A dengue no Rio de Janeiro: repensando a participa??o popular em saúde

Abstract: this paper discusses popular participation in health from the point of view of an experience in dengue epidemic control in the late 1980s and early 1990s in the city of rio de janeiro. it refers specifically to a social movement called "se liga leopoldina", combining professionals (from health services and public schools) and members of popular organizations from the suburban neighborhoods served by the leopoldina railway. the paper reviews the ideas motivating actions undertaken by the movement, principally the role of popular organizations in the control of endemics. it calls attention to the danger of preconceived notions when analyzing participation by members of popular organizations in the social movement. the author also calls attention to the fact that social scientists researching social movements should focus on being part of a wider social network of conviviality rather than seeing their role as educating members of the popular organization. such networks should seek to strengthen and expand social support for the endemic control process.

Abstract: O artigo apresenta uma discuss o sobre a participa o popular em saúde, a partir de uma experiência vivida durante a epidemia de dengue no município do Rio do Janeiro. Refere-se, especificamente, ao movimento "Se Liga Leopoldina", que congregou profissionais de saúde (academia e servi os) e membros de organiza es populares da regi o dos subúrbios da Leopoldina. Busca rever as idéias que nortearam as a es do movimento, principlamente no que se refere à participa o popular no controle de endemias. Atenta-se para a necessidade de se refutar os (pre)conceitos que envolvem as análises sobre as a es dos sujeitos que integram os movimentos sociais. Identifica-se que a investiga o científica na sua rela o com os movimentos populares, antes de se colocar como formadora desses movimentos, pode e deve se colocar como integrante de uma ampla rede de convivência, buscando, antes de tudo, o fortalecimento e amplia o das redes sociais de apoio no controle dos processos endêmico-epidêmicos.

Abstract: using narratives of an experience with popular mobilization during the 1986-91 dengue epidemic in the city of rio de janeiro, the authors discuss the scientific research and technical counseling involving basic sanitation conditions for vulnerable social groups. they present research results on water distribution in the slums from the leopoldina area of the city. the research stemmed from demands by community leaders at local forums discussing health conditions. gathering, systematizing, and analyzing the data were based on what they call "shared knowledge construction", resulting by crossing accumulated scientific knowledge with popular knowledge produced as a result of living conditions and life experiences among working-class groups. finally, the authors comment on the need for local health professionals to be aware of relationships between epidemic and endemic processes and protection of life.

Abstract: Partindo do relato da experiência de mobiliza o popular durante a epidemia de dengue ocorrida no município do Rio de Janeiro, entre os anos de 1986 e 1991, o artigo busca apresentar e discutir o processo de investiga o científica e assessoria popular em torno da situa o de saneamento básico de grupos sociais vulneráveis. S o apresentados resultados da investiga o sobre o problema de abastecimento de água nas favelas da regi o, tendo como ponto de partida reclama es de seus representantes em fóruns locais de debate público da situa o de saúde. A coleta e sistematiza o dos dados e informa es analisados tiveram como base a "constru o compartilhada do conhecimento", compreendida como uma produ o onde se intercruzam o saber científico acumulado e o saber popular produzido a partir de condi es e experiências de vida da popula o. Abstract: this study analyzes the characteristics of one of the main foci for cutaneous leishmaniasis transmission in the city of rio de janeiro, brazil, examining its territorial configuration and the relations with spatial organization processes. an analytical model was applied to the process of occupation and organization of urban space on a local scale, considering the new functions acquired by the spatial elements expressed by different work relations, land use, and land value. the study employed geoprocessing techniques and classification of images obtained by remote sensing, localization of households, and cases of cutaneous leishmaniasis, associated with qualitative data on the historical process of land occupation and use. the analysis detected areas with distinct conditions of vulnerability and showed that changes in these conditions allowed production of the epidemic in a given time period and its subsequent reduction. the study contributes to monitoring of the disease at the local level and application of effective measures for cutaneous leishmaniasis surveillance and control.

Abstract: this study analyzes the specific conditions involved in dengue transmission in various areas in itaipu, a coastal neighborhood in the city of niterói, rio de janeiro state, brazil, with a focus on socio-environmental determinants and conditioning factors. four areas were selected with similar dengue incidence rates but different urban planning and socioeconomic patterns. the socio-spatial characteristics of each area were obtained through interviews with key informants and systematic observation. two distinct factors were identified that may potentially condition the risk of dengue transmission. the first related to the limited water supply and scarce financial resources in a lower-income population. the second was associated with a group having better socioeconomic status, which allowed them to store water in larger tanks. the implementation of a housing infrastructure generated by real estate speculation was a determining factor for the creation of socio-spatial segregation, resulting in different forms of receptiveness and vulnerability to dengue. in this sense, the incomplete and unequal installation of housing infrastructure is a determining factor for the differentiated generation of vector breeding sites and thus for dengue transmission.

Abstract: the reemergence, spread, and persistence of dengue are currently challenging the brazilian health system. factors related to living conditions have been addressed to understand different health outcomes. this study examines the occurrence of dengue and its relationship to living conditions in the city of nova igua?u, rio de janeiro state, from 1996 to 2004. data on dengue occurrence were obtained from the brazilian national disease notification system (sinan). a composite indicator of socioeconomic and urban infrastructure variables was created to characterize the prevailing living conditions, using 2000 census data. operations between layers were used to identify spatial associations between the composite indicator and dengue incidence by neighborhood. the results do not show a linear relationship between poor living conditions and disease occurrence, but the spatial patterns indicated greater susceptibility of areas with inequalities in living conditions and behind highway access routes. the results also suggest that such inequalities can influence the dengue time trend. thus, models that consider the interaction between socioeconomic variables (and not only the quantification of social indicators) can be useful for dengue surveillance.
Paracoccidioidomycosis mortality in the State of Paraná, Brazil, 1980/1998

Abstract: the paracoccidioidomycosis mortality rate in the state of paraná, brazil (1980/1998) was analyzed using the death registry data from the brazilian mortality information system and the estimated population from the brazilian institute of geography and statistics. to qualify the deaths, we deployed icd-9 for 1980/1995 and icd-10 for the more recent years. during this period there were 551 deaths from paracoccidioidomycosis in the state of paraná. most of the deaths were in male in the 30-59-year age group. the average annual mortality rate was 3.48 per million inhabitants, showing a tendency to stabilize during the study period. paracoccidioidomycosis was the fifth cause of mortality among the predominantly chronic infectious diseases and had the highest mortality rate among the systemic mycoses. paracoccidioidomycosis was observed in 184 counties in the state of paraná. most deaths were recorded in the north-central meso-region, and the highest mortality rate occurred in the west meso-region.

Abstract: this paper characterizes a local area in terms of sanitary conditions and environmental degradation processes that may be related to dengue transmission, and discusses the construction of new social and environmental indicators aiming at epidemiological surveillance. the studied area was ilha da concei??o, in the municipality of niterói, state of rio de janeiro. key informants were interviewed to obtain information that could be used in the construction of new indicators. comparing the data from the ibge with those provided by the informants, we found some differences. the data provided by the latter are more reliable regarding the local reality, and they enable, according to the area, the observation of different modus operandi of garbage collection and public cleansing. the cases were concentrated on flat slum and hill slum areas. it was concluded that it is important to analyze indicators referring to the frequency of water supply and garbage collection in local level studies associated with the incidence of dengue.
