Imunofluorescência direta no diagnóstico de casos suspeitos de penfigóide cicatricial ocular

Abstract: OBJETIVOS: O objetivo deste estudo é caracterizar por meio do exame oftalmológico os pacientes com suspeita clínica de penfigóide cicatricial ocular e apresentar os resultados obtidos com a técnica de imunofluorescência direta. MéTODOS: Estudo prospectivo realizado no setor de Córnea e Doen as Externas da Universidade Federal de S o Paulo. Foram examinados 18 pacientes com suspeita clínica de penfigóide cicatricial ocular e realizadas biópsias de conjuntiva para o processamento da imunofluorescência direta em 13 pacientes (26 olhos), que n o estavam na vigência de imunossupress o sistêmica ou terapêutica antiglaucomatosa tópica. RESULTADOS: Segundo o estadiamento de Foster, a propor o de olhos classificados de I a IV foram: 3,8%, 3,8%, 77%, 15,4% respectivamente. Apenas 3 (23%) pacientes apresentaram imunofluorescência direta positiva. CONCLUS O: A maioria dos pacientes encontravam-se em estágio avan ado da doen a, o que denota retardo no diagnóstico. A imunofluorescência direta apresentou baixa sensibilidade em detectar depósitos de imunocomplexos na membrana basal da conjuntiva, quando comparada aos dados da literatura.
Uso de mitomicina C tópico no tratamento da neoplasia intra-epitelial córneo-conjuntival e carcinoma espinocelular conjuntival: resultados preliminares

Abstract: OBJETIVO: Avaliar a seguran a, a eficácia e a recidiva tumoral após o uso de mitomicina C (MMC) tópica no tratamento de neoplasias intra-epiteliais (NIC) primárias e recidivadas e carcinomas espinocelulares (CEC) recidivados. MéTODOS: Dois grupos de pacientes foram tratados. Grupo 1 com diagnóstico de NIC primário ou recidivado fez uso do colírio de MMC 0,02%, 4 vezes ao dia por 28 dias. O grupo 2 com diagnóstico de CEC recidivado fez uso do colírio de MMC 0,02%, 4 vezes ao dia por 21 a 28 dias. Após o término do tratamento era feita citologia exfoliativa para controle ou biópsia nos casos de les o residual. RESULTADOS: Oito pacientes com NIC e 1 com CEC mostraram regress o total da les o e citologia exfoliativa de controle negativa para células neoplásicas. Em 1 paciente com NIC e 2 com CEC, houve regress o parcial das les es, tendo sido necessária exérese cirúrgica da les o residual, com exame anatomopatológico negativo para neoplasia. Os efeitos colaterais foram transitórios e desapareceram após a suspens o do colírio. N o houve recidiva tumoral num seguimento médio de 24,9 meses. CONCLUS O: Os resultados preliminares desse estudo su-gerem que o uso da MMC tópica é op o segura e eficaz como tratamento único de NIC primária ou recidivada e no tratamento do CEC recidivado, porém n o evitando a cirurgia na maioria dos pacientes com CEC. N o foram observadas recidivas tumorais durante o segmento. Mais estudos s o necessários, com maior número de pacientes e maior tempo de seguimento para confirmar esses resultados.

Abstract: purpose: to report the findings in patients with clinical suspicion of ocular cicatricial pemphigoid (ocp) and to present the results of direct immunofluorescence investigation. methods: prospective study performed at the federal university of s?o paulo. clinical examination and conjunctival biopsies were performed in thirteen patients (26 eyes) who presented clinical evidence of ocp without systemic immunosuppression therapy or topic anti-glaucoma drug use. immunofluorescence was immediately performed after obtaining biopsies. results: according to foster's staging system, the proportions of eyes classified as stage i to iv were 3.8%, 3.8%, 77% and 15.4%, respectively. only three (3) of thirteen (13) patients (23%) were positive on performing immunofluorescence technique. conclusion: this study shows that the diagnoses of ocular cicatricial pemphigoid were established at advanced stages of the disease and that direct immunofluorescence presented low sensitivity in detecting immunoreactant deposition along the basement membrane of the conjunctiva, when compared to the literature.
Cytological features of live limbal tissue donor eyes for autograft or allograft limbal stem cell transplantation

Abstract: purpose: to evaluate by impression cytology (ic) the corneal surface of live limbal tissue donor eyes for autograft or allograft limbal stem cell transplantation (lsct). methods: twenty limbal donors were enrolled (17 for autograft lsct and 3 for allograft). impression cytology was performed before transplantation of superior and inferior limbal grafts and after the third postoperative month. results: impression cytology analysis showed sheets of corneal epithelial cells and goblet cell absence beyond the edge of the keratectomy sites in all patients, suggesting that conjunctival invasion towards the center did not occur in any eye. partial conjunctivalization within 2 to 3 clock hours, confirmed by the presence of goblet cells, was limited to the keratectomy site in 10% of the cases. conclusion: a clear central corneal surface was demonstrated in all eyes following surgery leading to the conclusion that limbal donation was a safe procedure in this group of patients. a small percentage of eyes can have donor sites re-epithelized with conjunctival cells at the periphery of the cornea.

Abstract: purpose: to evaluate the efficacy of scleral contact lenses use on the management of ocular sequelae from stevens-johnson syndrome patients. methods: in a retrospective study, patients who suffered sequelae of stevens-johnson syndrome and started the use of scleral contact lenses were followed. patients were submitted to an evaluation of symptoms through a questionnaire; ophthalmologic exam (visual acuity measurement, biomicroscopy, ocular surface staining with fluorescein drops, schirmer test). results: ten eyes of seven patients were analyzed. visual acuity varied from hand movements to 20/25. all patients presented some degree of corneal opacity and slight symblepharon. in patients whose adaptation to scleral contact lenses was successful (90%), they all refered improvement of symptoms and sight. as for the biomicroscopic findings it was observed an improvement of conjunctival hyperemia and keratitis and a reduction of the mucous secretion in 90% the cases. conclusions: a successful adaptation to scleral contact lenses was feasible on most patients, with relief of symptoms and better visual acuity, probably due to regularization of the surface. scleral contact lenses represent an important and accessible alternative to reduce the limitations inferred by the damages from stevens-johnson syndrome.
Femtosecond assisted intrastromal corneal ring (ISCR) implantation for the treatment of corneal ectasia

Abstract: purpose: to assess the outcomes of intrastromal corneal ring (iscr) implantation for the treatment of corneal ectasia. methods: thirty-five consecutive patients with secondary corneal ectasia treated with keraring intrastromal corneal ring segment implantation aided by intralaser femtosecond technique (k-icrs-fs) were evaluated. visual acuity (logmar), refraction and astigmatism vector analysis were measured preoperatively and 3 months postoperatively using vector analysis as described by some authors. results: visual acuity improved from 0.40 ± 0.20 to 0.25 ± 0.13 logmar (p=0.0002), with reduction of the spherical equivalent from -5.41 ± 4.78 d to -2.83 ± 3.29 d (p=0.0002). mean astigmatism reduction was 3.92 ± 2.52 d. k maximum decreased from 51.65 ± 5.83 d preoperatively to 48.58 ± 5.54 d (p=0.001) 3 months after the surgery and k minimum decreased from 45.92 ± 4.64 d to 43.96 ± 5.06 d (p=0.0041). there were no intraoperative or postoperative complications. conclusion: intrastromal corneal ring implantation with the use of a femtosecond laser was a safe procedure, with low risk of complications and significant improvement on visual acuity and topographic data in this seeting of patients with secondary corneal ectasia.
Uso de mitomicina C tópico no tratamento da neoplasia intra-epitelial córneo-conjuntival e carcinoma espinocelular conjuntival: resultados preliminares

Abstract: purpose: the purpose of this study is to evaluate the safety, efficacy and recurrence after topical use of mitomycin c (mmc) for primary or recurrent corneal-conjunctival intraepithelial neoplasia (cin) and recurrent squamous cell carcinoma (scc). methods: two groups of patients were trated. group 1 patients with primary or recurrent cin were treated with topical 0.02% mmc, qid for 28 days. group 2, patients with recurrent scc, were treated with topical 0.02% mmc, qid for 21 to28 days. results: eight patients with cin and 1with scc had a total regression of the lesion and all of them had negative control exfoliative cytology for neoplastic cells. one patient with cin and 2 with scc had partial regression of the lesions, which were excised surgically, and the pathology was negative for neoplasia. side effects were transient and subsided after descontinuation of the treatment. there was no recurrence of the lesions on a mean follow-up of 24.9 months. conclusion: the preliminary results of this study suggest that the use of the topical mmc is safe and efficient for the treatment of primary and recurrent cin and recurrent scc, although it did not prevent surgery in almost all cases of cec. there were no recurrences during the follow-up. further studies with more patients and longer follow-up are necessary to confirm this results.
Transplante de membrana amniótica em casos agudos graves de queimadura ocular química e síndrome de Stevens-Johnson

Abstract: purpose: to study the therapeutic potential of amniotic membrane transplantation in cases of severe acute chemical ocular burn and stevens-johnson syndrome. methods: we retrospectively reviewed the records of eight patients, with a total of ten eyes, submitted to amniotic membrane transplantation for treatment of ocular chemical burns and stevens-johnson syndrome in the acute phase between january 1999 and may 2008 in the ophthalmology department of unifesp. data relating to sex, age, degree of chemical burns, etiology, affected eye, ophthalmological findings, extension of amniotic membrane, surgeries, additional time between the injury and surgery in days, visual acuity before and after surgery, epithelial defect healing (days), complications and time of follow-up in months were collected. results: the average age of patients was 35.7 ± 23.04 years, with six men and two women. three patients (four eyes) had stevens-johnson syndrome and five patients (six eyes) had chemical ocular burn. the epithelial defect was healed at an average of 27.8 ± 4.7 days (ranging from 20 and 35 days). all patients presented limbal stem cell deficiency in a median follow-up of 7.8 ± 2.8 months (ranging from six and twelve months) and four eyes developed symblepharon. conclusions: the results suggest that the amniotic membrane transplantation represents an additive that can be carried out in the serious cases of ocular chemical burn and stevens-johnson syndrome with the finality of promoting the epithelialization and abolishing the inflammation and its consequences, if compared with other studies that treated similar cases with medical therapy only. on the other hand, it is not possible to avoid the limbic deficiency in these cases, which in the future will need limbal stem cell transplantation or other surgeries for correction of the ocular surface.

Abstract: purpose: to establish current definition, classification and staging, and to develop diagnosis and treatment recommendations for ocular allergy, by using delphi approach. methods: ten latin american experts on ocular allergy participated in a 4-round delphi panel approach. four surveys were constructed and answered by panelists. a two-thirds majority was defined as consensus. definition, classification, staging and diagnosis and treatment recommendations were the main outcomes. results: "ocular allergy" was proposed as the general term to describe ocular allergic diseases. consensus regarding classification was not reached. signs and symptoms were considered extremely important for the diagnosis. it was consensus that a staging system should be proposed based on the disease severity. environmental control, avoidance of allergens and the use of artificial tears were recommended as first line treatment. the secondary treatment should include topical anti-histamines, mast cell stabilizers and multi actions drugs. topical non-steroidal anti-inflammatory and vasoconstrictors were not recommended. topical corticosteroids were recommended as third line of treatment for the most severe keratoconjunctivitis. consensus was not reached regarding the use of systemic corticosteroids or immunosuppressant. surgical approach and unconventional treatments were not recommended as routine. conclusion: the task of creating guidelines for ocular allergies showed to be very complex. many controversial topics remain unsolved. a larger consensus including experts from different groups around the world may be needed to further improve the current recommendations for several aspects of ocular allergy.
Correla??o clínico-laboratorial de úlceras infecciosas de córnea

Abstract: purpose: to evaluate the initial treatment of infectious corneal ulcers and their laboratory tests. methods: twenty four cases of infectious corneal ulcers seen in the external corneal disease section at the hospital do servidor público estadual - s?o paulo - brazil, underwent gram staining and culture for bacterial and fungus. treatment with fortified cefalotine (50 mg/ml) and gentamicin (14 mg/ml) was started. results: seventeen cultures (70.83%) were positive for bacterial infection and seven (29.17%) showed no growth. in three cases (15.76%) the gram staining showed correlation with the culture. three cases did not respond to the initial treatment and had the medications changed according to the results of the cultures. all patients responded to the clinical treatment. conclusions: ours results showed that combination of fortified cefalotine and gentamicin eye drops was efficient to treat most of the infectious corneal ulcers.
