Enfermería transcultural como método para la gestión del cuidado en una comunidad urbana: Camaguey 2008

Abstract: the madeleine leininger's cultural care theory poses that the knowledge of cultural and social structure of a community, group or subject, may define the achievement of objectives in nursing assistance practice. for the practical utilization of this theory in our environment, we conducted a qualitative and descriptive study in a urban community from "julio antonio mella" polyclinic to describe the cultural features of that population, and to identify the required cultural care , thus applying the rising sun model of this theory embracing the following variables: educational, economic, political and legal factors, cultural values and lifestyles, social and family factors and religious and philosophical factors. we determined that this population has a schooling mean level of 12 degree level, to practice mainly the protestant religion, is integrated to mass organizations and has a low rate of crime. most works with a low birthrate and the cultural values are identified fundamentally in the solidarity. unhealthy lifestyles were developed related to risk factors of non-communicable chronic diseases. we verified some popular myths that may be respected thus we designed a proposal of transcultural cares to this population.

Abstract: La teoría de cuidados culturales de Madeleine Leininger plantea que el conocimiento de la estructura cultural y social de una comunidad, grupo o individuo puede definir el logro de objetivos en las prácticas asistenciales de enfermería. Para la utilización práctica de ésta teoría en nuestro medio se realizó un estudio descriptivo cualitativo en una comunidad urbana del Policlínico "Julio Antonio Mella", con el objetivo de describir las características culturales de esa población e identificar los cuidados culturales que requieren para lo cual aplicamos el modelo del Sol Naciente de la teórica que abarca las siguientes variables: Factores educativo, económico, político y legal, valores culturales y estilos de vida, factores sociales y familiares y factores religiosos y filosóficos. Se determinó que ésta población tiene un nivel de escolaridad promedio de 12°, practica mayormente la religión protestante, está integrada a las organizaciones de masas y tienen un bajo índice de criminalidad. La mayoría trabaja, hay baja taza de natalidad y los valores culturales se identifican fundamentalmente en la solidaridad. Se desarrollan estilos de vida no sanos relacionados con factores de riesgo de enfermedades crónicas no transmisibles. Se identificaron varios mitos populares que pueden ser respetados y se confeccionó una propuesta de cuidados transculturales a ésta población. Causas de invalidantes laborales en el personal de enfermería

Abstract: the study of professions-related diseases is an activity that began in the last years. the nursing profession is not excluded from favoring the appearance of pathological processes if certain prevention measures are not taken in addition to the protection every cuban worker has. a descriptive and retrospective study of the 479 medical certificates given to nurses in 2002 and 2003 at "amalia simoni" hospital, that were the total of certificates issued in that period, was conducted. the objective of this study was to identify the causes of working disability and to propose an intervention strategy to attain a higher quality of life of the nurses. it was observed that in 2003, 248 medical certificates were submitted. the group of affections of the osteomyoarticular system predominated with 151 patients (31.5 %) and, within this group, it was observed a prevalence of low back pain in 45 patients (29.8 %). the most affected nurses were those working in the care field with 308 patients (64.3 %). the medicine service issued the highest number of certificates. data were obtained from the registry of medical certificates of the nursing subdivision. an intervention strategy based on the application, evaluation and control of a curriculum of corporal mechanics was proposed. it may contribute to improve the nurses' health problems that are supposedly related to the exercise of the profession.

Abstract: El estudio de las enfermedades relacionadas con las profesiones es una actividad que se inicia en a os recientes. La profesión de enfermería no está excluida de favorecer la aparición de procesos patológicos si no se tienen en cuenta ciertas medidas de prevención, además de la protección con que cuenta todo trabajador en Cuba. Se realizó un estudio descriptivo retrospectivo de los 479 certificados médicos expedidos a enfermeros en los a os 2002-2003 en el hospital "Amalia Simoni" y que constituyeron la totalidad de los certificados expedidos en este período. El objetivo del estudio fue identificar las causas de invalidantes laborales y proponer una estrategia de intervención para lograr una mayor calidad en la salud del trabajador de enfermería. En el a o 2003 el número de certificados médicos presentados, 248 (52 %), fue discretamente mayor que los de 2002. Predominó el grupo de afecciones del sistema osteomioarticular en 151 pacientes (31,5 %) y dentro de este grupo, fueron mayoritarias las sacrolumbalgias en 45 pacientes (29,8 %). Los enfermeros más afectados fueron los asistenciales y el servicio de medicina, el que más certificados médicos presentó. Los datos se obtuvieron del libro registro de certificados médicos de la vicedirección de enfermería. Se propone una estrategia de intervención que se basa en la aplicación, evaluación y control de un programa de clases sobre mecánica corporal para el mejoramiento de los problemas de salud que pudieran estar vinculados con el ejercicio de la profesión de enfermería. Abstract: self-care is a regulatory human function applied in a deliberate way by each subject to maintain its life and its health status. a cross-sectional and descriptive study was conducted to assess the care management in nursing in self-care deficit of dorothea orem's theory applied in patients admitted in "amalia simón" hospital in camaguey province from january to march, 2008. universe and sample included 187 patients fulfilling de inclusion criteria. primary datum was from medical records and from a questionnaire designed by nursing national management; both results were entered in a form including the following variables: breakdown of self-care actions of a major intervention; assessment of those actions taking into account the basic nursing systems according to a orem diagram, and assessment of satisfaction level, allowing to know a marked predominance in physiological needs in 100% of patients, the therapeutical self-care in 84 patients (44,91%), elimination action in 854 frequencies, the partially compensatory system in 1123 frequencies, and the satisfaction level in 180 patients (96,25%. data were processed in a microcomputer 11,5 version for windows, with a 95% of probability. present study allowed knowing that taking into account that oren's reflections value the interaction between patient and nurse, an essential feature in nursing professionals function and work to satisfy needs.
Gestión del cuidado enfermero en la teoría del déficit de autocuidado Management of nursing care in self-care deficit theory

Abstract: El autocuidado es una función humana reguladora que debe aplicar cada individuo de forma deliberada con el objetivo de mantener su vida y su estado de salud. Se realizó un estudio descriptivo transversal con el objetivo de evaluar la gestión del cuidado enfermero en la teoría del déficit de autocuidado de Dorothea Orem, en pacientes ingresados en el Hospital "Amalia Simoni" de Camagüey, de enero a marzo 2008. El universo y muestra estuvo integrado por 187 pacientes que cumplían los criterios de inclusión. El dato primario fue extraído de las Historias Clínicas y un cuestionario creado por la dirección nacional de enfermería, ambos resultados fueron llevados a un formulario que incluía las variables: Clasificación de las necesidades básicas según orden de prioridad; tipos de autocuidado; desglose de acciones de autocuidado de mayor intervención; evaluación de las acciones teniendo en cuenta los sistemas básicos de enfermería según esquema de Orem y evaluación del nivel de satisfacción, lo que permitió conocer que existió marcado predominio en las necesidades fisiológicas en el 100% de los pacientes, el autocuidado terapéutico en 84 (44,91%), la acción de eliminación en 854 frecuencias, el sistema parcialmente compensador en 1123 frecuencias y el nivel de satisfacción en 180 pacientes (96,25%). Los datos se procesaron en microcomputadora versión 11,5 para Windows, con una probabilidad del 95%. Nuestro estudio permitió conocer que tomando en consideración que las reflexiones de Oren valoran el interactuar del paciente y la enfermera(o), aspecto esencial en la función de los profesionales de enfermería, trabajan con el objetivo de satisfacer necesidades. Abstract: the production of popular services and health practices identifies an area developed within the master?s program in social practice and health as of 1993 in the school of dentistry, universidad central de venezuela. it allows us to tackle the issue of collective health from the frame of services and with the help of the community. we intend to analyze the practice and describe its links to the whole, to rescue its social aim and to provide an on-going feedback to the process whereby both the actors involved and the practice itself change radically. all of this successfully carried out based on decisions taken by the population and their actions fully committed with the health range. this can only be achieved through the joined efforts of diverse areas. most of which functions as cornerstones in the curriculum of the master?s program. without and within, close to the participants, monitoring their performance and with eyes glued on the community round the clock. what follows as subject matter of this paper adds up to our experience. a view from the area.

Abstract: La "Producción de Servicios y Prácticas Populares de Salud" identifica un área que se desarrolla en la Maestría en Práctica Social y Salud desde 1993 en la Facultad de Odontología de la U.C.V. Nos permite, desde el espacio concreto de los servicios y con la comunidad, el abordaje de la situación de la Salud Colectiva. Pretendemos desde el quehacer en los servicios, analizar la práctica y descubrir sus relaciones con la totalidad, rescatar su objetivo social y retroalimentar el proceso donde los actores involucrados y la práctica misma se transforman, a partir de las decisiones del colectivo y de su acción comprometida en el espacio social de la Salud. Ello, solo es posible, a través del esfuerzo conjunto desde diferentes áreas que componen el curso de maestría por la integración. Desde y hacia su interior, con los cursantes y donde el quehacer de los mismos con la comunidad y de la propia comunidad. A continuación presentamos en este papel parte de nuestra experiencia, una visión desde el área. Episiotomy in normal deliveries at the University Hospital "La Ribera".

Abstract: Aim: Describe the incidence of episiotomy in normal vaginal delivery and analyze risk factors that affect the performance of the practice by midwife.Material and Methods: Observational, descriptive, cross sectional, retrospective study from January 1 to December 31, 2011 at the University Hospital “La Ribera”. The variables are episiotomy, perineal tears, parity and technique, provided by the hospital itself.Results: The percentage of episiotomy in normal vaginal deliveries performed at the University Hospital "La Ribera" was 33.5% (2011). Statistically significant differences were found for variables perineal injury and mediolateral technique, increased the number of intact perineal and spontaneous tears for women without episiotomy. Among women who underwent episiotomy, there is a higher proportion than those who used a medial to lateral technique. Conclusions: The University Hospital "La Ribera" has a selective episiotomy policy in normal vaginals deliveries that favors humanized birth and helps to preserve the perineum intact.

Abstract: Volume stabilization in models with flat extra dimension could follow from vacuum energy residing in the bulk when translational invariance is spontaneously broken. We study a simple toy model that exemplifies this mechanism which considers a massive scalar field with non trivial boundary conditions at the end points of the compact space, and includes contributions from brane and bulk cosmological constants. We perform our analysis in the conformal frame where the radion field, associated with volume variations, is defined, and present a general strategy for building stabilization potentials out of those ingredients. We also provide working examples for the interval and the $T^n/Z_2$ orbifold configuration.
