Abstract: Chronische Krankheiten wie die rheumatoide Arthritis (RA) (auch als chronische Polyarthritis bezeichnet) ben tigen eine Langzeittherapie. Ist nun eine Schwangerschaft unter einer laufenden Therapie geplant oder tritt eine Schwangerschaft unter einer laufenden Therapie ein, so stellt sich die Frage der Teratogenit t oder Embryotoxizit t der angewendeten Substanzen. Dieser Artikel soll einen Beitrag zur Anwendung derzeit g ngiger Medikamente in der Behandlung der RA in der Schwangerschaft liefern.

Abstract: Schmerzen mit oder ohne Schwellungen einzelner oder vieler peripherer Gelenke, symmetrisch oder asymmetrisch auftretend, Schmerzen im Stammskelett führen den Patienten zum Arzt. Immer mu ein entzündlicher Rheumatismus, v.a. wenn die Symptomatik ohne Zusammenhang zu einem Trauma aufgetreten ist, wegen der therapeutischen und prognostischen Konsequenzen in die Differentialdiagnose eingeschlossen werden. Diese übersicht soll helfen, einzelne Krankheitsbilder aus der Gruppe des entzündlichen rheumatischen Formenkreises zu unterscheiden.

Abstract: Die Dokumentation chronischer Erkrankungen stellt eine besondere Herausforderung für den behandelnden Arzt dar, besonders dann, wenn eine chronische Erkrankung wie die rheumatoide Arthritis mit Schmerz, Gelenkzerst rung und zunehmender Behinderung einhergehen kann. Eine entsprechend aggressive Behandlung der rheumatoiden Arthritis ist daher heute gefordert. Laborparameter alleine sind für die Dokumentation des Verlaufes und den Einsatz moderner teurer Medikamente nicht ausreichend. Um die Effektivit t und Vertr glichkeit der Behandlung in allen oben genannten Aspekten genügend dokumentieren zu k nnen, gibt es ein Instrumentarium an Scores, die einzelne Aspekte dieser Erkrankung besonders gut hervorheben k nnen und dementsprechend zur Verlaufskontrolle angewendet werden sollten. Dieser Artikel soll einen überblick über einige heute verfügbare, evaluierte Scores zur Dokumentation des Verlaufes einer rheumatoiden Arthritis bringen.
A Comparison of the Modified Score for the Assessment of Chronic Rheumatoid Affections of the Hands and the Australian/Canadian Osteoarthritis Hand Index in Hand Osteoarthritis Patients

Abstract: Objectives. To compare the modified score for the assessment and quantification of chronic rheumatoid affections of the hands (M-SACRAH) with the Australian/Canadian osteoarthritis hand index (AUSCAN) in hand osteoarthritis (HOA). Both are self-administered patient questionnaires, being designed to assess functional status, stiffness, and pain in affected patients, despite some differences in format, compass and arrangement of questions. Methods. 66 HOA patients (51 females), attending the outpatient clinic, were included. Patients completed the AUSCAN (15 visual analogue scales) (VAS) and the M-SACRAH (12 VAS). Results. AUSCAN-pain amounted to a mean of 41.9 (±2.9 SEM), AUSCAN-stiffness to 53.1 (3.7) and AUSCAN function to 42.6 (3.2). M-SACRAH-function amounted to 25.4 (2.4), M-SACRAH-stiffness to 42.6 (3.0), and M-SACRAH-pain to 43.7 (3.1). The total mean M-SACRAH was 37.2 (2.4) (all 's<.0001). The three respective domains of the two scores correlated significantly: pain: =0.73, stiffness: =0.75, and function: =0.76 (all 's<.0001). The four identical items in both scores also correlated significantly. No significant gender specific differences were observed. Conclusion. Despite a different scope of items, a significant high correlation of these two scores evaluating HOA patients could be demonstrated. We conclude that both scores are equivalently valuable for the assessment of health status in these patients.
Molecular phylogenetic evidence that the Chinese viviparid genus Margarya (Gastropoda: Viviparidae) is polyphyletic

Abstract: We investigated the phylogeny of the viviparid genus Margarya, endemic to Yunnan, China, using two mitochondrial gene fragments (COI and 16S rRNA). The molecular phylogeny based on the combined dataset indicates that Margarya is polyphyletic, as two of the three well-supported clades containing species of Margarya also comprise species from other viviparid genera. In one clade, sequences of four species of Margarya even cluster indiscriminately with those of two species of Cipangopaludina, indicating that the current state of Asian viviparid taxonomy needs to be revised. Additionally, these data suggest that shell evolution in viviparids is complex, as even the large and strongly sculptured shells of Margarya, which are outstanding among Asian viviparids, can apparently be easily converted to simple smooth shells. The molecular data also indicate that the species status of the six extant species of Margarya should be re-assessed.
The Drosophila Forkhead transcription factor FOXO mediates the reduction in cell number associated with reduced insulin signaling

Abstract: Background Forkhead transcription factors belonging to the FOXO subfamily are negatively regulated by protein kinase B (PKB) in response to signaling by insulin and insulin-like growth factor in Caenorhabditis elegans and mammals. In Drosophila, the insulin-signaling pathway regulates the size of cells, organs, and the entire body in response to nutrient availability, by controlling both cell size and cell number. In this study, we present a genetic characterization of dFOXO, the only Drosophila FOXO ortholog. Results Ectopic expression of dFOXO and human FOXO3a induced organ-size reduction and cell death in a manner dependent on phosphoinositide (PI) 3-kinase and nutrient levels. Surprisingly, flies homozygous for dFOXO null alleles are viable and of normal size. They are, however, more sensitive to oxidative stress. Furthermore, dFOXO function is required for growth inhibition associated with reduced insulin signaling. Loss of dFOXO suppresses the reduction in cell number but not the cell-size reduction elicited by mutations in the insulin-signaling pathway. By microarray analysis and subsequent genetic validation, we have identified d4E-BP, which encodes a translation inhibitor, as a relevant dFOXO target gene. Conclusion Our results show that dFOXO is a crucial mediator of insulin signaling in Drosophila, mediating the reduction in cell number in insulin-signaling mutants. We propose that in response to cellular stresses, such as nutrient deprivation or increased levels of reactive oxygen species, dFOXO is activated and inhibits growth through the action of target genes such as d4E-BP.
A Snail Perspective on the Biogeography of Sulawesi, Indonesia: Origin and Intra-Island Dispersal of the Viviparous Freshwater Gastropod Tylomelania

Abstract: The complex geological history of the Indonesian island Sulawesi has shaped the origin and subsequent diversification of its taxa. For the endemic freshwater snail Tylomelania a vicariant origin from the Australian margin has been hypothesized. Divergence time estimates from a mtDNA phylogeny based on a comprehensive island-wide sampling of Tylomelania fit regional tectonic constraints and support the ‘out-of-Australia’ vicariance hypothesis. The Banggai-Sula region of the Sula Spur, the Australian promontory colliding with West Sulawesi during the Miocene, is identified as a possible source area for the colonization of Sulawesi by the ancestor of Tylomelania. The molecular phylogeny also shows a rapid diversification of Tylomelania into eight major lineages with very little overlap in their distribution on the island. Haplotype networks provide further evidence for a strong spatial structure of genetic diversity in Tylomelania. Distribution boundaries of the major lineages do at best partially coincide with previously identified contact zones for other endemic species groups on Sulawesi. This pattern has likely been influenced by the poor dispersal capabilities and altitudinal distribution limits of this strict freshwater inhabitant. We suggest that late Miocene and Pliocene orogeny in large parts of Sulawesi has been the vicariant event driving primary diversification in Tylomelania.

Abstract: 利用线粒体基因COI和16SrRNA构建了中国特有腹足类——螺蛳属(Margarya)的系统发育关系.系统发育结果揭示,螺蛳属并非是一个单系,其主要被分为3大支,即阳宗海支、抚仙湖-星云湖-杞麓湖支、滇池-洱海支.另外,滇池-洱海支中螺蛳属种类与圆田螺属种类交错聚在一起.这一结果暗示,亚洲田螺科的种类需要进一步详细的整理,螺蛳属现存种类的分类地位也需要重新确定.
