Avalia o perioperatória de pacientes em unidade de terapia intensiva Perioperative assessment of the patients in intensive care unit

Abstract: OBJETIVO: Avaliar as condi es pré-operatórias e o procedimento cirúrgico relacionando-os à morbidade e mortalidade de pacientes cirúrgicos em uma unidade de terapia intensiva geral de um hospital universitário. MéTODOS: Foram estudados os prontuários de pacientes submetidos a procedimentos cirúrgicos de médio e grande porte, admitidos na unidade de terapia intensiva geral. Foram analisados: dados demográficos, quadro clínico, registros de antecedentes pessoais e exames laboratoriais pré-operatórios e de admiss o na unidade de terapia intensiva, exames de imagem, relato operatório, boletim anestésico e antibioticoprofilaxia. Após a admiss o, as variáveis estudadas foram: tempo de interna o, tipo de suporte nutricional, utiliza o de tromboprofilaxia, necessidade de ventila o mecanica, descri o de complica es e mortalidade. RESULTADOS: Foram analisados 130 prontuários. A mortalidade foi 23,8% (31 pacientes); Apache II maior do que 40 foi observado em 57 pacientes submetidos à opera o de grande porte (64%); a classifica o ASA e" II foi observada em 16 pacientes que morreram (51,6%); o tempo de permanência na unidade de terapia intensiva variou de um a nove dias e foi observado em 70 pacientes submetidos à cirurgia de grande porte (78,5%); a utiliza o da ventila o mecanica por até cinco dias foi observada em 36 pacientes (27,7%); hipertens o arterial sistêmica foi observada em 47 pacientes (47,4%); a complica o mais frequente foi a sepse. CONCLUS O: a correta estratifica o do paciente cirúrgico determina sua alta precoce e menor exposi o a riscos aleatórios OBJECTIVE: To evaluate the preoperative condition and the surgical procedure of surgical patients in a general intensive care unit of a university hospital, relating them to morbidity and mortality. METHODS: We studied the medical records of patients undergoing medium and large surgical procedures, admitted to the general intensive care unit. We analyzed: demographic data, clinical records personal history and laboratory tests, both preoperatively and on admission to the intensive care unit, imaging, operative reports, anesthetic reports and antibiotic prophylaxis. After admission, the variables studied were: length of stay, type of nutritional support, use of thromboprophylaxis, mechanical ventilation, description of complications and mortality. Abstract: this work aims to describe the indications of a sample of 23 children (from 4 to 6 years old), from a settlement of mst, to an infantile education, that is in the planning phase, through drawings and explanations/descriptions. the data indicate: the interest for external environment and nature; suggestions of entertainment and fun elements and pedagogic activities or materials. it is necessary to emphasize the indication of building materials, mainly windows that are preoccupation justified by the precarious conditions of survival, because the families live for 8 years in cottages of canvas. the results indicate the children are able informants in regard to the educational reality they will attend to.

Abstract: this is a study of the hand's development - left or right depending on handedness - in 16 movements identified as prior to and basic for the reproduction of cursive handwriting. it is based on a sample of 256 male and female children, between the ages of 3 and 7, from lower or upper midd1e to upper socio-economic backgrounds. the children were divided into 8 age groups, an interval of 6 months between each. in each age group there was the same number of boys and girls and of each socio-economic group studied. results showed that 50% of the upper socio-econonúc groups were able to reproduce ali the movements against 40% of the other group. 4 movements were found as responsible for the difficulties of the children' s from both socioeconomics categories and these result allow to identify them as the most critical for these ages groups. the girls, as in an other pilot study, reproduce more frequently the movements than the boys, especially the girls of the upper socio-economic levei. several suggestions are made in given more attention to these aspects in children' s of the ages groups studied and in lower ages groups.

Abstract: a motor development in the reproduction of the 16 graphic components identified as prior to and basic for the cursive handwriting were analysed. 256 male and female children, among 3 and 7 years, from lower or upper middle to upper socio-economic backgrounds were studied. the children were divided into 8 groups, with intervals of 6 months between each one. in each group there was the same number of boys and girls and of each socio-economic group. results showed that the reproduction of the form of graphic components is possible since 3 years old, but precision and firmness of reproduction will depend on exercises in the use of graphic instruments, such as a pencil. suggestions are made to teachers to pay more attention to this skill, to better evaluate practice as a means of improving skills.

Objective: To evaluate the profile of the Brazilianvaginal ring user, reasons for choosing it, satisfaction, menstrual pattern, tolerability, efficacy, side effects and reasons for discontinuation. Methods: We reviewed the files of 148 users who attended the Hospital de Jacarepaguáand private clinics, between 2005 and 2011, in a total of 3107 cycles. Kaplan-Meier survival curves and regression models were used in statistical analysis. Results: The profile was: single, nulligravida, 30 years (mean) and with university education. The previous contraception used was mostly the oral hormonal contraception (75.7%). Women chose the ring for: practicality (32.4%), forgetting the pill (32.4%) and irregular vaginal bleeding (13.5%). The satisfaction degree was 80.5% (119). There were good cycle control and no pregnancies. Thirty-eight patients discontinued the use (25.7%). The main reasons were wish for pregnancy (6%), spotting (3.4%) and increased vaginal discharge (2.7%). At the end of the first year, use of the greatest reduction of continuity probability was observed (19.3%). Women with side effects were 68% less likely to be satisfied with the method than those without (p-value <0.0001). Conclusion: Nuvaring® is an excellent contraceptiveoption due to its sefficacy, cycle control, tolerability, practicality and safety.

Abstract: one of the outcomes of globalization has been a constant increase of the number of people living in the streets of metropolis such as s?o paulo: those are not only homeless, but also people who live in the streets and earn their living from it. we have surveyed the institutions working with adult homeless population in the central region of sao paulo. their goals and strategies were described, as well as how they associate with each other. our main hypothesis is that our reality is still formed by isolated actions, based on mere assistance policies that help to maintain the problem of homelessness and turn it into a chronicle situation. although, the participative methodology we chose has shown a small change from this model towards the idea of the emancipation of the subject. even so, the idea of a technical knowledge still looms large and keeps the voice of the population in the background. the constitution of a network based on inter-sectorial actions (according to the perspective of health promotion) indicates the possibility of building innovatory public policies.

Abstract: objective: among chronic diseases, end-stage kidney disease (esrd) is distinguished for its negative impact on quality of life (qol). this study intended to verify change of qol in esrd patients on hemodialysis during 12 months and identify factors associated with worsening or improvement of qol. methods: the sample comprised a cohort of 93 patients from a single hospital located in the state of ceará, brazil. the medical outcomes study questionnaire 36-item short form health survey (sf-36) was administered in 2004 and 2005 to measure qol in 8 dimensions and 2 components (physical and mental) scaled from 0 (worst) to 100 (best). the sample was stratified into three categories: no change, improvement and worsening of qol according to physical and mental components. results: there was positive mean variation (improvement) of the score due to the dimension role-emotional (36.0 ± 40.1 vs. 57.0 ± 45.7; p=0.004) and mental component (53.4 ± 22.2 vs. 61.6 ± 23.5; p=0.021). patients with a negative variation (worsening) due to mental component had been on hemodialysis for less time when compared to patients with no change (36.4 ± 32.9 vs. 74.0 ± 54.8 months; p=0.008). conclusions: in esrd patients a longer time on hemodialysis was associated with improvement in mental aspects of qol. psychological adaptation, which occurs in other chronic diseases, could be the explanation of this outcome.
Root canal filling using MTA in association with parendodontic surgery in the retreatment of an external apical root resorption – case report

Abstract: Introduction and objective: The treatment of external apical root resorption should be directed to eliminate the endodontic infection. In some cases, the clinical resolution by conventional endodontic treatment cannot be performed, due to instrumentation and root canal filling complexity. In these cases, alternative instrumentation and obturation techniques may be necessary,additionally to apical surgery. Case report: This case report shows an alternative to external apical root resorption retreatment, using obturation with MTA associated to parendodontic surgery. After the removal of obturation material and the root canal cleaning and shaping, calcium hydroxide therapy was performed during 45 days. After that the canal was filled with MTA, and in a posterior session apical curettage and apicoectomy were performed. After 34 months, the periapical healing could be radiographically noticed, with close contact between trabecular bone and MTA. Conclusion: The use of MTA as a root canal filling material associated with parendodontic surgery seemed to be an adequate alternative to external apical root resorptions.
