Sleep Disordered Breathing, Fatigue, and Sleepiness in HIV-Infected and -Uninfected Men

Abstract: Study Objectives We investigated the association of HIV infection and highly active antiretroviral therapy (HAART) with sleep disordered breathing (SDB), fatigue, and sleepiness. Methods HIV-uninfected men (HIV?; n = 60), HIV-infected men using HAART (HIV+/HAART+; n = 58), and HIV-infected men not using HAART (HIV+/HAART?; n = 41) recruited from two sites of the Multicenter AIDS cohort study (MACS) underwent a nocturnal sleep study, anthropometric assessment, and questionnaires for fatigue and the Epworth Sleepiness Scale. The prevalence of SDB in HIV- men was compared to that in men matched from the Sleep Heart Health Study (SHHS). Results The prevalence of SDB was unexpectedly high in all groups: 86.7% for HIV?, 70.7% for HIV+/HAART+, and 73.2% for HIV+/HAART?, despite lower body-mass indices (BMI) in HIV+ groups. The higher prevalence in the HIV? men was significant in univariate analyses but not after adjustment for BMI and other variables. SDB was significantly more common in HIV? men in this study than those in SHHS, and was common in participants with BMIs <25 kg/m2. HIV+ men reported fatigue more frequently than HIV? men (25.5% vs. 6.7%; p = 0.003), but self-reported sleepiness did not differ among the three groups. Sleepiness, but not fatigue, was significantly associated with SDB. Conclusions SDB was highly prevalent in HIV? and HIV+ men, despite a normal or slightly elevated BMI. The high rate of SDB in men who have sex with men deserves further investigation. Sleepiness, but not fatigue, was related to the presence of SDB. Clinicians caring for HIV-infected patients should distinguish between fatigue and sleepiness when considering those at risk for SDB, especially in non-obese men.

Abstract: Giant cell tumor of the tendon sheath is the second most common tumor of the hand often referred to as xanthoma. Histologically these tumors are composed of multinucleated giant cells, polyhedral histiocytes, fibrosis and hemosiderin deposits. Marginal excision of giant cell tumor of the tendon sheath is the treatment of choice. We present a case of xanthoma of flexor pollicis longus tendon presented as a single enlarging mass in volar aspect of left thumb. After clinical diagnosis, work-up is done with ultrasound, FNAC and excision biopsy.

Abstract:

We report results from the investigation of the nonlinear refractive index and nonlinear absorption coefficient of {(1Z)-[4-(Dimethylamino)phenyl]methylene} 4-nitrobenzocarboxy hydrazone mono-hydrate (DMPM4NBCHM) solution using Z-scan technique with a continuous wave (CW) Argon ion laser. The results show that this type of organic material has a large nonlinear absorption and nonlinear refractive index at 488 nm and 514 nm. The origin of the nonlinear effects was discussed. We demonstrate that the light induced nonlinear refractive index variation, leads to limiting effect. The results indicated that DMPM4NBCHM could be promising candidates for application on nonlinear photonic devices and optical limiters.

Abstract: Tonsillectomy once was a most common ENT procedure, being referred in past as the “bread and butter of otorhinolaryngologists” though not so commonly performed these days. The usual complications of this procedure have been haemorrhage-primary/reactionary/secondary, infection, tonsillar bed structures injury, and even death. With the advent of newer technologies like lasers, microdebriders, coblators, harmonic scalpel, radiofrequency, newer complications also arise. This case report focuses on one such complication of laser assisted tonsillectomy, not much spoken of in books.

Abstract: BOD measurements in presence of cobalt, nickel, copper, zinc, silver and cadmium are reported using wastewater from dairy industry as a seed. Extent of inhibition in BOD is studied for variables like, concen-tration of metal ions (0.2 mM to 1.4 mM), pH (3 to 8) and temperature of incubation (15℃, 20℃, 25℃, 30℃ and 35℃). Results of BOD inhibition are supported by absorbance measurement (OD600) studies of microbial matter preserved in Luriya broth medium. OD measurement results are used to derive minimum inhibitory concentration, i.e., threshold concentration of each metal showing toxicity towards microbes. Sil-ver is found to be the most toxic element.

A poly-L β-hairpin bent stereochemically as a boat-shaped protein of mixed-L,D structure is scrutinized in basis of ordering as minimum of energy specific for its sequenceand solvent. The model suitable for the scrutiny is accomplished by design. A terminally-blocked oligoalanine is nucleated overDPro6-Gly7 and DPro6-LAsp7 dipeptide structures as a twelve-residue β-hairpin and bent stereochemically as a boat-shaped fold. Abstract: A buck-type single-phase AC to DC converter is proposed in this paper. The conventional semiconverter which uses thyristors at line frequency has a very poor power factor (PF). Therefore, the semiconverter is modified to a new converter using metal oxide semiconductor field effect transistors (MOSFETs). This new converter is operated at constant switching frequency and variable duty cycle, using a very simple control scheme in voltage follower mode to achieve almost unity PF and low total harmonic distortion (THD). The output voltage is kept constant irrespective of variaions in the line voltage. The converter is analyzed and simulated to check the validity. The line current drawn by the converter is almost sinusoidal and harmonics are within the class A and class D limits imposed by harmonic standard IEC 61000-3-2.
