Abstract: se presenta la experiencia adquirida desde 1985 en el instituto nacional de gastroenterología, con la creación de una base de datos en la que se controlan los reprints solicitados por la bibliotecaria a autores de diversas partes del mundo con la información obtenida en el current contents, así como los recibidos por los especialistas de la institución. el fondo está compuesto por reprints que son de gran utilidad para todos los especialistas que allí se desempe?an, ya que en su selección se tuvieron en cuenta tanto los temas de investigación como los aspectos de interés para la docencia y asistencia de acuerdo con el perfil del centro. esta base de datos, denominada star, dispone ya de 6 000 reprints que pueden ser fácilmente recuperados con la ayuda de una computadora en la que se almacenan las referencias de la colección.

Influencia de factores epidemiológicos en la progresión de la fibrosis en la hepatitis crónica C

Abstract: approximately 80 % of hepatitis c evolves to chronicity. the epidemiological and viral factors as well as those of the guest take part in the progression of the disease. the behavior of the epidemiological variables associated with chronic hepatitis c was described and the possible association of these ones with the intensity of fibrosis was identified. 80 patients with hepatitis c were studied and the intensity of fibrosis was determined by the metavir scoring system. they were grouped into absence or little fibrosis (f0-f1) and marked fibrosis (f2-f4) seeking for the possible statistical association with the epidemiological factors. a significant association was found among the infection's oldness estimate, the way it was acquired, the patient's age on catching it and the intensity of fibrosis. the progression of hepatits c may be determined by some epidemiological factors influencing decisively the natural history of the disease.

Abstract: a review was made to explain how the diverse cellular processes occurring in the severely damaged liver tissue and in the mesenteric circulation influence on the different systemic alterations appearing in acute liver failure, and the close relation existing among them. the acute liver failure is a clinical syndrome resulting from a mass liver necrosis generating progressive dysfunction of the organ with the presence of deep alterations in its detoxicating, metabolic, sinthesizing and excreting functions.the hypothesis of critical mass, as well as that on endotoxins-cytokines and vasoactive substances explain somehow the different molecular changes supporting the physiopathological bases of this complex entity.
Influencia de factores epidemiológicos en la progresión de la fibrosis en la hepatitis crónica C

Abstract: Aproximadamente 80 % de la hepatitis C evoluciona a la cronicidad. Factores como los epidemiológicos, virales y del huésped intervienen en la progresión de la enfermedad. Se describió el comportamiento de variables epidemiológicas asociadas a la hepatitis crónica C y se identificó la posible asociación de estas con la intensidad de la fibrosis. Se estudiaron 80 pacientes con hepatitis C. Se determinó la intensidad de la fibrosis por el sistema de puntuación METAVIR. Se agruparon en: ausencia o poca fibrosis (F0-F1) y fibrosis marcada (F2 a F4) buscando la posible asociación estadística con los factores epidemiológicos. Se halló asociación significativa entre el estimado de antigüedad de la infección, la vía de adquisición y la edad al contraerla con la intensidad de la fibrosis. La progresión de la hepatitis C puede estar determinada por algunos factores epidemiológicos que influyen de forma decisiva en la historia natural de la enfermedad. Abstract: Se hace una revisión para explicar cómo influyen y la estrecha relación que tienen los diversos procesos celulares que ocurren en el tejido hepático, severamente da ado, y en la circulación mesentérica con las distintas alteraciones sistémicas que aparecen en la insuficiencia hepática aguda, síndrome clínico resultado de una necrosis hepática masiva que genera disfunción progresiva del órgano con la presencia de profundas alteraciones en sus funciones detoxificadora, metabólica, sintetizadora y excretora. La hipótesis de la masa crítica, así como la de las endotoxinas-citoquinas y sustancias vasoactivas explican de cierta forma los diferentes cambios moleculares que sustentan las bases fisiopatológicas de esta compleja entidad. Application of a biochemical and clinical model to predict individual survival in patients with end-stage liver disease

Abstract: AIM: To investigate the capability of a biochemical and clinical model, BioCliM, in predicting the survival of cirrhotic patients.METHODS: We prospectively evaluated the survival of 172 cirrhotic patients. The model was constructed using clinical (ascites, encephalopathy and variceal bleeding) and biochemical (serum creatinine and serum total bilirubin) variables that were selected from a Cox proportional hazards model. It was applied to estimate 12-, 52- and 104-wk survival. The model’s calibration using the Hosmer-Lemeshow statistic was computed at 104 wk in a validation dataset. Finally, the model’s validity was tested among an independent set of 85 patients who were stratified into 2 risk groups (low risk ≤ 8 and high risk > 8).RESULTS: In the validation cohort, all measures of fit, discrimination and calibration were improved when the biochemical and clinical model was used. The proposed model had better predictive values (c-statistic: 0.90, 0.91, 0.91) than the Model for End-stage Liver Disease (MELD) and Child-Pugh (CP) scores for 12-, 52- and 104-wk mortality, respectively. In addition, the Hosmer-Lemeshow (H-L) statistic revealed that the biochemical and clinical model (H-L, 4.69) is better calibrated than MELD (H-L, 17.06) and CP (H-L, 14.23). There were no significant differences between the observed and expected survival curves in the stratified risk groups (low risk, P = 0.61; high risk, P = 0.77).CONCLUSION: Our data suggest that the proposed model is able to accurately predict survival in cirrhotic patients.

Abstract: we study the cyclical behavior of labor productivity in eighty industries of the chilean manufacturing sector in the 1979-2001 period. we find that labor productivity at the sector-level is procyclical but it is a-cyclical when using aggregate data. we provide an analytical and empirical explanation for this divergence. we also use an econometric model to quantify the determinants of productivity. the results indicate that technology shocks account for one half of productivity growth, thus supporting the supply shocks hypothesis as the main source of business cycles in chile. the other 50% of the productivity changes is explained by reallocation of resources from less to more productive sectors as well as the presence of increasing returns. variations in factor utilization were insignificant

Abstract: for the definitive treatment of flail chest we have introduced a technical innovation to make an external fixative aimed at attaining a thoracic stabilization by traction through external support. only cheap surgical accesories and easily available materials are used for its construction. to place and withdraw it, it is only necessary a procedure of corporal penetration of minimun access. 40 patients treated with the device were presented. an effective skeletal traction, a low index of complications and a definitive resolution of the paradoxical movement of the costal wall were obtained. multiperforated plastic tubes for external support, kirschner's wires penetrating supraperiostically into the unstable thoracic wall, steiman's pins maintaining the structural stability of aggregate, as well as 50 cm of tubular elastic gum guaranteeing an adjustable accesory traction, form a single biomechanical model that allows to obtain the desired force of traction
