Abstract: O tumor de Buschke-L wenstein (TBL) é uma variante rara de condiloma acuminatum que possui comportamento maligno pelo crescimento local, mas n o apresenta potencial metastático. Afeta primariamente a área genital e, menos frequentemente a regi o perianal. The Buschke-L wenstein tumor (TLB) is a rare variant of condyloma acuminatum that presents local malignant behavior but no metastatic potential. It affects primarily the genital area and less frequently the perianal region. Risk factors for its development include immunosuppressive therapy and HIV infection. This tumor has a high rate of malignant transformation and the recommended initial treatment is surgery. We present a case of perianal and anal TBL in a HIV patient whose histology revealed no malignant transformation and that was treated successfully with surgery alone. We review the current knowledge about this entity taking into account its natural history in individuals infected with HIV.

Clinics , 2011, DOI: 10.1590/S1807-59322011000300011
Abstract: objective: to assess pulmonary function and the prevalence of atopy in school-age children who were very low birth weight as infants and to compare those who had bronchopulmonary dysplasia to those who did not. method: we studied 85 (39 male and 46 female) at a mean age of 84 (range, 62 to 107) months who were very low birth weight infants. bronchopulmonary dysplasia was defined as oxygen dependency at 36 weeks gestational age. we excluded 8 patients (4 for cerebral palsy and 4 for no collaboration). detailed perinatal and clinical data were collected. lung function was evaluated using conventional spirometry. atopy (assessed by the allergy skin-prick test) was considered when at least one positive skin test occurred in a panel of the most common environmental allergens in the local region. comparisons between the bronchopulmonary dysplasia and no bronchopulmonary dysplasia groups were performed using the mann-whitney, x2 and fisher's exact tests. results: we compared the bronchopulmonary dysplasia (n = 13) and no bronchopulmonary dysplasia (n = 64) groups. atopy was observed in 4 (30.8%) of the bronchopulmonary dysplasia patients and in 17 (26.6%) of the no bronchopulmonary dysplasia patients (p = 0.742). two (15.4%) patients with bronchopulmonary dysplasia had a family history of atopy vs. 17 (26.6%) in the no bronchopulmonary dysplasia group (p = 0.5). lung function tests showed airway obstruction in 2 (15.4%) of the bronchopulmonary dysplasia patients and in 10 (15.6%) of the no bronchopulmonary dysplasia patients (p = 1.0). four (33.3%) of the bronchopulmonary dysplasia patients had small airway obstruction vs. 14 (22.2%) of the no bronchopulmonary dysplasia patients (p = 0.466). conclusion: our data showed no significant differences in lung function between bronchopulmonary dysplasia and no bronchopulmonary dysplasia patients at school age and no evidence of an association between atopy and bronchopulmonary dysplasia.

Abstract: introduction: pleural effusions can complicate pneumonias in children and adolescents and are usually associated with a long hospital stay and increased morbidity. aims: to characterise a population of patients with parapneumonic pleural effusion and to establish possible prognostic factors on admission based on clinical, imaging and analytical data. to correlate treatment options with the outcome. methods: case review of patients under 18 years old with parapneumonic pleural effusion, admitted between july 1997 - june 2004 (7 years). results: 118 patients were included, 60% male, with mean age 7 years. the incidence of pleural effusion increased throughout the period of the study. the admissions occurred predominantly in autumn and winter. on admission 60% of patients had respiratory distress and 39% chest pain. in 40% loculations were found on admission and were associated with longer hospital stay, longer course of antibiotic therapy and more frequent need for surgery. thoracentesis was performed in 72% of patients (mean ph pleural fluid 7.24). the aetiologic agent was identified in 17% of cases: streptococcus pneumoniae (five), staphylococcus aureus (four) and streptococcus pyogenes (four). in our study, 52% of patients underwent pleural drainage and 18% surgery. median length of hospital stay was 15 days with mean 16.4 days (2 - 51). discussion: factors associated with worse prognosis were respiratory distress, loculations, empyema, low ph in pleural fluid, glucose or proteins in pleural fluid, high lactic dehydrogenase level in pleural fluid and high serum c-reactive protein. pleural drainage and/or surgery can shorten hospital stay and improve outcome. conclusion: complicated parapneumonic pleural effusions are managed successfully in centres with experience in the different types of procedure that might be necessary.

Abstract: p63 is a close relative of the p53 tumor suppressor and transcription factor that modulates cell fate. The full-length isoform of p63, containing a transactivation (TA) domain (TAp63) is an essential proapoptotic protein in neural development. The role of p63 in epithelial development is also well established; however, its precise function during neural differentiation remains largely controversial. Recently, it has been demonstrated that several conserved elements of apoptosis are also integral components of cellular differentiation; p53 directly interacts with key regulators of neurogenesis. The aim of this study was to evaluate the role of p63 during mouse neural stem cell (NSC) differentiation and test whether the histone H3 lysine 27-specific demethylase JMJD3 interacts with p63 to redirect NSCs to neurogenesis. Our results showed that JMJD3 and TAp63γ are coordinately regulated to establish neural-specific gene expression programs in NSCs undergoing differentiation. JMJD3 overexpression increased TAp63γ levels in a demethylase activity-dependent manner. Importantly, overexpression of TAp63γ increased β-III tubulin whereas downregulation of TAp63γ by specific p63 siRNA decreased β-III tubulin. Immunoprecipitation assays demonstrated direct interaction between TAp63γ and JMJD3, and modulation of TAp63γ methylation status by JMJD3-demethylase activity. Importantly, the demethylase activity of JMJD3 influenced TAp63γ protein stabilization and cellular distribution, as well as TAp63γ-regulated neurogenesis. These findings clarify the role of p63 in adult neural progenitor cells and reveal TAp63γ as a direct target for JMJD3-mediated neuronal commitment.
A Matrix Approach to the Socioeconomic Activity of a Country  [PDF]
Susana Santos
Theoretical Economics Letters (TEL) , 2018, DOI: 10.4236/tel.2018.85075
Abstract: A Social Accounting Matrix (SAM) is presented as a tool to study the socioeconomic activity of a country. This activity involves the monetary or nominal flows that are measured by the National Accounts, as well as production (organized in factors, industries and goods and services) and institutions (organized in households, general government, non-financial and financial corporations, non-profit institutions serving households, and rest of the world). In order to contribute to the definition of a methodology that can improve the knowledge of the different aspects of this activity, the potentialities of a SAM for its reading and interpreting are explored, as well as for carrying out experiments regarding its functioning. Through a SAM-based approach, how to construct more or less complex networks of linkages of the above mentioned flows is shown, from which structural features can be evidenced and the associated multiplier effects studied. Following an application to Portugal, it is shown that a numerical version of a SAM, enables an empirical description of the origin, use, and distribution of income, whereas, an algebraic version of a SAM allows one to carry out, for example, a deeper study of the multiplier effects associated with the institutional distribution of income. The crucial role of the factors of production accounts is identified in this study, namely when they establish the link between the generation and the distribution and use of income. In this process, the important role the complementary details that the Input-Output Matrix (IOM) can add is also identified. Thus, being the generation of income, the result of the output of goods and services and the associated costs, on the one hand, an industry by industry IOM can add details regarding domestic and imported intermediate consumption by and between industries and, on the other hand, a product by product IOM can add details regarding the domestic and imported intermediate consumption of goods and services.
