Evaluating the pre-, peri- and post-natal conditions of neonates affected by multiple congenital anomalies: a brief report

Abstract: Background: Neonatal deaths stand for almost two-thirds of all deaths occurring in infants under one year of age. Congenital anomalies are responsible for 24.5% of these cases forming a highly important issue for health policy-makers.Methods: We studied the pre-, peri- and post-natal conditions of 77 patients with multiple congenital anomalies (MCA) through genetic counseling at Several university Hospitals, in Tehran, Iran. The collected data were subsequently analyzed using SPSS software. Results: The patients did not have a good prognosis, demonstrating the need for the diagnosis of such diseases early in pregnancy to be of utmost importance. We screened for trisomy and nuchal translucency, which the first showed a low risk and the second showed normal results in most cases.Conclusion: Establishment of standards for prenatal diagnosis of congenital anomalies and monitoring their implementation seem to be necessary for the reduction of deaths due to congenital anomalies and infant mortality rate (IMR).
Comparison of Sonohysterography and Hysterosalpingography with Hysteroscopy in the Diagnosis of Intrauterine Lesions

Abstract: Background/Objective: One of the causes of inevitable abortion is structural abnormalities of the uterine cavity and endometrium, which interfere with the implantation of the embryo. We performed this study to compare the efficacy of sonohysterography and hysterosalpingography with hysteroscopy in the diagnosis of these abnormalities. Patients and Methods: This cross-sectional study was conducted on 72 infertile women who were candidates for hysteroscopy, attended to the Infertility Clinics of Vali-e-Asr Reproductive Health Research Center, affiliated to Tehran University of Medical Sciences. In this study, hysterosalpingography and sonohysterography were performed prior to hysteroscopy, which was considered as the gold-standard test for the diagnosis of the structural abnormalities of the uterine cavity and endometrium. Results: Comparing to hysteroscopy, sonohysterography had a sensitivity of 30%, a specificity of 100%, a positive predictive value of 100% and a negative predictive value of 30%; hys-terosalpingography had a sensitivity of 55%, a specificity of 68%, a positive predictive value of 41% and a negative predictive value of 60%. Conclusion: Due to the absence of the complications associated with hysteroscopy, being an uninvasive procedure, with high sensitivity, lower cost, and higher feasibility, sonohysterography seems to be a suitable choice for diagnosing intrauterine lesions.

Abstract: In this study, after power flow execution in several stages, values of input and output variables are obtained and the effect of the input parameters of system on output parameters is investigated. Each output is expressed as a linear function of input parameters and are calculated with using of linear regression based model for the IEEE 33 buses. The proposed model converges more quickly rather than power flow. Results of presented model are confirmed by the statistics analysis and power flow results (with 2% error-tolerance).
TGF-β Codon 25 Polymorphism and the Risk of Graft-Versus-Host Disease After Allogenic Hematopoietic Stem Cell Transplantation

Abstract: Some of the genotypes of cytokines are associated with acute graft versus host disease after bone marrow transplantation. The purpose of the present investigation was to find out the possible association between transforming growth factor beta-1 (TGF-β1) codon 25 polymorphism (rs:1800471) and acute graft versus host disease (aGVHD) after bone marrow transplantation from the sibling with the similar HLA among the Iranian population. "nIn this retrospective case-control investigation, 172 subjects including 86 Iranian patients and their siblings with the similar HLA as donor/recipient pairs were recruited. All of the patients were diagnosed with one group of blood disorder consisting of Acute Myeloid Leukemia (AML)=40, Acute Lymphoblastic Leukemia (ALL)=25 and Chronic Myeloid Leukemia (CML)=21. PCR-SSP method was carried out to ascertain TGF- β1 codon 25 G/C polymorphism genotypes. "nThe frequency of TGF- β1 codon 25 GG, GC and CC genotypes among all cases were 77.3%, 21.5% and 1.2%, respectively. Recipients with the GG genotype developed severe aGVHD significantly more than those with CC or GC genotypes (Odds Ratio =12.133, P=0.015). "nGenetic background of TGF-β1 may be involved in aGVHD development and/or severity in the patients who received Bone Marrow Transplantation (BMT) from their siblings with the similar HLA among the Iranian population.
Effect of Yeast (Saccharomyces cerevisiae) on Apparent Digestibility and Nitrogen Retention of Tomato Pomace in Sheep

Abstract: Twenty mature rams were used to determine the effects of yeast (Saccharomyces cerevisiae) on digestibility and nitrogen retention of tomato pomace. The animals were assigned randomly to one of the four different treatments (0, 2, 4 and 6 g/head/day yeast) with five rams per treatment in a completely randomized design. Each diet was fed for 14-day adaptation following a 7-day collection period whereas tomato pomace digestibility was measured by difference method using alfalfa as the basal diet. Yeast supplementation significantly (P < 0.05) increased digestibility of dry matter (DM), organic matter (OM), crude protein (CP), NDF and ADF of tomato pomace where the gross digestibility derived from the supplementation was superior in 4 gram yeast compared to the control group. In addition, sheep fed yeast had a marked increase in energy digestibility of tomato pomace at 4 gram level, however, yeast did not affect energy digestibility at 2 and 6 gram. Also, the nitrogen retention of tomato pomace was improved by Saccharomyces cerevisiae adding at 2 and 4 gram levels. The observed results were attributed to better digestibility and nitrogen retention of tomato pomace of 4 gram yeast group, possibly due to its better supply of rumen fermentation and microorganisms’ activity of digestive tract.
Effect of Electroplating Parameters on Microstructure of Nanocrystalline Nickel Coatings

Abstract: In order to achieve the optimum conditions for electroplating nanocrystalline nickel coating from Watts-type bath, the effect of some process parameters namely, bath temperature, current density, and saccharin addition on grain size and texture coefficient (TC= I(200) /I(111) ) of the deposits were investigated by X-ray diffraction (XRD). The results showed that in a bath containing 5 g/L saccharin, by increasing the bath temperature from 45°C to 55°C, the grain size decreased, whereas further increase of bath temperature resulted in a contrary effect. By increasing the current density from 10 to 75 mA/cm2, both the grain size and TC decreased, while further increase in current density had no significant effect on the grain size. At a given current density, the grain size and TC decreased rapidly by increasing the saccharin content before leveling off at 3 g/L of saccharin. Finally, based on the grain refining the optimum conditions for producing nanocrystalline nickel coating from Watts-type bath have been proposed.
The Relationship Between Psychological Climate and Organizational Commitment

Abstract: The aim of this study is to investigate the existence of relationships between components of psychological climate including autonomy, trust, pressure, cohesion, support, recognition, fairness and innovation and teachers, commitment to school, teaching occupation and work group. The study was conducted in one of the biggest cities (Ahvaz) in Iran, using a sample consisting of 170 teachers. Two methods were applied: Correlation analysis and multiple regression analysis. Support was found for the existence of statistically significant relationships between psychological climate and components of organizational commitment. The result of multiple regression analysis showed that trust, innovation support, fairness and recognition have significant effects on the explanation of variance of organization commitment.
Teaching reproductive endocrinology in Iran: Pilot assessment of hospital-based clinical modules for medical students at Tehran University of Medical Sciences  [PDF]

Abstract:

This pilot study evaluated effectiveness and acceptance of a new hospital-based reproductive endocrinology curriculum among Iranian medical students. A voluntary, anonymous questionnaire was used to compare two teaching methods as applied to junior medical students at Tehran University of Medical Sciences. Students were randomly assigned to one of two clinical teaching settings; no student experienced both modules. Coursework for the pilot (experimental) group (n = 19) utilized a teaching approach comprising lectures, genetics laboratory, pelvic ultrasound, small group sessions, and opportunities to observe advanced reproductive technologies such as in vitro fertilization, ICSI, and embryo transfer. A control group (n = 34) received reproductive endocrinology instruction by the ‘traditional curriculum’, consisting mainly of lectures. Students were sampled at baseline and again atthe conclusion of their reproductive endocrinology session. Pre- and post-test data were analyzed for both groups; post-test differences between groups were also compared. No significant differences in mean age or gender mix were identified between the two study groups. Overall, the ques- tionnaire did not identify any significant intergroup differences for any parameter investigated. Although student acceptance rate appeared similar for both educational modules, the ratio of students having a “favorable regard” for reproductive medicine declined only among students randomized to the control group (41.2% vs. 32.3%). This report offers the first data on teaching reproductive endocrinology to medical students in Iran. Both traditional and innovative approaches to teaching reproductive endocrinology were well-accepted by students, although negative post-test responses were more common among students in the control group. While periodic quality assessments for existing clinical teaching methods are necessary, introduction of alternative teaching approaches is also important. Additional studies are planned to evaluate the impact this initiative may have on results on standardized tests measuring reproductive endocrinology knowledge, as well as election of further specialization in training.

Prevalence of CHD in imperforated anus in patients referring toBooali Cina hospital from 1996 to 2003.(Is echocardiography required prior to surgery?)

Abstract: Background and purpose: Imperforated anus is a disease that requires surgery in the first days of life therefore, association with other diseases or anomalies is of great importance. This study aims to determine the prevalence of Congenital Heart Disease (CHD) in patients with imperforated anus.Materials and Methods: This is a descriptive study on patients with imperforated anus referring to Booali hospital of Sari, IRAN from 1996 to 2003. Previous patients medical records was reviewed and physical examination and concurrently echocardiography were done for new patients. The data were subjected to descriptive statistical analysis using SPSS software.Results: Ninty patients with imperforated anus were entered into the survey. Echocardiography revealed that 26 patients including 14 Female (57.2%) and 12 Male (42.8%) had CHD. 22 of CHD patients (84.6%) had imperforated anus(IA) of high type and 4 (15.3%) had IA of low type. CHD types consisted of : 7 patients (26.9%) ASD+PDA, 6 patients (23%) ASD+PDA+TR, 4 patients (15.3%) PDA, 4 patients (15.3%) MR, 1 patient (3.8%) TF, 2patients (7.6%) VSD and 2 patients (7.6%) ASD.Conclusion: Considering the high prevalence of CHD in IA, it seems that echocardiography is necessary in all those with IA particularly to predict outcome of surgery and prevent probable complications.
Elevated Ferrite Film Circulator with Different Permittivities for Layers: An Analytical Expression for the Input Conductance Employing Perturbation Method  [PDF]

Abstract: The main idea of this paper is to find an analytical formula for the input conductance of an elevated ferrite film circu-lator to match it systematically to the desired matching network. For solving the ferrite loaded dielectric resonator included in stripline elevated ferrite film circulator, the off diagonal components of the permeability tensor are taken as the perturbation. The electromagnetic fields computations are done for unperturbed structure. The dipolar resonant frequencies corresponding to harmonics of the resonant modes are then calculated using the perturbation integrals. The quality factor of the circulator is derived in terms of these dipolar resonant frequencies. Energy integrals are calculated to find the energy stored in the ferrite and dielectic layers. An analytical expression for the input conductance of the elevated ferrite film circulators is derived by using the quality factor and energy integrals. In this expression the ferrite and dielectric layers can have different permittivities. Some discussions about the effect of ferrite film thickness and permittivity mismatch on the bandwidth of the circulator are investigated by HFSS commercial software.
