Abstract: family health strategy represents one of the major public programs in brazil covering 5,601 municipalities in 2006 (91.8%). this study evaluated teresópolis family health strategy through information and perceptions of adults responsible for children around 0-5 years that used those health services. it was used evaluation forms answered by family in waiting rooms or during home visiting. results for urban and rural areas were compared and significant differences were observed in some issues. major findings include users' strong ties with the program and a highly positive perception about the children health status. access was satisfactory as to use criteria when relating to regular care proceedings. critical points observed include: low coverage of home visiting, bad access to labs and image exams and to systematic delivery of medicines. rural areas presented a high number of children who treated acute clinical conditions at home without doctor assistance. users satisfaction as to physicians, nurses, health community agents and services were high level. users not regularly visited at home presented lower access to medicine delivery.

Abstract: OBJETIVO: Avaliar o processo diagnóstico da tuberculose pulmonar em indígenas menores de 15 anos, por meio do Sistema de Pontua o do Ministério da Saúde Modificado (SP-MSm), em crian as e adolescentes com resultados negativos na baciloscopia. MéTODOS: Estudo descritivo retrospectivo de 49 casos de tuberculose em indígenas menores de 15 anos no estado do Mato Grosso do Sul entre 2007 e 2010. RESULTADOS: Dos 49 pacientes, 27 (56%) eram menores de 5 anos, 33 (67%) apresentavam sintomas sugestivos de tuberculose, 24 (49%) tinham baixo peso, e 36 (73,5%) haviam sido vacinados com BCG. O teste tuberculínico foi reator em 28 pacientes (57%). Dentre esses, 18 (64%) apresentaram endura o > 10 mm. Foram realizadas radiografias de tórax em 37 pacientes (76%), sendo que 31 (84%) fizeram apenas um exame. Desses 37 pacientes, os achados radiológicos eram sugestivos de tuberculose em 16 (43%), de infiltrado/condensa o em 10 (27%) e normais em 4 (11%). As Equipes de Saúde Indígena foram responsáveis pelo diagnóstico em 31 (63%) dos casos, mas o SP-MS original só foi utilizado em 14 (45%). Os escores do SP-MSm foram determinados em 30 pacientes (61%). Dos 30 casos pontuados, os resultados dos escores indicaram diagnóstico de tuberculose muito provável, possível e pouco provável em 16 (53%), 11 (37%) e 3 (10%), respectivamente. CONCLUS ES: A propor o de diagnóstico muito provável e possível foi concordante com o diagnóstico padr o do servi o (90%), evidenciando a aplicabilidade epidemiológica do SP-MSm para o diagnóstico da tuberculose pulmonar em indígenas, de forma compatível com a realidade do servi o de saúde prestado. OBJECTIVE: To evaluate the process of diagnosing pulmonary tuberculosis in smear-negative indigenous children and adolescents under 15 years of age with the modified Brazilian National Ministry of Health Scoring System (mBNMH-SS). METHODS: This was a retrospective descriptive study involving 49 indigenous patients under 15 years of age with tuberculosis, treated between 2007 and 2010 in the state of Mato Grosso do Sul, Brazil. RESULTS: Of the 49 patients, 27 (56%) were under 5 years of age, 33 (67%) had symptoms suggestive of tuberculosis, 24 (49%) were underweight, and 36 (73.5%) had been BCG vaccinated. The tuberculin skin test was positive in 28 patients (57%), 18 (64%) of whom had an induration > 10 mm. Chest X-rays were performed in 37 (76%) of the patients, 31 (84%) of whom had only one chest X-ray taken. Abstract: this study examines the relationships between brazilian psychiatric reform, the adoption of the centers for psychosocial care (caps) and the development of the unified health system (sus). the adherence of municipal governments was a variable determinant for the spread of reform, especially due to the continental scale and fragmentation of the brazilian federation. the article demonstrates the institutional stability of psychiatric reform in brazil over two decades. the institutional nature of the decision-making process in the public arena has permitted the implementation of new organizational formats through imitation and financial incentives. the psychiatric reform was successful in defending the advantages of caps in relation to the asylum and hospital model dominant in past decades. the inductive policies, strengthened and upheld by law 10.216/2001, transformed the agenda of psychiatric reform, limited to pioneering cities in a national public policy.

Abstract: La estructura actual del sistema financiero espa ol se generó en los últimos treinta a os. Era calificado, en aquellos tiempos, como uno de los más rígidos y reglamentados de toda la OCDE. La falta de sistemática y coherencia eran predominantes en las bolsas de valores. Las entidades financieras, esencialmente los bancos, operaban con márgenes regulados, rentabilidades elevadas y barreras a la entrada frente a competidores no deseados. A partir de esa situación inicial, el artículo va mostrando los ejes de desarrollo que han seguido, en su proceso de modernización, los mercados financieros y el sistema bancario.

Abstract: objective: to investigate operational aspects of tuberculin skin test (tst) use in tuberculosis control programs and at specialized brazilian national sexually transmitted diseases/aids and viral hepatitis program health care clinics in priority municipalities for tuberculosis control in the state of mato grosso do sul, brazil. methods: this was a descriptive, cross-sectional, epidemiological survey. data on qualifications/training of professionals administering tsts, timing of the tst, procedures in cases of loss to follow-up (reading), material availability, and material storage were collected through interviews and technical visits. for the 2008-2009 period, we determined the numbers of screenings in vulnerable populations, of tsts performed, and of patients treated for latent tuberculosis. results: we interviewed 12 program managers in six municipalities. some programs/clinics did not perform tsts. nursing teams administered the tsts, results were read by non-specialists, and specialization/refresher courses were scarce. the ppd rt23 was stored in 5-ml flasks under appropriate conditions. insulin syringes were commonly used. testing was available during business hours, three times a week. in cases of loss to follow-up, telephone calls or home visits were made. of the 2,305 tsts evaluated, 1,053 (46%) were performed in indigenous populations; 831 (36%) were screenings in prisons, performed for training; and only 421 (18%) involved contacts of tuberculosis patients or vulnerable populations. four vulnerable patients and 126 indigenous subjects were treated for latent tuberculosis. conclusions: these priority municipalities showed operational difficulties regarding human resources, materials, and data records.

Abstract: tuberculin screening aims at tracking tuberculosis infection. a one-to-three-week retest has been recommended when the tuberculin test is <10 mm in the first dose to check booster effect occurrence. the booster phenomenon occurs when the tuberculin screening is <10 mm in the first dose and after the second dose, in three weeks time, there is a >10 mm induration with an increase compared to the first dose of at least 6 mm. this study, aimed at estimating booster effect rate, involved health professionals from a university hospital in campo grande, a town in the brazilian state of mato grosso do sul. among the 194 participants, 65 (33,5%) were strong ppd reactors in the first test and 129 were weak or non-reactors. they went through a second test which revelled 10 were reactors. hence, the booster effect rate was 7,8%. in the two-step tuberculin test reactor tuberculosis infection prevalence was 38.7%. the authors concluded that identifying booster effect in tuberculin screening is important to avoid under estimated rates of tuberculosis infection. it is also important to follow up recently acquired infections and evaluate the results of tuberculin screening accurately to reduce the probability of false negative results, which would otherwise be misinterpreted as refering to recently infected health professionals.

Abstract: introduction: this study describes the epidemiological aspects of tb among brazilian native indians aged less than 15 years-old in mato grosso do sul, brazil, between 2000 and 2006. methods: a retrospective observational study based on secondary data collected from the health system of brazilian indigenous peoples, special indigenous sanitary district (dsei), and national system of notifiable diseases (sinan) databases. the variables analyzed were: frequency of tb according to sex, age, clinical presentation and outcome. the fisher test and the incidence tendency curve were calculated (p<0.01). results: tb prevalence was 20.4% (224/1,096). the incidence rate decreased 14% per year during the study period. tb was more prevalent among brazilian indigenous individuals aged less than 5 years-old and among those aged less than one year-old. more than half of tb cases were male and the most common clinical presentation was pulmonary tb (92.9%). cure was the most common outcome (91.1%), followed by abandoned treatment (3.6%) and death (2.2%). conclusions: the high cure rate, reduced mortality and the progressive decrease in tb incidence rate during the study period indicate the effectiveness of supervised treatment of the new tb control model implemented among brazilian indigenous peoples on 2000.
