Synthesis, ex Vivo and in Vitro Hydrolysis Study of an Indoline Derivative Designed as an Anti-Inflammatory with Reduced Gastric Ulceration Properties

Abstract: The compound 1-(2,6-dichlorophenyl)indolin-2-one (1), planned as a pro-drug of diclofenac (2), was easily synthesized in 94% yield by an intramolecular reaction in the presence of coupling agent (i.e., EDC). Compound 1 showed anti-inflammatory and analgesic activity without gastro-ulcerogenic effects. The chemical and enzymatic hydrolysis profile of the lactam derivative 1 does not indicate conversion to diclofenac (2). This compound is a new non-ulcerogenic prototype for treatment of chronic inflammatory diseases.

Abstract: The new compound 1-(2-chloro-6-fluorophenyl)-5-methylindolin-2-one (1), designed using the prodrug approach, was easily obtained in 85% yield and characterized by nuclear magnetic resonance, elemental analysis, mass spectrometry and infrared spectroscopy. The lactam 1 showed anti-inflammatory and analgesic activity comparable to that of the COX-2 inhibitor lumiracoxib, without gastro-ulceration effects. Stability studies demonstrated that the lactam function was stable and did not hydrolyze in pH 1.2 or 7.4. Furthermore, using a thioglycollate-induced peritonitis model, compound 1 was shown to inhibit cell migration by 50.4%, while lumiracoxib inhibited it by 18%. This compound represents a new non-ulcerogenic prototype for the treatment of chronic inflammatory diseases.

Abstract: Long-term nonsteroidal anti-inflammatory drugs (NSAIDs) therapy has been associated with several adverse effects such as gastric ulceration and cardiovascular events. Among the molecular modifications strategies, the prodrug approach is a useful tool to discover new safe NSAIDs. The 1-(2,6-dichlorophenyl)indolin-2-one is a diclofenac prodrug which demonstrated relevant anti-inflammatory properties without gastro ulceration effect. In addition, the prodrug decreases PGE 2 levels, COX-2 expression and cellular influx into peritoneal cavity induced by carrageenan treatment. Preliminary pharmacokinetic studies have shown in vivo bioconversion of prodrug to diclofenac. This prodrug is a new nonulcerogenic NSAID useful to treat inflammatory events by long-term therapy.
Dental caries in the primary dentition in public nursery school children in Juiz de Fora, Minas Gerais, Brazil

Abstract: the aim of this study was to assess the prevalence of dental caries in the primary dentition and associated variables in low socioeconomic preschool children enrolled in public nursery schools in juiz de fora, minas gerais, brazil. four public institutions were selected by geographic criteria (two in the central region and two in the peripheral region). the study population comprised 338 children (181 boys; 157 girls) aged 2-6 years old. dental caries was recorded using the decayed, missing, and filled teeth (dmf-t) index. among the examined children, 50.6% were caries-free. the mean dmf-t index was 2.03. it was higher in the peripheral nursery schools (p < 0.01). a trend towards a difference between sexes (p = 0.06) was observed. logistic regression analysis selected a previous child's visit to dentist (p < 0.001), geographic location of the public nursery school (p < 0.01), and age (p < 0.01) as predictive variables for the dmf-t index. the study showed the need for an oral health program for this population, including both curative and preventive measures in order to achieve the who/fdi goals for the year 2000, namely 50% of children free of caries at age 5-6 years.

Abstract: this work analyzes the consequences of the brazilian psychiatric reform to the care provided to the mentally ill in their family environment. it is a descriptive and exploratory study based on a qualitative approach, using the historical-dialectic materialism as its theoretical and methodological framework. the subjects of the research were 11 women that provided care to mentally ill persons. the analysis of their discourses revealed unique aspects related to caring for the mentally ill in their family environment and the difficulties resulting from the process of institutionalization of the mentally ill. the conclusion is that women who provide care for the mentally ill have an important social role that is indispensable to the consolidation of the psychiatric reform in the country.
A new Brazilian Passiflora leafminer: Spinivalva gaucha, gen. n., sp. n. (Lepidoptera, Gracillariidae, Gracillariinae), the first gracillariid without a sap-feeding instar

Abstract: Male, female, pupa, larva and egg of a new genus and species of Gracillariidae (Gracillariinae), Spinivalva gaucha Moreira and Vargas from southern Brazil are described and illustrated with the aid of optical and scanning electron microscopy. A preliminary analysis of mitochondrial DNA sequences including members of related lineages is also provided. The immature stages are associated with Passiflora actinia, P. misera and P. suberosa (Passifloraceae), and build mines on the adaxial leaf surface. Initially the mines are serpentine in shape, but later in larval ontogeny become a blotch type. Although the larvae are hypermetamorphic as in other Gracillariidae, there is no sap-feeding instar in S. gaucha; the larva feeds on the palisade parenchyma, thus producing granular frass during all instars. Pupation occurs outside the mine; prior to pupating, the larva excretes numerous bubbles that are placed in rows on the lateral margins of the cocoon external surface. This is the second genus of gracillariid moth described for the Atlantic Rain Forest, and the second gracillariid species known to be associated with Passifloraceae.

Abstract: low-cost alternatives that allow for the monitoring of population groups are important strategies in conducting cohort studies. this paper presents the procedures for the use of the national school census as a search tool for children and adolescents in follow-up studies. the study was conducted among adolescent students who took vaccinations in clinics in cuiabá, mato grosso state, brazil in 1999. a total of 86.8% were identified in the census and 0.2% in the mortality information system. of those identified, 1,716 were interviewed, representing a follow-up rate of 71.4%. in this study, the school census proved to be an effective method for finding students.

Abstract: the posterior probability of each instant being a change point was proposed by loschi & cruz (2005) as a measure of evidence that a change occurred in that instant. in this paper, the efficiency of such a measure is evaluated and it is compared with the measure proposed by hartigan (1990). poisson data sequences, with and without changes, are considered. the method is evaluated in the presence of both structural changes and atypical observations. the proposed method over performs hartigan's proposal. a series of violent crimes data from belo horizonte is analyzed by the proposed method. it was observed that the rate of violent crimes changes frequently throughout the time.

Abstract: the aim of this study was to identify expectations, prejudices, and feelings among medical students in relation to their experience in medical school at the state university in maringá, paraná, brazil. the study analyzed written interviews of 73 students who were currently taking the course in public health during the fourth year of undergraduate training, and who answered two open questions on the first day of classes. the idea was to examine the students'thoughts about medical training and the medical profession before they started the fourth year. five categories emerged from the initial reading of the answers: friendship and camaraderie as foundations during the course; medical teaching; the medical school and development of the course; physicians and the profession; and self-impressions. the results indicate that idealization of successful entry into medical school had already waned; by the fourth year, students had a better grasp of the course; they were still committed to and concerned with their training; and they were ready to enter internship with expectations of a conciliatory learning process. although they were more mature and satisfied with their choice, the study identified students who had still failed to find strategies to deal with the adversities of medical school.
