Health Care Discrimination in HIV Care  [PDF]

Abstract: Human Immunodeficiency Virus (HIV) infected population is experiencing enormous amount of social discrimination and stigmatization compared to other patients with any other chronic illness. Healthcare setup is not an exception where the HIV infected patients are shuttled from one place to another to get their basic services compared to HIV negative patients. This referral game of manipulation imparts additional stress to the already stressed HIV infected population. The physical and psychological impacts caused by other chronic conditions will be supplemented by social impact in the HIV infected population. This referral game in healthcare can cause the HIV infected to avoid their health seeking behavior and it may bring them back to their high risk activities, which can result in higher mortality/morbidity and failure in prevention and intervention strategies.
Solving Level Scheduling in Mixed Model Assembly Line by Simulated Annealing Method  [PDF]

Abstract: This paper presents an application of the simulated annealing algorithm to solve level schedules in mixed model assembly line. Solving production sequences with both number of setups and material usage rates to the minimum rate will optimize the level schedule. Miltenburg algorithm (1989) is first used to get seed sequence to optimize further. For this the utility time of the line and setup time requirement on each station is considered. This seed sequence is optimized by simulated annealing. This investigation helps to understand the importance of utility in the assembly line. Up to 15 product sequences are taken and constructed by using randomizing method and find the objective function value for this. For a sequence optimization, a meta-heuristic seems much more promising to guide the search into feasible regions of the solution space. Simulated annealing is a stochastic local search meta-heuristic, which bases the acceptance of a modified neighboring solution on a probabilistic scheme inspired by thermal processes for obtaining low-energy states in heat baths. Experimental results show that the simulated annealing approach is favorable and competitive compared to Miltenburg’s constructive algorithm for the problems set considered. It is proposed to found 16,985 solutions, the time taken for computation is 23.47 to 130.35, and the simulated annealing improves 49.33% than Miltenberg.
Psychosocial Profile of HIV Patients on HAART in Southern India  [PDF]

Abstract: BACKGROUND: Mortality of HIV/AIDS patients comes down in India after the National Aids Control Organization started providing free Antiretroviral Therapy (ART) since 2004. Madurai ART Center started providing free ART since 2004. Psychosocial profile of this population explored in this study. METHODS: A cross sectional interview conducted during the year 2008 with semi structured questionnaire on HIV positive clients who are on Highly Active Anti-Retro- viral Therapy (HAART) for more than 6 months. Convenience sampling done and totally 433 clients were interviewed with informed consent. RESULTS: Three fourth (73.4%) of the clients know about ART centre through Government Hospitals or Primary Health Centre. Most of the Clients (98.6%) on ART are feels back to normal life or back to near normal life. Pill burden/ Pill fatigue perceived by clients is 4.8%.Discrimination perceived was 8.8% and mostly in their own house or by neighbors, but most of their children (99.2%) were not discriminated as per the clients. Spouse positive rate for the married clients was 61.5% and unmarried in this population was 4.6%. Major or minor psychological problems in this group were 39.7% and attempted suicide was 8.8%. Drug abuse (alcohol and smoking) while started on ART was 40.4% and the current abusers are 7.6%. CONCLUSION: This study supports effectiveness of free ART program and effective case referral from government institutions. High percentage of major or minor psychological problem warrants further exploratory study and may need psychological interventions in this population.
Biochemical studies on the effect of Terminalia chebula on the levels of glycoproteins in streptozotocin-induced experimental diabetes in rats

Abstract: This study was designed to evaluate the effect of Terminalia chebula fruit extract on the levels ofplasma and tissue glycoprotein components in streptozotocin-induced-diabetic rats. Oral administrationof T. chebula fruit extract at a concentration of 200 mg/kg body weight for 30 days significantlyreduced the levels of blood glucose, glycosylated hemoglobin, urea, and creatinine as well as fucose,hexose, hexosamine and sialic acid in the diabetic rats treated with the fruit extract. The observeddecrease in the levels of plasma insulin and C-peptide in the diabetic rats was elevated to near normalby T. chebula fruit extract treatment. Histological observations made on the pancreatic tissue of controland experimental groups also revealed the beneficial effect of T. chebula fruit extract. The efficacy ofthe fruit extract was comparable with glibenclamide, a known hypoglycaemic drug.

Abstract: Introduction: Antineoplastic drug, Cyclophosphamide (CP), is a widely used drug that causes toxicity through its metabolites, phosphoramide mustard and acrolein. Squalene (SQ), an intermediate in the cholesterol metabolism has antioxidant and membrane stabilizing property. In the present study, the protective role of SQ towards the tissue defense system of the liver and kidney in the toxicity induced by CP was assessed. Material and methods: Normal Wistar albino rats were administered CP in a dose of 150 mg/kg b.wt., i.p., twice, for 2 consecutive days to induce toxicity. SQ, in a dose of 0.4 ml/day/rat p.o. was used to treat the toxicity induced by CP. Results: Significantly decreased activities of enzymic antioxidants [superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx), glutathione-s-transferase (GST) and glutathione reductase (GR)], decreased levels of reduced glutathione and increased levels of thiobarbutric acid reactive substance (TBARS) were observed. These pathological alterations were significantly normalized during the treatment of SQ. Conclusions: CP toxicity increased the free radical levels in the tissues and affected the activities of the enzymic antioxidants. Increased levels of TBARS [a measure of lipid peroxidation (LPO)] and decreased levels of GSH (due to utilization for detoxification process) evidenced the damage to these tissues. Protection exerted by SQ could be due to free radical quenching, providing additional alkylation site to CP metabolites and by inducing enzymic antioxidant production in these tissues. In conclusions improved antioxidant defense system in the liver and kidney of the experimental rats confirms the protective role of SQ against CP induced toxicities.
Association between primary hypothyroidism and metabolic syndrome and the role of C reactive protein: a cross–sectional study from South India

Abstract: A cross – sectional study from a tertiary care teaching hospital in Chennai city, South India. 420 patients with metabolic syndrome (NCEP – ATP III criteria) were included in the study group. 406 appropriately age and sex matched controls having no features of metabolic syndrome (0 out of the 5 criteria) were compared with the study group. The study extended over a 5 year period. TSH, FT4 were measured for both the groups using electrochemiluminescence immuno assay. HsCRP was measured for all the patients in the study group. The baseline characteristics between the groups were compared with Student's't' test. Chi-square test was used to analyze the association between metabolic syndrome and hypothyroidism (overt and sub-clinical). Logistic regression analysis was applied to identify the association between hypothyroidism and the patient characteristics in the study group.Of the 420 patients in the study group, 240 were females (57.1%), 180 were males (42.9%) with mean age 51 ± 9.4 years. Of the 406 patients in the control group, 216 were females (53.2%), 190 males (46.8%) with mean age 49 ± 11.2 years. In the study group, 92 had sub-clinical hypothyroidism (SCH) (21.9%), 31 were overtly hypothyroid (7.4%) and 297 were euthyroid (70.7%). In the control group 27 patients had sub-clinical hypothyroidism (6.6%), 9 patients had overt hypothyroidism (2.2%) and 370 patients were euthyroid (91.2%). On comparison SCH (P < 0.001) and overt hypothyroidism (P < 0.001) were significantly associated with the study group as compared to the control group. Logistic regression analysis recognized the association between female gender (P = 0.021) and HsCRP (P = 0.014) with sub-clinical hypothyroidism and female gender (P = 0.01) with overt hypothyroidism in the study group.Hypothyroidism is associated with metabolic syndrome and females are more at risk. Abstract: This paper presents a survey of single machine scheduling problem with uniform parallel machines. The single machine scheduling problem with uniform parallel machines consists of n jobs, each with single operation, which are to be scheduled on m parallel machines with different speeds. These parallel machines are also called proportional machines or related machines. There are several measures of performance which are to be optimized in uniform parallel machines scheduling. Since, this scheduling problem is a combinatorial problem; usage of a heuristic is inevitable to obtain solution in polynomial time. This paper gives a classification of the literatures of this scheduling problem in three major categories, viz. offline scheduling, online scheduling and miscellaneous scheduling. In total, the available literatures are classified into 17 subgroups. Under each of the first two categories, the available literatures are discussed under different groups based on different measures of performance and non-preemptive/preemptive nature of the jobs. In the last category, the literatures are discussed under three subgroups, namely non-preemptive jobs, preemptive jobs and periodic jobs.

Abstract: A mobile ad hoc network (MANET) is composed of mobile nodes, which do not have any fixed wired communication infrastructure. This paper proposes a protocol called “Delay, Jitter, Bandwidth, Cost, Power and Hop count Constraints Routing Protocol with Mobility Prediction for Mobile Ad hoc Network using Self Healing and Optimizing Routing Technique (QPHMP-SHORT)”. It is a multiple constraints routing protocol with self healing technique for route discovery to select a best routing path among multiple paths between a source and a destination as to increase packet delivery ratio, reliability and efficiency of mobile communication. QPHMP-SHORT considers the cost incurred in channel acquisition and the incremental cost proportional to the size of the packet. It collects the residual battery power of each node for each path; selects multiple paths, which have nodes with good battery power for transmission to satisfy the power constraint. QPHMP-SHORT uses Self-Healing and Optimizing Routing Technique (SHORT) to select a shortest best path among multiple selected paths by applying hops count constraint. It also uses the mobility prediction formula to find the stability of a link between two nodes.

In the first phase, a seed generation algorithm is given. Then, it is followed by three variations of the simulated annealing algorithms and their comparison using ANOVA in terms of their solutions on makespan.
GA Based Heuristic to Minimize Makespan in Single Machine Scheduling Problem with Uniform Parallel Machines  [PDF]

Abstract: This paper considers the single machine scheduling problem with uniform parallel machines in which the objective is to minimize the makespan. Four different GA based heuristics are designed by taking different combinations of crossover methods, viz. single point crossover method and two point crossover method, and job allocation methods while generating initial population, viz. equal number of jobs allocation to machines and proportionate number of jobs allocation to machines based on machine speeds. A detailed experiment has been conducted by assuming three factors, viz. Problem size, crossover method and job allocation method on 135 problem sizes each with two replications generated randomly. Finally, it is suggested to use the GA based heuristic with single point crossover method, in which the proportionate number of jobs allocated to machines based on machine speeds.
