Transient Osteoporosis of the Hip During Pregnancy

Abstract: Transient Osteoporosis is a well organised form ofregional osteoporosis, though rare. It affects the activitiesof daily living by its extended course of the disease anddisproportionate pain of the affected joint, even though itresolves spontaneously with a benign course. If notrecognised, the presentation can be alarming both clinicallyand radiologically1. Curtis and Kincaid2 introduced theterm “transient demineralisation of the hip” referring to arapidly developing, painful local osteoporosis of limitednature but of unexplained patho-physiology. Later on,Lequesne3 coined the term, transient osteoporosis of thehip in 1968.Transient osteoporosis can occur at any age, and in eithersex, but most commonly occurs in women during the thirdtrimester of pregnancy and in middle-aged men1. Hip isthe most commonly affected joint, though migratorypattern may affect the contra-lateral hip joint or the ankleand foot joints. Higher incidence of transient osteoporosishas been reported in patients with osteogenesis imperfect.Here, we report a case of transient osteoporosis affectingthe hip joint during the third trimester of pregnancy.

Abstract: The discourse between the Dutch Reformed Church (DRC) and race is a difficult and intense one. The aim of this article is to give a descriptive-empirical description of the relationship between the DRC and race by using the Church Mirror surveys. An altered social distance scale is used to measure church acceptance. In the discourse on race, acceptance and unity in the DRC with regard to racial prejudice and attitudes towards the “other” group have a stronger voice than theological or religious arguments. As a normative task in this discourse, at least two aspects can be pointed out: developing a framework for forgiveness and a confession on racism.
Open tibia fractures in HIV positive patients

Abstract: Open tibia fractures are common injuries, particularly in developing countries.Pedestrian or bicycle to motor car contact is the most common mechanism.These injuries result in high morbidity and often long-term disability. HIV infection complicates open fractures by raising the incidence of infectionin the open wound (5 of 7 patients in our series). This risk may be compounded if internal fixation techniques are used (5 of 12 HIV patients with internal fixation of any open fracture). There is also a suggestion that HIV may delay bone union (4 of 7 patients united at 6 months). External fixation offers an alternative method of fracture stabilisation. It avoids the risks associated with putting metal-ware in the wound, but creates a new issue of pin track sepsis. We found that pin track infection was more common in patients with HIV, but the rate at which pins required removal was 7%. We consider external fixation to be a lower-risk strategy than internal fixation in such patients but open fracture wound sepsis remains a problem. We have not yet demonstrated a difference in severity or frequency of complications in patients of low CD4 count, but logically one expects septic complications to increase as CD4 count falls. Antiretroviral medication decreases viral load and elevates the CD4 count. Research is underway regarding potential effectiveness of such drugs in reducing wound and fracture healing complications. Above all, meticulous and timely all-round care is required to achieve satisfactory results in immunecompromised patients. This includes, debridement, bony stability, and soft-tissue reconstruction.

Abstract: The publication in 2004 of twenty-six lines of verse by the ancient Greek poet Sappho solves several questions surrounding Fragment 58 Lobel-Page, based on a papyrus dating from the 3rd century A.D. and published in 1922. This papyrus preserved only the righthand side of a poem or poems. The new discovery, on the papyrus cartonnage of a mummy at the University of Cologne, dates from the 3rd century B.C., contains the oldest Sappho text to date and offers what is virtually the lefthand side of one poem and some lines of two other poems. From the integration of the two texts an entire new poem of 12 lines has been recovered. The two papyri were written about 500 years apart – an indication of the sustained interest in this great poet. The present article gives some of the background on the poetess and the new discovery, and offers an Afrikaans translation and analysis of the new poem. The poem deals with the physical and psychological effects of advancing age, and in its directness and realism recalls Sappho's Fragment 31 Lobel-Page. To enhance her theme, she uses the mythological example of the dawngoddess Eos who loved the mortal Tithonus and obtained immortality for him – but without eternal youth. So Tithonus grew ever older while Eos remained young. The example is meant to prove human mortality, and poignantly contrasts Sappho's situation with that of the young girls in her care.

The loss of the functional capacity of the cardiovascular system with space flight is over ten times faster than the course of aging on Earth. Mg is an antioxidant and calcium blocker and in space there is oxidative stress, insulin resistance, and inflammatory conditions with evidence in experimental animals of significant endothelial injuries and damage to mitochondria. The aging process is associated with progressive shortening of telomeres, repetitive DNA sequences, and proteins that cap and protect the ends of chromosomes. Telomerase can elongate pre-existing telomeres to maintain length and chromosome stability. Low telomerase triggers increased catecholamines while the sensitivity of telomere synthesis to Mg ions is primarily seen for the longer elongation products. Mg stabilizes DNA and promotes DNA replication and transcription, whereas low Mg might accelerate cellular senescence by reducing DNA stability, protein synthesis, and function of mitochondria. Telomerase, in binding to short DNAs, is Mg dependent. On Earth, in humans, a year might be required to detect changes in telomeres, but in space there is a predictably much shorter duration required for detection, which is therefore more reasonable in time and cost. Before and after a space mission, telomere lengths and telomerase enzyme activity can be determined and compared with age-matched control rats on Earth. The effect of Mg supplementation, both on maintaining telomere length and extending the life span, can be evaluated. Similar studies in astronauts would be fruitful.

Neurologic Complications of Critical Illness

Abstract: The book is arranged in two different ways, first into chapters of neurologic symptoms such as altered mental status, seizures or weakness. Later it is organized into a list of neurologic complications found in specific disease processes (e.g. vasculitis, acid–base derangements, acute renal or hepatic failure) and complications of pregnancy and following environmental injuries and procedures (e.g. aortic or cardiac surgery or organ transplantation).The strength of this book is its comprehensive nature and extensive referencing, which is more than one might expect from a single author. One is hard pressed to find a neurologic entity in the intensive care unit (ICU) that is not covered. Each chapter is very thorough, but one of the strongest chapters in the book is that devoted to complications following organ transplantation.The weakness of this book is the same as that for any textbook and relates to the publishing process itself, whereby information presented in a book rapidly becomes outdated. For example, the potential of hypothermia as a neuroprotectant is alluded to, but more recent data reported in 2002 provide evidence that the use of hypothermia may improve outcomes [1,2]. In addition, not included in the book is recent information showing that tight glucose control in the ICU decreases the incidence of critical care polyneuropathy [3]. Practical management tips could be more clearly stated. This is a pitfall in writing a text that focuses on recognition and a description of complications rather than one that also encompasses management. The weakest chapter is that devoted to multisystem trauma. For example, in patients with head injury, it cannot be overstated that hypertension and/or increasing agitation may be indicators of increasing intracranial pressure, which should be addressed before the symptoms or signs themselves are treated with antihypertensive medication or sedation. An intragenic distribution bias of DNA uptake sequences in Pasteurellaceae and Neisseriae

King Jordan.Gene repertoires are not stable features of prokaryotes [1], and it has been suggested that most genes have been affected by horizontal gene transfer at some point during their evolutionary history [2]. Environmental DNA not only serves as a potential nutrient for bacteria [3], but also represents an important substrate for the acquisition of new coding sequences via incorporation into the genome [4]. The ability to take up DNA and thereby acquiring new genes from the environment is termed natural competence, but this trait is not shared between all bacteria to the same extent. This implies that for some bacteria, or in selected environments, natural competence is a more favourable ability.In the Pasteurellaceae and Neisseriaea, natural competence has been linked to specific DNA motifs that are substantially overrepresented in the genome. The DNA Uptake Sequence (DUS) 5'-ATGCCGTCTGAA-3' in Neisseria species occurs almost 1,500 times in the genome [5,6], and represents approximately 2% of the genome. The Uptake Signal Sequence (USS) in most Pasteurellaceae is 5'-AAAGTGCGGT-3' [7,8], although a different motif is encountered in a subclade of this family [8]. If the conspecific DNA uptake sequence (or US, as I refer to them collectively) is present in the donor DNA, it increases the transformation efficiency many fold [9]. The exact mechanism how these motifs enhance DNA uptake remains unclear. It has been postulated that USs may be involved in some sort of transformation-barrier, as they selectively increase DNA uptake from a source containing the same US [10], even though this US can be shared by distantly related Pasteurellaceae species [11]. Alternatively, these genomically overrepresented motifs were suggested to be associated with the replication and repair mechanisms due to their genome distribution [12], but no general orientation bias has ever been observed for these sequences in the genome seque
