Abstract: In January 2005 the Andalusian Health Service Improvement Plan prepared: Caring for the Caregiver include actions to be taken to promote equity, to recognize and facilitate the work of family carers. From our perspective of formal caregivers, it seems necessary to consider not only themselves need care patients with the disease, but also makes it mandatory caring individuals usually relatives, facilitators of the provision of care. In the Unit of Neurology, the daily observation of these family situations, has guided and network relationship between the two formal and informal systems of care, and we have tried the approach of the caregivers as clients to treat them as co-participants the experience of caring. Hypothesis: The Implementation of Functional Plan caregiver positive impact on hospitalization decreased anxiety and improved quality of life of caregivers of a dependent patient. Overall objective: To determine the effect of applying functional caregiver Plan on anxiety and quality of life of family caregivers of dependent people with neurological disorders. Study Design: Experimental study of the clinical trial such an intervention group and a control group randomly assigned.

Abstract: the relation ship between the psychic development and the nutritional condition from the fetal stage up to the teenage years is the innermost objective of this study. the importance of eating and having an adequate diet during the pregnancy period, the training of the future mother to breast feed in the first months of the baby′s life and the subsequent application of the ablactación process and its relation with psychomotor development and the personality progression in the different stages of the psychological, physiological and social development of the infant-adolescent period. the results shown were obtained from various studies carried out in the nutritional clinical service. they are related to the reinduction of breast feeding in children less than 4 months of age with protein energetic malnutrition, as well as other stages of the infant-juvenile obese and other chronic and genetic diseases related to the nourishment and nutrition (diabetes, fenilcetonuria, hiperamonemia, homocistinuria y fibrosis quística), in which the close relationship betweeen the psychology, and the nutrition stands, all through the psychotherapeutic and educational treatments and based on the application of the clinic psychology in the prevention, promotion and treatment of the nutritional alterations and other chronic and genetic diseases related to nourishment and mal nutrition. aspects related to the psychological and social characterizations as well as the personality evolution of these patients and their relatives environment are established.

Abstract: La relación del desarrollo psíquico con el estado nutricional, desde la etapa fetal, hasta la adolescencia, es el objetivo central de este artículo. La importancia de la alimentación y la nutrición adecuada durante el embarazo, la preparación de la futura madre para la práctica de la lactancia materna, la consecuente aplicación del proceso de ablactación, y su relación con el desarrollo psicomotor y formación de la personalidad en las diferentes etapas del desarrollo psicológico, fisiológico y social del período infanto-juvenil. Se exponen los resultados obtenidos en diversos estudios desarrollados dentro de un Servicio de Nutrición Clínica, sobre reinducción de la lactancia materna en ni os menores de cuatro meses con desnutrición proteica energética, así como otras etapas de las edades infanto-juvenil, obesos y otras enfermedades crónicas y genéticas relacionadas con la alimentación y la nutrición (diabetes, fenilcetonuria, hiperamonemia, homocistinuria y fibrosis quística), en los que se fundamentan la estrecha relación entre la Psicología y la Nutrición, a partir de tratamientos psicoterapéuticos y educativos, basados en la aplicación de la psicología Clínica en la prevención, promoción y tratamiento de las alteraciones nutricionales, y de otras enfermedades crónicas y genéticas relacionadas con la alimentación y la nutrición. Cromomicosis en área no endémica de Venezuela

Abstract: La cromomicosis es una infección crónica causada por hongos dematiáceos del tipo cromomicetos, caracterizado por nódulos y placas verrugosas de evolución lenta y prolongada. Los agentes causales penetran por inoculación traumática. En Venezuela, los estados de Lara, Falcón y Zulia de la región noroccidental son considerados como áreas endémicas. Se presenta el caso de un paciente masculino de 65 a os de edad, natural del estado Apure y procedente de Cazorla, estado Guárico, dedicado a labores de agricultura desde la infancia, quien refiere lesiones nodulares que evolucionan a placa de bordes definidos, circinados, eritematodescamativos e infiltrados en ambas regiones glúteas de 21 a os de evolución. Se realiza estudio micológico observándose al examen directo la presencia de células fumagoides, y al cultivo en medio Sabouraud se logra aislar Cladophialophora carrionii. Recibió tratamiento con itraconazol 200mg diarios por 6 meses con excelente evolución clínica. El presente caso, representa el primer reporte de esta patología en el Estado Guárico, el cual no ha sido reportado hasta ahora como área endémica, debido a la ausencia de factores epidemiológicos como la cría de chivos y presencia de vegetación.

Abstract: OBJETIVO: Identificar os fatores preditores de infec o ou coloniza o por micro-organismos resistentes. MéTODOS: Foi realizado estudo quantitativo de coorte prospectivo. Foram realizadas a análise descritiva, para conhecimento da popula o do estudo, e a análise discriminante, para identifica o dos fatores preditores. RESULTADOS: Foram incluídos 85 pacientes com infec es por micro-organismos resistentes: Pseudomonas aeruginosas resistente aos carbapenêmicos (24,7%), Acinetobacter resistente aos carbapenêmicos (21,2%), Staphylococcus aureus resistente à meticilina (25,9%), Enterococcus spp. resistente à vancomicina (17,6%) e Klebsiella pneumoniae resistente aos carbapenêmicos (10,6%). A análise discriminante identificou transferências de outros hospitais e interna o na Unidade de Terapia Intensiva como fatores preditores para ocorrência de infec o pelos grupos S. aureus resistente à meticilina, Acinetobacter resistente aos carbapenêmicos e K. pneumoniae resistente aos carbapenêmicos. Nenhuma das variáveis estudadas foi discriminante para Enterococcus spp. resistente à vancomicina e P. aeruginosas resistente aos carbapenêmico. CONCLUS O: Os fatores preditores encontrados foram: interna o na UTI e a transferências de outros hospitais. OBJECTIVE: Identifying predictors of infection or colonization with resistant microorganisms. METHODS: A quantitative study of prospective cohort was carried out. A descriptive analysis was performed in order to know the population of the study and a discriminant analysis was performed to identify the predictors. RESULTS: In this study were included 85 patients with infections caused by resistant microorganisms: carbapenem-resistant Pseudomonas aeruginosas (24.7%); carbapenem-resistant Acinetobacter (21.2%); methicillin-resistant Staphylococcus aureus (25.9%), vancomycin-resistant Enterococcus spp (17.6%) and carbapenem-resistant Klebsiella pneumonia (10.6%). The discriminant analysis identified transfers from other hospitals and hospitalization in intensive care unit as predictors for the occurrence of infections by the following groups: S. aureus resistant to methicillin, Acinetobacter resistant to carbapenems and K. pneumoniae resistant to carbapenems. None of the studied variables was discriminant for vancomycin-resistant Enterococcus spp. and carbapenem-resistant P. aeruginosas.
Estudio clínico-genético molecular de la fibrosis quística en la V Región, Chile Clinical and molecular genetic study of cystic fibrosis in the 5th Region of Chile

Abstract: Background: Cystic fibrosis (CF) is the most common autosomal recessive disease in Caucasian population. More than 900 mutations have been detected in the Cystic Fibrosis Transmembrane Regulator (CFTR) gene. The most common worldwide, is a deletion of phenylalanine 508 (deltaF508). Aim: To analyze the presence of mutations F508, G542X, N1303K, G551D, R553X and S549N in patients from the 5th Region of Chile, with a clinical diagnosis of CF. Patients and methods: We studied 17 non-related patients, presenting frequent respiratory tract infections, malabsorption and positive sweat tests, or meconial ileum. Serum immunoglobulins (IgG, IgA, IgM), and total, CD3+ and B-lymphocytes, were determined to discard the presence of an immune deficiency. The molecular study of the gene was performed by Polymerase Chain Reaction amplification and restriction analysis. Results: Immunological parameters were normal in all patients. The deltaF508 mutation was detected in 11 chromosomes and the mutation G542X in 3 chromosomes. Conclusions: The mutation G542X was the second most frequent mutation found in this sample of Chilean CF patients. Since this mutation has a high frequency in Spanish CF patients, we suggest that this mutation might have had its origin in Spain (Rev Méd Chile 2002; 130: 850-8)

Abstract: Background: The X syndrome, related to coronary disease in adults, could be possibly programmed priory to delivery, in children with intrauterine growth retardation. Aim: To measure serum lipids in newborns with symmetrical or asymmetrical intrauterine growth retardation. Patients and methods: One hundred thirty five newborns with intrauterine growth retardation and 116 normal term newborns, with 38 to 41 gestational weeks, were studied. Total, HDL, and LDL cholesterol, triglycerides and apoproteins. A1 and B were measured in imbilical cord blood samples. Results: No differences in total, HDL, LDL cholesterol, apoproteins A1 and B were observed between the study groups. Triglycerides were higher in newborns with intrauterine growth retardation, compared to normal term newborns (45 ± 27 and 36 ± 19 mg/dl respectively, p<0,001). Differences in serum triglyceride levels respect to controls were observed in both male and female newborns with asymmetrical growth retardation. Likewise the differences respect to controls were observed in newborns with mild or severe but not with moderate growth retardation. Conclusions: Newborns with intrauterine growth retardation have higher triglyceride levels than normal term newborns. (Rev Méd Chile 2000; 128: 741-8).
Embarazo en la adolescencia y su relación con la deserción escolar The relationship between teenage pregnancy and school desertion

Abstract: In Chile, the prevalence of teenage pregnancy is 17%. Aim: To assess relationship between adolescent pregnancy and school desertion. Patients and methods: At the Hospital Guillermo Grant Benavente's Departament of Obstetrics and Gynecology, in Concepción, Chile, 2001 a comparative, cross sectional and correlational study was conducted. The study group were pregnant adolescents who deserted from school system, divided in two subgroups: 86 adolescents who deserted before pregnancy and 130 who deserted during pregnancy. Results: Twenty percent of teenagers that deserted from school before pregnancy belonged to a sublevel of poverty, compared with 5% of those who deserted during pregnancy. Flunk was frequent in both but higher in girls that deserted before pregnancy (46.5 and 36.9% respectively, (p <0.001). Economic problems were the main cause of desertion before pregnancy (27.6%). Shame (41.6%) and obstetric complications (31.7%) were the main reasons for deserting during pregnancy. Seventy percent of adolescents who deserted before pregnancy had no educational, working or recreational activities. The parental educational level of both groups was low. Conclusions: There is a relationship between teenage pregnancy and school desertion. Adolescents who deserted from school before pregnancy are more vulnerable (Rev Méd Chile 2004; 132: 65-70).
Leadership and Faith in a School Tragedy: A School Principal’s Perspective  [PDF]

Abstract:

On April 15, 2008, six students (aged 16 years) and one teacher (aged 29 years) from a New Zealand school lost their lives in a river canyoning tragedy. The present study investigated the school principal’s perspective of how he led his school through the tragedy, and the role of faith in the school’s coping. The school principal was interviewed two years after the event. The school’s Christian foundation was the fundamental source of strength and guidance for the principal, as well as for students, staff, teachers, and families in the immediate aftermath of the tragedy and in the two years following (i.e., to the time of the present study), the Christian culture of the school guiding decision-making. Support from outside the school (e.g. critical incident support; teaching support from other schools; social support from community agencies and civic leaders) also played an important role in assisting the school through the tragedy, particularly in the immediate aftermath of the event. Further studies are required that allow the voices of children, families and school staff to be heard regarding leadership strategies that impact on them through a disaster.
