Abstract: objectives: cancer is a complex process in which cytokines play an important role. cytokines are low-molecular weight soluble proteins involved in cellular transmission signals and several disorders. pro-inflammatory cytokines (il-1, tnf-a and il-6) are involved in prostate cancer development. the aim of this study was to relate the expression (analyzed by western blot and immuno-histochemistry) of several pro-inflammatory cytokines (il-1, tnf-a and il-6) with serum levels of prostate-specific antigen (psa) in normal (no pathological samples) as pathological samples (hyperplasia and cancer), in order to elucidate their possible role in tumor progression. we are also discussing the possible use of these cytokines as a potential therapeutic target. methods: this study was carried out in 5 normal, 25 benign prostatic hyperplastic (bph) and 17 prostate cancer (pc) human prostates. immunohistochemical and western blot analysis were performed. serum levels of psa were assayed by a psa dpc immulite assays (diagnostics products corporation, los angeles, ca) results: in bph, il-1a, il-6 and tnf-a were only expressed in patients with psa serum levels of 0-4 ng/ml or 4-20 ng/ml, but not in the group >20 ng/ml. in pc these cytokines were only expressed in patients with psa serum levels >4 ng/ml, although the expression of these cytokines was elevated when psa levels were >20 ng/ml. conclusions: in pc there might be an association between high expression of pro-inflammatory cytokines (il-1, tnfa and il-6), elevated serum levels of psa and cancer progression. a better understanding of the biologic mechanism of this association may improve the finding of new targets for therapy in these patients.

Abstract: during the last decade there has been a rapid development in flexible nephroscopy, flexible ureterorenoscopy, laser lithotripsy and instruments for stone manipulation. we are going to review the use of laser in the management of lithiasis in different situations. efforts should be made to minimize renal injury and lasers play a significant role in patients with urolithiasis and horseshoe kidneys, chronic renal failure, neurological patients.

Abstract: Este artículo está dirigido a profesores de inglés y estudiantes de ense anza del inglés como lengua extranjera o como segundo idioma (EFL) (ESL) principalmente, pero lectores de otras disciplinas son bienvenidos. El propósito del artículo es presentar una panorámica general de la ciencia cognoscitiva en relación con la ense anza y aprendizaje de un idioma. áreas como la psicología cognitiva, la ense anza y el aprendizaje cognitivos, y la adquisición de un segundo idioma se incluyen como parte de lo que se quiere dar a conocer. Los objetivos son el delinear someramente algunos puntos visibles e importantes de la ciencia cognitiva y relacionarlos con las características de las tendencias en cuanto a ense anza y aprendizaje de un idioma hoy en día. El autor propone tomar en cuenta dos áreas de ESL/EFL simultáneamente: la cognitiva y la afectiva, ya que esta última pareciera haberse dejado de lado últimamente, dándosele más énfasis a elementos cognitivos como las estrategias de aprendizaje. En la conclusión, el autor insiste en dar el mismo status al desarrollo eventual de ambos programas: a los del desarrollo de la autoestima y los de estrategias de aprendizaje que eventualmente se puedan estar dise ando tanto en escuelas como en colegios. This article is directed to English language teachers and / or students of teaching English as a Foreign/Second Language (EFL) (ESL) mainly, but readers from other disciplines are welcome aboard. Its purpose is to present an overview of cognitive science in relation to language teaching and learning. Areas like cognitive psychology, cognitive teaching and learning, and second language acquisition are explored as part of the panorama that is portrayed. The objectives are to briefly delineate some visible historic points and remarkable features of cognitive science, and to relate these characteristics to today`s language teaching and learning trends. The author′s proposal involves taking into account two areas in ESL/EFL simultaneously: the cognitive and the affective, since the latter seems to be overlooked nowadays and more emphasis is being given to cognitive elements like learning strategies. In the conclusion, the author insists on giving the development of eventual school or high school programs on self- esteem the same status as the designing of future learning strategies programs or the teaching / training of learning strategies in relation to subjects like English.

Abstract: El artículo Autoestima e idioma inglés: una primera discusión , trata sobre la importancia que reviste la parte afectiva en el aprendizaje, en este caso, de una lengua extranjera como el inglés, en el medio costarricense. En ese sentido, se da una visión de lo que la ciencia cognoscitiva y las corrientes humanísticas proponen en cuanto a aprendizaje, rendimiento académico y aprendizaje de una lengua extranjera se refiere. Seguidamente, se hace un recorrido histórico del tema autoestima y se definen términos como autoestima, autoestima baja y alta. Además se analiza el aspecto adquisición/aprendizaje de un idioma y se ubica el tema en el sistema educativo costarricense. También se discute la situación de la ense anza-aprendizaje de idiomas en nuestro medio y los aspectos que relacionan lo afectivo con el aprendizaje del inglés

Abstract:

With the objective to evaluate the properties, curatives of kombucha extract as an alternative treatment were used; 20 sheep creoles weaned with stunting, respiratory diseases, parasitic and malnutrition were divided into control and experiment n = 10 groups; the first one was administered by ivermectin 1% (2 mg/kg) subcutaneous, with a reinforcement of closantel 5% (0.5 mg/kg) by oral route and enrofloxacin at 5% (3 mg/kg) intramuscularly for three consecutive days and the second group by oral route 20 ml of extract kombucha per animal each three days for about 4 months, was weighed at the beginning of each month; clinical tests were conducted and statistical data indicated no significant differences in weight gain 30.370 kg/p control, 31.290, 3.503 kg/p experimentally, some showed small differences in blood samples and in stool tests for example, control ended with Eimeria 1.00 ± 0.00, creatinine, 0.17 ± 0.04 mg/dl, uric acid 23.20 5.37 ± mg/dl, 46.12 ± 4.70 MCV, 7.21 ± 0.25 g/dl total protein, 38.20 10.06 ± 1.40 and segmented neutrophils ±1.07, and on the other hand the experimental in erythrocytes present 9783000.00 ± 1036072.17 million, in hemoglobin 13.43 ± 0.51 g/dl, hematocrit 38.87 ± 2.13, and platelet 422320.000 ± 52442.578 thousands. Both groups did not exceed normal parameters, however, physically the experimental animals were better and had no parasites or respiratory problems as opposed to controls.

Abstract: In order to assess the impact of the early and late tissue repair of Kombucha extract on incised wounds in animal models, 24 Wistar male rats were used and divided into three groups: one of the groups received a topical cure on the wound with Brosin, and the other received the Kombucha extract. The third group did not receive any treatment, working as a control group. The objective was to evaluate and compare the Kombucha extract with a commercial healing product. Each one of the animals took a 1 cm wound in depth and length at a femur level on a side of the left thigh. The healing process was evaluated on an early and late phase by performing a morphometric and morphological analysis under the assumption of a faster recovery with the use of Kombucha. The results showed that there was a poor recovery in the control group. On the other hand, the other two groups, Brosin and Kombucha, were similar, with little inflammation and a high cell proliferation and migration along with basal array pulls of elastin and collagen which served for angiogenesis, repair and renovation of the new tissue.

Abstract: ResumenLa cirugía cardiovascular video-asistida incluye procedimientos extremadamente complejos, como la revascularización miocárdica y las sustituciones valvulares, no exentos de complicaciones fácilmente prevenibles y tratables. En este artículo se discuten las complicaciones de la cirugía cardíaca video-asistida, propias de la posición del paciente: respiratorias, cardiovasculares, quirúrgicas, infecciosas, entre otras. La cirugía cardíaca video-asistida brinda al paciente innumerables ventajas e implica un riesgo potencial de complicaciones evitables. El desarrollo de estos procedimientos es posible gracias a los adelantos tecnológicos y al dise o de nuevas técnicas anestésicas, ventilatorias, farmacológicas y de monitorización.AbstractThe video-assisted cardiac surgery includes extremely complex procedures such as myocardial revascularization and valve replacements, which are not exempt from easily preventable and treatable complications. In this article, the complications of video-assisted cardiac surgery, typical of patient positioning: respiratory, cardiovascular, surgical, infectious, among other complications, are discussed. The video-assisted cardiac surgery offers many advantages to the patient and involves a potential risk of avoidable complications. The development of these procedures is possible thanks to technological advances and the design of new anesthetic, ventilatory, pharmacologic and monitoring techniques.
El cólera en un distrito de Perú

Abstract: Considerando el bajo reporte de enfermos de cólera y con el propósito de conocer la realidad del problema en el Distrito Villa El Salvador, de la introducción de Vibrio cholerae (V. cholerae) en Perú, se tomó una muestra de 101 casos con enfermedad diarreica aguda (EDA) seleccionados por un muestreo sistemático a partir de un arranque aleatorio definido para cada unidad asistencial del Distrito, según la ocurrencia promedio diaria de casos con EDA atendidos respectivamente durante la semana precedente a la realización del estudio. A todos les fue llenada una encuesta epidemiológica, se les tomó también una muestra por hisopado rectal para aislamiento de V. cholerae, se encontraron 53 casos positivos (52,2 % y un intervalo de confianza entre 42,29 y 62,5 %) con diferencias significativas (p < 0,01) entre la frecuencia en adultos (67,3 %) respecto a los ni os (34,8 %). Sólo en 13 (61,9 %) de los 21 casos con antecedentes de contacto con enfermos de cólera se aisló V. cholerae, para un riesgo relativo de 1,24 (0,83Taking to consideration the low report of cholera patients and with the main knowing the reality about the introduction of Vibrio cholerae (V. cholerae) in Peru, a sample of 101 cases with acute diarrheal disease (ADD) was taken at the Distrito Villa El Salvador. They were selected by a systematic randomized sampling defined for each health care unit in the District, according to the daily average occurence of ADD cases attended a week before the beginning of the study. All of them took part in a epidemiological survey. A sample was taken by rectal swab in order to isolate V. cholerae. 53 positive cases were found (52.2 % and a confidence interval from 42.29 to 62.5 %) with significant differences (p < 0.01) between the frecuency in adults (67.3 %) and children (34.8 %). V. cholerae was isolated only in 13 (61.9 %) of the 21 cases who had contact with cholera patients, for a relative risk of 1.24 (0.83 < RR < 1.85). A high positivity was also found, 21 cases (72.4 %) among those who had raw food. A significant difference (p < 0.01) was observed in connection with those who had cooked food. In the multivariate logistic regression analysis it was only found a significant relationship with age and with the ingestion of raw food as regards the occurrence of cholera.

Abstract: an exhaustive review and update on the teaching care visit was carried out as one of the education modalities at work in the teaching-learning process of higher medical education, on the basis of the experience acquired by the authors of this article in health care during the hospitalization and observations made in the teaching categorization exercises over two decades in santiago de cuba province. this article deals with the importance of the teaching care visit, teaching care objectives, relation to clinical method, main characteristics, and stage development. it was concluded that the aforementioned care visit constitutes the primary teaching activity for the post-and pre-graduate personnel training, taking into account the large spectrum of communicative possibilities, acquisition and consolidation of skills, strengthening of values, performance and competence, as well as varieties of interpersonal relationships, and the application of didactical principles to medicine.

Abstract: Se realizó una revisión y actualización exhaustivas sobre el pase de visita médico docente asistencial como una de las modalidades de la educación en el trabajo, en el proceso de ense anza-aprendizaje de la educación médica superior sobre la base de la experiencia acumulada por los autores de este artículo en la asistencia durante la hospitalización y las observaciones efectuadas por más de 2 décadas en los ejercicios de categorización docente en la provincia de Santiago de Cuba, donde se han abordado su importancia, objetivos docentes y asistenciales, relación con el método clínico, características fundamentales y su desarrollo por etapas. Se concluyó que el mencionado pase constituye la principal actividad docente para la formación del personal en pregrado y posgrado, dado su amplio espectro de posibilidades comunicativas, adquisición y consolidación de habilidades; fortalecimiento de valores, desempe o y competencias, así como variedades de interacciones interpersonales y aplicación de principios didácticos a la medicina.
