Cryostimulation improves recovery from oropharyngeal dysphagia after stroke

Abstract: Introduction: Stroke is considered one of the most frequent neurological causes of oropharyngeal dysphagia. Aim: To determine the effect of cryostimulation on oropharyngeal sensitivity and, subsequently, on the swallowing reaction and premature escape of food in patients with neurogenic dysphagia after stroke. Methods: Clinical and experimental study. The study enrolled 7 adult subjects, 6 men and 1 woman ranging from 28 to 64 years of age, with a diagnosis of stroke and current oropharyngeal dysphagia without any other underlying disease. The selected subjects underwent speech-language pathology evaluation and videofluoroscopic assessment of the dysphagia. The subjects were then treated with cryostimulation consisting of 10 applications to each structure (anterior faucial pillar, posterior oropharyngeal wall, soft palate, and back tongue) 3 times a day (for a total of 30 daily applications per structure) for 4 consecutive days. The patients were then re-evaluated based on the same criteria. The pre- and post-cryostimulation results of the clinical and videofluoroscopic evaluations were analyzed descriptively and statistically using Student's t-test and Fisher's exact test. Results: Cryostimulation had beneficial effects on oropharyngeal sensitivity in 6 of the 7 subjects. There was also a significant improvement in swallowing and in the premature escape in six subjects. Conclusion: Cryostimulation increased sensitivity and subsequently improved the swallowing reaction and premature escape of food in patients with neurogenic dysphagia after stroke. These effects were evident by both speech-language pathology and videofluoroscopic evaluation.

Abstract: OBJETIVO: caracterizar o perfil de pacientes com paralisia cerebral em uso de gastrostomia e verificar o efeito que tal procedimento ocasiona nos cuidadores. MéTODO: foi realizado estudo transversal de caráter qualitativo e quantitativo. A pesquisa foi feita com pacientes do Hospital da Crian a Santo Ant nio - Complexo Hospitalar Santa Casa que tivessem diagnóstico médico de Paralisia Cerebral (PC) em uso de gastrostomia. Foi aplicado um questionário aos cuidadores contendo quest es quantitativas, qualitativas e dados sobre a caracteriza o da amostra. RESULTADOS: houve predominancia de renda mensal de até dois salários mínimos, e uma configura o familiar caracterizada pelo pai trabalhar e a m e n o atuar no mercado de trabalho. Dentre os motivos para a indica o do procedimento, a dificuldade de degluti o esteve presente entre todos os sujeitos, seguido das pneumonias de repeti o e baixo peso. Observa-se um grande número de pacientes que fizeram uso de sonda antes da gastrostomia, com tempo de permanência acima de um mês. Destacam-se os principais benefícios observados com a coloca o da gastrostomia: ganho de peso, diminui o dos problemas respiratórios e redu o de interna es. A partir dos relatos dos cuidadores percebe-se a ocorrência de sentimentos como de medo do desconhecido, resistência e aceita o, dificuldades e benefícios. CONCLUS O: observa-se que problemas de alimenta o s o frequentemente encontrados como indica o para coloca o de gastrostomia em crian as com PC. Os cuidadores sentem medo quanto à impossibilidade de alimentar a crian a pela via oral. Porém, após a cirurgia, grande parte deles relatou benefícios, como por exemplo, ganho de peso e redu o das interna es. Abstract: purpose: to identify language deficits in a group of elderly patients with dementia in tasks of free lexical retrieval, lexical retrieval with orthographic and semantic criteria, interpretation of metaphors and of narrative discourse, from the montreal communication evaluation battery - mac battery, as well as to verify the frequency of deficits. methods: participants were 13 patients with mild to moderate dementia, treated at the neurology sector of the ambulatory of movement disorders and dementia of the hospital santa clara of the irmandade santa casa de misericórdia de porto alegre (iscmpa). the neuropsychological assessment instrument was the mac battery, and the tasks were applied in the following order: free lexical retrieval, lexical retrieval based on orthographic criteria, lexical retrieval based on semantic criteria, interpretation of metaphors, and interpretation of narrative discourse. results: the group of patients with dementia showed impaired performance in all tasks from mac battery that were evaluated, with significant difference. furthermore, participants presented higher frequency of deficits, in descending order, in partial retelling of narrative discourse tasks (essential and present information), followed by the tasks of lexical retrieval with orthography and semantic criteria, and full retelling of narrative discourse. at least two patients showed deficits in some of the five tasks performed from the mac battery. conclusion: language evaluation using the mac battery allowed the verification of language processing deficits in elderly with dementia, characterizing that this instrument is also applicable to this clinical population.

Abstract: Este estudo objetiva realizar levantamento bibliográfico para verificar a possível influência da Levodopa sobre a fase oral da degluti o de indivíduos com Doen a de Parkinson. Foi realizado levantamento bibliográfico em periódicos indexados no Science Direct, periódicos Capes, LILACS, SciELO, MedLine e Scopus e livros. Para busca foram utilizados os termos disfagia, medicamentos, levodopa, Parkinson, dysphagia, drugs, swallowing disorders, Parkinson's disease, disfagia, las drogas e enfermedad de parkinson. Foram identificados 16 artigos internacionais e 25 nacionais, após a leitura de todos os artigos identificados foram selecionados 32, utilizaram-se ainda quatro livros e um resumo de anais de congresso. Para tal sele o utilizou-se como critério de inclus o aqueles artigos que considerassem o comprometimento causado pela Doen a de Parkinson, especialmente na fase oral da degluti o, bem como os efeitos da levodopa sobre a mesma, e como critério de exclus o a influência do fármaco na fase faríngea, pois geralmente ele parece ter maior efeito na fase oral. Existe discordancia na literatura quanto aos possíveis efeitos da Levodopa na degluti o. Quando estes ocorrem, geralmente afetam a fase oral desta fun o, uma vez que as dificuldades que o portador da Doen a de Parkinson já apresenta s o maximizadas pela xerostomia decorrente da administra o deste fármaco. Em virtude da grande diversidade dos resultados dos estudos encontrados, faz-se necessária a realiza o de estudos controlados para uma elucida o mais detalhada dos efeitos da Levodopa na degluti o. The purpose of this study is to conduct a literature review in order to verify the possible influence of Levodopa on the oral phase of deglutition in individuals with Parkinson disease. The survey was conducted in journals indexed in Science Direct, Capes periodicals, LILACS, SciELO, MedLine and Scopus, as well as in books. For this survey, the terms disfagia/dysphagia/disfagia, medicamentos/drugs/las drogas, levodopa, Parkinson/Parkinson disease/ enfermedad de Parkinson and swallowing disorders were used. It was possible to identify 16 international and 25 national scientific articles. After reading every identified papers, 32 were selected in addiction to four books and one abstract published in scientific conference proceedings were used. For such selection, the inclusion criteria were the papers which considered the impairments caused by Parkinson′s disease, especially during the oral phase of swallowing, as well as the effects of levodopa about it. Disfagia e acidente vascular cerebral: rela??o entre o grau de severidade e o nível de comprometimento neurológico

Abstract: the aim of this case study was to verify the occurrence of dysphagia in acute ischemic stroke within 48 hours after the onset of the first symptoms, in order to establish a possible relationship between the level of neurologic impairment and the severity degree of dysphagia. after emergency hospital admission, three patients underwent neurological clinical evaluation (general physical examination, neurological examination, and application of the national institute of health stroke scale - nihss), and clinical assessment of swallowing using the protocolo fonoaudiológico de avalia??o do risco para disfagia (pard - speech-language pathology protocol for risk evaluation for dysphagia). one of the patients presented functional swallowing (nihss score 11), while the other two had mild and moderate oropharyngeal dysphagia (nihss scores 15 and 19, respectively). the service flow and the delay on the patients' search for medical care determined the small sample. the findings corroborate literature data regarding the severity of the neurological condition and the manifestation of dysphagia.

Abstract: purpose: to analyze the concept of interdisciplinarity among teachers of the speech therapy course at the university of vale do itajaí. methods: the research was guided by a qualitative approach, through a discourse analysis following the french model, using an in-depth interview as the data collection tool. results: following the analysis, different subject positions were identified, in which knowledge of interdisciplinarity, the possible pedagogical situations in which it occurs, and the obstacles to interdisciplinary practices, were identified. conclusion: it was realized that there is still a lack of firmly-grounded knowledge on the subject of interdisciplinarity that can lead to a work model with sufficiently defined and consistent theoretical and methodological principles. finally, this work reveals a need for greater investment on research that can support the principles of interdisciplinarity in the area of speech therapy, in order to expand the horizons of this professional who needs, above all, to understand him or herself as a health professional whose daily practice requires a plural perspective.

Abstract: introduction: the act of swallowing depends on a complex and dynamic process which uses common structures to the act of breathing; respiratory problems can cause swallowing difficulties. aim: to assess the swallowing pharyngeal phase in patients with chronic cough. method: retrospective study with 15 patients of both genders, patients with chronic cough and risk factors for aspiration defined by the pneumologic diagnosis. the patients were submitted to anamnesis on complaints related to swallowing, chewing and breathing, or related to food and to videofluoroscopic examination. results: it was observed that 33.3% had normal and functional swallowing, being the last one of most prevalence. the mild dysphagia was observed in 20% of the patients, the mild to moderate dysphagia in 6.7% of them. in relation to the rosenbek scale, 73.3% of patients presented degree 1, 6.7% presented degrees 2 and 3, and 13.3% presented degree 8. the most found pathology was the chronic cough with 40%, followed by asthma with 20%; 69.2% of patients presented stasis and of these, five used protection maneuvers, of these, seven were effective and only three were used in the presence of stasis. the most used maneuver was the multiple swallowing, being effective in 100%. conclusion: there are peculiarities in the patients' swallowing with chronic cough that, although not presenting complaints relating to swallowing, it presents an important aspiration risk due to the presence of changes in breathing pattern that can intervene in the coordination between breathing and swallowing, which is essential to protect the lower airway.

Trifuntional Oligoetherols and Polyurethane Foams with Carbazole Ring  [PDF]

Abstract: The method of synthesis of trifuntional oligoetherols with carbazole ring from 9-(2,3-epoxypropyl)carbazole and die- thanolamine and oxiranes like ethylene and propylene oxide was presented. Structure and some physicochemical prope- rties of the products were described. The oligoetherols were demonstrated as good substrates for synthesis of polyure- thane foams of enhanced thermal resistance. It has been found that the obtained foams are rigid at room temperature and their apparent density was 34 kg/m3- 44 kg/m3. The water uptake was low, maximum to 10.5 mass%. Dynamic thermal analysis of this foams showed that 5% mass loss starts at 200?C - 220?C, while temperature of 50% mass loss was 340?C - 370?C. Concomitantly the increase of compression strength was observed.
