25 gauge vitrectomy under topical anesthesia: A pilot study

Abstract: Aims: To evaluate the safety and efficacy of transconjunctival 25 gauge vitrectomy under topical anesthesia. Settings and Design: A pilot study of consecutive cases which underwent 25 gauge vitrectomy under topical anesthesia. Materials and Methods: Seven eyes of 7 patients underwent 25 gauge vitrectomy under topical anesthesia with a pledget soaked in anesthetic, for vitreous hemorrhage (2 eyes), retained cortex (1 eye) and postoperative endophthalmitis (4 eyes). Subjective pain and discomfort were graded from 0 (no pain or discomfort) to 4 (severe pain and discomfort). Patients underwent an immediate postoperative assessment, followed by day one and one week postoperative evaluation. Results: All patients had grade 0 pain during the surgery. Five patients had grade 2 pain during the placement of the sclerotomies. None of the patients required any sedation during the procedure. No inadvertent eye movements were noted during surgery. Except one patient, none required postoperative analgesics. Five eyes had a favorable outcome. No eyes in this pilot study had any procedure-related complications. Conclusion: With appropriate case selection, topical anesthesia is a safe and effective alternative to infiltrative anesthesia for 25 gauge vitrectomy. A larger series of patients with a longer follow-up is required to validate the findings of this pilot study.

Abstract: Currently most ophthalmic operating rooms are equipped with an analog video recording system [analog Charge Couple Device camera for video grabbing and a Video Cassette Recorder for recording)]. We discuss the various advantages of a digital video capture device, its archiving capabilities and our experience during the transition from analog to digital video recording and archiving. The basic terminology and concepts related to analog and digital video, along with the choice of hardware, software and formats for archiving are discussed.

Abstract: Endogenous fungal endophthalmitis is most commonly caused by Candida species and usually occurs in patients with chronic diseases such as diabetes mellitus and renal insufficiency. Voriconazole, a broad-spectrum triazole antifungal agent, attains therapeutically significant concentrations in the vitreous cavity after systemic administration. We report, the successful management of presumed endogenous Candida endophthalmitis in a patient with multiple diseases and unstable systemic status with oral voriconazole. Though fungal endophthalmitis has been successfully treated with a combination of intravenous and intravitreal voriconazole, to the best of our knowledge this is the first report in ophthalmic literature (Medline Search) on the treatment of fungal endophthalmitis with only the oral route of administration of voriconazole.

Abstract: A 59-year-old man with adenocarcinoma of stomach was prescribed capecitabine as adjuvant chemotherapy. After two cycles of therapy, patient developed hyperpigmentation on hands and feet. Examination revealed a peculiar distribution of hyperpigmentation on hands and feet and in addition, hyperpigmented spots on the dorsum of tongue. Although hand-foot syndrome (HFS) to capecitabine solely manifesting as palmoplantar hyperpigmentation has been described earlier, this is probably the first instance wherein oral pigmentation has also been found in association. In addition, this finding lends support to the growing argument of hyperpigmentation being a separate entity: different from HFS, both therefore being separate adverse effects of the same drug.

Abstract: For network computers to communicate to one another, they need to know one another's IP address and MAC address. Address Resolution Protocol (ARP) is developed to find the Ethernet address that map to a specific IP address. The source computer broadcasts the request for Ethernet address and eventually the target computer replies. The IP to Ethernet address mapping would later be stored in an ARP Cache for some time duration, after which the process is repeated. Since ARP is susceptible to ARP poisoning attacks, we propose to make it unicast, centralized and secure, along with a secure design of DHCP protocol to mitigate MAC spoofing. The secure protocol designs are explained in detail. Lastly we also discuss some performance issues to show how the proposed protocols work.

Abstract: Supported tritonX100 polyaniline nano-porous electrically active film has been fabricated successfully onto indium-tin-oxide conducting probe using electrochemical polymerization process. The doping of TX-100 in the polymeric network of PANI was suggested using cyclic voltammeter, UV-vis spectroscopy, and Fourier Transform Infrared spectroscopy. The change in the surface morphology of PANI thin film due to incorporation of tritonX-100 was investigated using Atomic Forced Microscopy and porosity has been confirmed scanning electron microscopy, respectively. The surface morphology, uniformly disperse hexagonal close packing of TX-100 in PANI matrices due to the incorporation of TX-100 in polymeric network of PANI was confirmed by Atomic Force Microscopy. The electrical conductivity of PANI-TX-100 increases from 1.06 x 10-2 S/cm-1 to 4.95 x 10-2 S/cm-1 as the amount of TX-100 increases during the polymerization. The change in the morphology and electrical conductivity of PANI due to incorporation of TX-100 prove as a promising material for the sensing application.
