Update on the natural history of intracranial atherosclerotic disease: A critical review

Abstract: Intracranial atherosclerotic disease (ICAD) contributes to a significant number of ischemic strokes. There is debate in the recent literature concerning the impact of the location of stenosis in ICAD on outcome. Some reports have suggested that disease processes and outcomes vary by vessel location, potentially altering the natural history and indications for intervention. Here we have performed a comprehensive, critical review of the natural history of ICAD by vessel in an attempt to assess the differences in disease specific to each of the vascular territories. Our assessment concludes that only minor differences exist between patients with different vessels affected in vessel-specific ICAD. We have found that middle cerebral artery disease confers a lower mortality than vessel-specific ICAD in other intracranial vessels, asymptomatic disease follows a more benign course than symptomatic disease, and that plaque progression or the detection of microemboli on transcranial Doppler may predict poor outcome. Given the expanding indications for treatment of ICAD and rapidly developing endovascular techniques to confront this disease, a thorough understanding of the natural history of ICAD aids the interventional neuroradiologist in determining when to treat and how to predict outcome in this patient population.

Abstract: the mach-planck controversy (1910-1911) about the cognitive status of scientific theories lead us to the following conclusions. (1) not always the one who maintains some theses that look better (planck's defense of the reality of the atoms) is the one who advanced the best arguments for criticizing his opponent (e.g. planck misinterpreted mach's views about the scientific image, its concepts and laws). (2) their views were grounded in different philosophies, and consequently, in different valuesystems. (3) there was no absolute winner-loser; each one might look like a winnerloser depending upon de historical and scientific context relative to which the choice is being made. (4) there is no algorithm and definitive argument for declaring a final winner-loser. (5) standard philosophical dichotomies like realism vs. instrumentalism are insufficient for grasping the complexity of the epistemological and ontological positions of scientists. finally, the controversy emphasizes the decisive role played by philosophy in the theoretical issues of physics.

Abstract: hypertension alters arterial wall function. the application of knowledge of the principles of arterial mechanics allows making more adequate diagnoses with better indications about how to treat patients. the development of large clinical and treatment trials produced the so-called evidence based medicine. these studies have represented a significant advance in the knowledge of the natural history of diseases and allowed establishing the best therapeutic options. however, the patient seeking medical advice has personal, geographical, cultural and emotional conditions that, in most cases, differ from those of the patients selected in the trials, and require a variant of the therapeutic approaches used in those trials. treatment guidelines were designed on the basis of the results of evidence based medicine and are useful to reduce mistakes made by those who are not experts in the topic. the specialist should consider the particular patient he/she is facing using the knowledge provided not only by evidence based medicine but also by his/her personal experience and other ways of evaluation. in this way, the specialist can justify why he/she does not follow the recommendations suggested by the guidelines for his/her patient's benefit. briefly, this would mean moving from only evidence-based medicine to personalized evidence-based medicine.

Abstract: this paper examines the assumption of a specific knowledge, which has been attributed to latin american cultural studies practitioners and constitutes its specific contribution in the political arena. i maintain here that such knowledge is possible, even though it is not supported by a mere institutional endorsement. in order to support this position, i will discuss the notions of human reason, episteme and epistemology, so to characterize the birmingham center's foundational contribution in terms of the materialistic principle to subsume any knowledge practice to data provided by concrete human beings in their concrete relationships. following this approach, i analyze the idealistic risk to fall in essentialisms and therefore i'll offer several methodological inquiries that would guide work on latin american cultural studies, providing political and epistemological coherence.

Abstract: Examino aquí la presuposición de un saber específico que se atribuye a los practicantes de los estudios culturales latinoamericanos y que constituye su aportación específica en la arena política y defiendo la posición de que ese saber es posible, pero no está garantizado por el mero aval institucional. Para sustentar esta postura, discuto las concepciones de razón humana, episteme y epistemología y luego caracterizo la aportación fundacional del Centro de Birmingham en términos del principio materialista de subordinar toda práctica de conocimiento a los datos brindados por los seres humanos concretos en sus relaciones concretas. Desde esta perspectiva, analizo el riesgo idealista de los esencialismos y propongo algunas inquisiciones metodológicas que orientarían el trabajo en estudios culturales latinoamericanos con coherencia política y epistemológica.
