Variability in Surgical Quality among Surgeons in Breast Cancer Surgery  [PDF]

Abstract: Introduction: Quality assurance is an essential aspect of cancer care. Assessment of surgical quality in breast cancer is still evolving. Variability in surgical care among surgeons has been well documented in literature and we sought to investigate such variation between two groups of surgeons referring patients to our oncology center. Methods: A prospective review of patient records of all breast cancer referrals to our department was made. Two groups were identified and segregated based on the performance of mastectomy by a general surgeon (GS) or by a surgical oncologist (SO). Patients treated with modified radical mastectomy for clinical stages 1 - 3 were included for the study. Patient demographic data and disease related information were collected in addition to thorough evaluation of the surgical pathology report. Margin positivity, mean nodal harvest, nodal ratio, inadequate axillary clearance, revision surgery and the use of radiotherapy for inadequate nodal dissection were the parameters evaluated in the study. Results: A total of 142 patient records were evaluated 72 designated as group 1 (general surgeons) and 70 as group 2 (surgical oncologist). The median age was 52 years and both groups were evenly balanced for age, laterality of breast lesion, histological type and grade. Loss of Keratin 8 Phosphorylation Leads to Increased Tumor Progression and Correlates with Clinico-Pathological Parameters of OSCC Patients

Abstract: Background Keratins are cytoplasmic intermediate filament proteins expressed in tissue specific and differentiation dependent manner. Keratins 8 and 18 (K8 and K18) are predominantly expressed in simple epithelial tissues and perform both mechanical and regulatory functions. Aberrant expression of K8 and K18 is associated with neoplastic progression, invasion and poor prognosis in human oral squamous cell carcinomas (OSCCs). K8 and K18 undergo several post-translational modifications including phosphorylation, which are known to regulate their functions in various cellular processes. Although, K8 and K18 phosphorylation is known to regulate cell cycle, cell growth and apoptosis, its significance in cell migration and/or neoplastic progression is largely unknown. In the present study we have investigated the role of K8 phosphorylation in cell migration and/or neoplastic progression in OSCC. Methodology and Principal Findings To understand the role of K8 phosphorylation in neoplastic progression of OSCC, shRNA-resistant K8 phospho-mutants of Ser73 and Ser431 were overexpressed in K8-knockdown human AW13516 cells (derived from SCC of tongue; generated previously). Wound healing assays and tumor growth in NOD-SCID mice were performed to analyze the cell motility and tumorigenicity respectively in overexpressed clones. The overexpressed K8 phospho-mutants clones showed significant increase in cell migration and tumorigenicity as compared with K8 wild type clones. Furthermore, loss of K8 Ser73 and Ser431 phosphorylation was also observed in human OSCC tissues analyzed by immunohistochemistry, where their dephosphorylation significantly correlated with size, lymph node metastasis and stage of the tumor. Conclusion and Significance Our results provide first evidence of a potential role of K8 phosphorylation in cell migration and/or tumorigenicity in OSCC. Moreover, correlation studies of K8 dephosphorylation with clinico-pathological parameters of OSCC patients also suggest its possible use in prognostication of human OSCC.
Fascin overexpression promotes neoplastic progression in oral squamous cell carcinoma

Abstract: To understand the role of fascin in OSCC development and/or progression, fascin was overexpressed along with vector control in OSCC derived cells AW13516. The phenotype was studied using wound healing, Boyden chamber, cell adhesion, Hanging drop, soft agar and tumorigenicity assays. Further, fascin expression was examined in human OSCC samples (N = 131) using immunohistochemistry and level of its expression was correlated with clinico-pathological parameters of the patients.Fascin overexpression in OSCC derived cells led to significant increase in cell migration, cell invasion and MMP-2 activity. In addition these cells demonstrated increased levels of phosphorylated AKT, ERK1/2 and JNK1/2. Our in vitro results were consistent with correlative studies of fascin expression with the clinico-pathological parameters of the OSCC patients. Fascin expression in OSCC showed statistically significant correlation with increased tumor stage (P = 0.041), increased lymph node metastasis (P = 0.001), less differentiation (P = 0.005), increased recurrence (P = 0.038) and shorter survival (P = 0.004) of the patients.In conclusion, our results indicate that fascin promotes tumor progression and activates AKT and MAPK pathways in OSCC-derived cells. Further, our correlative studies of fascin expression in OSCC with clinico-pathological parameters of the patients indicate that fascin may prove to be useful in prognostication and treatment of OSCC.Oral squamous cell carcinoma (OSCC) is the sixth most common malignancy in the world and ranks as the first in males in the Indian subcontinent. Abstract:

Surface and groundwater are related systems. They can be used conjunctively to maximize the efficient use of available resources. Groundwater may be used to supplement surface water to cope with the irrigation demands to meet the deficits in low rainfall periods. The parameters involved in the present study are groundwater availability, surface water availability, water requirement of crops and crop area. The inclusion of such uncertain parameters leads to accept the decision making process beyond the consideration of economic benefits. In the present study, an irrigation planning model is formulated by considering the conjunctive use of surface and groundwater. The resources in the present model, i.e. the area, surface water and groundwater availability are represented by fuzzy set. The linear membership function is used to fuzzify the objective function and resources. The model is applied to a case study of Jayakwadi project and solved for maximization of the degree of satisfaction (l) which is 0.546.

Finite Element Analysis of Sound Transmission Loss in One-Dimensional Solids  [PDF]

Abstract:

A higher-order acoustic-displacement based finite element procedure is presented in this paper to investigate one-dimensional sound propagation through a solid and the associated transmission loss. The acoustic system consists of columns of standard air and a solid, with the upstream column of air subjected to a sinusoidal sound source. The longitudinal wave propagation in each medium is modeled using three-node finite elements. At the interfaces between the air and the solid medium, the continuity in acoustic displacements and the force equilibrium conditions are enforced. The Lagrange multipliers method is utilized to assemble the global equations of motion for the acoustic system. Numerical results obtained for various test cases using the procedure described in the paper are in excellent agreement with the analytical solutions and other independent solutions available in the literature.

Stability and Vorticity Production in Stratified Astrophysical Disks  [PDF]

Abstract:

We study local linear non-axisymmetric perturbations in fully stratified 3D astrophysical disks. Radial stratification is set to be described by power law, while vertical stratification is set to be exponential. We analyze the linear perturbations in local shearing sheet frame and derive WKB dispersion equation. We show that stratification laws of the disk matter define not only the thermal stability of the disk, but also the efficiency of the potential vorticity production by rotationg convective turbulence in astrophysical disks. Taken developed convective turbulence we assume nonlinear tendencies set by linear spectrum and show that vortices are unlikely to be generated in rigid rotation flows. In contrast, differential rotation yields much higher vortex production rate that depends on the disk thickness, distance from the central object and the spectral characteristics of the developed thermal turbulence. It seems that measurements of the temperature and density distribution in accretion disks may indicate the efficiency of the turbulence development and largely define the luminosity characteristic of accreting flows.

Abstract: The present work aims to optimize the foaming conditions for tomato juice using three level Box-Behnken Experimental Design of Response surface Methodology. In the following study three process parameters namely concentration of egg albumin (EA) as foaming agent, Carboxy methyl cellulose (CMC) as foam stabilizer and whipping time (WT) was optimized. The levels for various input variables were EA: 5% - 15%, CMC: 0.10% - 0.60% & WT: 3 - 7 min. The responses measured were expansion volume (EV), foam density (FD) and drainage volume (DV) which are an indication of foaming ability and foam stability. The predicted levels of responses as generated by software were EV: 91.49%, FD: 0.558 g/cc & DV: 10 ml, which on validation was found closer to experimental value.

Abstract: Low profile antenna in communication is a new methodology. Fractal geometry is a methodology through which size reduction is achieved. A Self-similar fractal antenna using multicantor technique is proposed and experimentally studied. Space-filling cantors and self-similarity properties of fractal geometry have been adopted in the proposed antenna to miniaturize the size of antenna. The antenna is designed in such a way to operate at MICS band (Medical Implant communication Service) for wireless telemedicine application. The prototype antenna exhibits wideband characteristics and provides a good agreement of return loss (S11). Experimental return loss has been compared with that which is obtained using method of moments. The objective of using self-similar concept in antenna makes it flexible in terms of controlling the resonance and bandwidth. In this paper, the Self-similar property of fractal geometry is examined by maintaining return loss (S11) more than –30 dB approximately in all the iterations (n) and the prototype antenna has return loss greater than –10 dB and exhibits wideband characteristics.

Abstract:

Phyllanthus emblica L. is an important constituent of Ayurvedic medicine and a fresh fruit species in the market in Sri Lanka. Therefore, it has a high potential to be established as a commercial fruit crop. The seeds of P. emblica are semi orthodox and exhibit a long dormancy period hindering the natural sexual propagation. Therefore, it still remains as an underutilized fruit crop in Sri Lanka due to its lack of quality planting material and poor propagation techniques. Long term dormancy also causes a big challenge in germinating seeds to create populations in breeding programs. In order to promote P. emblica from its underutilized status, what seems most feasible is to develop a method to break up the seed dormancy artificially. In order to do so, the methods of breaking the dormancy of P. emblica seeds have to be studied. Therefore, the aim of this study was to identify a method to break up the seed dormancy of P. emblica. The seeds were extracted from 21 trees belonging to three different districts in Sri Lanka. The selected viable seeds were subjected to four different pre treatments: none treated seeds (i.e. control), seeds scarified, seeds scarified and treated with 1% gibberellin and seed coat removed and followed by seeds treated with 1% gibberellin. From the four treatments, seed dormancy was overturned with a germination percentage of 43% by the seed pre treatment where the seeds were scarified and treated with 1% gibberellin and no other pre treatment methods were successful in breaking the dormancy. This suggests that the natural germination potential of P. emblica seeds is very low and it can be overridden by seed scarification and gibberellin pre treatment.

Effect of flunixin meglumine alone and in combination on haemodynamics during bovine endotoxic shock and after treatment  [PDF]

Abstract: To investigate the effect of Flunixin meglumine- a NSAID; alone and in combination with hypertonic saline on endotoxemic buffalo calves, two groups of five apparently healthy male buffalo calves aged be-tween 6-8 months were subjected to I.V. infusion of E.coli endotoxin at the rate of 5μg/kg BW per hour for 3 hours. A highly significant (P < 0.01) fall in mean systolic,diastolic, pulse, mean arterial pressure (M.A.P), central venous pressure (C.V.P) and haemo-globin was observed till the end of endotoxin infusion while respiratory rate was significantly elevated along with a non-significant alteration in rectal tem-perature and hematocrit during the infusion of en-dotoxin. Immediately at the end of endotoxin infusion, flunixin meglumine at the rate of 1.1 mg/kg B.W was infused i.v. in group-I animals and group-II animals were infused with hypertonic saline solution (H.S.S.) at the rate of 4 ml/Kg BW as one time infusion fol-lowed by flunixin meglumine at the rate of 1.1 mg/kg B.W which resulted in increase of various parameters either to normal or very close to normal value while the rectal temperature and haematocrit decreased non-significantly throughout the observation period of 7 hours. No improvement in Hb and respiration was observed consequent to FM administration. Both treatments successfully raised systolic, diastolic, pulse pressure, C.V.P & M.A.P to normal pre-infusion val-ues. From the results of the present investigation, it can be concluded that i.v. infusion of FM alone and in combination with hypertonic saline solution in en-dotoxemic buffalo calves effectively restores the various hemodynamic parameters close to normal pre-infusion values and it can be used as immediate resuscitation measure to provide the clinician valu-able time to plan further long term treatment.
