Cecal vanishing tumor associated with cytomegalovirus infection in an immunocompetent elderly adult

Abstract: Gastrointestinal involvement in cytomegalovirus (CMV) infection is well documented among immunocompromised patients and is also observed in immunocompetent individuals. The presentation of this infection can sometimes mimic those of other diseases, thus making accurate diagnosis difficult. We herein report a rare case of an immunocompetent elderly adult with gastrointestinal CMV infection that presented as a vanishing tumor at the cecum. A 76-year old man initially presented with lower abdominal pain. Colonoscopy revealed a tumorous lesion with irregular ulceration observed at the ileocecal valve. Histological findings of a biopsy specimen revealed intranuclear inclusions which were positive for CMV on immunohistochemical staining. However, this tumorous lesion disappeared within 7 wk from the initial hospital visit without any further treatment. Inflammatory pseudotumors associated with CMV infection should be considered as a differential diagnosis of tumorous lesions in the colon, even in immunocompetent adults.
Health Survey of Numbness/Pain and Its Associated Factors in Kotohira, Japan

Abstract: We conducted a survey of adults in Kotohira, a town of about 10,000 people located in the Nakatado District of Kagawa Prefecture, Japan. The survey was distributed to 8184 individuals, and effective responses were received from 3863 persons (response rate, 47.2%) during the survey period. Results regarding numbness and pain showed numbness alone in 7.7%, pain alone in 7.2%, both numbness and pain in 6.0%, and neither numbness nor pain in 79.6%. Spine and spinal cord damage was reported present by 5.4%, and absent by 94.6%. Analysis using the Short-Form Health Survey questionnaire, with comparison between subjects reporting both numbness and pain in the extremities and subjects with either numbness or pain alone, showed lower scores for in Short-Form Health Survey subscales (physical functioning, role [physical, emotional], bodily pain, vitality, and mental health). Subjects with numbness alone generally reported no disability in daily life. In a secondary survey, analysis of neurological findings by specialists identified 6 cases of “pain following spinal cord damage” in which spinal cord-related pain developed in the hands or feet. This represented 0.15% of the survey population starting from the primary survey.
Exchange Rate and Current Account Dynamics with Habits over Consumption and Money Holdings  [PDF]

Incorporating two independent habits over consumption and money holdings into a small country model, we examine the adjustment dynamics of the current account and the exchange rate to expansionary monetary and fiscal shocks under two alternative policy regimes: (1) the endogenous income transfer regime; and (2) the endogenous fiscal spending regime. In response to the shocks under regime (1), the exchange rate depreciates on impact and in the long run whereas it appreciates (depreciates) in transition if preferences for real money balances exhibit distant (adjacent) complementarity. Under regime (2), the consumption habits and the monetary habits jointly generate possibly non-monotonic current account dynamics. An induced increase in fiscal spending in regime (2) can generate a current account surplus in the case where the monetary habits exhibit strong distant complementarity.

C/EBPβ Promotes Transition from Proliferation to Hypertrophic Differentiation of Chondrocytes through Transactivation of p57Kip2

Abstract: Background Although transition from proliferation to hypertrophic differentiation of chondrocytes is a crucial step for endochondral ossification in physiological skeletal growth and pathological disorders like osteoarthritis, the underlying mechanism remains an enigma. This study investigated the role of the transcription factor CCAAT/enhancer-binding protein β (C/EBPβ) in chondrocytes during endochondral ossification. Methodology/Principal Findings Mouse embryos with homozygous deficiency in C/EBPβ (C/EBPβ?/?) exhibited dwarfism with elongated proliferative zone and delayed chondrocyte hypertrophy in the growth plate cartilage. In the cultures of primary C/EBPβ?/? chondrocytes, cell proliferation was enhanced while hypertrophic differentiation was suppressed. Contrarily, retroviral overexpression of C/EBPβ in chondrocytes suppressed the proliferation and enhanced the hypertrophy, suggesting the cell cycle arrest by C/EBPβ. In fact, a DNA cell cycle histogram revealed that the C/EBPβ overexpression caused accumulation of cells in the G0/G1 fraction. Among cell cycle factors, microarray and real-time RT-PCR analyses have identified the cyclin-dependent kinase inhibitor p57Kip2 as the transcriptional target of C/EBPβ. p57Kip2 was co-localized with C/EBPβ in late proliferative and pre-hypertrophic chondrocytes of the mouse growth plate, which was decreased by the C/EBPβ deficiency. Luciferase-reporter and electrophoretic mobility shift assays identified the core responsive element of C/EBPβ in the p57Kip2 promoter between ?150 and ?130 bp region containing a putative C/EBP motif. The knockdown of p57Kip2 by the siRNA inhibited the C/EBPβ-induced chondrocyte hypertrophy. Finally, when we created the experimental osteoarthritis model by inducing instability in the knee joints of adult mice of wild-type and C/EBPβ+/? littermates, the C/EBPβ insufficiency caused resistance to joint cartilage destruction. Conclusions/Significance C/EBPβ transactivates p57Kip2 to promote transition from proliferation to hypertrophic differentiation of chondrocytes during endochondral ossification, suggesting that the C/EBPβ-p57Kip2 signal would be a therapeutic target of skeletal disorders like growth retardation and osteoarthritis.
Relationship among coronary plaque compliance, coronary risk factors and tissue characteristics evaluated by integrated backscatter intravascular ultrasound

Abstract: Non-targeted plaques with moderate stenosis (plaque burden at the minimal lumen site: 50-70%) located proximal to the site of the percutaneous coronary intervention target lesions were evaluated by IB-IVUS. Thirty-six plaques (less calcified group: an arc of calcification ≤10°) in 36 patients and 22 plaques (moderately calcified group: 10°?<?an arc of calcification ≤60°) in 22 patients were evaluated. External elastic membrane volume (EEMV) compliance, lumen volume (LV) compliance, plaque volume (PV) response (difference between PV in systole and diastole), EEM area stiffness index were measured at the minimal lumen site. Relative lipid volume (lipid volume/internal elastic membrane volume) was calculated by IB-IVUS.In the less calcified group, there was a significant correlation between EEMV compliance and the relative lipid volume (r?=?0.456, p?=?0.005). There was a significant inverse correlation between EEM area stiffness index and the relative lipid volume (p?=?0.032, r?=??0.358). The LV compliance and EEM area stiffness index were significantly different in the diabetes mellitus (DM) group than in the non-DM group (1.32?±?1.49 vs. 2.47?±?1.79%/10 mmHg, p =0.014 and 28.3?±?26.0 vs. 15.7?±?17.2, p =0.020). The EEMV compliance and EEM area stiffness index were significantly different in the hypertension (HTN) group than in the non-HTN group (0.77?±?0.68 vs. 1.57?±?0.95%/10 mmHg, p =0.012 and 26.5?±?24.3 vs. 13.0?±?16.7, p =0.020). These relationships were not seen in the moderately calcified group.The present study provided new findings that there was a significant correlation between mechanical properties and tissue characteristics of coronary arteries. Moreover, our result can be applied to show the existence and uniqueness of solutions for fractional differential equations with multiple delays. A Method for the Detection of Decrease in Power in PV Systems Using Satellite Data  [PDF]

Abstract: In this paper, a method to detect a decrease in the output power of photovoltaic systems is proposed. This method is based on using satellite irradiance data. In addition, fault detection is carried out with only one day’s data in this method. Thus, the time elapses since the decrease in output is shorter than with the other methods. In order to mitigate the error in satellite data and improve the accuracy of fault detection, data extraction is carried out, which consists of two steps. In the first step, effective data are extracted by setting a lower irradiance limit. In the second step, “Calculation day” is determined depending on the number of effective data in one day. Fault detection, which is only conducted on the Calculation day, is conducted by comparing the expected power and the measured power. The parameters used in this study were optimized by testing 45 systems that appear normal. Subsequently, 340 systems were analyzed with the proposed method, using optimized parameters. The results showed the effectiveness of our method from the viewpoints of both accuracy and time required. In addition, three data extraction methods were considered to distinguish between the permanent decrease caused by failure, and the temporary decrease caused by partial shade. Fuzzy cluster analysis showed the best result among the three methods used.
Otolith features and growth of juvenile Opsaridium microcephalum (Pisces: Cyprinidae) from the southwestern shoreline of Lake Malawi

Abstract: Sanjika,Opsaridium microcephalum, were collected from two sites (Nkhotakota and Chia) along the southwestern shoreline of Lake Malawi. The sagitta of the otolith was arrowheadshaped with an obvious nucleus. Increments in the sagittae were observable until the bases of the rostra but invisible in the rostra. The asteriscus was oval-shaped with an ambiguous nucleus, leading to difficulty in identifying the first increment. The lapillus was ‘a’-shaped with an obvious nucleus and increments were observable from the nucleus to the margin. The lapillus was therefore considered the most suitable structure for interpreting daily growth increments in this species. The estimated hatching months of fish based on lapilli increment counts were from November to July, suggesting that this species has a long breeding period of at least eight months per year. The growth rate of fish born in the rainy season (0.74mmTL per day) was higher than those in the dry season (0.56mmTL per day). This growth difference was considered to be due to the difference in water temperature in the lake.
Postdiction: its implications on visual awareness, hindsight, and sense of agency

Abstract: There are a few postdictive perceptual phenomena known, in which a stimulus presented later seems causally to affect the percept of another stimulus presented earlier. While backward masking provides a classical example, the flash lag effect stimulates theorists with a variety of intriguing findings. The TMS-triggered scotoma together with “backward filling-in” of it offer a unique neuroscientific case. Findings suggest that various visual attributes are reorganized in a postdictive fashion to be consistent with each other, or to be consistent in a causality framework. In terms of the underlying mechanisms, four prototypical models have been considered: the “catch up,” the “reentry,” the “different pathway” and the “memory revision” models. By extending the list of postdictive phenomena to memory, sensory-motor and higher-level cognition, one may note that such a postdictive reconstruction may be a general principle of neural computation, ranging from milliseconds to months in a time scale, from local neuronal interactions to long-range connectivity, in the complex brain. The operational definition of the “postdictive phenomenon” can be applicable to such a wide range of sensory/cognitive effects across a wide range of time scale, even though the underlying neural mechanisms may vary across them. This has significant implications in interpreting “free will” and “sense of agency” in functional, psychophysical and neuroscientific terms.

Abstract: We investigate $O(N)$-symmetric vector field theories in the double scaling limit. Our model describes branched polymeric systems in $D$ dimensions, whose multicritical series interpolates between the Cayley tree and the ordinary random walk. We give explicit forms of residual divergences in the free energy, analogous to those observed in the strings in one dimension.
