Postoperative Nausea and Vomiting Prophylaxis with Ondansetron in Diagnostic Gynecologic Laparoscopy: Preemptive versus Preventive Method  [PDF]

Abstract: Background and Objective: Post-operative nausea and vomiting (PONV) is a common adverse effect of the anesthesia in laparoscopic surgery. Ondansetron has been used for prevention and treatment of the PONV. The purpose of the present study was to compare the effects of preemptive and preventive intravenous ondansetron on PONV in patients undergoing diagnostic gynecologic laparoscopy. Materials & Methods: In a randomized double-blind clinical trial, 80 women candidate of diagnostic laparoscopy, were enrolled to study in two preemptive or preventive groups (n = 40). Ondansetron 4 mg IV was administered 5 min before anesthesia induction or 5 min before extubation in preemptive or preventive groups, respectively. The frequency and severity of the PONV were compared at post-anesthetic care unit (PACU), 3th, 6th and 24th postoperatively in two groups. Also the first time of need for the antiemetic drug was studied. Results: Demographic data were similar but duration of anesthesia was shorter in preventive group. The PONV rate was similar in two groups [(37.5% and 32.5% in preemptive and preventive groups, respectively (P = 0.815)]. In preemptive group it was more intense at PACU and 24 hours after surgery (P-value <0.05) and rate of vomiting was high (11 vs. 3, P-value 0.037). Abstract: Teaching grammar has been a complex issue for centuries. The effectiveness of deductive and inductive approaches, aiming at maximizing the students' opportunity to practice thinking skills, has been investigated in empirical studies. The aim of this study is to investigate whether various rule explication techniques should precede or follow a focus on the use of grammatical forms.The researcher framed this study around the following research questions: 1) Which instructional approach, deductive or guided inductive, will be more effective for EFL learners’ on short-term learning of grammatical structures? 2) Which instructional approach, deductive or guided inductive, will be more effective for EFL learners’ on long-term learning of grammatical structures? In order to investigate the afore-mentioned questions, the sample populations were 40 students. The grammar pretest was administered to participants at the beginning of the semester, prior to the treatment phase, in order to assess the comparability of grammar knowledge between the six sections. The grammar posttest was identical to the grammar pretest. At the end of the semester, 6 weeks after the pretest, the grammar posttest was administered to the participants to measure the long-term learning of the grammatical structures as well as the effectiveness of each presentational approach.To analyze the obtained data, a t-test computation was applied, in which the scores obtained for multiple-choice questions were the dependent measure, the data showed that:1. deductive approach is more effective for EFL learners’ on short-term learning of grammatical structures.2. deductive and guided inductive approach are similar for EFL learners’ on long-term learning of grammatical structures.3. deductive and guided inductive approach are similar for EFL learners’ on short-term learning of grammatical structures.
Equivalent Theory and Retrieval of Effective Metamaterials Parameters

Abstract: We apply the equivalent theory to orthorhombic anisotropic materials and provide a general unit-cell design criterion for achieving a length-independent retrieval of the effective material parameters from a single layer of unit cells. We introduce a graphical retrieval method and phase unwrapping techniques. The graphical method utilizes the linear regression technique. Our method can reduce the uncertainty of experimental measurements and the ambiguity of phase unwrapping. Moreover, the graphical method can simultaneously determine the bulk values of the six effective material parameters, permittivity and permeability tensors, from a single layer of unit cells.

Dissipation-Induced Super Scattering and Lasing PT-Spaser

Abstract: Giant transmission and reflection of a finite bandwidth are shown to occur at the same wavelength when the electromagnetic wave is incident on a periodic array of PT-symmetric dimers embedded in a metallic film. Remarkably, we found that this phenomenon vanishes if the metallic substrate is lossless while keeping other parameters unchanged. When the metafilm is adjusted to the vicinity of a spectral singularity, tuning substrate dissipation to a critical value can lead to supper scattering in stark contrast to what would be expected in conventional systems. The PT-synthetic plasmonic metafilm acts as a lasing PT-spaser, a planar source of coherent radiation. The metallic dissipation provides a mean to couple light out of the dark modes of the PT- spaser. Above a critical gain-loss coupling, the metafilm behaves as a meta-gain medium with the meta-gain atoms made from the PT-plasmonic dimers. This phenomenon implies that super radiation is possible from a cavity having gain elements by tuning the cavity dissipation to a critical value.
Omnidirectionally Bending to the Normal in epsilon-near-Zero Metamaterials

Abstract: Contrary to conventional wisdom that light bends away from the normal at the interface when it passes from high to low refractive index media, here we demonstrate an exotic phenomenon that the direction of electromagnetic power bends towards the normal when light is incident from arbitrary high refractive index medium to \epsilon-near-zero metamaterial. Moreover, the direction of the transmitted beam is close to the normal for all angles of incidence. In other words, the electromagnetic power coming from different directions in air or arbitrary high refractive index medium can be redirected to the direction almost parallel to the normal upon entering the \epsilon-near-zero metamaterial. This phenomenon is counterintuitive to the behavior described by conventional Snell's law and resulted from the interplay between \epsilon-near-zero and material loss. This property has potential applications in communications to increase acceptance angle and energy delivery without using optical lenses and mechanical gimbals.
