Abstract: En Costa Rica el dengue constituye la principal enfermedad de transmisio n vectorial. Su control requiere un conocimiento integral de su vector Aedes aegypti, el cual utiliza gran variedad de recipientes artificiales para su cri a. Objetivos: Identificar y describir los principales sitios de cría del A. aegypti en la provincia de Limón. Materiales y Métodos: El muestreo se realizo por el personal del programa de control de vectores durante el a o 2011; de forma simulta nea en los seis cantones que conforman la provincia de Limón. Resultados: Se identifico la presencia de A. aegypti en los seis cantones de la provincia de Limón. Los sitios de cría mas frecuentes fueron las llantas, los tarros o depósitos inservibles, los baldes y los tanques. Conclusiones: La proliferación del A. aegypti está directamente relacionado con el hombre, sus malos hábitos, y por la calidad de los servicios de salud de protección y mejoramiento del ambiente humano. Estos resultados demuestran que el abordaje del control del A. aegypti debe ser integral. Dengue is the main disease due to vectorial transmission in Costa Rica. Its control requires comprehensive knowledge of its vector which uses a wide variety of artificial containers for breeding. Objectives: To identify and describe Aedes aegypti breeding sites present in the Limon province of Costa Rica. Materials and methods: this survey was carried out during the year 2011 by personnel of the Vector Control Program working simultaneously in six municipalities of the studied region. Results: We identified the presence of Aedes aegypti in six municipalities in the province of Limon. The most frequent breeding sites were tires, cans or unusable deposits, buckets and tanks. Conclusions: Proliferation of Aedes aegypti is directly related to man, his bad habits, and to the quality of health care models regarding protection and improvement of the human environment. These results show that the approach to control Aedes aegypti must be comprehensive.
Propuesta de un algoritmo para evaluar la causalidad de eventos adversos en los Ensayos Clínicos de Vacunas.

Abstract: En el transcurso de los ensayos clínicos usualmente se designa a una comisión, la cual se encarga de evaluar la causalidad de los eventos adversos que se reportan y transcriben en los cuadernos de recogida de datos. Para llegar a contestar la pregunta de: si una vacuna que se está estudiando ha causado un evento adverso?, los decisores pueden tomar diferentes caminos, por lo que obviamente la conclusión final podría variar mucho y la evaluación deficiente puede generar conclusiones erróneas, lo que confirma la necesidad de disponer de un procedimiento para definir las categorías que se utilizarán para analizar y clasificar la relación de causalidad. Una forma de contestar a la pregunta es mediante la construcción de un algoritmo. En relación con los ensayos clínicos no encontramos en la literatura un algoritmo que sea el “patrón clave” para establecer o descartar la causalidad, sin embargo es factible dise arlo partiendo de la todología internacional utilizada poscomercialización, lo cual fue el propósito de este trabajo. Se dise ó una primera propuesta que fue analizada individual y colectivamente por el grupo multidisciplinario de la Gerencia Médica del Instituto Finlay. Finalmente se aprobó por consenso el algoritmo que puede ser útil para aplicar en los ensayos clínicos dentro y fuera de la institución.

Abstract: the study of the economical behaviour has left sideways all what is not considered "formal". they have ignored the people, whose economic behaviour, did not follow the rules of the market. economic contributions of the women along the history has been underestimated; since the contribution from the "collecting woman" (greater than the "hunting man"), to our days, where the change of cultural values and the incorporation of the woman to the public sphere, has caused an increase of the sanitary expense and a search in the sphere of resources to cover the needs that before were covered by the private means. economic behaviour that was part of the domestic thing, contributed mainly by the women, now is the state of welfare who has to guarantee it. this article pretends to articulate key concepts like family caretakers and nursing, busy mainly by women has been influenced by the collective conscience that linked the "care" to somewhat maternal, lower and opposite to method and not generate capital gain. all that is a heavy weight that difficult the change toward a new paradigm where the "care" is based on the scientific method and recognized as efficient.

Abstract: introduction: costa rica has undergone a gradual increase of dengue incidence, including some hemorrhagic dengue cases. objective: to analyze the bahaviour of the entomological indexes before and after the control interventions on the vector as well as to identify the breeding sites of aedes aegypti. methods: the source of information was the database from the vector control campaign workers in el cantón de limón in 15 visited locations, before and after the interventions from september through december 2007. the interventions comprised non-conventional garbage collection, destruction of breeding sites, use of abate as larvicide and heat treatment for adult vectors inside and outside the house. results: the first survey showed house-index values over 5 in 12 localities, whereas 5 exhibited increased values after the interventions during the second survey. values exceeding 30% were found due to non-visited closed houses and to families that rejected to be visited. positivity percentages in swampy places were low, 3,6 and 2,9% before and after the interventions, respectively. nine hundred one reservoirs with larvae and pupas, being the most positive cans, jars, tyres, buckets and canoes. conclusions: the critical assessment of the results showed a significant impact on the entomological indexes after the interventions; however, for future interventions, it was recommended to make a detailed review of the in situ work so as to reduce problems in the implementation and introduction of new measures.

Abstract: INTRODUCCIóN: Costa Rica ha sufrido un incremento paulatino en la incidencia del dengue, con la presencia de algunos casos hemorrágicos. OBJETIVO: analizar el comportamiento de los índices entomológicos antes y después de intervenciones de control sobre el vector, así como identificar los sitios de cría de Aedes aegypti. MéTODOS: se utilizó como fuente de información la base de datos de los trabajadores de vectores del Cantón de Limón en 15 localidades visitadas, antes y después de intervenciones sobre el vector durante septiembre-diciembre de 2007. Las intervenciones realizadas fueron: recogida de basura no convencional, destrucción de criaderos, uso de abate como larvicida y tratamiento térmico adulticida intradomiciliario y extradomiciliario. RESULTADOS: la primera encuesta mostró valores de índice casa por encima de 5 en 12 localidades, mientras que, 5 mostraron un aumento de sus valores después de las intervenciones durante la segunda encuesta. Se encontraron valores por encima de 30 % pertenecientes a viviendas cerradas y renuentes a la visita. Los porcentajes de positividad en los terrenos baldíos fueron bajos, 3,6 y 2,9 % antes y después de las intervenciones, respectivamente. Se reportaron 901 recipientes con larvas y pupas, los más positivos resultaron: diversos, tarros, esta ones, llantas, baldes, cubetas y canoas. CONCLUSIóN: la evaluación crítica de los resultados mostró un impacto significativo sobre los índices entomológicos después de las medidas de intervención, sin embargo, se recomienda para futuras intervenciones una revisión detallada del trabajo de terreno, para disminuir fallos en su implementación e incorporar nuevas medidas. Abstract: In this work, erosion tests were performed to study the behavior of an aluminium alloy known as AISI 6061-T6 against the impact action of aluminium oxide abrasive particles. This material was selected because of its high ductility and tenacity. An erosion rig based on that in ASTM G76-95 was used to conduct the tests. The alumina particles had a particle size between 300 and 400 μm. Four incident angles, 30°, 45°, 60° and 90° were used to conduct the erosion tests. The particle velocity and the abrasive flow rate were 24 ± 2 m/s and 63 ± 0.5 g/min. The room temperature was between 35°C and 40°C. Chemical analysis of the material and abrasive particles were obtained using energy dispersive X-ray analysis (EDS). SEM images were employed to identify the wear mechanisms, which were characterized by high plastic deformation. The erosion rates were obtained and the results indicated that the maximum erosion damage was achieved at 30° reducing progressively at normal incidence.

Abstract: Here we report a four-year-old female with CdLS due to a frameshift mutation in the NIPBL gene (c.1445_1448delGAGA), who also had a tissue-specific mosaic 45,X/46,XX karyotype. The patient showed a severe form of CdLS with craniofacial dysmorphism, pre- and post-natal growth delay, cardiovascular abnormalities, hirsutism and severe psychomotor retardation with behavioural problems. She also presented with minor clinical features consistent with TS, including peripheral lymphedema and webbed neck. The NIPBL mutation was present in the two tissues analysed from different embryonic origins (peripheral blood lymphocytes and oral mucosa epithelial cells). However, the percentage of cells with monosomy X was low and variable in tissues. These findings indicate that, ontogenically, the NIPBL mutation may have appeared before the mosaic monosomy X.The coexistence in several patients of these two rare disorders raises the issue of whether there is indeed a cause-effect association. The detailed clinical descriptions indicate predominant CdLS phenotype, although additional TS manifestations may appear in adolescence.

Abstract: objective: to know the clinical and histological association in presence of microcalcifications birads 4. method: retrospective analysis of 204 patients with microcalcifications birads 4, under mastectomy in barros luco-trudeau hospital between january 1993 and december 2004. the patients were marked previously with metallic harpoon in zone of microcalcifications and later radiological control of the surgical piece. results: the location of birads 4 microcalcifications was the left breast in 52% (106 cases), right breast 42% (85 cases), and both 6% (13 cases). the deferred biopsy showed 17 cases with mammary cancer (8.3 %), fibrocystic condition 197 cases (96.6%), non atypical hyperplasia 49 cases (24%), atypical hyperplasia 17 cases (8.3%), duct ectasia 7 cases (3.4%), papillomatosis 6 cases (2.9%), chronic mastitis 4 cases (2%), fibroadenoma 4 cases (2%), galactophoritis 2 cases (1%) and fat necrosis 2 cases (1%). conclusion: the birads 4 microcalcifications constitute an important mammography finding to be associated with breast cancer and high risk injuries in a high percentage
Comportamiento de parámetros dialíticos y nutricionales en peritoneo diálisis pediátrica

Abstract: behaviour of dialysis and nutritional parameters in paediatric peritoneal dialysis (pd) the management of children on pd requires constant monitoring of the dose of dialysis and nutritional support in order to maximise growth and development in the uraemic child. the measurement of a series of dialytic and nutritional variables allow the adjustment of treatment to the childs needs. objectives: 1) to measure the different variables in child undergoing pd, 2) calculate the dialysis dose (kt/v), the protein catabolic rate (pcr), the urea equivalent of nitrogen appearence (pna), the nitrogen balance, the peritoneal equilibrium test (pet) and, 3) to determine the correlation between them. patients and methods: a prospective study of 15 children undergoing chronic pd, with 59 serial measurements in a 6 month period. monthly measurements of weight/height, body surface area, urine volume, dialysis volume in 24 hrs, and the values of creatine, protein, albumin and urea in plasma, urine and dialysate, and in plasma the acid-base and electrolytes. a diet containing proteins according to rda was given. kt/v urea peritoneal and residual, pcr, pna and nitrogen balance were measured monthly, pet every 6 months. results: the mean kt/v residual and total was 1.49 ± 1 and 3.41 ± 0.86 respectively. a positive correlation between kt/v and residual creatine clearence was found (p < 0.05). there were no significant differences between the pet for creatinin (4th hour) or glucose d4/d0 at the beginning and end of the study, 0.78 ± 0.01 and 0.74 ± 0.13 for creatinine and 0.35 ± 0.11 and 0.34 ± 0.08 respectively. dietary protein intake was 3.32 ± 1.05, protein catabolism 1.32 ± 0.47 gm/kg/day, with a nitrogen balance of 2 gm/kg/day and a pnan of 0.94 ± 0.33. total and residual kt/v was correlated with the pnan, and a negative correlation between protein intake and plama bicarbonate was found. conclusions: the variables studied show a positive protein balance and a kt/v greater than the recom
