Abstract: the characteristics of the unified health system (sus) and the family healthcare strategy (esf) bring them close to complex thought, a term that refers to the theoretical and epistemological framework rather than to an adjective describing phenomena with multiple features. indications of inadequacy in the training of healthcare professionals, for them to work within family healthcare, led to a proposal to survey the training of these professionals for the expanded approach to healthcare implicated in sus/esf. focus groups among undergraduate nursing, medical and dental students at the federal university of amazonas were conducted, with analysis from a hermeneutic-dialectic perspective. the data indicated that the training for these three professions was reductionist and biomedical, focusing on individual work rather than on multi-professional teamwork. this is an inadequate framework for sus/esf, for which professional action focusing on the complexity of the socioeconomic and anthropological context of the system's users is recommended.

Abstract: medical training in public health is a topic that has gained increasing visibility in the literature, to the extent that such professionals have worked under the family health strategy (fhs). to investigate whether medical training in the state of amazonas, brazil, is consistent with the guidelines for working under the fhs, a qualitative empirical study was conducted, using a focus group and semi-structured individual interviews with students from the last year of medical school at a public university in that state. since recent medical graduates fill the fhs positions in the interior of the state, it is important to study their training in primary care. the resulting information points to gaps in the pedagogical model in medical school, in terms of training in public health and medical practice in the fhs.

Abstract: the establishment and implementation of the brazilian universal healthcare system (sistema único de saúde [sus]) guarantees universality, fairness, and establish healthcare as a duty of the state. several strategies have been developed, including the family health program which in theory redefines primary care. however, there are weaknesses in the professional education of nurses, physicians, and dentists to provide primary care to the individual, family, and community as defined by the sus. therefore, humanization of care becomes a paradox. the authors discuss those issues using narrative review and emphasizes freire's theoretical framework as an approach for college education of healthcare professionals to overcome difficulties in adopting and implementing primary care according to ideals and principles of the sus.

Abstract: the family health strategy (fhs) is based on teamwork and demands changes in the work process of the health care units, breaking the traditional model centralized on the physician's role. the present study aims to propose a theoretical model to assess power relations within the multidisciplinary team, grounded on hannah arendt's work. based on an analysis of legal documents of the fhs and on arendt's assumptions, a theoretical model for fhs evaluation was set up using a qualitative and hermeneutic-dialectic approach. in order to test the model, focus groups with health care teams and individual interviews were conducted in the municipality of manacapuru - state of amazonas (northern brazil). the results show that each professional performs his/her role in isolation, without sharing with others. the local management policy limits the collective work and does not use leadership principles; it is characterized as tyranny in arendt's perspective, which brings discredit to the fhs.

Abstract: professional action in the family health program requires that dentists have an attitude that breaks with traditional clinical practices, such as the articulation with the agente comunitário de saúde (community health agent - acs) in this context of teamwork. these questions refer to the field of sociology of health professions. this article investigates the perception of dentistry academics about the acs performance in oral health work. concerning the methodology, we used 40 interviews with dentistry academics from a federal university, and the interviews were treated by a hermeneutical-dialectical process. the results point to lack of training in multiprofessional teamwork, difficulties in working in public health, and also lack of knowledge about the tasks of the family health team members, in particular about the acs's tasks within oral health.

Abstract: objective: to describe the teaching and learning process of a pedagogical model to educate nurses for the brazilian universal health care system / family health program (uhs/fhp) in use at a higher education institution in the north of brazil. methods: focus groups and individualized semi-structured interviews with 6 senior nursing students were used to collect the data. the education theory of paulo freire and the uhs/fhp literature guided this hermeneutic study. results: there was a vertical relationship between nursing faculty and students, a lack of autonomy among nursing students, a lack of power for articulation among nursing students, and a lack of articulation of nursing faculty regarding the relationship between the teaching and learning process and the nursing practice in the uhs/fhp. conclusion: there is a need for re-orientation of the teaching and learning process of nursing at the institution studied.

Abstract: the objective of this study was to validate the alcohol use disorders identification test (audit) for a river population in the brazilian amazon. the original english version of audit was translated into portuguese, using the procedure recommended by the world health organization. the text was then back-translated and submitted to a native english translator, who approved the translation. audit was administered to 361 inhabitants for a total of three times in two weeks. data were analyzed for test/retest reliability and internal consistency. cronbach's alpha was 0.87 at the first interview, 0.87 at the second, and 0.86 at the third. test/retest reliability assessed via the intra-class correlation coefficient for the total audit scale was 0.93. area under roc was 0.805 for a cutoff of seven (sensitivity 76.4%; specificity 75%). the validated audit proved to be internally consistent and stable over time, but requires further investigation of its psychometric properties.

Abstract: this article focuses on the implementation of the family health program (fhp) in municipalities (counties) in the state of amazonas, brazil, analyzing potential population coverage, evidence of change in the health care model, and impact (2004 and 2008). implementation was classified as fair versus unsatisfactory. the findings showed an increase in the number of municipalities with the fhp implemented, as well as in the program's coverage (which nevertheless was less than 50% of the population). concerning change in the health care model, no municipalities were observed with a high rate of change. as for impact, there was a decrease in low impact and an increase in high impact. the final classification did not show a statistically significant change from one year to the next, and unsatisfactory implementation prevailed in the municipalities of the state of amazonas. evaluation of the fhp is not simple, since there are various factors involved in the issue. further studies could reveal the factors that explain the program's suboptimal implementation in the state of amazonas.

Abstract: alcohol abuse is one of the main public health problems on the list of priorities in brazil's family health strategy. the objectives of this study were to learn the views of family health nurses and physicians concerning alcohol abuse, focusing on their understanding of this issue and the way it is handled routinely by health services in the amazon. this was a qualitative study with 14 recently graduated nurses and physicians from the family health strategy in a county in the interior of the state of amazonas. two focus groups were used. the data were analyzed through thematic analysis, based on the principles of the unified national health system (sus) and human resources training. the health professionals identified risk factors for alcohol abuse, were aware of the consequences for the individual and society, and described the criteria used by disease classification systems, but they did not know how to act on this issue, thus revealing gaps in the respective university training.

Abstract: the family health strategy (fhs) in brazil has employed multidisciplinary teams, precisely for this health care model to allow linking knowledge in the provision of care. objective: this study's objective was to investigate the perceptions of physicians, nurses, and dentists concerning the functioning of multidisciplinary family health teams in the municipality (county) of coari, amazonas state, brazil. methods: this was a qualitative study with a semi-structured questionnaire applied through individual interviews with all the primary care professionals in the municipality, compared to interviews with physical therapists and psychologists from secondary care services, analyzed through dialectic hermeneutics. results: except for the psychologists' discourse, the perceptions by physicians and other family health professionals revealed weaknesses in the development of a joint intervention project, particularly necessary for the fhs, thereby revealing that multidisciplinary teamwork only occurs in theory and not according to the underlying logic of the work process in family health care.
