Abstract: the present article purports to analyze aspects related to the concept of quality of life, taking into account the relevance of setting down parameters that allow for the evaluation of those aspects, especially in patients with chronic diseases. the analysis considered quality of life as one of the essential components of medical care, and pointed out the precautions that should be taken to choose instruments of evaluation, that should be sensitive and reliable to the dimensions they intend to evaluate.

Abstract: the aim of this study was to stratify 305 elderly (> 65 years) treated at an academic health center (botucatu, s?o paulo, brazil) according to risk of repeated hospitalization. data collection used an instrument to evaluate risk of repeated hospital admissions, including eight health indicators: self-rated health, hospitalizations, doctor visits, diabetes, cardiovascular disease, gender, social support, and age. 56.4% of interviewees presented low probability of repeated hospitalization, as compared to 26.9% medium, 10.5% medium-high, and 6.2% high probability. combining health indicators with the probability of repeated hospitalization, for elderly classified as medium, medium-high, and high risk compared to low, the relative risk was significant: medium or bad health (2.31); hospitalization (2.38); > 3 doctor visits (1.75); diabetes (2.10); cardiovascular disease (2.76); male gender (1.68); and > 75 years of age (1.62). the instrument was able to stratify elderly at risk of repeated hospitalization and is thus useful for reorganizing public health services.

Abstract: the article discusses the development of the physician's identify, starting with professional choice and education. based on the authors' experience in medical education, especially in the discipline of medical psychology, as well as on the literature in this area, certain elements are discussed, such as: the idealization of the physician's role, the conscious and unconscious motivations behind the choice of a profession, the difficulties in the first years of medical school, the beginning of didactic activities in the hospital and the psychological defensive mechanisms triggered by contact with patients. it is essential that medical schools and their professors be aware of these aspects. they should not only be concerned with curricular and pedagogical issues, but also pay special attention to the teacher-student relationship, given its fundamental role in the establishment of a medical identity.

Abstract: introduction: there are few published articles on group psychotherapy for patients with panic disorder (pd) and usually they are restricted to the cognitive-behavioral approach. objective: to describe an experience of group psychodramatic therapy for pd patients, started in 1996 at botucatu medical school - unesp, and discuss psychodynamic aspects of the panic disorder. methods: two-hour monthly psychodramatic sessions, with an average of 8 to10 patients were carried out. the patients were usually receiving concomitantly psychopharmacological treatment. the subjects to be discussed were freely chosen, ranging from issues directly related to pd (most common symptoms, hurdles to get to a diagnosis, hypochondriac concerns, limitations and dependency, family reactions, strategies of exposure and coping, and effects of drug treatment) to individual conflicts and problems. results: the psychodramatic approach favors the identification and elaboration of psychodynamic aspects. the most common feelings observed were helplessness and other related ones, such as: insecurity; vulnerability and fear; anger and guilt; isolation and difficulty in identifying and expressing emotions (which manifested physically); and difficulty in assuming the role of caretaker and other responsibilities. conclusions: valuable aspects for the treatment of pd, such as mutual support, partnership, confidence, modeling and encouragement, are favored in a group setting, where sharing experiences and sufferings helped to improve the participants' self-esteem, their ability to express emotions and to play different social roles. besides the clinical improvement, the symptoms could be better understood and elaborated, and meaningfully integrated into their life context.
O ciúme enquanto sintoma do transtorno obsessivo-compulsivo

Abstract: morbid jealousy (mj) is an important problem in psychiatry, which involves some risks and much distress, and may occur in many mental disorders. phenomenologically, it can be manifested in different ways, such as an obsessional, overvalued or delusional idea of infidelity. however, its manifestation in obsessive-compulsive disorder (ocd), as an obsession usually associated with checking rituals, is not much documented. to address this specific issue, considering that little has been published about mj in ocd, this article describes and discusses four selected clinical cases, emphasizing diagnostic and therapeutic aspects. the comprehension of some cases of mj as a manifestation of ocd, even when not clearly ego-dystonic, enhances therapeutic possibilities and may improve the prognosis.

Abstract: background: obsessive-compulsive disorder (ocd) has been considered by the world health organization as the 10th leading cause of years lived with disability, but studies on quality of life (qol) in ocd are still scant. objectives: a literature review of studies on quality of life in ocd. method: a medline, psychoinfo and lilacs literature review from 1980 to 2007 was conducted, using the following key-words: obsessive-compulsive disorder, quality of life, functional impairment and disability. results: population-based studies have shown indirect indicators of qol impairment in ocd sufferers, such as more unemployment, lower income, lower number of marriages, besides considerably high rates of suicidal thoughts and attempts. clinical studies found more qol impairment in ocd patients compared to individuals with some chronic physical conditions, other anxiety disorders, depressive disorders and even schizophrenia, in some aspects. conclusion: the negative impact of ocd on qol can be severe, affecting several life domains and affecting family members as well. psychosocial rehabilitation should be an essential part of ocd treatment and it is necessary to increase both public awareness of this disorder and the availability of appropriate treatments to minimize such impact, which can be devastating.

Abstract: INTRODU O/OBJETIVOS: Há poucos trabalhos publicados sobre psicoterapia grupal para pacientes com transtorno de panico (TP); além disso, esses estudos geralmente s o restritos a abordagens cognitivo-comportamentais. O objetivo deste trabalho é relatar uma experiência de atendimento psicoterápico psicodramático grupal para portadores de TP, iniciada em 1996 na Faculdade de Medicina de Botucatu/Unesp, e discutir aspectos psicodinamicos desse transtorno. MéTODOS: Atende-se uma média de oito a dez pacientes em sess es mensais de duas horas de dura o, nas quais se utilizam técnicas psicodramáticas. O uso associado de psicofármacos é a regra. A temática é aberta, centrando-se tanto em aspectos próprios do TP (sintomas mais comuns; peregrina o em servi os médicos até o diagnóstico; preocupa es hipocondríacas; limita es e dependência; rea es dos familiares; estratégias de exposi o e enfrentamento; efeitos e rea es dos medicamentos) quanto em problemas individuais específicos. RESULTADOS: A abordagem psicodramática favorece a identifica o e a elabora o de aspectos psicodinamicos. Entre estes, destacaram-se os sentimentos de desamparo e seus desdobramentos: inseguran a; fragilidade e medo; raiva e culpa; sentimentos de desamor e rejei o; vergonha e inferioridade; isolamento e dificuldade de pedir ajuda; dificuldade de identificar e de expressar sentimentos - manifestados somaticamente -; e dificuldade de assumir o papel de "cuidador" e outras responsabilidades. CONCLUS ES: Aspectos valiosos para o tratamento do TP, como apoio mútuo, companheirismo, confian a, modelo e estímulo, s o favorecidos pelo contexto grupal homogêneo. O compartilhar de experiências e sofrimentos comuns propiciou rápida coes o e suporte grupal, melhora da capacidade de expressar sentimentos e da auto-estima e o aprimoramento de papéis sociais. Além da melhora dos sintomas, possibilitou-se a melhor compreens o e elabora o destes, que passam a ser integrados significativamente no contexto existencial.

Abstract: O ciúme patológico (CP) é um problema importante para a psiquiatria, que envolve riscos e muito sofrimento, podendo ocorrer em diversos transtornos mentais. Psicopatologicamente pode se apresentar de formas distintas, tais como idéias obsessivas, prevalentes ou delirantes sobre infidelidade. Entretanto, sua apresenta o no transtorno obsessivo-compulsivo (TOC), como uma obsess o em geral associada a rituais de verifica o, é relativamente pouco documentada. Para discutir esse aspecto específico do CP, este trabalho é ilustrado com quatro casos clínicos selecionados, em que o CP tem tais características, abordando aspectos de diagnóstico e tratamento. A compreens o de alguns casos de CP como uma manifesta o do TOC, mesmo quando n o claramente egodist nica, amplia as possibilidades terapêuticas e pode melhorar o prognóstico.
Um estranho à minha porta: preparando estudantes de medicina para visitas domiciliares

Abstract: this study describes the experience in training first-year students at the botucatu school of medicine/unesp to conduct home visits as part of the course on university-health service-community interaction. faculty members from the departments of psychology and psychiatry coordinated the activity, with two groups of 45 students each, by means of the sociodrama technique, which includes warm-up, dramatization, and shared reflections. the main themes brought up by students in the dramatizations were: a) strife and violence; b) poverty and need; c) disappointment and disinterest; d) disempowerment and failure; and e) rejection and distrust. in just a few hours, it was thus possible to experience and elaborate on numerous potentially frightening situations and emotions that are rarely approached during undergraduate medical training. this experience emphasizes the need to prepare undergraduates to make contact with multiple, complex determinants of the health-disease process and act in health care from an interdisciplinary and inter-sector perspective. it is essential for students to have an expanded view of patients within the family and socio-cultural context, drawing on partnership, collaboration, and mutual understanding.
