Abstract: Amyloidosis is the result of fibrous and insoluble amyloid protein deposition in extracellular spaces of tissues and organs, including the skin. Through the analysis of medical records, clinical condition and laboratory tests, this article proposes a rare case report of an adult patient with symptoms and diagnosis of polymyositis, histopathology of nodular amyloidosis with skin lesions suggestive of lupus. The patient is still being followed at the dermatology clinic of Valença Medical School, in order to improve the prognosis and prevent the progression of existing symptoms.

Abstract: Introduction: Syphilis consists in a systemic infect contagious pathology with a chronic character. The etiological agent consists of an anaerobic spirochete bacterium, Treponema pallidum. Laboratory diagnosis can be made through a direct investigation of the etiologic agent or non-treponemal (VDRL) and treponemal (FTA-Abs) serological tests. False-negative results are rare. Objectives: The present report presents a rare case of a man with a secondary syphilis syndrome who was initially undiagnosed due to the prozone effect, since he presented VDRL in low titers, and was later confirmed with a new diluted serum sample and VDRL and FTA-Abs, which were positive. Conclusion: A close look should be given to patients who, although the clinical condition is compatible, complementary exams may not be, and clinical cases should be carefully evaluated so that the patient is not treated late.

Abstract: Introduction: The involvement of undergraduate student at the beginning of their training in activities based on Primary Health Care (PHC), provides preventive and health promoting actions, and ensures the evaluation of the vulnerability of communities and people. Objectives: To analyze the opinion of students and teachers of two medicine higher education institutions on the relevance of Primary Health Care in this course. METHODS: Students and teachers should be effectively enrolled in order for the study to be conducted, being chosen in a random manner and without interferences on the part of the researchers so that the work was as reliable as possible. A cross-sectional study was conducted with interviews structured in questionnaires about what they thought about PHC. To some students and teachers of the medical course of Valen?a and Grande Rio University (UNIGRANRIO). Before receiving the questionnaire, all interviewed signed a Free and Informed Consent Term, which included the objectives and methodology of the work. Results: 310 students and 51 teachers participated in the study, where 91.43% of the students and 100% of the teachers affirmed that primary care plays a fundamental role in medical education. 94.86% of students and 97.91% of teachers, claimed that the discipline allows a greater contact between undergraduates and community. When questioned about the encouragement that teachers give to students to pursue a career in PHC, 57.87% of the students and 70.83% of the professors said they lacked in such a stimulus. Conclusion: PHC is still very neglected by governmental policies, educational institutions and by the own teachers, that often, turn they attention only to tertiary medicine and, as a consequence, a discouragement of this area occur. Therefore, a higher PHC value is necessary, allowing a more humane look at the patient, valuing their feelings, anguishes and their pathological framework.
Achados histológicos na parede posterior da laringe em pacientes com refluxo gastroesofageano (GERD)

Abstract: introduction: the mechanisms of the otolaryngologic manifestations of gerd are most likely the result of intermittent nocturnal gastroesophageal-pharyngeal reflux and contact or upper airway structures with refluxate. the contact of the gastric juice with the pharynx and larynx, result in edema and inflammation, even with minimal acid exposition. although, histological findings in the posterior wall of the larynx in patients with gerd, may be a important diagnostic tool in these patients. the aim of this study is to describe the results of the biopsies of the posteiror wall of the larynx in patients with gerd. study design: prospective no randomized. material and method: we performed biopsies in the laryngeal posterior wall in seven patients complaints of cronical pharyngeal symptoms with gastroesophageal reflux disease (gerd). all the patients were investigated with digestive endoscopy and all had erosive esophagitis. laryngoscopy were performed looking for lesions sugestive of gerd. two groups were created: group i - normal laryngoscopy; and group ii - laryngoscopy with posterior laryngitis. results: the results showed that 42.85% of the patients presented with scamous epithelim in the laryngeal posterior wall, 42.85% presented areas of scamous metaplasia among the respiratory epithelium and 28.57% of the patients presented with respiratory epithelium. 71.4% of the patients presented with histologic alterations of the gerd that are found in the mucosal esophagus. conclusions: the results ot this study suggest that the gerd can be related to the epithelium changes in the laryngeal posterior wall and inflamatory histologic alterations in this region.

Abstract: Systemic lupus erythematosus (SLE) is an autoimmune disease that affects mainly women, presents several clinical manifestations and has no known exact cause. The skin is one of the organs that can be affected, with subacute cutaneous lupus erythematosus (SCLE) being one of the classifications according to cutaneous lupus erythematosus lesions. SCLE is commonly found in young to middle age women and carries characteristics related to sun exposure. In this report, there is a rare case of a male and elderly patient that presents SCLE subgroup initially asymptomatic, with subsequent appearance of macular skin lesions and vitiligo appearance. This is an important case to alert health professionals that the disease can manifest itself and open its clinical profile even in patients of sex and unusual ages.

Abstract: Henoch-Schönlein purpura is a vasculitis of small vessels, characterized by tissue deposition of IgA, affecting predominantly the skin, intestine, joints and kidneys. It mainly affects the pediatric age group, especially from 2 to 11 years old. Most often, it is preceded by episode of upper airway infection from days to weeks. The most common clinical manifestations are purpuric cutaneous lesions located in the lower limbs and buttocks, abdominal pain in colic, nausea, vomiting, diarrhea, in addition to arthralgia/arthritis, usually with spontaneous resolution. Renal impairment usually manifests with hematuria and proteinuria, and renal function may also be impaired. The case described refers to a female patient, 86 years old, with a previous history of infection of the upper airways, followed by purpura in the lower limbs and renal impairment: serum creatinine 3.1 mg/dL (N 0.5 - 1.2 mg/dL); glomerular filtration rate 15 ml/min (N > 60 ml/min); urea 118 mg/dL (N < 40 g/dL); proteinuria 4.497 mg/24h. The present report aims to draw attention to the occurrence of Henoch-Schönlein purpura in an atypical age group, with significant renal impairment, improvement only with conservative treatment, thus avoiding the use of immunosuppressants and the risks of immunosuppression and toxicity related to such drugs.
Efeito da Suplementa o de Lipídios na Ra o sobre a Produ o de ácido Linoléico Conjugado (CLA) e a Composi o da Gordura do Leite de Vacas

Abstract: O objetivo deste trabalho foi verificar o efeito de fontes de lipídios (soja integral moída ou óleo de soja) adicionados à dieta sobre o perfil de ácidos graxos da gordura do leite, especialmente sobre o aumento do CLA. Foram utilizadas 6 vacas multíparas 7/8 holandês-zebu, 30 dias após o parto, com peso vivo médio de 500 kg e produ o média de 20 quilos de leite ao dia. Os animais foram divididos em três grupos para recebimento das dietas (tratamentos), isoprotéicas e isoenergéticas, em que o tratamento controle continha 3% de extrato etéreo na ra o total e os demais 7%. Os tratamentos contendo fontes de lipídios, quando comparados com o controle, diminuíram o percentual de ácidos graxos de cadeia curta e diminuíram os teores dos ácidos butírico, capróico, caprílico, cáprico, láurico, mirístico e o percentual de ácidos graxos saturados. Os mesmos tenderam, ainda, a reduzir os teores de ácido palmítico e palmitoléico e tenderam a aumentar o percentual de ácidos graxos insaturados e de cadeia longa. Por outro lado, esses tratamentos aumentaram o teor de ácido esteárico e tenderam a aumentar os teores de ácido oléico e CLA. O óleo de soja, comparado ao gr o de soja moído, diminuiu os teores de ácido linoléico e linolênico e aumentou o CLA, tendeu a aumentar o teor de ácidos graxos n o identificados e a diminuir os ácidos graxos saturados. Concluindo, a inclus o de óleo de soja na dieta, e n o soja gr o, aumenta o teor de CLA na gordura do leite.

Foram utilizadas seis vacas multíparas 7/8 holandês-zebu, 30 dias após o parto, com peso vivo médio de 500 kg e produ o média de 20 litros de leite ao dia. Os animais foram divididos em três grupos para recebimento das dietas, isoprotéicas e isoenergéticas, em que o tratamento controle continha 3% de extrato etéreo na ra o total e os demais, 7%. N o foi verificado efeito de tratamento sobre os parametros avaliados. Os resultados das análises físico-químicas do leite est o dentro da faixa de valores recomendados pelo Regulamento de Inspe o Industrial e Sanitária de Produtos de Origem Animal.
Achados histológicos na parede posterior da laringe em pacientes com refluxo gastroesofageano (GERD)

Abstract: Introdu o: O mecanismo de manifesta es otorrinolaringológicas da doen a do refluxo gastroesofageano é provavelmente o resultado de refluxo faringogastroesofageano noturno intermitente. O contato da faringe e laringe com o suco gástrico resulta em inflama o e edema, mesmo com pouca ou mínima exposi o ácida. Porém, estudos histológicos na parede posterior da laringe, em pacientes com exposi o cr nica ao suco gástrico, n o têm sido realizados mas podem representar importante informa o diagnóstica. Este estudo tem como objetivo descrever biópsias de parede posterior de laringe em pacientes portadores de doen a do refluxo gastroesofageano. Forma de estudo: Prospectivo n o randomizado. Material e método: No período compreendido entre junho de 1998 a setembro de 1998, foram realizadas biópsias da parede posterior da laringe em sete pacientes portadores de doen a do refluxo gastroesofageano (GERD), que apresentavam sintomas cr nicos de faringe e laringe. Todos os pacientes realizaram investiga o do refluxo gastroesofageano através da endoscopia digestiva alta, e também laringoscopias diretas na procura de les es laringológicas sugestivas de etiologia relacionada à GERD,e foram classificados em grupo I e grupo II, respectivamente: grupo com laringoscopia normal e grupo com laringoscopia com les o. Resultados: Os resultados mostraram que 42,85% dos pacientes estudados apresentavam epitélio escamoso como revestimento da parede posterior da laringe; 42,85% apresentaram áreas de metaplasia escamosa em permeio ao epitélio pseudoestratificado respiratório; e apenas 28,57% dos pacientes apresentaram epitélio respiratório. Além do mais, 71,4% dos pacientes apresentaram altera es histológicas decorrentes da GERD que s o encontradas no es fago. Conclus es: Os resultados deste estudo sugerem que o refluxo gastroesofageano pode estar relacionado com a mudan a do epitélio da parede posterior da laringe, assim como estar também relacionado com altera es histológicas inflamatórias dessa regi o.

Abstract: caucasian male patient, 43 years old, presented two years ago with one single lesion in the right knee, in addition to desquamation in the plantar region. direct mycological exam of the plantar damage showed filaments of dermatophytes, but it was negative for the knee injury. however, histopathology of the area showed presence of foreign body granuloma with hyphae of dermatophytes, confirming the diagnosis of majocchi's granuloma.
