Urethral reconstruction in severe hypospadias using buccal mucosa graft and penile skin flap

Abstract: Introduction. Hypospadias represents the most frequent penile anomaly. The most challenging part of hypospadias surgery is urethral reconstruction. Many various tissues are used (local skin flaps, bladder mucosa grafts, buccal mucosa grafts etc.) for the reconstruction of the neourethra. Objective. Our aim was to evaluate advantages and disadvantages of combined buccal mucosa graft and penile skin flap in urethral reconstruction in severe hypospadias repair. Methods. Between December 2005 and August 2009, 48 patients with severe hypospadias, aged from nine months to 12 years (mean age 23 months) underwent surgery. Thirty-four (71%) had penoscrotal and 14 (29%) scrotal hypospadias. Ventral penile curvature was present in all cases, and was corrected by incision of the short urethral plate. Urethroplasty was performed using a buccal mucosa graft combined with dorsal penile skin flap. Results. Mean follow-up was 22 months (range from 9 months to 3 years). Satisfactory functional and aesthetic results were achieved in 40 (83%) patients. Fistula was noted in six cases and was solved by minor revision. Urethral stenosis in two cases was resolved by simple dilatation. Conclusion. Urethral reconstruction using the buccal mucosa graft and dorsal penile skin flap in severe hypospadias repair could be the method of choice. Using this technique, single stage urethral reconstruction is possible with satisfactory esthetical and functional results.
Incidence of penile curvature in various forms of hypospadias

Abstract: Introduction. Hypospadias is a congenital anomaly of the penis, characterised by ectopically positioned urethral meatus and associated anomalies (cryptorchidism, inguinal hernia, penile curvature). Proximal forms of hypospadias, as severe cases, are particularly accompanied by penile curvature (chordee). Distal types are considered to be mild degrees. Objective. To determine the incidence of congenital curvature within various forms of hypospadias in order to signify preoperative and intraoperative diagnosis of chordee as a part of hypospadias repair. Methods. The total of 454 patients with hypospadias were treated surgically in a five-year period (2001-2006.) at the University Children's Hospital of Belgrade. The patients were divided into two groups according to the surgeon who had treated them. Only the first group of patients was tested for chordee as a part of standard procedure and complete treatment. In both groups we analyzed the number of patients treated for penile curvature within various types of hypospadias. We also compared scores in the two groups using Fisher test and χ2-test. Results. Scanning retrospective, 104 cases (22.9%) of diagnosed and surgically corrected chordee were determined. In 31.6% of patients from the first group and 11.6% of patients from the second group we diagnosed and corrected some form of penile curvature was. Chordee was significantly more frequent in the first group, regarding hypospadias in general (p<0.01), as well as distal (p<0.05) and mid shaft forms (p<0.01). Conclusion. Penile curvature is not uncommon in hypospadias. In this study we report a significantly higher frequency as related to the patients in the second group who were not tested for curvature during hypospadias treatment. This is why standard techniques in hypospadias repair should definitely include the diagnosis and surgical correction of penile curvature.
Parallelizing Wang-Landau algorithm in the field: the micromagnetic ensemble

Abstract: It is shown in this work how the Wang-Landau algorithm can be parallelized through the concept of the micromagnetic ensemble, when the Hamiltonian contains both spin interaction and the external field terms, and thus energy-magnetization plane is used for characterizing the density of states. Within this framework random walk is performed on mutually independent micromagnetic lines, and can thus be paralellized on a computer grid, without need for shared memory among individual processes. This approach pushes forward significantly the size of the systems that may be addressed on current computer hardware (from currently reported $42\times 42$ to at least $256\times 256$ in the case of two dimensional systems), and should turn out important for diverse studies where field dependent behavior is essential.
Selection of the appropriate methodology for the deterministic seismic hazard assessment on the territory of the Republic of Serbia

Abstract: This paper presents a discussion regarding the most common approaches to the deterministic seismic hazard analysis, as well as their relation with the probabilistic hazard analysis. Different methodologies for estimation of the strong earthquake ground motion at a site of interest on the territory of the Republic of Serbia are also discussed. When generation of the synthetic ground motion time histories on the territory of the Republic of Serbia is concerned, a method developed by Trifunac and his associates is suggested having in mind that this approach uses only those input parameters that can be easily and accurately defined while at the same time being able to model all properties of strong earthquake ground motion that are presently known as well as to consider the probabilistic nature of earthquake occurrence.

Abstract: This paper presents a review of the approaches to the probabilistic seismic hazard analysis as well as discussion regarding the selection of the appropriate methodology of probabilistic seismic hazard analysis for the territory of the Republic of Serbia. Use of the deductive approach has been suggested, as well as that the new Serbian seismic hazard maps should be expressed through the values of the peak ground acceleration, having in mind that the new hazard maps for the Republic of Serbia should be compiled in compliance with the recommendations of the Eurocode 8.
Fast random number generation using 128 bit multimedia extension registers on Pentium class machines

Abstract: In this work it is shown how 128 bit SSE2 multimedia extension registers, present in Pentium IV class 32 bit processors, may be used to generate random numbers at several times greater speed then when regular general purpose registers are used. In particular, a 128 bit algorithm is presented for the Marsaglia MWC1616 generator from the DIEHARD battery of random number generator tests, and its performance is compared to that of the conventional approach.
X Chromosome Inactivation and Differentiation Occur Readily in ES Cells Doubly-Deficient for MacroH2A1 and MacroH2A2

Abstract: Macrohistones (mH2As) are unusual histone variants found exclusively in vertebrate chromatin. In mice, the H2afy gene encodes two splice variants, mH2A1.1 and mH2A1.2 and a second gene, H2afy2, encodes an additional mH2A2 protein. Both mH2A isoforms have been found enriched on the inactive X chromosome (Xi) in differentiated mammalian female cells, and are incorporated into the chromatin of developmentally-regulated genes. To investigate the functional significance of mH2A isoforms for X chromosome inactivation (XCI), we produced male and female embryonic stem cell (ESC) lines with stably-integrated shRNA constructs that simultaneously target both mH2A1 and mH2A2. Surprisingly, we find that female ESCs deficient for both mH2A1 and mH2A2 readily execute and maintain XCI upon differentiation. Furthermore, male and female mH2A-deficient ESCs proliferate normally under pluripotency culture conditions, and respond to several standard differentiation procedures efficiently. Our results show that XCI can readily proceed with substantially reduced total mH2A content.
Ethnographic museum exposition as a function of communication process: An example of the permanent exposition in the Ethnographic museum in Belgrade

Abstract: Research about the meanings of the permanent exposition in the Ethnographic museum in Belgrade, which communication-wise points to the individual cultural projects as a function of incorporating and establishing specific viewpoints into one's own cultural system. Within developments of museum logy science, which resulted as a recent museum exposition, can be seen the ways and byways of the pre-memocentric expographic model. Three floors where the exposition was placed indicate multi-layered ideationally-conceptual syndrome, which - based on specific and inadvertent messages - point to several segments in real and symbolic space for communication between exposition's author and consumers. As a function of cultural mediators, authors present objects which represent public communication space as opposed to the objects from the private communication space. Such approach points to the functional symbolic reality of two models - proclaimed mainstream as a function of wanted model and everyday functional model of conduct. That way semantic premise is underlined, which in each community separates communication channels into mythologized stereotyped and prejudiced semantic space and realistic existence.

Abstract: The wars and subsequent crisis in the former Yugoslavia have brought about a different, distressed value system to the populations in Serbia. One of its reflections is seen in an establishment of the so-called alternative healing systems. The contemporary, modern medicine holds that illnesses are caused by various psycho-somatic agents, therefore, I take contemporary healers to be alternative psychiatrists and therapists which balance and cure individual distresses. Crisis in societies are psycho-physical triggers that initiate structural disturbances in personalities of active and passive participants, and initiate a search for psycho-therapeutic methods which include transcendental. The processes of globalization and urbanization have helped clear up the fact that the official/established and alternative/traditional medicines have structural determination and corresponding status with the prevailing value system and religious affiliation of the population. Cultural-historic processes are often established in the alternative, and the opposite.

Abstract: The paper analyses mythical places in careers of two singers of new folk music, who marked the time periods after Tito and Milo evi . The analysis shows that the development and establishment of a certain value system in "newly created culture", could point out to important consequences to socio-cultural changes in periods after death (political or otherwise) of "leaders". The establishment of newly created culture into local community was facilitated by mixing of very different value systems.
