Abstract: septic arthritis of the pubic symphysis is an unusual disease. its clinical presentation includes pubic pain, fever and alteration of serum inflammatory markers in most cases. we report a 23 year-old male presenting with pubic pain and fever. magnetic resonance showed an arthritis of the pubic symphysis and a 18 x 8 mm liquid collection. the patient was treated with antibiotics with a good clinical response. two weeks after discharge, he was completely asymptomatic (rev méd chile 2007; 135: 1040-3)

Abstract: Antecedentes: El tumor de células de la granulosa (TCG) ovárico es considerado una neoplasia de la cuerda sexual, de baja frecuencia, en su mayoría debuta en etapas tempranas, con patrón de crecimiento lento y puede presentar metástasis tardías. Objetivo: Presentar un caso clínico de metástasis pulmonares de TCG ovárico 26 a os después del diagnóstico. Caso clínico: Mujer de 70 a os, con antecedentes de TCG ovárico diagnosticado en 1976, tratado con histerectomía, salpingooforectomía bilateral y quimioterapia triasociada. En buenas condiciones generales hasta el 2002, detectándose múltiples nodulos tumorales pulmonares bilaterales correspondientes a metástasis de TCG. Recibió quimioterapia con platino y etopó-sido. Actualmente la paciente se encuentra viva, con enfermedad tumoral pulmonar de lento crecimiento. Conclusión: A lo mejor de nuestro conocimiento, se trata del tercer caso a nivel mundial de un TCG ovárico con metástasis pulmonares tardías, 26 a os posdiagnóstico del tumor primario. Se se ala el curso clínico de lenta evolución de esta neoplasia y la relevancia de los controles a largo plazo incluyendo niveles de estradiol o inhibina. Background: Granulosa cell tumor of the ovary (GCT) of the ovary is a sex cord related tumor. It is a low frequency neoplasm with low clinical progression but it may show late metastases. Objective: To present a 70 year old female with a GCT of the ovary with multiple pulmonary metastases after 26 years from the initial diagnosis of the primary tumor. Clinic case: The patient had an ovarian GCT diagnosed by histology in 1976 and underwent hysterectomy, bilateral salpingoophorectomy and chemotherapy. The clinical course was satisfactory until 2002 when she presented with respiratory symptoms and images showing multiple bilateral pulmonary nodules that were histologically confirmed to be GCT metastases. The patient underwent chemotherapy with platinum and ethoposide. Actually the patient is alive with metastatic disease. Conclusion: To the best of our knowledge, this is the third case in medical literature of a GCT of the ovary with late pulmonary metastases 26 years after the initial diagnosis. This case illustrates the protracted clinical course of the neoplasm and the importance of the long term follow up including levels of estradiol and inhibin.
Valores plasmáticos de proteína C reactiva (PCR) en cirugía ortopédica electiva

Abstract: c reactive protein (crp) is used as a marker of inflammatory, infectious and surgical processes. aim: to measure post operative plasma crp levels after elective orthopedic surgery. materials and methods: four groups of 25 patients each were defined according to the type of surgical procedure performed. group i corresponded to pure arthroscopic meniscectomy. group ii to arthroscopic anterior cruciate ligament reconstruction or knee osteotomy. group iii to elective primary total hip or knee arthroplasty and group iv to instrumented elective spine surgery. plasma crp levels were measured prospectively at the preoperative day and at the 1st, 2nd, 3rd, 7th and 15th postoperative day. patients that had any complication were evaluated independently. results: in all uncomplicated patients, a similar shape and distribution of plasma crp curves were observed, with maximum postoperative levels observed between the second and third day. patients with more complex surgical procedures had higher crp values. in all groups, crp levels decreased significantly at 7th and 15th day, with respect to the higher measured value. in patients with surgical complications, pcr values were higher or did not decrease significantly at the seventh postoperative day, as in patients without complications. conclusion: plasma crp levels fluctuate after elective orthopedic surgical procedures, with maximum values observed between the 2nd and 3rd postoperative day followed by an ulterior decrease. this behavior changes if postoperative complication appear. serial measurements of crp levels may be useful in the early detection of surgical complications

Abstract: starting from a theoretical revisión of the concept of decentralization, this paper proposes a definition which underlines the requirement of an effective transferring of power,functions and financial resources to autonomous subnational governments. this definition exeludes other phenomena usuallypresented as decentralization, such as privatization, delegation, deconcentration, and regional and local development. this article offers a novel systematization of legislative bilis approved in chile between 1990 and 2008. the analysis of 174 laws relevant on regional and local matters allows to asses the so called chilean decentralization process, concluding that its real decentralizing effect is debatable. in addition, the review ofthe chilean case compels a revisión of the initial working definition.

Abstract: this work analyses the chilean decentralization process by performing a thorough revision of press sources regarding the political debate revolving on decentralizing bills passed between 1991 and 2005. the study describes both the actors and the context in which reforms were approved in chilean congress. in discussing the literature on the politics of decentralization, this paper confirms the relevance of two existing hypotheses: the effects of democratization, and the prospects of electoral performance. at the same time, it reassesses a third theory concerning decentralization as a top-down process by stressing the mayors' increasing power in chilean politics. in addition, this paper provides a novel systematization of municipal and regional electoral results, by party and by coalition, for the period under study

Abstract: the year 2007 was the second in michelle bachelet’s presidencial term. it was a year free of elections, exhibiting a fairly good economic performance, despite the high rate of inflation shown during the last months. public discussion on corruption escandals was frequent; however, the most important issue was the disestrous beginning of the reform on the public transportation system of the country’s capital: transantiago. this has placed a veil over the important achievements on the pension system and education, questioning not only the government’s capacity, but also opening and edge of indiscipline within the ruling coalition (concertación), which is a symtom of its erosion after 17 years in the presidential office.

Abstract: the paper describes the political events of 2006, featuring the election and assumption of michelle bachelet as chile's president in march, and the death of the former dictator augusto pinochet in december. even though the new government augured in tune with the citizenship, and despite good economic performance, the political institutions exhibit a growing distrust from citizens, which started early with demonstrations of high school students, and a rollover of corruption scandals. in part, these problems can be seen as the government's own fault, including a weak cabinet of ministers. however, the main reason for most of the wear away is the current moment of the concertación, the ruling coalition with 17 years in power
