Unexpected Airway Obstruction Caused by Bilateral Hypoglossal Nerve Palsy Following Second Radical Neck Dissection  [PDF]

A 67-year-old woman underwent right radical neck dissection for cervical lymph node metastasis from maxillary gingival carcinoma. Two months later, metastasis in the left superior internal jugular lymph nodes were discovered, and left radical neck dissection was performed. Postoperatively, airway obstruction occurred despite performing extubation after confirming that the patient had fully recovered from anesthesia. Bilateral hypoglossal nerve palsy was diagnosed and the patient was reintubated. After extubation on the following day, airway obstruction was relieved, but slurred speech and impaired swallowing were persistent. In view of this, hypoglossal nerve function should be examined before the second radical neck dissection on the contralateral side.

Sitting position improves consciousness level in patients with cerebral disorders  [PDF]

Abstract: Cerebral disorders are often associated with disturbance of consciousness. Since the latter could alter the prognosis of physical function, early improvement of consciousness level is important. The present study tested the hypothesis that sitting position improves disturbances of consciousness in patients with cerebral disorders. The subjects were 17 patients with cerebral disorders and disturbances of consciousness. The consciousness level was evaluated in supine position and following a change to sitting position. The consciousness level was evaluated by the Glasgow Coma Scale (GCS). The total, eye opening, and motor GCS scores were significantly higher in the sitting position compared with the supine position. In conclusions, changing from supine to sitting position improves consciousness level in patients with cerebral disorders.

The shallow reflection surveys were carried out in 2007 and 2010 austral summers in East Ongul Island, the Lützow-Holm Complex (LHC), East Antarctica. LHC is identified by geologically as one of the Pan-African terrains of Eastern Dronning Maud Land. The multi-channel reflection surveys targeted to achieve the image of laminated layering of metamorphic rocks near the surface (the depths down to a few hundreds of meters) of the crystalline crust. Two surveys were conducted in total length of the profiles about 500 m along a main traffic load across the East Ongul Island. The multi-channel acquisition systems were utilized with combining the dense geophones along the profiles. Seismic sources were adopted by combining the boom of a power shovel, a weight drop and hammer shots with their intervals in a few tens of meters. The obtained data include clear first P-arrivals in far offset distance. The energy of P-S converted waves was enhanced because of the characteristics of the seismic sources. Pre-stacked images could be expected to the information on metamorphic layering for several lithological structure composed by hornblende gneiss, garnet gneiss and pyroxene gneiss appearing as the surface bedrocks. The conducted shallow reflection surveys would give rise to one step for understanding tectonic formation of LHC, as one of the Pan-African mobile belts in Gondwana super-continent.

Severe Mediastinal Emphysema and Tension Pneumothorax Caused by Cough-Induced Intercostal Lung Herniation  [PDF]

Abstract: We report a case of cough-induced intercostal lung herniation with severe mediastinal emphysema and pneumothorax. A 60-year-old overweight man was admitted with dyspnea, chest pain, and chest wall bulging. Imaging confirmed left pneumothorax, fractures of the 8th to 10th ribs, and a defect in the 8th intercostal muscles. The presence of protrusion of lung parenchyma outside the intercostal space was unclear. Although the symptoms of tension pneumothorax disappeared after drainage, the subcutaneous emphysema was uncontrollable. Emergency surgery revealed lung injury near the 8th rib wedge not directly related to the rib fracture, and lung herniation was observed. We performed direct approximation of the ribs, and the lung parenchymal surface was encased by absorbable materials, avoiding infection risk. Although the lung herniation recurred 5 months later, the patient did not desire additional surgical repair. Non-absorbable materials should be considered for repair of lung herniation, excluding patients at high infection risk.

Ontological Approach toward Cybersecurity in Cloud Computing

Abstract: Widespread deployment of the Internet enabled building of an emerging IT delivery model, i.e., cloud computing. Albeit cloud computing-based services have rapidly developed, their security aspects are still at the initial stage of development. In order to preserve cybersecurity in cloud computing, cybersecurity information that will be exchanged within it needs to be identified and discussed. For this purpose, we propose an ontological approach to cybersecurity in cloud computing. We build an ontology for cybersecurity operational information based on actual cybersecurity operations mainly focused on non-cloud computing. In order to discuss necessary cybersecurity information in cloud computing, we apply the ontology to cloud computing. Through the discussion, we identify essential changes in cloud computing such as data-asset decoupling and clarify the cybersecurity information required by the changes such as data provenance and resource dependency information.

The relationship between changes of elderly people’s participation in social activities and their cognitive functions was examined. Healthy, middle and upper-middle aged people (n = 407) responded to the Nagoya University Cognitive Assessment Battery and to a questionnaire inquiring about their participation in social activities. The results suggested that those whose participation in social activities increased in the last 6 months showed better verbal functioning than those whose participation was unchanged or decreased. Moreover, those whose social participation decreased showed inferior memory function compared to those whose social participation was unchanged, or increased. Furthermore, those whose social participation increased had better information processing speed than those whose social participation decreased. These results suggest that changes in social activities might act as an important signal that is indicative of the cognitive decline in elderly people, which would be useful to local health care managers in the elderly people’s community.

Abstract: Analysis of the natural abundance 14C content of dibutyl phthalate (DBP) from two edible brown algae, Undaria pinnatifida and Laminaria japonica, and a green alga, Ulva sp., revealed that the DBP was naturally produced. The natural abundance 14C content of di-(2-ethylhexyl) phthalate (DEHP) obtained from the same algae was about 50-80% of the standard sample and the 14C content of the petrochemical (industrial) products of DBP and DEHP were below the detection limit.
Triadic Synchrony: Application of Multiple Wavelet Coherence to a Small Group Conversation  [PDF]

Abstract: By applying multiple wavelet coherence (MWC) to data from human body movements in triadic interaction, this study quantified triadic synchrony, rhythmic similarity among three interactants. Thirty-nine Japanese undergraduates were randomly assigned in a triad, and engaged in a brain-storming task. Triadic synchrony was quantified by calculating MWC to the time-series movement data collected by Kinect v2 sensor. The existence of synchrony was statistically tested by using a pseudo-synchrony paradigm. Results showed that the averaged value of MWC was higher in the experimental participant trio than in those of the pseudo trio in the frequency band of 0.5 - 1 Hz. The result supports the possible utility of applying multiple wavelet coherence to evaluate triadic synchrony in a small group interaction.
A Case of Liposarcoma With Peritonitis Due to Jejunal Perforation

Abstract: A 21-year-old man, who had been treated for congenital dilatation of the bile duct 13 years previously, presented with an acute abdomen. The physical examination suggested peritonitis, and an emergent laparotomy was performed. A perforation was foundin the jejunum approximately 100 cm distal to the ligament of Treitz, followed by resection of a 60-cm jejunal segment. No tumorous lesions were found during the operation, and the resected jejunal segment showed only focal myxomatous thickening of the serosa. Despite intensive therapy, he died of uncontrollable septic shock 2 days after the operation. Unexpectedly, however, histological examination revealed a liposarcoma, showing an unclassifiable histology. From the distribution of the lesion and the histological findings, it is thought that a primary lesion was somewhere else, covered by severe adhesions due to the previous operation, and that the tumor cells spreading from it could have caused the jejunal perforation through vascular involvement. Although extremely rare, liposarcomas in the abdomen can cause intestinal perforation. It is important for both clinicians andpathologists to carefully investigate the cause of an unusual clinical presentation such as intestinal perforation.
