Análises visual e volumétrica por ressonancia magnética das forma??es hipocampais em um grupo de pacientes com diagnóstico clínico de epilepsia do lobo temporal

Abstract: the purpose of this study is to test the sensitivity of the volumetric analysis compared to the visual analysis of the hippocampal formations of a group of 153 patients with clinical diagnosis of temporal lobe epilepsy and candidates to temporal lobectomy, evaluated by magnetic resonance (mr), using a 0.5 tesla machine and inversion-recovery t1-weighted images and 5 mm coronal slices. there was agreement between the prospective visual analysis and another retrospective visual analysis carried out by two independent observers (c=0.748 and c=0.720). there was also agreement between the retrospective analysis of the two investigators (c=0.733). there was genuine agreement (c=0.788) between the results of the qualitative and quantitative analyses carried out prospectively. volume measurements showed a nonsignificant tendency to lateralize more cases of clinically presumed hippocampal atrophy. our results confirm the reliability of a qualitative visual analysis and indicate the utility of hippocampal volumetry as a supplementar, objective and quantitative measure of hippocampal sclerosis.

Abstract: Visando comparar a sensibilidade da avalia o volumétrica com a análise visual na avalia o por ressonancia magnética (RM) das forma es hipocampais de pacientes com epilepsia do lobo temporal refratária e candidatos à lobectomia temporal, estudamos 153 casos com diagnóstico clínico de epilepsia do lobo temporal, utilizando um equipamento de 0,5 Tesla, com técnica de inversion-recovery ponderada em T1, com cortes de 5 mm no plano coronal. Houve boa concordancia entre a análise visual prospectiva e outra retrospectiva, realizada por dois observadores independentes (C=0,748 e 0,720). Houve também concordancia entre a análise retrospectiva dos dois observadores (C=0,733). Houve ainda concordancia genuína (C=0,788) entre os resultados das análises quantitativa e qualitativa realizadas prospectivamente. A análise quantitativa demonstrou uma tendência n o-significativa a lateralizar mais casos de atrofia hipocampal presumida clinicamente. Nossos resultados confirmam a confiabilidade da análise visual qualitativa e indicam a utilidade da volumetria hipocampal como uma medida suplementar, objetiva e quantitativa, de esclerose hipocampal.
Liver transplantion in a patient with rapid onset parkinsonism-dementia complex induced by manganism secondary to liver failure

Abstract: bilateral and symmetric globus-pallidus hyperintensities are observed on t1-weighted mri in most of the patients with chronic liver failure, due to manganese accumulation. we report a 53-year-old man, with rapid onset parkinsonism-dementia complex associated with accumulation of manganese in the brain, secondary to liver failure. a brain mri was performed and a high signal on t1-weighted images was seen on globus-pallidus, as well as on t2-weighted images on the hemispheric white-matter. he was referred to a liver-transplantation. the patient passed away on the seventh postoperative day. our findings support the concept of the toxic effects of manganese on the globus-pallidus. the treatment of this form of parkinsonism is controversial and liver-transplantation should not be considered as first line treatment but as an alternative one.
The role of the irritative zone and of the number and distribution of calcifications in the severity of epilepsy associated with intracranial calcifications

Abstract: objective: to determine the influence of the location of the irritative zone, and the number and the distribution of the intracranial calcifications in the severity of epilepsy associated with intracranial calcifications. method: we studied 47 patients with epilepsy and intracranial calcifications, 24 with normal (group a) and 23 with abnormal interictal eegs (group b), a control group (n=21) with abnormal interictal eegs and normal ct-scans (group c). clinical, electroencephalographic and neuroradiological features were compared among groups. results: temporal lobe interictal eeg abnormalities were found in 23/24 group b patients, and in all group c patients. most group b and group c patients presented temporal lobe seizure symptomatology, whereas in most group a patients symptomatology was rolandic (p=0.0001). epilepsy was more severe in group b and group c patients than in group a patients (p=0.0001 and p=0.0054). no relationship was found between the number of calcifications and epilepsy severity. conclusion: an irritative zone at the temporal lobe is more relevant in determining the severity, symptomatology and frequency of seizures than the number and location of calcifications.

Abstract: we studied 5 patients with medically refractory temporal lobe epilepsy using special methods of mri, in order to localize the epileptogenic area. coronal sequences were obtained with 5 mm sections from the posterior margin of the brainstem to the base of the frontal lobe using: "inversion recovery t1" (tr=2500 msec, te=26 msec, t1=600 msec) to analyse loss of internal structure and atrophy of the hippocampus, and morphology of the temporal lobe; and spin-echo t2w (tr=250 msec, te=120 msec) looking for abnormal high intensity signal. in two cases with left focal eeg the mri showed atrophy, loss of internal structure and abnormal high intensity signal in the hippocampus, and atrophy of the anterior temporal lobe, ipsilateraly. one case with independent bilateral foci at the eeg showed atrophy, loss of internal structure and abnormal high intensity signal in the right hippocampus. other case with right focal eeg showed bilateral atrophy of the anterior temporal lobes. another case was normal. we conclude that this methodology can facilitate the surgical indication in temporal lobe epilepsy.

Abstract: Os autores apresentam o caso de paciente do sexo masculino, 62 anos de idade, com emagrecimento há quatro meses e diminui o da for a muscular associada a parestesias em membros inferiores há dois dias. Foi submetido a mielotomografia, que demonstrou massa no mediastino posterior com destrui o dos corpos vertebrais e invas o do canal medular, além de espessamento irregular das paredes do es fago. Na evolu o, foi submetido a estudo contrastado do es fago, que demonstrou falha de enchimento irregular. A biópsia confirmou a presen a de carcinoma de células escamosas. Este é o primeiro relato na literatura latino-americana (Lilacs) de carcinoma de es fago com invas o de canal medular e manifesta o inicial de síndrome de compress o medular.

Abstract: ossification of the posterior longitudinal ligament (opll) is an uncommon cause of compressive myelopathy in the caucasian population. a case of spastic paraparesis in a caucasian man whose radiological investigation showed opll is presented. the radiographs of the cervical spine showed a strip of bony density posterior to the vertebral bodies, extending from c2 to t1. computerized tomography (ct) and ct myelography showed opll at the same level. magnetic resonance showed an area of increased signal on t2-weighted sequences at c7-t1 level suggestive of myelomalacia. the patient underwent an open-door laminoplasty (c2 to c7) with improvement of the paraparesis. opll should be included in the differential diagnosis of cervical myelopathy. it can be easily detected by plain radiographs and ct of the cervical spine. a review of the clinical and radiological features and the treatment of opll is presented.

Abstract: the authors report the case of a 62-year-old male with a 4 month history of weight loss and a 2 day complaint of weakness and paresthesia on the lower limbs. a computed tomography myelogram revealed a mass in the posterior mediastinum associated with destruction of the vertebral body, spinal canal extension and irregular esophageal wall thickening. the patient was later submitted to a barium esophagogram that showed an irregular filling defect. a biopsy confirmed the presence of a squamous cell carcinoma. this is the first report in the latin-american literature (lilacs) of a patient with an esophageal carcinoma with spinal canal extension and spinal cord compression syndrome at initial presentation.

Abstract: This paper analyzes the Indigenous people’s struggle for an intercultural, specific, differentiated school intended for the valorization of their culture and identity. Aiming to catechize and adapt Indigenous peoples to the western culture, settlers introduced school education as a means to make the Indians give up their identities and integrate them into the national culture. This paper evidences that the Indians have opposed the colonial school right from the start, and by means of struggle and organization, they obtained the right to an intercultural, specific, differentiated Indigenous school. However, obtaining this right had not put an end to their struggle, as the end of colonial school did not mean the end of coloniality.

Abstract: In this work numerical codes carried out in a multiconfiguration Har-tree-Fock relativistic (HFR) approach for the P IV ion are used to obtain the oscillator strengths of each transition as well as the lifetimes of each energy level. With the existing data from several authors that contributed to the spectrum using different light sources, and optimizing the electrostatic parameters by a least-squares procedure when replacing the theoretical values by the experimental ones in the energy matrices, one obtains closer values and according to the observations for the intensities, and also of the lifetimes closer to those that would be obtained experimentally.
