Benefits from Long-Term Treatment in Irritable Bowel Syndrome

Abstract: It is known that irritable bowel syndrome (IBS) is a chronic disease of cyclic nature characterized by recurrent symptoms. IBS patients should receive, as initial therapeutic approach a short course of treatment which, if effective, has the additional value of confirming the diagnosis. Long-term treatment should be reserved to diagnosed IBS patients with recurrent symptoms. Clinical trials with stabilized therapies and new active treatments showed an improvement of the symptoms over placebo that is often time-dependent but with high relapse rates (around 40%–50% when stopping treatment). Relapse is not always immediate after stopping treatment and the recent data from OBIS trial with otilonium bromide or with psychotherapy, showed that due to different chemico-physical characteristics of the drugs or the psychosomatic impact to the disease not all treatment gave the same relapsing rate if compared to placebo. Results of IBS clinical trials with different therapies tailored to the patient needs indicate that a cyclic treatment therapy is advisable to counteract the nature of the disease.
Effect of ricinoleic acid in acute and subchronic experimental models of inflammation

Abstract: Observational studies indicate that topical application of ricinoleic acid (RA), the main component of castor oil, exerts remarkable analgesic and anti-inflammatory effects. Pharmacological characterization has shown similarities between the effects of RA and those of capsaicin, suggesting a potential interaction of this drug on sensory neuropeptide-mediated neurogenic inflammation. The aim of this study was to assess RA anti-inflammatory activities in comparison with capsaicin in several models of acute and subchronic inflammation. The acute inflammation was induced by intradermal injection of carrageenan in the mouse or by histamine in the guinea-pig eyelid. In either experiment, the extent of the oedema thickness was measured. Subchronic oedema was induced by complete Freund's adjuvant injection in the ventral right paw of mice. Tissue substance P (SP) was measured in the carrageenan experiments by radioimmunoassay (RIA). It was found that the acute topical application of RA (0.9 mg/mouse) or capsaicin (0.09 mg/mouse) significantly increased the mouse paw oedema induced by carrageenan, while an 8-day repeated topical treatment with the same doses of both compounds resulted in a marked inhibition of carrageenaninduced paw oedema matched by a reduction in SP tissue levels. Similar effects were found against histamine-induced eyelid oedema in guinea-pigs after acute or repeated application of RA or capsaicin. RA and capsaicin given for 1–3 weeks reduced the established oedema induced by Freund's adjuvant, a subchronic model of inflammation, particularly if given by the intradermal route. Either in mouse paw or in guineapig eyelid, capsaicin but not RA by itself produced a slight hyperemia and activation of a behavioural response (e.g. scratching of the eyelids). On the basis of the present results, RA may be seen as a new capsaicin-like, non-pungent anti-inflammatory agent suitable for peripheral application.

Abstract: Angiotensin converting enzyme inhibitors (ACE-I) are known to affect the endothelial function decreasing the formation of the potent vasoconstrictor angiotensin II and increasing the bioavailability of the vasodilators bradykinin and nitric oxide. In order to test the potential differences among the class of ACE-I, we have evaluated their effects on endothelin-1 and nitric oxide availability, and on reactive oxygen species generation in human vascular endothelial cells (HUVECs) obtained from umbilical cord veins. HUVECs were cultured in the presence of either sulfhydryl-containing ACE-I (zofenoprilat or captopril) or nonsulfhydryl-containing ACE-I (enalaprilat or lisinopril). After 24 hours, all ACE-I reduced endothelin-1 secretion and increased nitric oxide production. However, zofenoprilat (?42% after 8 hours of incubation) was more effective (<.05) than enalaprilat (?25%), lisinopril (?21%), and captopril (?30%) in reducing endothelin-1 secretion. Abstract: The aim of this study is to investigate human left ventricular heart morphological changes in time among 17 healthy subjects. Preliminarily, 2 patients with volumetric overload due to aortic insufficiency were added to our analyses. We propose a special strategy to compare the shape, orientation and size of cardiac cycle’s morphological trajectories in time. We used 3D data obtained by Speckle Tracking Echocardiography in order to detect semi-automated and homologous landmarks clouds as proxies of left ventricular heart morphology. An extended Geometric Morphometrics toolkit in order to distinguish between intra- and inter-individual shape variations was used. Shape of trajectories with inter-individual variation were compared under the assumption that trajectories attributes, estimated at electrophysiologically homologous times are expressions of left ventricular heart function. We found that shape analysis as commonly applied in Geometric Morphometrics studies fails in identifying a proper morpho-space to compare the shape of morphological trajectories in time. To overcome this problem, we performed a special type of Riemannian Parallel Transport, called “linear shift”. Whereas the two patients with aortic insufficiency were not differentiated in the static shape analysis from the healthy subjects, they set apart significantly in the analyses of motion trajectory’s shape and orientation. We found that in healthy subjects, the variations due to inter-individual morphological differences were not related to shape and orientation of morphological trajectories. Principal Component Analysis showed that volumetric contraction, torsion and twist are differently distributed on different axes. Moreover, global shape change appeared to be more correlated with endocardial shape change than with the epicardial one. Finally, the total shape variation occurring among different subjects was significantly larger than that observable across properly defined morphological trajectories.

Abstract: this article presents results of a research on teacher university education in brazil in the 1930s. the investigation was carried out gathering information sources from that period, including documents produced by the institution under study, as well as from periodicals, legislation, newspapers, and educational literature, amongst others. from the documental corpus so gathered it was observed that in 1933, in the state of s?o paulo, the first higher education school for the formation of teachers created was the instituto de educa??o (institute of education), stemming from the educational reform of fernando de azevedo. in 1934 it was incorporated to the university of s?o paulo and, together with the school of philosophy, sciences and literature, was responsible for the higher education of teachers. at the institute of education of the university of s?o paulo was established the first generation of university teachers devoted to the education of teachers for the primary and secondary school, a project that had its roots in the intellectual debate that took place especially in the 1920s. although the institute constituted one of the answers to the demand for teacher professionalization and had been a model for other initiatives, it was closed down in 1938. political forces linked to the interventionist government of ademar de barros and to the catholic church put an end in 1938 to the first brazilian experience of higher education for teachers.

Abstract: The relationship between man and nature denotes many problems in Spinoza’s context, where necessity plays an essential role. This essay tries to define the status of the possibility and its relationship to the natural necessity. The man-nature relationship reveals itself as conflictual space, where the human being faces the possibility of death and looks for the possibility of survival. The political action and the constitution of community offer an answer to the uncertainty of human position. Community is not an artificial monster; it is rather the adequate place where the human right and the human power express themselves. Nature and culture are not dissociated, since the society affirms the natural potentia of the human being and that of the multitude.

Abstract: The World Anti-Doping Agency has assumed responsibility for international doping control from the International Olympic Committee. It has revised, reformed and now presented a new World Anti-Doping Code, which became effective in March 2004, before the Olympic Games Athena's 2004. . The World Anti-Doping Code contains the List of Prohibited Substances and Prohibited Methods. Two important differences from the previous list are that caffeine has been removed from the banned list, and that a new category of 'specified substances' which may produce inadvertent positive tests has been added.

Abstract: O presente estudo aborda a rela o entre o conceito de Sustentabilidade e o sucesso empresarial, seja em termos de resultados operacionais ou em termos de valor agregado junto aos seus stakeholders. Procura-se compreender como a Sustentabilidade pode ser transformada em uma forte vantagem competitiva e, consequentemente, geradora de mais lucros. Os resultados foram obtidos a partir da aplica o de um questionário a dez empresas portuguesas e de entrevistas com quadros exercendo fun es de rela es-públicas e com empresários. Por meio da análise de dados, constatou-se que quando os investimentos em actividades, ac es corporativas ou programas s o aliados às práticas de gest o sustentável e de boa comunica o, aumentam as hipóteses de sucesso empresarial. Este estudio explora la relación entre el concepto de la sostenibilidad y el éxito empresarial, ya sea en términos de resultados operativos o en términos de valor a adido a sus stakeholders. Se trata de comprender cómo la Sostenibilidad se puede transformar en una fuerte ventaja competitiva y consecuentemente asi generando más beneficios. Los resultados fueron obtenidos a partir de la aplicación de un cuestionario a diez empresas portuguesas y entrevistas con el personal que ejerce funciones de relaciones públicas y los empresarios. A través de análisis de datos, se encontró que cuando las inversiones en actividades, acciones corporativas o programas se han aliado a la práctica de gestión sostenible y la buena comunicación, las posibilidades de éxito empresarial aumenta.
