Etiopathogenesis of disk displacement of the temporomandibular joint: A review of the mechanisms

Abstract: Disk displacement of the temporomandibular joint (TMJ) is a clinically important condition, showing a high prevalence in both patient and non-patient populations. Despite its clinical importance, there is incomplete understanding of the etiopathogenic mechanisms leading to disk displacement. A number of possible risk factors have been identified. This article analyzes the etiopathogenesis from both the clinical and the biomechanical viewpoints and also reviews the literature on the association between disk displacement and the main risk factors (i.e., trauma, altered disk shape and/or dynamic properties, occlusal abnormalities, steepness of the articular eminence, hyperactivity of the lateral pterygoid muscle, joint hypermobility, etc.). According to our interpretation of available data, an impairment of joint lubrication may be a common finding in cases of disk displacement, thus suggesting the need for future studies addressing both local and systemic neuroendocrine aspects influencing the friction coefficient of the TMJ. A full comprehension of the etiopathogenesis of disk displacement is far from being achieved, and clinicians must take into account this consideration when treating patients with temporomandibular disorders.
Finite Size Scaling and Running Coupling Constant in CP(N-1) models

Abstract: In this work I present a numerical study of the Finite Size Scaling (FSS) of a correlation length in the framework of the $CP ^{N-1}$ model by means of the 1/N expansion. This study has been thought as propedeutical to the application of FSS to the measure on the lattice of a new coupling constant $f_{x}(1/R)$, defined in terms or rectangular Wilson Loops. I give also a perturbative expansion of $f_{x}(1/R)$ in powers of the corresponding coupling constant in the $\overline{MS}$ scheme together with some preliminary numerical results obtained from the Polyakov ratio and I point out the conceptual problems that limit this approach.
Aspects of Non-Equilibrium Dynamics in Quantum Field Theory

Abstract: This work is devoted to the study of relaxation--dissipation processes in systems described by Quantum Field Theory. In the first part, I focus on the phi^4 scalar quantum field theory in finite volume in the large N limit. I find that the zero-mode quantum fluctuations cannot grow macroscopically large starting from microscopic initial conditions, while the long-wavelength fluctuations do scale with the linear size of the system, signalling dynamical infrared properties quite different from the equilibrium ones. I also introduce a non-gaussian Hartree-Fock approximation (tdHF) and I derive the mean-field coupled time-dependent Schroedinger equations for the modes of the scalar field. The dynamical evolution of a uniform condensate is studied in a further controlled gaussian approximation, for N=1. Results similar to the large N case show an internal inconsistency of this approximation. I also study the dynamics of the system in infinite volume in the broken symmetry phase. The data show that the fixed points of the evolution cover at most the classically metastable part of the static effective potential. I finally study the dynamical evolution for the O(N+1) nonlinear sigma model in 1+1 dimensions in the large N limit. I first of all verify that the large coupling limit of the O(N+1) phi^4 model, which renders the model non linear, commutes with the large N limit, so that the O(infty) nonlinear sigma model is uniquely defined. I then study the evolution of several observables, like the spectrum of produced particles, finding no evidence for parametric resonance. I conclude with some remarks on the ``state of art'' in gauge theories and some comments about the open issues in the subject.

Abstract: one of the major parameters that influence the feasibility of using bioremediation in contaminated sites is the biodegradability of the compounds of concern. the bartha's respirometric method, adapted from a dutch norm, is a common method used in brazil to determine such biodegradability. however, tropical soils present different characteristics as compared to soils from regions of temperate climate. in this research, its applicability was studied for a latosoil, which is the predominant soil type in s?o paulo state, brazil. based on the results of this study, it can be concluded that abiotic reactions in latosoil generated carbonic gas in significant concentrations. furthermore, this research verified the difficulty of sterilizing the soil, which would make it difficult to evaluate the removal of contaminants by other mechanisms or the biodegradation by exogenous microorganisms. therefore, the application of bartha?s respirometric method is not recommended to determine the biodegradability of pollutants in latosoils.

Abstract: two concomitant movements occur in the first decade of the xxi century within the private and public dental services in brazil: the entrance of oral health on the agenda of political priorities of the federal government and the vigorous growth of additional dental care. we analyzed the occurrence of these phenomena in the city of s?o paulo, by seeking information in official documents and electronic databases in the municipality of s?o paulo, the ministry of health and national health agency (ans), and also in scientific literature. during the studied period - january 2000 to december 2009 - and with basis on indicators such as coverage of first consultation program and dental coverage population potential, percentages were found that characterize low public assistance and a situation far short of the constitutional principle of universal access to dental care. the growing number of beneficiaries of additional services through exclusively dental coverage insurance plans and other types of private insurance plans in the same period was significant, accounting for a major expansion of population coverage in this mode of care. it was found that, compared to the overall national framework, the city of s?o paulo offers poor access to public dental care, with reduced supply of services to adults and aged people. furthermore, considering the limitations of market additional services to provide dental care to all brazilians, it reinforces the need for continuity and expansion of brasil sorridente, which is the programmatic expression of the national oral health politics.

Abstract: The present work is a result of the analysis of the data obtained by the project named: NA/OS: from information and education to preventions. The project was performed by the Departments of Social Work, Social Sciences and Medical Clinic of the University of Londrina, together with the Interdisciplinary Association of A/DS of Londrina, from February to December 1992. The aims of the project were the raising of awareness about the seriousness of the pandemic and the need of its prevention and it was carried out by means of educational talks. The aim of this essay is to evaluate the razing of the level of information and the wíll to change the attitude through strategies such as the one used in this project, that is, educational talks as a means of prevention of AIDS. The methodology used to evaluate the results was based on questionnaires filled in before and after the educational lecture. The analysis indicates that some basic knowledge about AIDS is absorbed, although pre-conceived notions remain. The conclusion points out the importance of the educational lecture in the initial stage of the work. It is, however, from the point view of prevention, a strategy of limited efficacy, demanding complementary strategies. O trabalho apresentado é resultado da análise dos dados obtidos no projeto de extens o: "AIDS: da informa o e educa o à preven o", realizado pelos Departamentos de Servi o Social, Ciências Sociais e Clínica Médica da Universidade Estadual de Londrina -UEL, em convênio com a Associa o Londrinense Interdisciplinar de AIDS-ALIA, de fevereiro a dezembro de 1992. Os objetivos do projeto foram a sensibiliza o da popula o para a gravidade da pandemia e a necessidade da ado o de práticas preventivas e seu desenvolvimento se deu através de palestras educativas. O objetivo do presente texto é avaliar o aumento do grau de conhecimento e a predisposi o para mudan a de comportamento, através de estratégias como a utilizada, ou seja, palestra educativa como mecanismo de preven o da AIDS. A metodologia utilizada para mensura o dos resultados foi baseada na aplica o de questionários com quest es fechadas antes e depois da palestra educativa. A análise dos dados indica que informa es básicas sobre AIDS s o absorvidas, embora no es pré-concebidas permane am. A conclus o aponta para a validade de palestras educativas no estágio inicial de sensibiliza o para a quest o da AIDS. S o, no entanto, do ponto de vista do trabalho preventivo, de eficácia limitada, demandando estratégias complementares.
Breast reconstruction with permanent expander: a different approach

Abstract: background: immediate or delayed breast reconstruction with tissue expanders can be performed in one or several surgical sessions. we opted to perform breast reconstruction over several sessions. the aim of this study was to report our experiences with the breast reconstruction technique involving becker permanent tissue expanders and complete expander coverage with a flap comprising the pectoralis major, pectoralis minor, serratus anterior, and rectus abdominis aponeurosis. methods: the medical records of 21 patients who underwent postmastectomy breast reconstruction with becker permanent tissue expanders were retrospectively analyzed. results: during muscle flap preparation, the aponeurotic dissection was performed 6-8 cm below the inframammary crease, aiming at full expander coverage without tension, enabling lower suturing in the inframammary crease, and avoiding disruption of the pectoralis major attachments. only two patients developed infection, one in the first postoperative week and the other in the third postoperative month. conclusions: the flap comprising the pectoralis major, pectoralis minor, serratus anterior, and rectus abdominis aponeurosis is a good choice for breast reconstruction with permanent tissue expanders because it ensures adequate expander and skin-flap protection. moreover, the technique enables tissue expansion without confining the expanders in the submuscular cavity

Abstract: This article analyses processes of dynamics in urban space of Limeira city, S o Paulo State, taking by agents the public power and private initiative in the conformation or configuration of the central area’s urbanization, especially in the period between the 1960s and the first decade of the XXI century. Were focused to this analysis three cuttings of the urban center, identified as decadent center, old center and new center. Stand out in these portions some processes and features the material use of this spaces in context of occupation and use of urban land. As part of current interest to the urban occupation, the new center appears as the main focus of this study with the promotion of relevant issues regarding their current urbanization.
