanced computed tomography revealed a huge mass in the right abdominal cavity, and a large accumulation of fluid in the pelvic cavity, suggesting hemoperitoneum. We diagnosed the rupture as an intra-abdominal tumor, and an emergency tumorectomy was performed with resection of the greater omentum. This tumor was located in the distal right side of the greater omentum, and showed no continuity with the gastric wall. The tumor occurred primarily in the greater omentum. The resected tumor was about 19 cm × 12 cm × 14 cm in diameter, and weighed 1529 g. Histologically, the tumor was composed of epithelioid-shaped cells with high cellularity, and was positive for CD117 and CD34, and negative for S-100, α-smooth muscle actin. The mitosis was 6/50 under high power field. This case showed exon 18 mutation of PDGFRA with 846 (Asp to Glu) substitution, 848 (Asn to Lys) substitution. This is the first report of this PDGFRA mutation in omental GIST, and this might play an important role in the tumorigenesis of this case. Based on these findings, the tumor was diagnosed as high risk GIST primarily occurring in the greater omentum. The patient was treated with imatinib at a dose of 400 mg/d as adjuvant chemotherapy, and has been followed up for 24 mo with no evidence of recurrence.

Abstract: This note gives a counterexample on Reis [1]. Using a certain family of utility functions, this note not only gives a sharper representation than that of Reis but also demonstrates that interest rate inelastic money demand does not lead to superneutrality. This implies that superneutrality does not exist when uncerinty is introduced.

Abstract: It is argued that incentives for employees in the public service agencies will necessarily be weak because of the multiple dimensions of products, multiple principals, incomplete contract, and socializing. Some empirical studies refer to incomplete contracting situations as part of the cause of the diminishing of the public sector. This work investigates the effects of privatization and ownership shares on incentive schemes for employees who work for public or privatized firms under incomplete contracting situations. Two main results are obtained. First, the incentive intensity of public firms decreases as the government has more ownership shares, and the social benefit declines. Second, privatized firms offer their employees higher-powered incentive contracts than do public firms.

Abstract: This paper reviews the literature on the sustainability of Japanese government debt/deficit. First, we offer an overview of the approaches and the key findings on the sustainability analysis. Second, we introduce the arguments of Hoshi and Ito [1], which they predict that foreign investors’ share of JGBs could exceed beyond domestic ownership. Finally, we discuss the coordination problem in the JGB market based on the findings of Onji et al. [2] that examine how government withdrawal from the JGBs could roil the market.

Within this cohort, 20 met the study’s a priori selection criteria, which included a new diagnosis of colorectal cancer, no prior therapy, and a total bilirubin of ≥3.0 mg/dL. All 20 patients had liver metastases, and as a whole the group had a median serum bilirubin of 6.4 mg/dL (range 3.1, 28 mg/dL). Six patients received chemotherapy with an oxaliplatin-containing regimen, and four subsequently sustained a drop in their bilirubin. In one instance, a drop from 27.2 to 2.5 mg/dL occurred. These six patients lived a median of 71 days (range 23+, 283 days), but one treatment-related death occurred. In contrast, patients who received only supportive care lived a median of 28 days.Conclusion: Chemotherapy appears to provide modest benefit to newly diagnosed colorectal cancer patients with severe hyperbilirubinemia.Keywords: colorectal cancer, liver metastases, hyperbilirubinemia, chemotherapy, oxaliplatin

Abstract: preferential trade agreements have been grown in asia pacific since 1998, hence after this region faced financial crisis. the growing of those trade areas is called new asian economic regionalism. this paper shows this new wage of regional economic integration, focusing the trade, whose main features are: include countries that were not previously member of any preferential trade area; economies are members of more than one regional trade agreements; some of new agreements cross world's geographic regions and many of the new agreements are bilateral. the paper gives economic integration definition and classification and the benefits and the limits that preferential trade agreements lead to economies. it is analyzed the areas of economic integration have existed yet and those in negotiation process in asia pacific region. causes of the new asian economic regionalism are analyzed: financial crisis of asia, regional organisms and multilateral trade system lost of credibility, increasing preferential trade in other regions of the world and economic and political competition between countries of asia. conclusion is that if really there is integration between the economies, there will be a new cycle of asian economic growth, mainly in east and southeast asia, continuing the economic model led by exports.
Inflation, Growth, and Impatience in a Generalized Cash-in-Advance Economy

Abstract: In this note, we consider a cash-in-advance (CIA) economy in which the CIA constraint applies to not only consumption but also to all or part of investment and where the discounting rate is a function of consumption. We use this economy to investigate the effects of monetary growth on capital, money, consumption, and welfare. We find that as long as the condition assuring the uniqueness of the steady state holds, the effect of monetary growth on all of these variables is negative, though mitigated by the positive slope of the discounting function. Using this result, we establish a qualitative equivalence between the money-in-the-utility, transaction-cost, and CIA models.
Increasing Visual Search Accuracy by Being Watched

Abstract: In daily life, huge costs can arise from just one incorrect performance on a visual search task (e.g., a fatal accident due to a driver overlooking a pedestrian). One potential way to prevent such drastic accidents would be for people to modify their decision criterion (e.g., placing a greater priority on accuracy rather than speed) during a visual search. The aim of the present study was to manipulate the criterion by creating an awareness of being watched by another person. During a visual search task, study participants were watched (or not watched) via video cameras and monitors. The results showed that, when they believed they were being watched by another person, they searched more slowly and accurately, as measured by reaction times and hit/miss rates. These findings also were obtained when participants were videotaped and they believed their recorded behavior would be watched by another person in the future. The study primarily demonstrated the role of being watched by another on the modulation of the decision criterion for responding during visual searches.
Search for Lepton Flavor and Lepton Number Violating tau Decays into a Lepton and Two Charged Mesons

Abstract: We search for lepton flavor and lepton number violating tau decays into a lepton (ell = electron or muon) and two charged mesons (h,h' = pi^\pm or K^\pm), tau- -> ell-h+h'- and tau- -> ell+h-h'-, using 671 fb^{-1} of data collected with the Belle detector at the KEKB asymmetric-energy e+e- collider. We obtain 90% C.L. upper limits on the branching fractions in the range (4.4-8.8)x10^{-8} for tau -> ehh', and (3.3-16)x10^{-8} for tau -> muhh' processes. These results improve upon previously published upper limits by factors between 1.6 to 8.8.
