Instrumento para classifica??o de recém-nascidos de acordo com o grau de dependência de cuidados de enfermagem

Abstract: objective: to deal about reconstruction of the instrument for classification of neonates proposed for toledo and fugulin in 2000. methods: it was considered 16 caring areas and the neonate classified in three levels of the nursing care complexity. for the content validation was utilized the delphi technique. a team of fivs experts nursing professionals of neonate, obstetric and pediatric area whom attends patients or teaches in the medical school. results: the obtained results have showed the experts agreement concerned to: the maintenance of the 16 caring area, property and intelligibility of the area contents and presence of an increasing level in the nursing care complexity.

Abstract: this study aimed to determine the reliability of perroca's instrument model for classifying patients. fifty patients randomly chosen were assessed by the nurses of the general intensive unit care of a medical school hospital. kappa's quotient was used to observe the concordance degree of the total scores by the nurses. the reliability analysis showed a kappa between 0.68 and 0.90 with 95% of ci, showing a good concordance. therefore, this study showed that this instrument model has drawn evidence of reliability.

Abstract: this is an exploratory, descriptive, prospective field study with a quantitative approach carried out at a general/adult intensive care unit in the city of s?o paulo. objectives: to evaluate the nas - nursing activities score - as a tool for measuring nursing workload, its use in measuring shifts, and its correspondence to the number of effective nursing personnel. thirty-three patients, with mean age of 70 years (+/-16.5), were classified. most were males (66.7%). length of stay in the icu was 17 days (+/- 20.4); sapsii was 41.7 (+/-17.9), with mean probability of death of 33.5% (+/- 26.8); 63.6% were transferred to intermediate care units and 18.2% died during icu stay. three hundred and ninety six nas measures (shifts) were performed (134-morning, 132-afternoon, 130-evening), with averages of 55.4 (+/-12.3) and 147 in 24-hour nas and mean of 69.6 (+/- 18.2). nas performed better in 24-hour application than in shifts, and proved to be an interesting tool for patient and nursing workload classification in intensive care.

Abstract: the purpose of this research was to check the reliability of perroca's instrument model for classifying patients, based on the particular needs of nursing care. this patients' classification instrument was applied in 50 patients randomly chosen by the nurses of the general intensive care unit of a medical school hospital. pearson's quotient correlation was used to analyze the concordance degree of the total scores by the nurses. the reliability analysis showed a high and positive correlation in relation to scores given by the nurses.

Abstract: home care brings up issues related to the number of daily hours and days of nursing service from users of these services. this study was carried out in order to identify the criteria adopted by managers and nurses for calculating the nursing staff needed in home care services. data were collected through semi-structured interviews and analyzed according to the method of content analysis. three categories were identified: patient's eligibility, time spent in care, and professional competency profile. the steps considered were: identification of daily average work load, choice of the proportion of professional categories, nursing workers' daily journey, and identification of the technical safety index for covering planned and unplanned absences. from these variables a model for calculating nursing personnel needed in home care is proposed in order to speed up the decision making process.

Abstract: this study had in its aim the construction and the validation of a patient classification instrument which has been based on the patient's individual necessities that require the nursing care. it was considered in the instrument 13 critical indicators : mental state and level of conciousness, breathing, vital signs, nutrition and hydration, movement, locomotion, corporeal hygiene, eliminations, therapy, health teaching, behavior, communication and skin. integrety. each one of these indicators ha, a 1 to 5 gradation denoting an increasing level in the nursing care complexity. the patient is classified in all the indicators in one of the five levels, in the option that better describes his/her situation. the'content validation of the instrument was done by the delphi tecnique aplication through 2 rounds. a team of 15 nursing experts who attend patients or teach in the medical school in s?o josé do rio preto were participants in this research. the obtained results have showed the experts' agreement concerned to : the maintenance of the 13 critical indicators in the instrument; property and intelligibility of the critical indicator contents and the presence of an increasing level in the nursing care complexity.

Abstract: the goal of this exploratory-descriptive study was to measure and compare nursing staff turnover levels at the university hospital of s?o paulo university. the population consisted of 255 nursing workers - 78 nurses, 20 technicians and 156 auxiliaries - who resigned from the hospital between 1997 and 2000. turnover levels were measured by means of global indicators, admission and resignation, net replacement on the job and variation rates. quantitative data analysis indicated a low nursing staff turnover rate at this hospital.
Estimating the nursing staff required in a new hospital

Abstract: opening a new hospital poses a complex and consequential set of challenges. one of these challenges is to estimate the nursing staff. the aim of this article is to report the entire process adopted to estimate the required nursing staff for a new hospital in brazil. the nursing staff was projected according to the brazilian federal nursing council (cofen). we applied an equation to estimate nursing staff and compared the results with two other existing hospitals. a significant difference (p<0.05) was observed when comparing the nurse-license practice nurse ratio recommended by cofen between the new hospital and other hospitals. this statistical difference is mostly due to reduced nurse staff in intensive care units. almost one year after the hospital opened its doors, it is necessary to review nursing staff hours with the real information to reinforce the expenditure on these personnel and to evaluate the decisions made so far.

Abstract: this paper starts from a research on the reliability and validity assessment of perroca's patient classification instrument. its purpose is to analyze the construct validity of this instrument, as well as to check its use in nursing management practice. study participants were a hundred forty-one patients, allocated in intensive care and hospitalization units of a school hospital in the interior of s?o paulo state, brazil. several statistical techniques were used to show the construct validity of the instrument. research results revealed the importance of all critical indicators, each of which has its own role in determining the patient's complexity degree in relation to nursing care. therefore, the study showed that the instrument shows evidence of validity. hence, it is useful for nursing management practice, as a guide to the patient's care needs as well as to the nursing team's workload.

Abstract: Este artigo parte de uma pesquisa sobre a avalia o da confiabilidade e validade do instrumento de classificac o de pacientes, elaborado por Perroca, e tem por propósito analisar a validade de constructo desse instrumento, bem como verificar sua aplicabilidade na prática gerencial do enfermeiro. Integraram o estudo 141 pacientes, alocados em Unidades de Internac o e Unidades de Terapia Intensiva, de um hospital de ensino no interior de S o Paulo. A validade de constructo do instrumento foi realizada por um conjunto de tratamentos estatísticos. O resultado do estudo evidenciou todos os indicadores críticos como importantes, tendo cada um papel próprio na determina o do grau de complexidade do paciente em rela o à aten o de enfermagem, assim como mostrou que o instrumento apresenta evidências de validade, estando em condi es de ser aplicado na prática gerencial do enfermeiro como norteador das necessidades de cuidado do paciente, bem como da carga de trabalho da equipe de enfermagem.
